Globe penetration during loco-regional anesthesia: prevalence and review of cases

AIM: To describe prevalence and different clinical signs and management of cases with penetrating eye injuries during loco-regional anesthesia for ophthalmic surgery. METHODS: A retrospective review of clinical records was carried out, identifying cases of globe penetration secondary to peribulbar anesthesia injection during 5 y activity in Centro de Oftalmología Barraquer. RESULTS: A total of 17 460 needle-based ocular anesthesia procedures were performed in our centre and 4 cases of globe penetration were recorded with an estimated prevalence of 0.024%. Globe penetrations were always detected in the first 24 h after surgery. Vitreous haemorrhage was found in all the cases. Two eyes presented retinal detachment and two eyes choroidal detachment(CD). The initial surgical approach was performed within the first 48 h. Silicone oil was used as tamponade in three eyes and the fourth case remained only with air. Detachments were solved successfully in all the cases. Functional results varied among cases, depending on ocular remarkable antecedent and globe penetration with or without retinal or ***: Prevalence of globe penetration during loco-regional anesthesia is low in our centre. Physicians should consider the possibility of globe penetration in eyes with postoperative atypical appearance after locoregional anesthesia. Immediate B-scan ultrasonography is recommended in suspicious cases with a dense vitreous haemorrhage. An early vitrectomy surgery in conjunction with laser or cryotherapy at the penetration sites is essential for good anatomical and functional results.

Liver transplantation(LT) after neoadjuvant chemoradiotherapy in patients with unresectable hilar cholangiocarcinoma(HC) is an accepted treatment strategy [1]. neoadjuvant therapy is associated with an increased risk of arterial and portal complications after LT [1,2]. In most cases, radiation therapy makes the use of the native hepatic artery inadvisable, and an aortic anastomosis is needed, either with or without a graft [2].

Exploring seven hundred years of transhumance, climate dynamic, fire and human activity through a historical mountain pass in central Spain

A high-altitude peat sequence from the heart of the Spanish Central System(Gredos range) was analysed through a multi-proxy approach to determine the sensitivity of high-mountain habitats to climate, fire and land use changes during the last seven hundred years, providing valuable insight into our understanding of the vegetation history and environmental changes in a mountain pass close to a traditional route of transhumance. The pollen data indicate that the vegetation was dominated by shrublands and grasslands with scattered pines in high-mountain areas, while in the valleys cereals, chestnut and olive trees were cultivated. Strong declines of high-mountain pines percentages are recorded at 1540, 1675, 1765, 1835 and 1925 cal AD, which may be related to increasing grazing activities and/or the occurrence of anthropogenic fires. The practice of mountain summer farming and transhumance deeply changed and redesigned the landscape of the high altitudes in central Spain(Gredos range) since the Middle Ages, although its dynamics was influenced in some way by climate variability of the past seven centuries.

Lipid droplets as metabolic determinants for stemness and chemoresistance in cancer

Previously regarded as simple fat storage particles,new evidence suggests thatlipid droplets(LDs)are dynamic and functional organelles involved in keycellular processes such as membrane biosynthesis,lipid metabolism,cellsignalling and ***,an increased LD content is one of the mostapparent features resulting from lipid metabolism reprogramming necessary tosupport the basic functions of cancer *** have been associated to differentcellular processes involved in cancer progression and aggressiveness,such astumorigenicity,invasion and metastasis,as well as ***,all of these processes are controlled by a subpopulation of highly aggressivetumoral cells named cancer stem cells(CSCs),suggesting that LDs may befundamental elements for stemness in *** the key role of CSCs onchemoresistance and disease relapse,main factors of therapy failure,the design ofnovel therapeutic approaches targeting these cells may be the only chance forlong-term survival in cancer *** this sense,their biology and functionalproperties render LDs excellent candidates for target discovery and design ofcombined therapeutic *** this review,we summarise the currentknowledge identifying LDs and CSCs as main contributors to cancer aggressiveness,metastasis and chemoresistance.

Chronic intestinal pseudo-obstruction(CIPO) is a rare disease due to a severe gastrointestinal motility disorder which may mimic,on both clinical and radiological grounds,mechanical *** report a case of a 26-year-old woman who presented to our institution for plain abdominal radiography for referred long-lasting constipation with recurrent episodes of abdominal pain and *** X-ray,performed both in the upright and supine position,an isolated air-fluid level was depicted in the left flank,together with a number of radiological signs suggestive of ***,subphrenic radiolucency could be observed in the upright ***,the intestinal wall of some jejunal loops appeared to be outlined in the right ***,the inferior cardiac border was clearly depicted in the upright *** patient however had no evidence ofperitoneal signs but only hypoactive bowel *** multi-detector computed tomography(MDCT) of the abdomen and pelvis was therefore *** revealed abnormal air-driven distension of the small and large bowel,without evidence of extraluminal *** radiological signs of pneumoperitoneum turned out to be false-positive *** patient was submitted to pan-colonoscopy and to anorectal manometry to rule out Hirshprung's disease,and was finally discharged with a diagnosis of CIPO.

