Aim:The anatomical study and clinical application for the vascularized corticoperiosteal fl ap from the medial femoral condyle have been performed and described *** prior studies have described the composite osteomyocutaneous fl ap from the medial femoral condyle,a detailed analysis of the vascularity of this region has not yet been fully ***:This anatomical study described the variability of the arteries from the medial femoral condyle in 40 cadaveric ***:The descending genicular artery(DGA)was found in 33 of 40 cases(82.5%).The superomedial genicular artery(SGA)was present in 10 cases(25%).All 33 cases(100%)of the DGA had articular branches to the periosteum of the medial femoral *** branches and saphenous branches of the DGA were present in 25 cases(62.5%)and 26 cases(70.3%),***:The current study demonstrates that the size and length of the vessels to the medial femoral condyle are suffi cient for a vascularized bone fl ap.A careful preoperative vascular assessment is essential prior to use of the vascularized composite osteomyocutaneous fl ap from the medial femoral condyle,because of the considerable anatomical variations in different branches of the DGA.

Background:The prediction of biogeographical patterns from a large number of driving factors with complex interactions,correlations and non-linear dependences require advanced analytical methods and modeling *** study compares different statistical and machine learning-based models for predicting fungal productivity biogeographical patterns as a case study for the thorough assessment of the performance of alternative modeling approaches to provide accurate and ecologically-consistent ***:We evaluated and compared the performance of two statistical modeling techniques,namely,generalized linear mixed models and geographically weighted regression,and four techniques based on different machine learning algorithms,namely,random forest,extreme gradient boosting,support vector machine and artificial neural network to predict fungal *** evaluation was conducted using a systematic methodology combining random,spatial and environmental blocking together with the assessment of the ecological consistency of spatially-explicit model predictions according to scientific ***:Fungal productivity predictions were sensitive to the modeling approach and the number of predictors ***,the importance assigned to different predictors varied between machine learning modeling *** tree-based models increased prediction accuracy by more than 10%compared to other machine learning approaches,and by more than 20%compared to statistical models,and resulted in higher ecological consistence of the predicted biogeographical patterns of fungal ***:decision tree-based models were the best approach for prediction both in sampling-like environments as well as in extrapolation beyond the spatial and climatic range of the modeling *** this study,we show that proper variable selection is crucial to create robust models for extrapolation in biophysically differentiated *** allows for reducing the dime

Over the last 20 years, laparoscopic colorectal surgery has shown equal efficacy for benign and malignant colorectal diseases when compared to open surgery. However, a laparoscopic approach reduces postoperative morbidity and shortens hospital stay. In the quest to optimize outcomes after laparoscopic colorectal surgery, reduction of access trauma could be a way to improve recovery. To date, one method to reduce access trauma is natural orifice specimen extraction(NOSE). NOSE aims to reduce access trauma in laparoscopic colorectal surgery. The specimen is delivered via a natural orifice and the anastomosis is created intracorporeally. Different methods are used to extract the specimen and to create a bowel anastomosis. Currently, specimens are delivered transcolonically, transrectally, transanally, or transvaginally. Each of these NOSEprocedures raises specific issues with regard to operative technique and application. The presumed benefits of NOSE-procedures are less pain, lower analgesia requirements, faster recovery, shorter hospital stay, better cosmetic results, and lower incisional hernia rates. Avoidance of extraction site laparotomy is the most important characteristic of NOSE. Concerns associated with the NOSE-technique include bacterial contamination of the peritoneal cavity, inflammatory response, and postoperative outcomes, including postoperative pain and the functional and oncologic outcomes. These issues need to be studied in prospective randomized controlled trials. The aim of this systematic review is to describe the role of NOSE in minimally invasive colorectal surgery.

The BETA application-specific integrated circuit(ASIC)is a fully programmable chip designed to amplify,shape and digitize the signal of up to 64 Silicon photomultiplier(SiPM)channels,with a power consumption of approximately~1 mW/*** to its dual-path gain,the BETA chip is capable of resolving single photoelectrons(phes)with a signal-to-noise ratio(SNR)>5 while simultaneously achieving a dynamic range of~4000 ***,BETA can provide a cost-effective solution for the readout of SiPMs in space missions and other applications with a maximum rate below 10 *** this study,we describe the key characteristics of the BETA ASIC and present an evaluation of the performance of its 16-channel version,which is implemented using 130 nm *** ASIC also contains two discriminators that can provide trigger signals with a time jitter down to 400 ps FWHM for 10 *** linearity error of the charge gain measurement was less than 2%for a dynamic range as large as 15 bits.

