Inflammatory bowel diseases(IBD),Crohns disease and ulcerative colitis,are chronic conditions associated with high morbidity and healthcare *** natural history of IBD is variable and marked by alternating periods of flare and *** though the use of newer therapeutic targets has been associated with higher rates of mucosal healing,a great proportion of IBD patients remain symptomatic despite effective control of *** symptoms may include but not limited to abdominal pain,dyspepsia,diarrhea,urgency,fecal incontinence,constipation or *** this setting,commonly there is an overlap with gastrointestinal(GI)motility and absorptive *** recognition of these conditions greatly improves patient care and may decrease the risk of ***,in this review we describe the prevalence,diagnosis and treatment of GI motility and absorptive disorders that commonly affect patients with IBD.

Mortality in cirrhosis is mostly associated with the development of clinical decompensation,characterized by ascites,hepatic encephalopathy,variceal bleeding,or ***,it is important to prevent and manage such ***,the pathophysiology of decompensated cirrhosis was explained by the peripheral arterial vasodilation hypothesis,but it is currently understood that decompensation might also be driven by a systemic inflammatory state(the systemic inflammation hypothesis).Considering its oncotic and nononcotic properties,albumin has been thoroughly evaluated in the prevention and management of several of these decompensating *** are formal evidence-based recommendations from international medical societies proposing that albumin be administered in individuals with cirrhosis undergoing large-volume paracentesis,patients with spontaneous bacterial peritonitis,those with acute kidney injury(even before the etiological diagnosis),and those with hepatorenal ***,there are a few randomized controlled trials and meta-analyses suggesting a possible role for albumin infusion in patients with cirrhosis and ascites(long-term albumin administration),individuals with hepatic encephalopathy,and those with acute-on-chronic liver failure undergoing modest-volume *** studies are necessary to elucidate whether albumin administration also benefits patients with cirrhosis and other complications,such as individuals with extraperitoneal infections,those hospitalized with decompensated cirrhosis and hypoalbuminemia,and patients with hyponatremia.

Background:Thrombocytopenia is a possible risk factor for bleeding after band ligation of esophageal ***,elevated von Willebrand factor(VWF)in cirrhosis improves platelet function and could decrease this *** objective was to assess platelet function in patients with cirrhosis undergoing esophageal variceal ligation(EVL).Methods:The assessment consisted of platelet count,antigen and activity of VWF and VWF-cleaving protease ADAMTS-13 activity,and a platelet adhesion and aggregation test simulating vascular flow in vivo(Impact-RR)prior to ***:Totally 111 patients were divided into three groups according to platelet count:(1)100×109/L(n=26,23.4%).No statistically significant difference was found in the aggregate size of platelets[group 1:41.0(31.8–67.3)μm 2;group 2:47.0(33.8–71.3)μm 2;and group 3:47.0(34.0–66.0)μm 2;P=0.60]and no significant correlation was found between aggregate size and platelet count(Spearman r=0.07;P=0.47).Surface coverage was 4.1%(2.8%–6.7%),8.5%(4.0%–10.0%),and 9.0%(7.1%–12.0%)(P<0.001)in groups 1,2 and 3,respectively and correlated with platelet count(Spearman r=0.39;P<0.0001).There was no significant difference between groups in VWF or ***-EVL bleeding occurred in six(5.4%)patients(n=2 in group 1,n=1 in group 2,and n=3 in group 3;P=0.32).Patients with bleeding had higher MELD scores[15.0(11.3–20.3)versus 12.0(10.0–15.0);P=0.025],but no difference was demonstrated for platelet function ***:Platelet function is preserved even in the presence of thrombocytopenia,including in the patients with post-EVL bleeding.

Primary prophylaxis of variceal bleeding in patients with cirrhosis:A comparison of different strategies

Patients with cirrhosis and esophageal varices bleed at a yearly rate of 5%-15%,and,when variceal hemorrhage develops,mortality reaches 20%.Patients are deemed at high risk of bleeding when they present with medium or large-sized varices,when they have red signs on varices of any size and when they are classified as Child-Pugh C and have varices of any *** order to avoid variceal bleeding and death,individuals with cirrhosis at high risk of bleeding must undergo primary prophylaxis,for which currently recommended strategies are the use of traditional non-selective beta-blockers(NSBBs)(i.e.,propranolol or nadolol),carvedilol(a NSBB with additional alpha-adrenergic blocking effect)or endoscopic variceal ligation(EVL).The superiority of one of these alternatives over the others is *** EVL might be superior to pharmacological therapy regarding the prevention of the first bleeding episode,either traditional NSBBs or carvedilol seem to play a more prominent role in mortality reduction,probably due to their capacity of preventing other complications of cirrhosis through the decrease in portal hypertension.A sequential strategy,in which patients unresponsive to pharmacological therapy would be submitted to endoscopic treatment,or the combination of pharmacological and endoscopic strategies might be beneficial and deserve further investigation.

