Profile of SARS-CoV-2 Infected Patients Hospitalized at the Epidemic Treatment Center (ETC) in Saint-Louis, Senegal during the First Two Waves

Introduction: The SARS-CoV-2 infection is a major public health emergency. Several risk factors are involved in the occurrence of respiratory distress that can lead to death despite resuscitation measures. Objectives: The aim of this study was to describe the epidemiological, clinical, paraclinical, therapeutic, and evolution profile of patients infected with SARS-CoV-2 hospitalized at the CTE of Saint-Louis (Senegal) during the first two waves. Patients and Methods: We conducted a retrospective, cross-sectional, descriptive, and analytical study that included all patients hospitalized at the ETC of Saint-Louis (Senegal) with SARS-CoV-2 infection from March 2020 to April 2021. Results: A total of 358 cases were collected, 256 (71.5%) during the first wave and 102 (28.5%) during the second wave. The mean age was 49.5 years (19.5). There was a male predominance (58.4%), with a sex ratio of 1.4. Hypertension was the main comorbidity, with 87 cases (24.3%). The most common functional signs were cough in 194 cases (54.2%), dyspnea in 143 cases (40%) and ageusia in 134 cases (37.4%). Thoracic CT scans were performed on 20 patients (5.6%), with severe involvement (50% - 75%) observed in 50% of cases. Hydroxychloroquine-azithromycin was prescribed to 351 patients (98%). Overall, 338 (94.4%) recovered and 17 (4.7%) died. In multivariate analysis, factors associated with death were male sex [OR = 2.645;95% CI: 1.530 - 4.785;p = 0.011], age 60 years [OR = 1.039;95% CI: 0.564 - 1.914;p = 0.002], the presence of comorbidities [OR = 2.171;95% CI: 0.564 - 3.429;p = 0.033], SpO2 (ambient air) 95% [OR = 2.061;95% CI: 0.616 - 3.827;p = 0.03], acute respiratory distress syndrome (ARDS) [OR = 0.635;95% CI: 0.316 - 1.275;p = 0.001], severe form [OR = 1.664;95% CI: 0.298 - 2.478;p = 0.016], occurrence of complications [OR = 0.521;95% CI: 0.287 - 0.944;p = 0.032], high creatinine levels [OR = 2.061;95% CI: 1.616 - 3.827;p = 0.026], and lymphopenia [OR = 0.485;95% CI: 0.370 - 0.636;p = 0.0

Introduction: COVID-19 is a global public health emergency that can cause acute respiratory distress syndrome. In countries where tuberculosis (TB) is endemic, coinfection of COVID-19 and TB is often encountered, which increases the risk of developing severe forms of COVID-19. Objectives: The aim of this study was to determine the prevalence of COVID-19/TB coinfection at the Epidemic treatment center (ETC) in Saint-Louis (Senegal) and to describe the epidemiological, clinical, paraclinical, and outcome profile of co-infected patients. Patients and Methods: This is a retrospective, cross-sectional, descriptive cohort study based on the records of COVID-19/ TB co-infected patients who were hospitalized at the ETC in Saint-Louis (Senegal) over an 18-month period from March 2020 to September 2021. Results: Out of a total of 454 hospitalizations, we collected records of 9 patients co-infected with COVID-19/TB, resulting in a prevalence of 2%. The study included patients with a median age of 34 years (range: 10-86 years), with a male predominance (7 cases) and a sex ratio of 3.5. The majority of patients (88.9%) had severe forms of COVID-19. Dyspnea and cough were reported in all patients (100%). Pulmonary TB was the most frequent localization, with 9 cases. The diagnosis of COVID-19 was confirmed by nasopharyngeal PCR in all patients (100%). Bacilloscopy was positive in 3 out of 5 cases. One patient tested positive for GeneXpert? MTB/RIF without rifampicin resistance. All patients were prescribed the hydroxychloroquine-azithromycin combination and anti-tuberculosis treatment. Out of the nine patients, four recovered (44.4%) and five died (55.5%). Conclusion: COVID-19/TB coinfection had a low prevalence in our cohort, but was associated with a high mortality due to the frequent occurrence of severe forms of the disease.

Introduction: Entero bacteria are mainly found in the gut of man and animals. The frequent acquisition of antibiotic resistance mechanisms explains why they are the bacteria most often implicated in human infectious pathology. It is estimated to be involved in 50% of sepsis, 60% of enteritis, 70% of urinary tract infection case. Objective: To determine the prevalence of enterobacterial infections diagnosed at Fann Infectious diseases Clinic, and describe their epidemiological, clinical, therapeutic and evolutionary aspects. Patients and Methods: This is a retrospective and descriptive study, on patients hospitalized from January 2013 to December 2014, at Fann Infectious diseases Clinic, with bacteriological confirmation of an enterobacteria infection. Results: A total of 129 cases were collected during the study period. The average age was 41 years, and female were predominant (60%) with a sex ratio of 0.67. Comorbidity was found in 88.4% of the cases, most of which were HIV infection. The most common clinical signs were infectious syndrome (53.49%) and general impairment (40.31%). The main gateway was urinary (55.8%). Samples were monomicrobial in 76.7% of cases. Klebsiella and Escherichia were the most common and 68.7% of the subjects had probabilistic treatment. Most enterobacterial strains were resistant to third generation cephalosporins (C3G), aminoglycosides to ciprofloxacin and cotrimoxazole. Aside from 4% of them, all were sensitive to imipenem. Conclusion: The advent of antibiotics has brought hope in the treatment of enterobacterial infections. However, an increase in their resistance to the usual antibiotics has been noted in recent years. As a result, the fight against antibiotic resistance must be a priority.

