Deep vein thrombosis of the upper limb is a rare location of venous thromboembolic disease. Data on this form of thrombosis are limited. We report six cases collected in Dakar. The sex ratio was 0.5 and the average age was 27.6 years with extremes of 17 and 39. Painful limb edema was the most constant sign and involved the non-dominant limb in almost all patients. Thrombosis occurred in a field of peripartum cardiomyopathy in two patients and SS sickle cell disease in one patient taking oral contraception. There was neither cancer nor venous catheter in medical history. The diagnosis was made by venous Doppler ultrasound in all patients. Subclavian involvement was the most common. The thrombophilia assessment, done for two patients, was normal. It was about exertion thrombosis on three (3) patients. All the patients had received antivitamin K treatment relaying a low molecular weight heparin. Venous limb compression was associated with anticoagulation for all patients. The treatment duration was six (6) months. There were no reports of pulmonary embolism or death. One patient presented a post-thrombotic syndrome.

Background: Urinary tract infections (UTIs) are a frequent reason for consultation and lead to a significant and sometimes inappropriate prescription of antibiotics. The latter favors antibiotic resistance and an increase in mortality as well as the cost of treatment. The present study aims to contribute to the fight against antibiotic resistance of enterobacteria. Methods: This is a prospective study from January to December, 2021 in the Microbiology laboratory of the General Reference Hospital (GRH) of Niamey including 3369 urine samples. The antibiotic resistance of enterobacteria was determined using the Viteck-2 method. Results: At least 280 strains of Enterobacteriaceae were isolated from the patient’s urine. Among these strains, Escherichia coli was the most predominant (74.64%), followed by Klebsiella pneumoniae (16.07%) and Enterobacter cloacae (7.14%) and other enterobacteria 2.15%. These Enterobacteriaceae are more common in community patients than in hospitalized patients. The average age of patients is 52 years and the age group most affected by these enterobacteria is 46 - 60 years (23.33%). The female sex is the most affected sex with (51.07%) against (48.97%) for the male sex with 1.04 as sex ratio. The hospitalization departments most affected by these enterobacteria are Nephrology (29.23%) and Endocrinology (21.54%). Up to 75% of the Enterobacteriaceae isolates show high resistance to ampicillin, amoxicillin-clavulanic acid, ticarcillin, piperacillin-tazobactam, cefoxitin, cefixime, ceftazidime, ceftriaxone, cotrimoxazole, nalidixic acid and ofloxacin. Conclusion: The high rate of antibiotic resistance among enterobacteria in urine is of concern. Only a few Enterobacteriaceae show low resistance to ertapenem, imipenem, amikacin, gentamicin, fosfomycin and nitrofurantoin. Therefore, these antibiotics are recommended as first line treatment for urinary tract infections.

This work studies the influence of mechanical and geometrical characteristics of the concrete and the soil on the stresses in a mat foundation. In this study, the soil-structure interaction is modeled by two parameters, the modulus of subgrade vertical reaction (k) and the modulus of subgrade horizontal reaction (2T). These two parameters are dependent on the geometrical and mechanical characteristics of the system. Results of this study show a sensitivity of solicitations to variations of geometrical and mechanical characteristics of the model. Although solicitations in the plate are sensitive to mechanical properties of concrete, these solicitations are strongly influenced by the mechanical and geometrical characteristics of the soil mass. However, it should be noted that the influence of Eb is denoted in the center of the plate whereas the Es feels almost in the same manner over the entire extent of the plate. This study also shows that for the same load cases, the values of the torsion moment and shear stress are not significant those of bending moments and normal stresses, respectively.

Vector-borne diseases are highly sensitive to environment and to environmental changes. Rift Valley Fever (RFV) is a mosquito-borne zootic virus associated with severe diseases in human beings and economic consequences to livestock sector. Animal and human movements have a fundamental impact on RVF transmission. In recent years, there has been a growing interest in the use of mathematics and agent based models to represent and analyze the dynamic of RFV transmission. However, no previous study has taken into consideration animal herds’ mobility and precipitation factors to understand the disease spread. This limitation underlines the necessity to use computational model approach based on multi-agent system in the study of vector-borne diseases transmission and diffusion. In this paper, a multi-agent system combining conceptual model expressiveness is used to study animal herds’ mobility and the precipitation parameter impact on the Rift Valley Fever outbreak in Ferlo Barkedji in Northern Senegal. Simulation scenarios with various parameters, including rain quality, hosts, vectors, camp dispersal around ponds, etc., are unrolled. The different results we have obtained show that the evolution of the number of infected hosts and infected vectors depend on the degree of animal herds’ mobility and on precipitations. Our model provides a framework that permits predicting the spread of the disease associated with the mobility of animal herds.

Rise of Urban Water Table as a Cause of Flooding: Improving Knowledge in the City of Niamey (Niger Republic)

Niamey, the capital of Niger, has experienced continuous demographic growth (+4%), accompanied by rapid urban expansion that is insufficiently controlled. This growth, combined with the effects of climate change as well as a drastic change in land use (urbanization of cultivated fields, deforestation of plateaus and erosion of slopes) disrupts the water cycle, thus leading to the superposition of three types of floods: 1) rain floods (monsoon period);2) river floods (Niger river);and 3) flooding caused by rising water table. In several neighbourhoods, the water table is now out in a sustainable manner and degrades already fragile sanitary conditions. This study aims to clarify the functioning of aquifers in the city of Niamey due to the combination of geological, geophysical and hydrogeological data. Hydrogeological investigations make it possible to identify, in areas flooded by the water table, a shallow aquifer with low capacitance (effective porosity of a few %) and low permeability (2 × 10-6 to 1 × 10-4 m/s), overlying at a level shallow clay (~10 m) and lying on the Precambrian base (schists, granites). This configuration limits flows and has led to the appearance of permanent pools created by the water table in the valley thalweg in and around the city. Thus, in Niamey, an increase of up to twenty to forty meters was observed between 1961 and 2021 with seasonal piezometric fluctuations of a pluri-metric order following the rainy season. Beyond the health impacts, this trajectory negatively impacts land and locally causes degradation or displacement of traffic axes. Containing the level of the water table appears to be essential in the long term for sustainable sanitation in the city of Niamey.

