Evaluation of the Frequency of Prolonged Fevers and Exploration of Their Etiologies in the Internal Medicine Department of the Point “G” University Hospital in Bamako from 2009 to 2013

Objective: The aim was to evaluate the frequency of prolonged fevers and to determine their etiologies. Methods: We carried out a cross-sectional study extending from the period of 2009 to 2013 in the Internal Medicine department of the “G” Point University Hospital in Bamako. Included were all records of hospitalized patients with a central temperature greater than 37°C in the morning and 37°C in the evening, resting for 15 minutes, fasting for more than 2 hours, and absence of antipyretic treatment. We include all the patients of the study period with fever greater than 37.5°C in the morning and 37.8°C in the evening, resting for 15 minutes, fasting for more than 2 hours, and absence of antipyretic treatment, which have more than 21 days and measured on several occasions. The data were collected on a survey sheet. Data entry and analysis was done on SPSS software. Results: We recorded 243 fever cases out of 2155 hospitalizations, a prevalence rate of 11.2%. There were 128 men and 115 women with an average age of 43 years (range, 15 to 84 years), a modal class of 37 to 47 years, and a sex ratio of 1.11. The infectious etiologies accounted for 81% followed by neoplastic causes 09.6% and inflammatory 01.2% of cases. HIV infection was found in 26.4% of patients, malaria 13.5% and urinary tract infections 10.2%). Gram negative bacilli 88% consisted mainly of Escherichia coli (56%) and Klebsiella pneumoniae (20%).

Electrocardiographic Changes in Patients with Type 2 Diabetes Multicenttic Cross Sectional and Descriptive Study in Dakar

Introduction: The resting electrocardiogram is generally called upon in the evaluation of cardiovascular risk in diabetics. Thus we proposed in this work to evaluate electrocardiographic changes in patients with type 2 diabetes. Methods: This was a cross-sectional and descriptive observation that took place from 1 January 2014 to 1 October 2014 in the Internal Medicine/Endocrinology Departments of Pikine National Hospital and Cardiology National Hospital of Grand-Yoff. Results: One hundred (100) patients consisted of 43 male and 57 female diabetics. The sex ration male/female ratio was 0.7. The average age was 58.3 years. Men were twice as likely to be active as women. Rhythm disorders were noted in 20% of patients, 13% of whom were women. Right atrial hypertrophy was found in 11% of patients and left hypertrophy in both men and women (25%). Wolf-Parkinson White syndrome was present in 2% of patients. A complete right bundle block was present in 11% of patients. Primary repolarization disorders were noted in 17% of patients and secondary in 18% of patients, postero-diaphragmatic necrosis in 14% of patients, real posterior necrosis in 2% of patients, extensive anterior necrosis in 15% of patients. The QT interval was lengthened in 21% of patients including 14 women. Left ventricular hypertrophy was present in 33% of patients and right ventricular hypertrophy was found in 10% of patients. The bivariate analysis showed that electrocardiographic abnormalities were more correlated with the association of certain risk factors such as hypertension and dyslipidemia, the feminine gender, but also the poor glycemic balance. On the other hand, physical activity and treatment had a protective effect. Conclusion: The ECG is certainly insufficient for an exhaustive exploration of the heart of the diabetic patient, but still very useful in our conditions of exercise to improve the care of our patients.

Clinical and Therapeutic Aspects of Genital Prolapse at Hôpital du Mali about 100 Cases

To study the epidemio-clinical aspects, and the therapeutic attitudes of genital prolapse (GP) in the gynecology department of Hôpital du Mali (HDM). This is a descriptive retro-prospective study over five (5) years from January 2015 to December 2019, conducted in the gynecology department of HDM. We had collected 100 cases of GP out of 989 surgeries, with a frequency of 9.89%. The age group of 60 years and over accounted for 33% of our patients, with a mean age of 50 years. Multiparous were the most affected (89%). The notion of obstructed labor was observed in 52% of patients. The most found reason for consultation was the feeling of lump in a vagina, with 65%. Grade III according to BADEN-WALKER classification system (BWCS) concerned 72% of our patients. Triple perineal surgery and hysterectomy involved 56% of our patients. Spinal anesthesia was performed in 96% of cases. Per and postoperative complications were dominated by urinary retention in 4% and by infection of the surgical site in 2%. The average hospital stay was 3.2 days. We recorded 88% of satisfied patients after the intervention. No deaths were recorded during the study. The management of genital prolapse remains essentially surgical;it requires a semiological analysis and a mastery of the surgical technique.

