Supervised vs home-based exercise program in kidney transplant recipients: A pilot pragmatic non-randomized study

BACKGROUND Although the benefits of exercise for kidney transplant recipients(KTRs)have been widely demonstrated,these patients experience several barriers in undertaking a structured exercise program in hospital and non-hospital *** To compare the effects of a supervised moderate-intensity gym-based intervention with a home-based low-intensity walking program on exercise capacity in *** KTRs were asked to choose between two six-month *** first group performed a low-intensity interval walking intervention at home-based exercise intervention (HBex). The second group performed a supervised training program at an adapted physical activitygym (Sgym), including aerobic and resistance training. The outcomes, collected at baseline and at the end of theprograms, included the 6-minute walking test, the peak oxygen consumption (VO_(2)peak) during a treadmill test,the 5-time sit-to-stand test, and blood *** patients agreed to participate and self-selected into the HBex (n = 9) and Sgym (n = 8) groups. Twopatients in the Sgym group dropped out because of familial problems. At baseline, patients in the HBex group weresignificantly older and had lower walking distance, VO_(2)peak, and lower limb strength. Primary outcome changeswere significantly greater in the HBex group than in the Sgym group (52 ± 23 m vs 8 ± 34;P = 0.005). No othersignificant differences between groups were observed. Both groups improved most of the outcomes in the withingroupcomparisons, with significant variations in VO_(2) ***-month moderate-intensity supervised or low-intensity home-based training programs effectively improvedexercise capacity in KTRs. Gym-based programs combine aerobic and resistance training;however, in-homewalking may be proposed for frail KTRs.

The Global boundary Stratotype Section and Point （GSSP） for the base of the Ladinian Stage （Middle Triassic） is defined in the Caffaro river bed （45°49＇09.5＂N, 10°28＇15.5＂E）, south of the village of Bagolino （Province of Brescia, northern Italy）, at the base of a 15-20-cm-thick limestone bed overlying a distinct groove （＂Chiesense groove ＂） of limestone nodules in a shaly matrix, located about 5 m above the base of the Buchenstein Formation. The lower surface of the thick limestone bed has the lowest occurrence of the ammonoid Eoprotrachyceras curionii （base of the E. curionii Zone; onset of the Trachyceratidae ammonoid family）. Secondary global markers in the uppermost Anisian include the lowest occurrence of conodont Neogondolella praehungafica and a brief normal-polarity magnetic zone recognized in closely correlated sections including the principal auxiliary section at Seceda in the Dolomites. The GSSP-level is bracketed by U-Pb single zircon age data from volcaniclastic horizons, indicating a boundary age of ca 241 Ma.

Immunohistochemistry panel segregates molecular types of hepatocellular carcinoma in Brazilian autopsy cases

AIM: To assess the distribution of proteins coded by genes reported as relevant for the molecular classification of hepatocellular carcinoma (HCC).METHODS: In this retrospective cross-sectional study, the following clinicopathological data were analyzed in 80 autopsied HCC patients: sex, age, ethnicity, alcohol intake, infection with hepatitis B and/or C virus, infection with human immunodeficiency virus, prior treatment, basic and immediate causes of death, liver weight, presence of cirrhosis, number and size of nodules, gross pattern, histological grade and variants, architectural pattern, invasion of large veins, and presence and location of extrahepatic metastases. The protein products of genes known to be involved in molecular pathogenesis of HCC, including epidermal growth factor receptor (EGFR), MET, keratin 19 (K19), vimentin, beta-catenin, mechanistic target of rapamycin (mTOR), extracellular signaling-related kinase (ERK)1, ERK2, Ki67, cyclin D1, caspase 3 and p53, were detected by immunohistochemistry on tissue microarrays. The expression levels were scored and statistically assessed for correlation with HCC ***: Infection with hepatitis C virus was identified in 49% of the 80 autopsy patients, cirrhosis in 90%, advanced tumors in 95%, and extrahepatic metastases in 38%. Expression of K19, p53 and ERK1 correlated to high-grade lesions. Expression of ERK1, nuclear beta-catenin, cyclin D1 and ERK2 correlated to higher rates of cell proliferation as determined by Ki67. Expression of MET, EGFR (> 0) and caspase 3 correlated with lower histological grades. Expression of EGFR correlated to that of caspase 3, and overexpression of EGFR (≥ 200/300) was observed in low-grade tumors more frequently (grades 1 and 2: 67% vs grade 3: 27% and grade 4: 30%). Expression of ERK1 was associated with that of K19 and vimentin, whereas expression of ERK2 was associated with that of cyclin D1, MET and membrane beta-catenin. Expression of vimentin

High-sensitivity integrated devices based on surface plasmon resonance for sensing applications

A metallic nanostructured array that scatters radiation toward a thin metallic layer generates surface plasmon resonances for normally incident light. The location of the minimum of the spectral reflectivity serves to detect changes in the index of refraction of the medium under analysis. The normal incidence operation eases its integration with optical fibers. The geometry of the arrangement and the material selection are changed to optimize some performance parameters as sensitivity, figure of merit, field enhancement, and spectral width. This optimization takes into account the feasibility of the fabrication. The evaluated results of sensitivity(1020 nm/RIU)and figure of merit(614 RIU^(-1)) are competitive with those previously reported.

