Emerging roles of the intestine in control of cholesterol metabolism

The liver is considered the major “control center” for maintenance of whole body cholesterol homeostasis. This organ is the main site for de novo cholesterol synthesis, clears cholesterol-containing chylomicron remnants and low density lipoprotein particles from plasma and is the major contributor to high density lipoprotein (HDL; good cholesterol) formation. The liver has a central position in the classical definition of the reverse cholesterol transport pathway by taking up periphery-derived cholesterol from lipoprotein particles followed by conversion into bile acids or its direct secretion into bile for eventual removal via the feces. During the past couple of years, however, an additional important role of the intestine in maintenance of cholesterol homeostasis and regulation of plasma cholesterol levels has become apparent. Firstly, molecular mechanisms of cholesterol absorption have been elucidated and novel pharmacological compounds have been identified that interfere with the process and positively impact plasma cholesterol levels. Secondly, it is now evident that the intestine itself contributes to fecal neutral sterol loss as a cholesterol-secreting organ. Finally, very recent work has unequivocally demonstrated that the intestine contributes significantly to plasma HDL cholesterol levels. Thus, the intestine is a potential target for novel anti-atherosclerotic treatment strategies that, in addition to interference with cholesterol absorption, modulate direct cholesterol excretion and plasma HDL cholesterol levels.

Prevalence of diverticulosis in recurrent Clostridium difficile infection

AIM:To re-evaluate the theory that colonic diverticulosis is associated with relapse of Clostridium difficile associated disease (CDAD) in light of data suggesting increasing rates of CDAD infection and ***: Charts were reviewed for patients with recurrent CDAD who had also had a prior colonoscopy or flexible sigmoidoscopy. An age and gender matched control group was used to compare the prevalence of ***: Twenty-two patients met the study criteria, and the prevalence of diverticulosis in patients with CDAD relapse was 23% compared to 32% in age and sex matched controls (P=0.44). A significant proportion of patients with CDAD relapse had comorbidities associated with immune ***: Diverticulosis does not appear to be associated with CDAD relapse.

The proliferation of internet communication channels has increased telecom fraud,causing billions of euros in losses for customers and the industry each *** constantly find new ways to engage in illegal activity on the *** reduce these losses,a new fraud detection approach is *** fraud detection involves identifying a small number of fraudulent calls from a vast amount of call *** an effective strategy to combat fraud has become *** much effort has been made to detect fraud,most existing methods are designed for batch processing,not real-time *** solve this problem,we propose an online fraud detection model using a Neural Factorization Autoencoder(NFA),which analyzes customer calling patterns to detect fraudulent *** model employs Neural Factorization Machines(NFM)and an Autoencoder(AE)to model calling patterns and a memory module to adapt to changing customer *** evaluate our approach on a large dataset of real-world call detail records and compare it with several state-of-the-art *** results show that our approach outperforms the baselines,with an AUC of 91.06%,a TPr of 91.89%,an FPr of 14.76%,and an F1-score of 95.45%.These results demonstrate the effectiveness of our approach in detecting fraud in real-time and suggest that it can be a valuable tool for preventing fraud in telecommunications networks.

Hepatic lipogranulomas in patients with chronic liver disease: Association with hepatitis C and fatty liver disease

AIM: To study the significance and clinical implication of hepatic lipogranuloma in chronic liver diseases, including fatty liver disease and hepatitis C. METHODS: A total of 376 sequential, archival liver biopsy specimens were reviewed. Lipogranuloma, steatosis and steato-fibrosis were evaluated with combined hematoxylin and eosin and Masson’s trichrome staining. rESULTS: Fifty-eight (15.4%) patients had lipogranuloma, including 46 patients with hepatitis C, 14 patients with fatty liver disease, and 5 patients with other diseases. Hepatic lipogranuloma was more frequently seen in patients with hepatitis C (21%) and fatty liver disease (18%), and its incidence was significantly higher than that in control group (P respectively). In addition, 39 out of the 58 patients with lipogranuloma were associated with steatosis and/or steato-fibrosis. Of the 18 lipogranuloma patients with clinical information available for review, 15 (83%) had risk factors associated with fatty liver disease, such as alcohol use, obesity, hyperlipidemia, and diabetes mellitus. Although the incidence of these risk factors was greater in patients with lipogranuloma than in control group (60%), it did not reach statistical significance. CONCLUSION: Hepatic lipogranuloma is not limited to mineral oil use and commonly associated with hepatic steatosis, hepatitis C and fatty liver disease. With additional histological features of steato-fibrosis, lipogranuloma can also be used as a marker of prior hepatic steatosis.

Nanoscale optical resolution with a large field of view is a critical feature for many research and industry areas,such as semiconductor fabrication,biomedical imaging,and nanoscale material *** scanning microscopes have been developed to resolve the inverse relationship between the resolution and field of view;however,those scanning microscopes still rely upon fluorescence labeling and complex optical *** overcome these limitations,we developed a dual-camera acoustofluidic nanoscope with a seamless image merging algorithm(alphablending process).This design allows us to precisely image both the sample and the microspheres simultaneously and accurately track the particle path and ***,the number of images required to capture the entire field of view(200×200μm)by using our acoustofluidic scanning nanoscope is reduced by 55-fold compared with previous ***,the image quality is also greatly improved by applying an alpha-blending imaging technique,which is critical for accurately depicting and identifying nanoscale objects or *** dual-camera acoustofluidic nanoscope paves the way for enhanced nanoimaging with high resolution and a large field of view.

The progressive ageing of the population and the high incidence of acute coronary syndromes(ACS)in the elderly is leading to an important increase in the number of elderly patients admitted for ACS.[1]Nevertheless,the information about the optimal clinical management in this age group is scarce due to the exclusion of older patients from clinical trials.[2]Higher comorbidity and frailty are common in this clinical setting and they are associated with higher rates of complications and consumption of healthcare resources.[3]These are the main reasons for launching this special issue about management of ACS in the elderly,in which several experts in the field address some of the most important questions about how to deal with ACS in patients at older ages.