Investigating the Depth and the Geometry of the Quarzitic Panafrican Basement Using Near-Surface 3D Seismic Refraction Tomography: Case Study of the Locality of Bakel (Senegal)

Seismic refraction investigations have been carried out in Bakel, Eastern Senegal. The purpose was to map geometrical relationship between the existing rock types and the Panafrican quarzitic basement, which is valuable information for the project of the Bakel fluviatile port construction. Four seismic refraction profiles were acquired. The obtained data have been processed by inversion. The obtained four seismic P-wave velocity profiles have been integrated to obtain a 3D model. By comparing the outcropping geological formations with the observed seismic data at the surface, it was possible to identify the lithology corresponding to each measured range of seismic velocity for the alluvium, the weathered bed rock, and the fresh rock. The results showed that the depth of the fresh rock of the basement varies from 0 to 18 meters above the sea level, with a deepening toward the Senegal River and toward the Northern part of the studied area. The presence of alluviums and their thickness are linked to the existence of bays and gulfs. The results of this study give valuable information for the river bed dredging cost assessment prior to the port construction phase.

Objective: To describe the clinical and etiological characteristics of hemiplegia in the northern region of Burkina Faso. Methodology: This was transversal with a descriptive and analytical study of one hundred and ninety-six hemiplegic patients in charge of general medicine at the Ouahigouya Regional University Hospitalfrom November 2015 to November 2017. Results: The majority of patients had a brutal hemiplegia predominant on the right. The sample consisted mainly of male patients with a mean age of 58 years. High blood pressure was the main factor of vascular risk. The cerebrovascular accident (58.7%) was the main pathology diagnosed. Most patients received symptomatic management. Physiotherapy was performed in 29.6% of patients. The clinical outcome at discharge was marked by the persistence of hemiplegia in more than 90.8% of patients. There was an improvement in the average MIF score at the exit. Conclusion: The lack of computed tomography and neuro-resuscitation unit were the main factors limiting the management of hemiplegia in our context.

Management and Outcome of Ureteropelvic Junction Obstruction in Senegalese Children

Introduction: The ureteropelvic junction obstruction is a rare congenital malformation of the upper urinary tract. We report its management and outcome at Senegal’s main pediatric surgical department. Patients and Methods: We conducted a descriptive review of all patients managed in our pediatric surgical department at Albert Royer National Children’s Hospital Centre from January 1st, 2013 to December 31st, 2017. Results: Thirty patients were included in our study. The mean age was three years. Males were more affected (sex ratio of 2.75). The prenatal diagnosis was made in 23% of cases using prenatal ultrasonography. An abdominal mass was the circumstance of discovery in 36% of cases, and lumbar contact and renal sloshing were found in 66% of cases. There was no renal function impairment in 93% of cases. Urinary tract infection occurred in 50% of cases. Ultrasonography (100%) and urinary computed tomography (56%) were used to diagnose. A temporary nephrostomy was performed in 23% of cases, and as definitive treatment, an open Anderson-Hynes procedure was performed in 96% as a nephrectomy was made in a patient. Early postoperative complications were encountered in 56% of cases: urinary tract infection (36%), surgical site infection and anastomotic stenosis (6% both), and urinoma and textiloma (both in 3%). A patient died from sepsis. After a mean follow-up of 12 months, no additional complication was reported. Conclusion: Ureteropelvic junction obstruction was frequently diagnosed late, and its management carried unneglectable morbidity in our environment. A laparoscopic approach would be a solution to improve its outcome.

Evaluation of Practice of Prescription of the Corticotherapy in the Department of Internal Medicine of the University Hospital of Point G from 2009 to 2013

Introduction: It is estimated that about 0.2% to 0.5% of French populations received a long-term systemic corticosteroid therapy. In Mali, from March, 2007 to February, 2008, in National Center for Disease Control and Prevention, 19.2% of hospitalized patients (24/125) received long-term corticosteroid therapy. Until now, the department of internal medicine of the university hospital of Point G hasn’t done any research on the corticosteroid therapy specially. Therefore, we initiated this work to evaluate our practice of prescription of corticosteroid. The target of the study was to evaluate practical activity of prescription of corticosteroid of internal medicine. Methods: Based on all hospitalized patient materials of the department of internal medicine of Point G regional university central hospital during January 2009-December 2013, we initiated this retrospective and descriptive study. This study was based on all hospitalized patients receiving corticosteroid in the time of researching, without distinguishing their sex or age. Results: During the period of study, the department of internal medicine of point G regional central hospital had received 2155 patients;64 were eligible: prevalence of 2.97%. Most patients were female, accounting for 87.5% with a sex-ratio of 0.14. The average age of our patients was 39.11 ± 16.92 years, ranging from 14 to 97 years. Physicians enrolled in the study Specialized Diploma (DES) and Internal represented 79.7% of prescribers. As pretreatment assessment, only 2 patients (3.12%) had carried out all of the standard balance (NFS, electrolytes, calcium, blood glucose and serum protein). The most common glucocorticoide was prednisone. Solid tumors represented 31.25% (20 cases) of the diseases treated, followed by systemic lupus erythematosus with 15.62% (10 cases).
