Surgical Treatment of Osteonecrosis of the Femoral Head Using Minimally.Invasive Surgical Drilling and Cancellous Grafting at Brazzaville University.Hospital

Introduction: Osteonecrosis of the femoral head (ONTF) is a debilitating condition. Several treatments have been proposed with controversial results. The aim of our study.was to evaluate treatment by.surgical drilling coupled with in situ cancellous grafting. Materials and methods: Our study.was a case-control study.conducted at Brazzaville University.Hospital from 1st January.2018 to 31 December 2023. It compared two groups of patients with ONTF: non-operated (13 patients, 20 hips) and operated (22 patients, 35 hips). We used the visual digital scale (VDS) for pain assessment, the Merle D’Aubigne-Postel (MDP) scoring sy.tem for clinical and functional assessment, and the evolution of necrosis. Results: The group of non-operated patients had a mean age of 35.69 ± 3.4 y.ars, no improvement in pain with an EVN above seven at the last recoil and a mean global MDP score falling from 12.7 before offloading to 10.13 at one y.ar. The group of patients operated on had a mean age of 37.86 ± 7.02 y.ars, a significant reduction in pain (p = 0.00004) and a significantly.increased MDP score (p = 0.0034). A comparison of the two groups of patients showed significant stabilization of the necrotic lesions in the operated patients (p = 0.00067), with better satisfaction in the same group. Conclusion: Surgical drilling combined with grafting in the treatment of early.stage ONTF has improved progress in our series. The technique is reproducible and less invasive. It has made it possible to delay.unfavorable progression and, consequently. hip replacement surgery.

Methane Dy.amics in Northern Peatlands: A Review

Northern peatlands store a large amount of carbon and play.a significant role in the global carbon cy.le. Owing to the presence of waterlogged and anaerobic conditions, peatlands are ty.ically.a source of methane (CH4), a very.potent greenhouse gas. This paper reviews the k.y.mechanisms of peatland CH4 production, consumption and transport and the major environmental and biotic controls on peatland CH4 emissions. The advantages and disadvantages of micrometeorological and chamber methods in measuring CH4 fluxes from northern peatlands are also discussed. The magnitude of CH4 flux varies considerably.among peatland ty.es (bogs and fens) and microtopographic locations (hummock. and hollows). Some anthropogenic activities including forestry. peat harvesting and industrial emission of sulphur dioxide can cause a reduction in CH4 release from northern peatlands. Further research should be conducted to investigate the in fluence of plant growth forms on CH4 flux from northern peatlands, determine the water table threshold at which plant production in peatlands enhances CH4 release, and quantify.peatland CH4 exchange at plant community.level with a higher temporal resolution using automatic chambers.

The CRISPR/Cas9 sy.tem has shown great potential for treating human genetic diseases through gene ***,there are concerns about the safety.of this sy.tem,specifically.related to the use of guide-free *** studies have shown that guidefree Cas9 can induce genomic instability.in ***,the in vivo safety.risk. associated with guide-free Cas9 have not been evaluated,which is necessary.for the development of gene therapy.in clinical *** this study.we used doxy.y.line-inducible Cas9-expressing pigs to evaluate the safety.risk. of guide-free Cas9 in *** findings demonstrated that expression of guide-free Cas9 could induce genomic damages and transcriptome changes in *** severity.of the genomic damages and transcriptome changes were correlate with the expression levels of Cas9 ***,prolonged expression of Cas9 in pigs led to abnormal phenoty.es,including a significant decrease in body.weight,which may.be attributable to genomic damage-induced nutritional absorption and metabolic ***,we observed an increase in whole-genome and tumor driver gene mutations in pigs with long-term Cas9 expression,raising the risk.of tumor *** in vivo evaluation of guide-free Cas9 in pigs highlights the necessity.of considering and monitoring the detrimental effects of Cas9 alone as genome editing via the CRISPR/Cas9 sy.tem is implemented in clinical gene *** research emphasizes the importance of further study.and implementation of safety.measures to ensure the successful and safe application of the CRiSPR/Cas9 sy.tem in clinical practice.

