Effect of age on visual and refractive results after LASIK: mechanical microkeratome versus femtosecond laser

AIM: To evaluate the effect of age on visual and refractive results after laser in situ keratomileusis(LASIK) obtained with a mechanical microkeratome or a femtosecond ***: Retrospective, nonrandomized, cohort study. A total of 3826 eyes were included in the study(1725 eyes treated with mechanical LASIK and 2101 eyes treated with femtosecond LASIK). The relationship between patient age and the 3-month postoperative visual and refractive results of both procedures were analyzed by linear regression ***: Three months postoperatively, we found a significant correlation between age and the postoperative spherical equivalent(SE; r2=0.004, P=0.006), efficacy(r2=0.006, P=0.001), and safety indexes(r2=0.05, P=0.0001) in the mechanical LASIK group. On the other hand, we found a significant correlation between age and the postoperative SE(r2=0.02, P=0.0001) and the efficacy index(r2=0.01, P=0.0001) but not the safety index in the femtosecond laser group. Mechanical LASIK provided slightly but significantly better efficacy and predictability in patients 18 to 40 years of age and femtosecond LASIK did so in patients older than 40 years of age. The femtosecond laser provided better safety results than the mechanical microkeratome in both age groups. CONCLUSION: A tendency toward undercorrection and less predictability is found with aging after myopic LASIK regardless of whether the flap was created with a mechanical microkeratome or a femtosecond laser. However, femtosecond laser provides significantly betteroutcomes in terms of efficacy, safety and predictability compared to mechanical microkeratome for the correction of myopia in patients over 40 y.

Prophylaxis with ketotifen in rats with portal hypertension:involvement of mast cell and eicosanoids

BACKGROUND:Since we have previously shown an increase of mast cells in the small bowel and in the mesenteric lymph nodes in the rats with prehepatic portal hypertension,it can be hypothesized that this essential inflammatory cell would be involved in the pathogeny of the splanchnic changes related to portal hypertension. METHODS:To verify this hypothesis,we first studied mast cell infiltration in the ileum and in the mesenteric lymph nodes in sham-operated male Wistar rats(n=12) and in short-term prehepatic portal hypertensive rats (n=12),and the serum levels of rat mast cell protease Ⅱ(RMCP-Ⅱ)by *** a second set of experiments ketotifen,a mast cell stabilizer drug,was administered to sham-operated(n=10)and portal hypertensive(n=12) rats 24 hours before the intervention and prostanoids (PGE2,PGI2,TXB2)and leukotrienes(LTC4,LTB4)were assayed by RIA,mast cell infiltration in the ileum and in the mesenteric lymph nodes and the serum levels of RMCP-Ⅱwere also studied,to show its effectiveness to prevent the mesenteric alterations produced by the inflammatory mediators released by the mast cell. RESULTS:Forty-eight hours after the intervention RMCP-Ⅱ (P<0.05),PGE2(P<0.001)and LTC4 serum levels decreased and mast cell number and RMCP-Ⅱlevels increased in mesenteric lymph nodes in portal hypertensive *** administration of ketotifen reduced portal pressure(P<0.001),serum levels of PGE2(P<0.001)and RMCP-Ⅱ(P<0.001)in mesenteric lymph nodes. CONCLUSIONS:In acute portal hypertension in the rat,the mast cell translocation from intestinal mucosa to mesenteric lymph nodes,where they are activated and degranulates,would represent a defence mechanism to avoid the activation of an acute and massive inflammatory response in this *** administration of ketotifen is able to reduce the splanchnic inflammatory changes related to acute portal hypertension in the rat.

Background: The main objective of this study is to analyse the change in the type of lesions developed by HPV-infected patients after the introduction of the vaccine in three different periods;2002-2006 (years previous to the implementation of the vaccine in Spain), 2009-2011 (shortly after the vaccination) and 2020-2021 (years where the vaccine was well established) at a single hospital. Methods: This is an observational, descriptive, retrospective study based on the review of the results of the biopsies of patients with HPV lesions at a single large tertiary hospital, Hospital Clínico San Carlos, in Madrid, Spain. We have collected the data from three different time periods: 2002-2006, 2009-2011, 2020-2021 to try to understand the potential changes in these lesions after vaccine introduction. Results: In this time we have reviewed the data from 946 women. In these three periods, a decreasing trend in the rate of squamous cell carcinoma was noted, the rate of adenocarcinoma remains stable, and the rate of cervical intraepithelial neoplasia grades 2 - 3 (CIN 2-3) lesions shows an increasing trend. We have also found a change in the mean ages of the patients with these lesions, as this increased in the three lesions caused by HPV after the implementation of the vaccine. Our study indicates that the identification of other high risk serotypes, apart from 16 and 18, as well as those with indeterminate risk, has undergone a progressive increase, increasing from 24.24% and 14.11% respectively in 2002-2006 to 40.42% and 28.34% in 2020-2021. Conclusion: Our study confirms the effectiveness of the vaccines developed so far, against the HPV serotypes they contain. This is demonstrated by the evidence, in our population, of a decrease in the incidence of squamous cell carcinoma in uterine cervix. Changes in the Rate of Human Papilloma Virus Serotypes after Vaccine Implementation: A Descriptive Study

Background: The main objective of this study is to describe the rate of the different serotypes of HPV in cervical cytologies and biopsies in three different periods: 2002-2006 (prior to the implementation of the vaccination programs in Spain), 2009-2011 (shortly after this implementation) and 2020 (almost 15 years after introduction of the vaccine) at a single hospital. Methods: This is an observational, descriptive, retrospective study based on the review of the results of the determination of the HPV serotype using the commercial kit (Genomica®;PharmaMar LTD) in cervical liquid-based cytologies and biopsies at a single large tertiary hospital, Hospital Clínico San Carlos, in Madrid, Spain. We have collected the data from three different time periods: 2002-2006;2009-2011, and 2020 to try to understand the potential changes associated with the use of the vaccine. Results: In these time periods we have reviewed the data from 1420 women. In the three periods the most frequent serotype was HPV 16, followed by HPV 18 or a combination of both. The most frequent low risk serotype was HPV 6 followed by the combination of HPV 6 and 11. It has been verified in our study that the prevalence of the category “others”, constituted by the three risk groups, has undergone a progressive increase, beginning with an infection rate of 65.43% in 2002-2006 to finally rise up to 90.92% in the year 2020. Conclusions: Our study reveals an increase in the number of infections by the HPV serotypes that are not included in the tetravalent vaccine.