The author proves a global existence result for strong solutions to the quasilinear dissipative hyperbolic equation (1.1) below, corresponding to initial values and source terms of arbitrary size, provided that the hyperbolicity parameter ε is sufficiently small. This implies a corresponding global existence result for the reduced quasilinear parabolic equation (1.4) below.

AIM: To describe the role of Transanal total mesorectal excision(Ta TME) in minimally invasive rectal cancer surgery, to examine the differences in patient selection and in reported surgical techniques and their impactson postoperative outcomes and to discuss the future of Ta TME. METHODS: MEDLINE(Pub Med), EMBASE, and The Cochrane Library were systematically searched through the 1st of March 2015 using a predefined search strategy. RESULTS: A total of 20 studies with 323 patients were included. Most studies were single-arm prospective studies with fewer than 100 patients. Multiple transanal access platforms were used, and the laparoscopic approach was either multi- or single port. The procedure was initiated transanally or transabdominally. If a simultaneous approach with 2 operating surgeons was chosen, the operative time was significantly reduced. CONCLUSION: Ta TME was also associated with better TME specimens and a longer distal resection margin. Ta TME is thus feasible in expert hands, but the learning curve and safety profile are not well defined. Longterm follow-up regarding anal function and oncological outcomes should be performed in the future.

The Urban Street Segment Chapter of the Highway Capacity Manual(HCM)includes a methodology for evaluating the level of service urban street segments provide to automobile *** methodology does not account for pedestrian activity at un-signalized midblock crosswalk on an urban street *** activity at un-signalized midblock crosswalk on urban street segments causes friction conditions between automobiles and *** a consequence,the average time it takes vehicles to travel along the segment is *** segment running time decreases both the travel speed of automobiles and the level of service provided to automobile *** is an inverse relationship between the delay incurred by interrupted vehicles and the speeds at which pedestrians walk while crossing at *** account for this delay,there is a need to investigate pedestrian walking speeds at un-signalized midblock *** study measured pedestrian walking speeds by age-group at two un-signalized midblock crosswalks on urban street *** first objective of this paper is to perform statistical analyses to examine the measured free-flow pedestrian walking *** second objective is to demonstrate how the findings of this study can be incorporated into the Urban Street Segment Analysis Chapter of the *** walking speeds were recorded and analyzed for 2937 *** results show teenagers walk at an average speed of 1.45 m/s,young adults walk at an average speed of 1.55 m/s,middle age pedestrians walk at a speed of 1.45 m/s,older pedestrians walk at speed of 1.09 m/s,and elderly or physically disabled pedestrians walk at a speed of 1.04 m/s.

Mutations in genes encoding key players in oncogenic signaling pathways trigger specific downstream gene expression profiles in the respective tumor cell *** regulation of genes related to cell growth,survival,and death has been extensively studied,much less is known on the regulation of drug-metabolizing enzymes(DMEs)by oncogenic ***,a comprehensive review of the available literature is presented summarizing the impact of the most relevant genetic alterations in human and rodent liver tumors on the expression of DMEs with a focus on phasesⅠandⅡof xenobiotic *** few data are available with respect to DME regulation by p53-dependent signaling,telomerase expression or altered chromatin *** contrast,DME regulation by constitutive activation of oncogenic signaling via the RAS/RAF/mitogen-activated protein kinase(MAPK)cascade or via the canonical WNT/β-catenin signaling pathway has been analyzed in greater depth,demonstrating mostly positive-regulatory effects of WNT/β-catenin signaling and negativeregulatory effects of MAPK *** studies have revealed molecular interactions between oncogenic signaling and nuclear xeno-sensing receptors which underlie the observed alterations in DME expression in liver *** of altered DME expression and inducibility in liver tumors with a specific gene expression profile may impact pharmacological treatment options.