Late leaf spot caused by Cercosporidium personatum is one of the most widespread groundnut leaf diseases. Along with early leaf spot and rust, it is one of the main fungal diseases hampering groundnut production worldwide. Late leaf spot accounts for significant yield losses throughout the world where groundnuts are grown. This reduction in yield caused by the disease could result in yield losses of between 50% and 70%. In Burkina Faso, the disease is present throughout the country, with incidence varying according to region and season. Could the variability in the incidence of the disease be linked to the nature of the isolates or to the conditions from each agro-ecological zone? In this study, the aim was to assess the capacity of three isolates from three agroclimatic zones of the country to defoliate and reduce groundnut yield. To this end, three isolates of Cercosporidium personatum (Berk. Et M.A Curt.) were collected in these zones and evaluated on three contrasting groundnut varieties. A split-splot design was used for the experiment. Isolates were prepared from samples collected in farmers’ fields. After incubation in the laboratory, leaf spots showing good sporulation were scraped off with a scalpel after immersing the leaves in distilled water. Inoculations were carried out under controlled environment. After inoculation with the isolate, the percentage of defoliation and the reduction in yield of these varieties were then evaluated. Inoculation was carried out from the 30th day after sowing with Cercosporidium personatum spore suspensions at 105 spores/ml. The study showed that the percentage of defoliation (P = 0.0001) and the reduction in yield (P = 0.0001) were significant. The study revealed that, whatever the variety, isolate I3TF from the Upper Basins region in the South Sudanese zone caused the greatest defoliation and the greatest reduction in yield. Li-ion batteries (Libs) are a mature technology widely used for energy storage in various electronic devices. Nowadays, this technology has become a leading candidate for the portable electronics market and for electric vehicles due to its good performance. As a result, the demand for Libs containing critical metals, rare earth elements and precious metals is increasing day by day with the accelerated upgrades of consumer electronics, which promotes the supply risk of many mining resources. In addition, the problems associated with the production of end-of-life Lib are increasing on a global scale. Used Libs are e-waste containing significant levels of critical raw materials (such as Co, Li, Mn and Ni) along with harmful substances. Without proper management of Lib waste, these precious metals and toxic substances may end up in nature and cause environmental and public health problems. In order to preserve nature, ensure sustainable resource management and stimulate the circular economy, it has become crucial to properly manage and recycle end-of-life Li-ion batteries. By the way, conventional methods focusing on pyrometallurgical treatments combined with hydrometallurgical treatment are widely studied to recover design metals from Libs waste. It is in this context that we have conducted this bibliographic synthesis, focusing on the efficiency of the solvents employed and their competitiveness for a more environmentally friendly economic management. In this manuscript, recent leaching, solvent extraction, electrodeposition and precipitation strategies to recover precious metals from end-of-life Li-ion battery designs are reviewed and the evolution of these processes is discussed.

Frequency and Factors Associated with Depression in Rheumatoid Arthritis in African Black Patients: Case-Control Study

Objective: To study the frequency and factors associated with depression in rheumatoid arthritis (RA) among Black African patients. Patients and methods: It was a case-control study with a group of patients with RA (case) and a group of healthy subjects without rheumatoid arthritis (con-trols) matched by age and sex. All patients admitted to the rheumatologic department during February 2015 to July 2015 for rheumatoid arthritis meeting the criteria ACR/EULAR 2010, were included. Patients with high blood pressure, diabetes mellitus, renal failure or HIV infection have been excluded. Depression was assessed by the Hamilton Rating Scale for Depression. Results: Fifty patients with RA were included and compared to 100 healthy controls. For RA patients, it was 39 (78%) female and 11 (22%) male with a sex ratio of 0.28. In the control group, it was 77 (77%) female and 23 (23%) male with a sex ratio of 0.29. Twenty-seven (54%) patients with RA had depression compared with 17 (17%) subjects in the control group (p = 0.000). Impaired quality of life and a DAS 28 score greater than 3.2 were statistically associated with depression during RA with a p value respectively of 0.021 and 0.0000231. Conclusion: Depres-sion is significantly more common during RA than in non-RA controls. RA patients should be screened routinely for depression in order to ensure improved treatment and management.

