Trends in HIV-2 Seroprevalence at the National Reference Center of HIV from 2005 to 2014 in Lome, Togo

Background: In Togo, the HIV/AIDS epidemic is characterized by the circulation of the 2 subtypes of HIV. Thus, patients infected with HIV-2 are diagnosed and monitored in the care centers. Objective: To document the trend of HIV-2 prevalence over a decade of activities of the National Reference Center for HIV tests and screening (CNR-VIH). Methods: A cross sectional study was carried out from the screening data archived from January 2005 to December 2014 at the CNR-VIH, a laboratory located in the Sylvanus Olympio University Hospital (CHU SO) Lome. The sampling consisted of adults and children outpatients or those who were hospitalized in CHU SO, subjects presenting for voluntary testing, pregnant women and patients or samples referred for HIV confirmation. All samples were tested for HIV-1 and HIV-2 infections by combining ELISA and rapid diagnostic tests. Result: During the decade, 34,077 subjects were screened for HIV infection. The overall prevalence of HIV infection was 20.70% (7055/34077). In 10 years, the prevalence of HIV infection in CNR-VIH decreased significantly from 35.40% CI95% [34.50% - 36.20%] in 2005 to 14.20% CI95% [13.60% - 14.70%] in 2014 (p = 0.03). The prevalence of HIV-1, HIV-2 and dual HIV1&HIV-2 was respectively 20.40%, 0.23% and 0.07%, with annual prevalence of HIV-2 between 0.07% and 0.39%. The differences between the HIV-2 prevalence over the decade were not statistically significant (p > 0.15). A 4-year-old child from mother-to-child HIV-2 transmission was diagnosed. Sixty-five percent of adult patients were over 40 years of age with an average age of 43.5 ± 11.3 years. Conclusion: Data from the National Reference Center for HIV Tests in Togo over the last ten years confirm the existence of a weak epidemic of HIV- 2 infection with a tendency towards stability.

Epidemiology of Cardiovascular Mortality in Patients under 60 Years Old in a Cardiology Department in West Africa, Dakar-Senegal

Background: Our study aimed to examine cardiovascular mortality within the working-age population, exploring epidemiological, clinical, and paraclinical features, complications, and identifying etiological factors linked to mortality. Methods: We conducted a descriptive and analytical retrospective study from September 2019 to August 2022 at the General Hospital Idrissa POUYE in Dakar, we reviewed all the medical records of patients from 15 to 60 years old who died while admitted in the cardiology department. Data collected were socioeconomic status, clinical history, type of cardiovascular disaese, length of hospitalization, circumstances and timing of death. The data were analyzed with R. Studio version 2022.12.0 + 353 and Excel 2019, with a P-value Results: The study included 73 patients, indicating a specific mortality rate of 8.8% and a proportional mortality of 39%. Predominantly male (sex ratio 1.2), the average age was 44. Key cardiovascular risk factors identified were sedentarism (76.7%), hypertension (28.8%), and smoking (21.9%). The leading cause for consultation was dyspnea (72.6%). Notable findings included a majority of patients presenting with general condition deterioration (90%) and cardiovascular collapse upon admission (23.3%). Physical exam revealed signs of heart failure in 63%. Echocardiography showed left ventricular ejection fraction impairment (81%) and pulmonary hypertension (78%). Immediate causes of death were primarily cardiogenic shock (45.2%) and septic shock (37%). The analytical study indicates that the data most closely associated with mortality were age, socio-economic level, ischemic heart disease (p = 0.034), rheumatic valvulopathies, pulmonary embolism (p = 0.034), hypertension (HTA) (p = 0.009), smoking (p = 0.011), diabetes (p = 0.011), dyslipidemias, prolonged bedrest (p = 0.001), morbid obesity (p = 0.001), and COVID-19 infection (p = 0.017). Assessment of the Physical Capabilities of Heart Failure Patients before and after Cardiovascular Rehabilitation: A Study of 125 Patients from West Africa, Dakar, Senegal