Assessing the estrogenic potency in a Portuguese wastewater treatment plant using an integrated approach

The estrogenic potency of a wastewater treatment plant （WWTP） was evaluated using chemical and biological analyses, which showed that after the station treatment processes some of the selected endocrine disruptor compounds （EDCs） were still present in the treated effluent （e.g., bisphenol A, alkylphenols, estrone）. Thus, the most common endocrine EDCs were identified and quantified and the overall estrogenicity of the treated effluent assessed by integrating the results. Male goldfish （Carassius auratus） were used as biological indicators in a 28-day experiment. Vitellogenin （Vtg）, gonadosomatic and hepatosomatic indices, steroids （17β-estradiol and 11-ketotestosterone） and histopathology were biomarkers used in fish to evaluate WWTP treated effluent estrogenicity, in combination with instrumental analyses. The results showed a significant increase （P 〈 0.01） in plasma and liver Vtg, which were significantly correlated （r = 0.66; P 〈 0.01）. The gonadosmatic index was significantly （P 〈 0.01） reduced in exposed fish. The steroid analyses revealed significant elevations in 17β-estradiol and depressed 11-ketotestosterone concentrations. The histological examinations show changes in exposed fish gonads, such as regressed testes and in some cases （43% to 75%） the development of ovo-testis in fish exposed to 50% and 100% treated effluent.

Treatment of Chronic Oxaliplatin-Induced Peripheral Neuropathy: A Systematic Review

Introduction: Oxaliplatin is a platinum-derivative chemotherapeutic agent used in digestive tumours, in the adjuvant and metastatic setting. Oxaliplatin can cause a chronic peripheral sensory neuropathy which impacts the quality of life and is dose limiting. To date, no therapeutic strategies have proved effective in the treatment of oxaliplatin-induced peripheral neuropathy (OIPN). Methods: A computerized search of the literature on PubMed database was performed. Publisher original articles were included if they focused on treatment of peripheral neuropathy among patients submitted to oxaliplatin. Eleven out of 242 reviewed papers met our inclusion criteria and were subjected to a 19-item quality checklist. Results: The included studies differed with respect to study design, patient population and sample size, neuropathic symptoms assessment and efficacy measure. Most studies had an adequate quality. Ten trials tested one drug, and one pilot study tested a non-pharmacological treatment—the neurofeedback. Of these, 3 trials included only patients submitted to oxaliplatin-based chemotherapy. Duloxetine showed moderate efficacy in 3 trials. Topical treatment with capsaicin or 10% amitriptyline was promisors in 2 single-arm trials with a few samples.

Using Annuli on the Opercula Bones and Scales of African Pike （Hepsetus odoe） for Age Determination in Ado Ekiti Reservoir, Nigeria

Age of African pike Hepsetus odoe from Ado Ekiti Reservoir Ekiti State, Nigeria was studied. Annuli on the opercula bones and scales were used to determine the age. The standard length of specimens examined varied between 12.50 and 29.80 cm and body weight 10-385 g. Annuli （growth rings） observed on the opercula bones of specimens with mean standard lengths 21.70 ± 1.4 cm had one real annulus, 24.80 ± 1.1 cm had two real annuli, 27.4 ± 0.9 cm had three real annuli indicating age one, two, and three （1 ＋, 2＋, 3＋） respectively. Growth rings observed on scales appeared as interruptions in the normal pattems of scales. Specimens with mean standard lengths of 21.3 ± 1.5 cm, 24.7 ± 0.8 cm and 27.35 ± 11 cm were interpreted to be ages 1＋, 2＋, and 3＋ respectively. The maximum age of samples, derived from annuli on both opercula bones and scales was 3 years. Specimens below 18 cm standard length did not register any growth mark. The mean lengths at the estimated ages obtained using both opercula bones and scales were not significantly different （P 〉 0.05, P 〉 0.01）.

Evaluation of virus-specific cellular immune response in transplant patients

Virus-specific immune responses have a major impact on the outcome of the infection. Viral agents that are characterized by latency, such as herpesviruses and polyomaviruses, require a continuous immune control to reduce the extent of viral reactivation, as viral clearance cannot be accomplished, independently from the anti-viral treatment. In transplant patients, morbidity and mortality related to viral infections are significantly increased. In fact, the key steps of activation of T-cells are major target for anti-rejection immunosuppressive therapy and anti-viral immune response may be altered when infected cells and cellular effectors of immune response coexist in a transplanted organ. The role of cellular immune response in controlling viral replication and the main methods employed for its evaluation will be discussed. In particular, the main features, including both advantages and limitations, of available assays, including intracellular cytokine staining, major histocompatibility complex- multimer-based assays, Elispot assay, and Quanti FERON test, will be described. The potential applications of these assays in the transplant context will be discussed, particularly in relation to cytomegalovirus and polyomavirus BK infection. The relevance of introducing viro-immunological monitoring, beside virological monitoring, in order to identify the risk profile for viral infections in the transplant patients will allows for define a patient-tailored clinical management, particular in terms of modulation of immunosuppressive therapy and anti-viral administration.