Mechanisms of radioresistance in hepatocellular carcinoma

Hepatocellular carcinoma(HCC), one of the most common cancers in the world, is characterized by poor prognosis and recurrence after resection. Its prevalence is highest in developing countries, particularly where there is high incidence of hepatitis B virus infection. Several curative treatments are available for early stage HCC; however, these options are not available for advanced disease. new techniques allowing the specific delivery of high-dose radiotherapy enable their use in the treatment of HCC, which has been avoided in the past due to low hepatic tolerance for radiation. This presents a new challenge—the development of resistance to radiotherapy and subsequent disease recurrence. Recently, the mechanisms controlling radioresistance have begun to be elucidated. Understanding the molecular basis of radioresistance is key to developing new strategies with better treatment response and increased patient survival.

Extracellular matrix components in peripheral nerve repair:how to affect neural cellular response and nerve regeneration?

Peripheral nerve injury is a serious problem affecting signiifcantly patients’ life. Autografts are the“gold standard” used to repair the injury gap, however, only 50% of patients fully recover from the trauma. Artiifcial conduits are a valid alternative to repairing peripheral nerve. They aim at conifning the nerve environment throughout the regeneration process, and providing guidance to axon outgrowth. Biocompatible materials have been carefully designed to reduce inlfamma-tion and scar tissue formation, but modiifcations of the inner lumen are still required in order to optimise the scaffolds. Biomicking the native neural tissue with extracellular matrix ifllers or coatings showed great promises in repairing longer gaps and extending cell survival. In addition, extracellular matrix molecules provide a platform to further bind growth factors that can be released in the system over time. Alternatively, conduit ifllers can be used for cell transplantation at the injury site, reducing the lag time required for endogenous Schwann cells to proliferate and take part in the regeneration process. This review provides an overview on the importance of ex-tracellular matrix molecules in peripheral nerve repair.

Uncertainties in the Mercury Mass Balance in a Coal-Based IGCC Power Plant (Puertollano, Spain)

Mercury(Hg)is a global pollutant that is subject to strict regulations to reduce anthropogenic *** production of energy represents an important activity that leads to Hg emissions into the *** all the systems used,IGCC plants are the most promising for reducing Hg emissions,since it is possible to remove Hg from syngas prior to *** aim of the present work was to evaluate the presence of Hg in the main streams of an experimental IGCC plant(ELCOGAS,Puertollano)in order to quantify Hg emissions and investigate the possibility of reducing *** main streams of the system were sampled for three consecutive days and both the solids,i.e.,raw material(coal and petroleum coke),fine and coarse slags,fly ash,sulphur,and the liquids,i.e.,slag system,Venturi scrubber and saturator,were *** results show that an average of 12.9%of the Hg that enters the IGCC power plant is eliminated with solid waste and only 0.08%with liquid *** is still an average of 87.12%of Hg that is not accounted for in the mass balance and that could remain in the system and/or be eliminated in the streams that were not *** it is impossible to offer an explanation for the final fate of the Hg lost in the system based on the obtained results,the data suggest that sulphur byproducts could be primarily responsible for the elimination of Hg from the syngas,and that a major proportion of Hg should be emitted via the chimney after the syngas combustion process.

Modeling the impact of surveillance activities combined with physical distancing interventions on COVID-19 epidemics at a local level

Physical distancing and contact tracing are two key components in controlling the COVID-19 *** their interaction at local level is important for *** propose a flexible modeling framework to assess the effect of combining contact tracing with different physical distancing *** scenario tree analyses,we compute the probability of COVID-19 detection using passive surveillance,with and without contact tracing,in metropolitan *** estimates of detection probability and the frequency of daily social contacts are fitted into an age-structured susceptible-exposed-infectious-recovered compartmental model to simulate the epidemics considering different physical distancing scenarios over a period of 26 *** the original Wuhan strain,the probability of detecting an infected individual without implementing physical distancing would have been 0.465,0.515,0.617,and 0.665 in designated age groups(0e14,15e49,50e64,and>65),*** the physical distancing measures were reinforced and the disease circulation decreased,the interaction between the two interventions resulted in a reduction of the detection probabilities;however,despite this reduction,active contact tracing and isolation remained an effective supplement to physical *** we relied solely on passive surveillance for diagnosing COVID-19,the model required a minimal 50%(95%credible interval,39e69%)reduction of daily social contacts to keep the infected population under 5%,as compared to the 36%(95%credible interval,22e56%)reduction with contact tracing *** simulation with the B.1.1.7 and B.1.167.2 strains shows similar *** simulations showed that a functioning contact tracing program would reduce the need for physical distancing and mitigate the COVID-19 epidemics.