Background: Cardiac rehabilitation represents a critical therapeutic strategy for patients suffering from chronic heart failure. The physical capacity of patients with heart failure, assessed using the exercise test and the 6-minute walk test, is the measure of the patient’s overall functional ability to perform physical activities and tolerate exercise loads. The objective of this study was to assess the impact of cardiac rehabilitation on patients’ physical capabilities and to conduct a thorough comparison of data obtained via exercise testing and the 6-minute walk test before and after the rehabilitation programme. Methods: This was a descriptive and analytical cross-sectional study, conducted from 1 February 2021 to 31 June 2022. Included were heart failure patients who had participated in an outpatient cardiovascular rehabilitation programme. The collected data included anamnestic, clinical, paraclinical data, and the 6-minute walk test. Informed consent was obtained. Data analysis, word processing, and charting were performed using Microsoft Word 2016, Excel 2013, and Sphinx version 5.***.0.2. Data analysis was performed using SPSS (Statistical Package for Social Sciences) version 24.0. Any difference less than 0.05 was considered statistically significant. Results: In a Senegalese study, heart failure patients undergoing rehabilitation in a cardiac unit represented 45.59% of all cases, with a prevalence rate of 3.21%. The average participant was 57.97 years old, with those aged 61 to 70 forming the largest group (35.5%). The study noted a male predominance (sex ratio of 2.1) and identified dyslipidaemia (80.6%) and sedentarism (71%), as prevalent cardiovascular risk factors. All participants initially suffered from NYHA stage 2 or 3 dyspnoea, yet 80.65% showed no symptoms following rehabilitation. Assessment of Groundwater Physico-Chemical Quality in the Ouémé Delta (Southern-Benin)

Groundwater resources are the main sources of water used to supply drinking water to the population of the Ouémé Delta via the Continental Terminal aquifer. Urbanization, population growth, and agricultural and industrial activities have resulted in a deterioration in the quality of these resources. To assess the quality of the delta’s groundwater and its suitability for human consumption and irrigation, a total of fourteen (14) physico-chemical parameters were analyzed in some forty existing water points between September 2020 and March 2021, using standard water analysis techniques. The values obtained were compared with the potability standards recommended by the World Health Organization (WHO) and the Republic of Benin and were subjected to statistical analysis (principal component analysis (PCA)). In addition, methods for determining the suitability of water for irrigation were used. The results showed that the waters are acidic to slightly neutral and influenced by ambient temperature. In addition, the waters are moderately mineralized, with conductivities (24 - 1205 μS/cm) in line with WHO standards. A comparison of the analytical results of the WHO (2017) and Benin (2001) standards indicates that the majority of the waters studied are of good quality for all the chemical parameters considered. Nevertheless, some samples show levels of nitrates (21%), potassium (14% to 16%), calcium (13%), ammonium (12%), nitrites (8%) and bicarbonates (10%) over their respective standards. The Wilcox and Riverside diagrams indicate that the majority of waters (90%) have excellent suitability for irrigation and no negative effect on soil fertilization.

Evaluation and Prediction of Groundwater Quality in the Municipality of Za-Kpota (South Benin) Using Machine Learning and Remote Sensing

Accessing drinking water is a global issue. This study aims to contribute to the assessment of groundwater quality in the municipality of Za-Kpota (southern Benin) using remote sensing and Machine Learning. The methodological approach used consisted in linking groundwater physico-chemical parameter data collected in the field and in the laboratory using AFNOR 1994 standardized methods to satellite data (Landsat) in order to sketch out a groundwater quality prediction model. The data was processed using QGis (Semi-Automatic Plugin: SCP) and Python (Jupyter Netebook: Prediction) softwares. The results of water analysis from the sampled wells and boreholes indicated that most of the water is acidic (pH varying between 5.59 and 7.83). The water was moderately mineralized, with conductivity values of less than 1500 μs/cm overall (59 µS/cm to 1344 µS/cm), with high concentrations of nitrates and phosphates in places. The dynamics of groundwater quality in the municipality of Za-Kpota between 2008 and 2022 are also marked by a regression in land use units (a regression in vegetation and marshland formation in favor of built-up areas, bare soil, crops and fallow land) revealed by the diachronic analysis of satellite images from 2008, 2013, 2018 and 2022. Surveys of local residents revealed the use of herbicides and pesticides in agricultural fields, which are the main drivers contributing to the groundwater quality deterioration observed in the study area. The results of the groundwater quality prediction models (ANN, RF and LR) developed led to the conclusion that the model based on Artificial Neural Networks (ANN: R2 = 0.97 and RMSE = 0) is the best for groundwater quality changes modelling in the Za-Kpota municipality.

Microbiological Quality of “Rabilé”, a Yeast Used for Fermentation of Dolo, a Local Beer in Dédougou, Burkina Faso

Sorghum beer or dolo is part of the eating habits of part of the population of Dédougou because of its low price compared with industrial beers. Its production is an ancestral tradition that uses traditional equipment and gives dolo organoleptic properties that are not found in industrial beers. The production process involves several stages, including fermentation, which itself comprises natural lactic fermentation followed by alcoholic fermentation using traditional yeasts, which are not controlled in any way. The general aim of this study is to assess the microbiological quality of these fermentative yeasts in the town of Dédougou, in order to contribute to the health safety of the population and the promotion of these local beers. Twenty samples of fermenting yeast were analyzed according to ISO standards, to isolate enterobacteria, total and faecal coliforms according to standard procedures for isolating these micro-organisms. The isolated strains were identified using the API20E gallery. Microbiological analyses revealed the presence of 51.17% enterobacteria, 45.38% total coliforms and 3.45% thermotolerant coliforms. We counted 40% Escherichia coli, 20% Enterobacter cloacae, 20% Klebsiella pneumoniae and 20% Klebsiella spp. All the strains detected are capable of surviving in hostile conditions and could harm the quality of the dolo, consumer health and cause real collective food poisoning in the town of Dédougou. This enabled us to assess the microbial quality of these yeasts and to propose more suitable measures for producing and preserving dolo under hygienic conditions to protect consumer health.