Jejuno-Ileal Ruptures after Blunt Abdominal Trauma at the Teaching Hospital of Bouake

Objective: The aim of this study is to determine the causes, pattern, management, and outcome of jejunal-ileal rupture following blunt abdominal trauma at the teaching Hospital of Bouake. Methods: The study included 27 patients who underwent laparotomy for jejunoileal injuries from blunt abdominal trauma at the Teaching Hospital of Bouake over a period of 14 years from January 2007 to December 2020. A retrospective study was conducted and the patients were analyzed with respect to patient demographics data, cause, injury mechanisms, presentation, anatomical distribution, diagnostic methods, associated injuries, treatment and outcomes. Results: During the 14 year period from 2007 to 2020, 27 patients with blunt small bowel injuries were treated at our Teaching Hospital. That is 2.9% of all blunt abdominal trauma. Male to female ratio was 4.4:1 and the average age was 26.3 years (range: 15 and 50 years). The majority (66.7% 18 cases) were Victims of road traffic accident. Median delay between injury and arrival at hospital for these patients was 10.1 hours (range: 1 - 72 h). A single intestinal injury was present in 22 patients, while 5 patients suffered from 2 injuries. There were 32 perforations of the small intestine in 27 patients with two perforations being transection. All perforations were located on the antimesenteric border of the intestine. Out of the 32 jejuno-ileal ruptures 18 were located on the jejunum while 14 were located on the ileum. Associated intra-abdominal injuries were present in 6 patients and nineteen associated extra-abdominal injuries were present in 10 (70.4%) patients. A one-stage therapeutic strategy was performed in 20 cases (74.1%) and a two-stage strategy was performed in seven cases (25.9%). Intestinal continuity was restored 93.8 days later (range 60 and 140 days). The average length of hospital stay of the operated patients was 11.4 days. The operative morbidity was 25.9%. Perinatal Deaths in Rural a Health Area, Case of the Health District of Kadiolo, Mali

The perinatal mortality is a public health issue in developing countries. In fact, the perinatal death rate seems the highest in a Malian rural area. The objective of this work was to study the perinatal mortality in a rural area of Mali (Kadiolo). Methodology: It was a cross-sectional descriptive study conducted in Kadiolo over 12 months. The study took place from April 1, 2016 to March 31, 2017. We have included in our study all dead fetuses in gestational age ≥ 28 weeks of amenorrhea (WA) or weight ≥ 500 grams (g) and infants died during the first week of life. Mothers who have experienced a perinatal death were included. Results: During the 12 months of study period, we have recorded a total of 2212 births out of which we have collected 205 perinatal deaths, a perinatal mortality rate of 205/2212 with 93 per 1000. The stillbirth rate was 152/2212 with 69‰ and the early neonatal mortality rate was 53/2212 with 24‰. The sociodemographic characteristic of the patients was: patient who has been evacuated 71.70%, unschooled 77.1%, coming out of the city Kadiolo 76%, young women 65% and patient who did not receive antenatal care 35%. Conclusion: Our study confirms the high rates of perinatal deaths in rural areas. The implementation of a coherent and efficient care strategy should help in reducing the still high rates.

Cerebral Venous Thrombosis: A Rare Complication of Spinal Anesthesia

Introduction: Cerebral venous thrombosis (CVT) is a rare complication of spinal anesthesia. The diagnosis is difficult when it occurs by isolated headache, following post spinal puncture headache (PSPH). We report a case of cerebral venous thrombosis complicating PSPH, following a spinal anesthesia. Observation: A 23-year-old man was admitted to the emergency room for PSPH appeared 6 hours after a spinal aesthesia for removal of osteosynthesis equipment from the right tibial plateau. The headache was fronto-occipital, relieved by lying down. Six days later, headache became permanent, not relieved by morphine, associated with vomiting. The neurological examination was normal. Brain magnetic resonance Magnetic venography showed superior sagittal sinus and right transverse sinus thrombosis. After administration of low molecular weight heparin at curative dose, and warfarin under INR control, the evolution was marked by a rapid regression of headaches on the same day. After a 1-year setback, the patient was asymptomatic. Conclusion: CVT is a rare complication of spinal anesthesia and potentially severe if diagnosed lately. Urgent brain imaging must be performed for any atypical headache or the onset of neurological disorders so as not to misunderstand cerebral vein thrombosis. Controlling the spine anesthesia would reduce the inadvertent perforation of the dura-mother that would lead to CVT complicating post spinal puncture headaches.