AIM:To investigate pro-atherosclerotic markers(endothelial dysfunction and inflammation)in patients one year after liver ***:Forty-four consecutive liver transplant(LT)outpatients who were admitted between August2009 and July 2010,were followed-up by for 1 year,exhibited no evidences of infection or rejection,all of them underwent tacrolimus-based immunosuppressive regimens were consecutively *** cytokines(TNFα,IFNγ,IL-8,and IL-10),endothelial biomarkers(sVCAM-1,sICAM-1,MPO,adiponectin,PAI-1,SAP,SAA,E-selectin,and MMP-9),high sensitive C-reactive protein,and Framingham risk score(FRS)were *** anthropometric data,aminotransferases,metabolic syndrome features,glucose and lipid profiles,and insulin resistance data were also *** LT recipients were compared to 22 biopsy-proven non-alcoholic steatohepatitis(NASH)patients and 20healthy controls(non-obese,non-diabetics,and nondyslipidemic).RESULTS:The LT recipients had significantly younger ages and lower body mass indices,aminotransferases,fasting glucose and insulin levels,glucose homeostasis model and metabolic syndrome features than the NASH *** cardiovascular risk markers,such as Hs-CRP and FRS[2.0(1.0-8.75)],were lower in the LT patients compared to those observed in the NASH patients(P=0.009).In contrast,the LT recipients and NASH patients had similar inflammatory and endothelial serum markers compared to the controls(pg/mL):lower IL-10 levels(32.3 and 32.3 vs 62.5,respectively,P=0.019)and higher IFNγ(626.1 and 411.9 vs 67.9,respectively,P<0.001),E-selectin(48.5 and 90.03vs 35.7,respectively,P<0.001),sVCAM-1(1820.6and 1692.4 vs 1167.2,respectively,P<0.001),and sICAM-1(230.3 and 259.7 vs 152.9,respectively,P=0.015)***:Non-obese LT recipients have similar pro-atherosclerotic serum profiles after a short 1-year follow-up period compared to NASH patients,suggesting a high risk of atherosclerosis in this population.

Acute renal failure, now termed acute kidney injury(AKI), is frequently found in patients with cirrhosis. The occurrence of AKI, irrespective of the underlying cause, is associated with reduced in-hospital, 3-mo and 1-year survival. Hepatorenal syndrome is associated with the worst outcome among AKI patients with cirrhosis. Several definitions for AKI that have been proposed are outlined and evaluated in this paper. Among these, the International Club for Ascites-AKI criteria substantially strengthen the quality of early diagnosis and intervention according to underlying cause of AKI.

This review aims to identify in vivo studies investigating the potential of plant substances and their natural molecules in managing inflammatory bowel disease(IBD).Specifically,the objective is to examine the impact of these substances on interleukins and other key inflammatory signaling *** articles published up to December 2022 were identified through a search of the PubMed,Scopus,Web of Science,and Embase *** search used keywords including“inflammatory bowel disease”,“medicinal plants”,“natural molecules”,“anti-inflammatory”,and“ulcerative colitis”,and identified 1,878 potentially relevant articles,of which 89 were included in this review after completion of the selection *** study provides preclinical data on natural products(NPs)that can potentially treat IBD,including ulcerative *** main actions of these NPs relate to their effects on nuclear factor kappa B(NF-κβ),the Janus kinase(JAK)/signal transducer and activator of transcription(STAT)signaling pathway,the regulation of T helper 17/regulatory T cells balance,and oxidative *** ability of these NPs to inhibit intestinal inflammation appears to be dependent on lowering levels of the pro-inflammatory cytokines tumor necrosis factor-alpha(TNF-α),interleukin(IL)-1β,and IL-17,via the Jun N-terminal kinase(JNK)1,NF-κβ-p65,and STAT3 *** addition,NPs were shown to reduce oxidative stress and the severity of ulcerative colitis,as well as increase the activity of antioxidant *** actions suggest that NPs represent a promising treatment for IBD,and potentially have greater efficacy and safety than current treatments.

Determination of the Antiretroviral Drug Zidovudine in Diluted Alkaline Electrolyte by Adsorptive Stripping Voltammetry at the Mercury Film Electrode

This paper describes a stripping method for the determination of zidovudine at the submicromolar concentration levels. This method is based on the controlled adsorptive accumulation of zidovudine at the thin-film mercury electrode, followed by a linear-sweep stripping voltammetry measurement of the surface species. Optimal experimental conditions include a NaOH solution of 2.0 × 10–3 mol●L–1 (sup-porting electrolyte), an accumulation potential of –0.30 V and a scan rate of 100 mV?s–1. The response of zidovudine is linear over the concentration range 0.01 - 0.08 ppm. After an accumulation time of 5 minutes, the detection limit was found to be 0.67 ppb (2.5 × 10–9 mol●L–1). More convenient methods to measure zidovudine concentration in the presence of the didanosine, acyclovir, nevirapine, lamivudine, and efavirenz, were also investigated. The presence of zidovudine together with ATP or ssDNA demonstrates the utility of this method.

Production Optimization of Biodiesel from Frying Oil Waste to Reduce the Environmental Impacts

The objective of this study was to reduce the environmental impacts of used frying oil waste through the production of biodiesel. A 2 2 factorial planning has been used to evaluate the influences of alcohol/oil and reaction time on the biodiesel production yield. The optimal condition to produce the biodiesel has been found by use of the response surface methodology and analysis of variance to obtain the fitting model. This study was conducted in Campinas city, Brazil, where were collected the waste oil. An analysis of ecological cost also has been developed. Cooking oils collected from Campinas homes were mixed with ethanol in planned proportions (1:9, 1:7 and 1:5) and were transesterified at 60 °C and planned reaction times (30, 60 or 90 min), in order to obtain biodiesel, using 0.1% NaOH as a catalyst. The results of the physical-chemical analyses demonstrated that the biodiesels obtained possessed characteristics close to those required by Brazilian standards. This fuel could be used in fleets of buses, trucks and machines, or even sold to fuel distributors, which results in a solving between US$0.8 and US$4.5 millions. Thus, Campinas would gain environmental credits and become a sustainable city.