Preliminary Study of Chemical Elements Distribution in Petroleum Source Rocks Donga and Yogou Formations of the Termit Sedimentary Basin (Est-Niger)

XRF and EDX analyses were carried out on 18 batches of representative raw samples to determine the distribution of major chemical elements in the petroleum source rocks of Donga and Yogou formations of Termit sedimentary basin. The chemical composition of these formations is dominated by silicon (Si), aluminum (Al) and iron (Fe). This is consistent with the oxide composition, which is also dominated by silicon oxide (SiO2), aluminum oxide (Al2O3) and iron monoxide (FeO). No less important chemical elements are calcium (Ca), potassium (K), sulfur (S), titanium (Ti), magnesium (Mg), manganese (Mn) and barium (Ba), as well as some of their oxides. All these major chemical elements are carried by silicate detrital minerals associated with pyrite and goethite and/or clay minerals such as kaolinite and interstratified illite, smectite and chlorite. This trend is illustrated by the values of the Si/Al and SiO2/Al2O3 ratios.

Understanding metal accumulation at organ level in roots, leaves and seeds in O. glaberrima (OG) is crucial for improving physiological and metabolic aspects in growing Asian and African rice in salted areas. The micro-analytical imaging techniques are required to reveal its accumulation and distribution within plant tissues. PIXE studies have been performed to determine different elements in rice plants. The existing microbeam analytical technique at the iThemba LABS will be applied for the 2D image mapping of fresh rice tissues to perform a concentration of low atomic mass elements (such as Al, Si, P, S, Cl, Ca, Ti, Mn, Fe, Cu, Br, Zn and K) with detection limits of typically 1-10 μg/g. Comparison of the distribution of the elements between leaves, root and seed samples using uptake and distribution of elements in particular environmental conditions with potential amount of salt in water have been performed. We are also expecting to indicate metal exclusion as salt tolerance strategies from leaves, root, and seed compartments using matrix correlation between samples and between elements on rice species.

The transformation of uterine fibroids is common in relation to their development. Giant forms of cystic degeneration are rare. They raise diagnostic difficulties with other pelvic tumors, such as ovarian tumors and leiomyosarcomas. Magnetic resonance imaging specifies the original organ, the volume and the main relationships of fibromyoma with adjacent structures. The diagnosis of certainty is based on laparotomy coupled with histology. The authors illustrate these difficulties by observing a giant cystic degenerative fibroma in a 26-year-old G1P1 woman in the postpartum period.

Effect of Cardiac Rehabilitation on Physical Capacity of Heart Failure Patients in Senegal

Background: Heart failure is a chronic and severe condition that often results from various heart diseases. Cardiac rehabilitation (CR) is currently a crucial component in managing this condition. The aim was to assess the effects of cardiac rehabilitation on physical capacity of heart failure patients. Methods: This was a cross-sectional study conducted from February 1, 2021, to June 30, 2023. We included all patients with heart failure who underwent cardiac rehabilitation. Data analysis was performed using SPSS software version 24.0, with a significance level set at p Results: The study included 87 heart failure patients, with a male-to-female ratio of 1.8. Mean age was 57.10 years (±11.75). Coronary artery disease was the primary cause of heart failure, accounting for 75.9% of cases. Atrial fibrillation was present in 4.7% of cases. Following cardiac rehabilitation, Left Ventricular Ejection Fraction increased from 40.15% to 49.48% (p = 0.001). Resting heart rate decreased significantly from 81.4 bpm to 68.3 bpm (p = 0.000), and the number of METS increased from 4.3 to 6.57 (+56.8%;p = 0.000). The mean distance covered in the 6-minute walk test significantly increased from 337.8 meters to 522.7 meters (p = 0.000), reflecting a gain of 183.5 meters. Moreover, the increase in the number of METS was more pronounced in females (p = 0.001), non-obese individuals (p = 0.000), non-diabetics (p = 0.001), non-sedentary individuals (p = 0.000), and non-smokers (p = 0.000). The study reported a low readmissions rate of 2.2% and a mortality rate of 1.1%. Conclusion: Our study demonstrates that cardiac rehabilitation is beneficial for black African heart failure patients, resulting in significant improvements in symptoms, physical and capacity.