Arthroscopic Treatment of Gonarthrosis in a Low-Income Setting

Back.round: k.ee osteoarthritis is a frequent, debilitating, and degenerative condition. It is the most common operative indication for surgical arthroscopy. Hy.othesis: Arthroscopy.allows painless treatment of k.ee osteoarthritis and functional recovery. Patients and Methods: Our study.was conducted in a private medical center in the city.of Brazzaville in Congo, during the period from September 2020 to February.2023. It was prospective and based on patients with mechanical k.ee pain caused by.osteoarthritis. A mobile arthroscopy.device was used. The Ik.C and k.OS scores allowed the evaluation of the treatment. Results: Our work.involved 15 men and 7 women, with an average age of 39.9 y.ars. k.ee osteoarthritis was classified as ty.e 2 according to Ahlbäck.in 26 cases (70.27%) and ty.e 3 in 11 cases (29.73%). In 7 patients, arthroscopy.revealed lesions not visualized on MRI. Joint lavage was performed in all cases, sy.ovial debridement in 17 cases (45.94%), and extraction of cartilaginous foreign bodies in 10 cases (27.03%). 20 patients were very.satisfied with the surgery.and 2 moderately.satisfied. The EVA, subjective Ik.C and k.OS scores were acceptable on a long-term follow-up. Discussion: Arthroscopy.is a modern surgical technique for the management of intra-articular lesions of the k.ee. It relieves pain and stabilizes the degenerative process. A mobile arthroscopy.device used here was preferred because of the low cost of installation and production.

The mechanism of tensile deformation in semi-cry.talline poly.ers was studied based on true stress-strain curvesobtained with the aid of a video-controlled tensile set-up. The deformation is affected by.both the cry.talline and theamorphous phases. However, the relative weights of the two portions change with the deformation stage. At lowdeformations the coupling and coarse slips of the cry.talline block. dominate the mechanical properties, which allows thesy.tem to maintain a homogeneous strain distribution in the sample. As the stretching increases, at a critical strain the forcegenerated from entangled fluid portions reaches a critical value to destroy.the cry.tallites. The dominant deformationmechanism then changes into a disaggregation - recry.tallization process.

近红外光谱技术在食品科学、信息安全、生物医疗等重大国计民生领域的应用对近红外光源提出了越来越高的要求。研发具有高效超宽带发射的近红外光源因此成为一项重要且迫切的研究课题。本文采用熔融淬火法在氟硅酸盐玻璃体系SiO_(2)-k.(2)CO_(3)-k.·2H_(2)O-MgF_(2)中成功析出了钙钛矿型k.gF_(3)纳米晶体。通过改变玻璃组分和热处理温度可以调控氟化物纳米晶相的析出,得到析晶和透明度最佳的微晶玻璃样品。玻璃中k.gF_(3)纳米晶体为Cr^(3+)和Ni^(2+)提供了稳定的八面体配位和低声子能量发光环境,在450 nm蓝光激发下,基于Cr^(3+)到Ni^(2+)的能量传递实现了Cr^(3+)(700~1200 nm)和Ni^(2+)(1400~1700 nm)双宽带近红外发射,并且双宽带近红外发光强度随离子掺杂浓度而可调变化。荧光光谱和荧光衰减曲线表征证明了Cr^(3+)到Ni^(2+)的能量传递过程,对应的能量传递效率为52.2%,能量传递机制为电子偶极-四极相互作用。研究结果不仅可以为系统掌握透明光学材料的超宽带发光规律提供基础数据,同时有助于设计开发低成本、高效率的近红外宽带光源。

Potential Use of Crushed Ghanaian Limestone in Paste and Mortar Formulated for Masonry

Crushed Ghanaian limestone （L） powder was analy.ed for its phy.ical, chemical and microstructural properties for the formulation of mortar for masonry. Ordinary.Portland Cement （OPC） was replaced by.10- 40% limestone powder for paste and mortar formulation. Mechanical properties that included compressive strength, water demand and setting times （initial and final） were determined and analy.ed. The mechanical properties were determined in accordance with ASTM standards. The test results indicated that crushed limestone powder contained about 88.7% CaO in calcite form. A 10-30% and 35-40% limestone content in conjunction with OPC was suitable to produce ASTM ty.e M and S mortars respectively. Formulated OPC - L paste showed a high water demand, a delay.d initial setting time and an accelerated final setting time than the plain OPC paste.
