BACKGROUND Metabolic syndrome is a multifactorial disease,and the gut microbiota may play a role in its ***,especially abdominal obesity,is associated with insulin resistance,often increasing the risk of type two diabetes mellitus,vascular endothelial dysfunction,an abnormal lipid profile,hypertension,and vascular inflammation,all of which promote the development of atherosclerotic cardiovascular *** To evaluate the outcomes of fecal microbiota transplantation(FMT)in patients with metabolic *** This was a randomized,single-blind placebo-controlled trial comparing FMT and a sham procedure in patients with metabolic *** selected 32 female patients,who were divided into eight groups of four patients *** of the patients were submitted to upper gastrointestinal *** each group,two patients were randomly allocated to undergo FMT,and the other two patients received saline *** patients were followed for one year after the procedures,during which time anthropometric,bioimpedance,and biochemical data were *** patients also had periodic consultations with a nutritionist and an *** primary end point was a change in the gut *** There was evidence of a postprocedural change in microbiota composition in the patients who underwent FMT in relation to that observed in those who underwent the sham ***,we found no difference between the two groups in terms of the clinical parameters *** There were no significant differences in biochemical or anthropometric parameters,between the two groups ***,there were significant postprocedural differences in the microbiota composition between the placebo *** date,clinical outcomes related to FMT remain uncertain.

Propofol vs midazolam sedation for elective endoscopy in patients with cirrhosis:A systematic review and meta-analysis of randomized controlled trials

BACKGROUND The progression of Barrett's esophagus(BE) to early esophageal carcinoma occurs sequentially; the metaplastic epithelium develops from a low-grade dysplasia to a high-grade dysplasia(HGD), resulting in early esophageal carcinoma and,eventually, invasive carcinoma. Endoscopic approaches including resection and ablation can be used in the treatment of this *** To compare the effectiveness of radiofrequency ablation(RFA) vs endoscopic mucosal resection(EMR) + RFA in the endoscopic treatment of HGD and intramucosal *** In accordance with PRISMA guidelines, this systematic review included studies comparing the two endoscopic techniques(EMR + RFA and RFA alone) in the treatment of HGD and intramucosal carcinoma in patients with BE. Our analysis included studies involving adult patients of any age with BE with HGD or intramucosal carcinoma. The studies compared RFA and EMR + RFA methods were included regardless of randomization *** The seven studies included in this review represent a total of 1950 patients, with742 in the EMR + RFA group and 1208 in the RFA alone group. The use of EMR +RFA was significantly more effective in the treatment of HGD [RD 0.35(0.15,0.56)] than was the use of RFA alone. Effect of drug therapies on self-reported chemosensory outcomes after COVID-19

Objective:The aim of this study was to assess the relative efficacy of medications used following severe acute respiratory syndrome coronavirus-2(SARS-CoV-2)infection on self-reported alterations in taste and/or smell ***:Seven hundred and fourteen persons with self-reported postcoronavirus disease 2019(post-COVID-19)chemosensory disorders were personally interviewed regarding specific medications they were administered following the acute phase of the *** dependent measure—self-reported total recovery of chemosensory symptoms—was subjected to stepwise logistic *** predictors included demographic and clinical variables,in addition to specific medications used to mitigate disease symptoms(i.e.,systemic corticosteroids,oseltamivir,vitamin C,ibuprofen,hydroxychloroquine,azithromycin,ivermectin,nitazoxanide,anticoagulants,and zinc).Results:The median time between COVID-19 symptom onset and the interviews was 81 days(interquartile range:60-104).Of the 714 subjects,249(34.9%)reported total recovery of their chemosensory function;437(61.2%)had at least one treatment since the beginning of the *** and those with more comorbidities had undergone more *** recovery rates of the treated and nontreated groups did not differ ***,respondents who had used nitazoxanide tended to have a higher rate of self-reported taste or smell *** who took oral zinc were less likely to ***:No medication employed during the first months after SARS-CoV-2 infection had a clear positive effect on returning self-reported smell or taste function to normal,although nitrazoxide trended in a positive *** zinc had a negative effect on the reported recovery of these senses.

There is a general concern that the increasing penetration of electric vehicles(EVs)will result in higher aging failure probability of equipment and reduced network *** electricity costs may also increase,due to the exacerbation of peak load led by uncontrolled EV *** paper proposes a linear optimization model for the assessment of the benefits of EV smart charging on both network reliability improvement and electricity cost *** objective of the proposed model is the cost minimization,including the loss of load,repair costs due to aging failures,and EV charging *** proposed model incorporates a piecewise linear model representation for the failure probability distributions and utilizes a machine learning approach to represent the EV charging *** two different test systems(a 5-bus network and the IEEE 33-bus network),this paper compares aging failure probabilities,service unavailability,expected energy not supplied,and total costs in various scenarios with and without the implementation of EV smart charging.