Contribution of Seismic and Magnetic Data for the Prevention of Geotechnical Risks Related to Building Cracking in Agadez Region (North Niger)

This study tries to highlight the role of magnetic and seismic data in the prevention of building cracking, which constitutes a geotechnical risk in Agadez City. This city is built on the faulted and fractured sandstone formations of the “Agadez Sandstones Formations”, which were deposited in unconformity on the basement of the Aïr Mountain. This study focuses on the prevention of geotechnical damages related to building cracking in the Agadez region through geophysical methods, which are well known for investigating tectonic structures and their directions on the surface and subsurface. A methodological approach integrated the seismic and magnetic data interpretation combined with field measurement on the cracked building and its underlying substratum, represented by Agadez sandstones and basement. The extraction of seismic lineaments from the West African seismic map showed the seismic directions oriented NW-SE (N135˚ to N160˚), passing through the studied area. The structural interpretation of the magnetic map shows that the Agadez region is also affected by the subsurface lineaments mainly oriented in NW-SE (N135˚) directions, which are similar to the identified seismic lineaments in the same zone. A structural study carried out on the Agadez sandstones and the underlying basement showed that faults and fractures oriented N120˚ to N165˚ affect both the basement of the Aïr Mountains and the sandstone formations on which the city of Agadez is built. These observations showed that building cracking in the Agadez region has a higher propagation tendency according to the directions ranging from NW-SE (N135˚) to NNW-SSE (N165˚). Therefore, the building’s cracking has a stronger propagation component according to these mean directions that are not recommended for building. Preparation and Characterization of Clay Soils for Applications in Cyanide Removal from Mining Wastewater

In Burkina Faso, water resources in mining areas are polluted by cyanide, which is released into the environment through mining activities. To mitigate this pollution, two clays named Koro and Kaya were collected in Burkina Faso and characterized using analytical technics. This work aimed to find out a cheap technology based on local soils for water treatment. The objective of the work was to prepare and characterize two local clays for application in mining effluent treatment. Specific surface area and pore dimensions were analyzed using Brunauer, Emmett and Teller method. In addition, anion exchange capacities, chemical composition through X-ray fluorescence and chemical functions by Fourier-transformed Infrared spectroscopy were determined. Mineralogical composition, thermal behavior, microstructure, and surface elemental composition of clays were examined using techniques of X-ray diffraction and scanning electron microscopy coupled with energy dispersive spectroscopy. Results showed that the surface areas of two anionic clays were 64 m2/g and 50 m2/g for Koro and Kaya, respectively. An anion exchange capacity of 126.68 cmol/kg for Koro and 125.13 cmol/kg at pH 11 was found. Chemical and mineralogical analysis showed that the clays were rich in minerals, mainly of illite (24 - 27)%, montmorillonite (23 - 24)% and kaolinite (17 - 26)%, orthosis (9 - 13)%, quartz (5 - 12)%, goethite (3 - 6)% and anorthite (2 - 6)%. Application of clays for the treatment of cyanide solution led to a decrease in intensities of chemical functions and a modification of the surface morphology. Consequently, those clay soils can be used as adsorbents in cyanide removal through column and batch experiments.

Introduction: diabetic ketoacidosis (DKA) is the most common metabolic complication of diabetes. Although historically reported in children, it is increasingly observed in adults. Short-term outcomes and prognosis vary widely across healthcare settings. Therefore, we aimed to determine the prevalence, diagnostic characteristics, and outcomes of newly diagnosed diabetic ketoacidosis in a hospital setting in Dakar. Methodology: We conducted a descriptive and analytical cross-sectional study from January 1, 2020, to January 31, 2021, in the Endocrinology-Metabolism Department of the Pikine National Hospital Center. All diabetic patients hospitalized for newly diagnosed diabetic ketoacidosis during this period were included. Results: A total of 54 patients were enrolled in the study. The prevalence of newly diagnosed DKA in the facility was 17.1%. The mean age was 38.54 years, with a male predominance and a sex ratio of 1.16. Thirty-five patients had a family history of diabetes. Upon admission, 14.9% of patients had altered consciousness, and 92.4% exhibited cardinal symptoms, with an average duration of 14.2 days. Kussmaul breathing and gastrointestinal symptoms were observed in 13% and 33% of cases, respectively. The mean capillary blood glucose level at admission was 3.43 g/L. Infection was identified as the precipitating factor in half of the cases, while no triggering factor was found in the remaining cases. The mean BMI was 25.27 kg/m², and the average glycated hemoglobin (HbA1c) level was 12.2%. We recorded the diabetes classification for all patients. Nine had type 1 diabetes, twelve had type 1b diabetes, thirty-one had type 2 diabetes, and two had diabetes secondary to corticosteroid therapy. All patients received continuous intravenous insulin therapy via a syringe pump. The mean time to ketosis resolution was 34.6 hours. The average total dose of regular insulin administered was 442.3 IU. The mean duration of hospitalization was 7.6 days. One patient died, a

Context: The presence of conjoined twins is a rare occurrence during pregnancy. Today, early diagnosis is possible thanks to advances in imaging, particularly three-dimensional ultrasound, and the intervention of a trained operator. This antenatal diagnosis can be used to decide whether or not to continue the pregnancy. Cases presentation: Based on the authors’ experience, five (05) cases of conjoined twins were collected over a period of six years (01 January 2018-01 January 2023) in the maternity wards of the Angre’s Teaching Hospital (study location), whose diagnosis was sometimes unexpected in the delivery room. Among the five cases, we described three (03) thoraco-omphalopages and two (02) parapages, with a female predominance of 4/5. None were candidates for separation because they were stillborn or died on the first day of life. Conclusion: This series of cases highlights the different characteristics of conjoined twins and the poor fetal prognosis due to diagnostic difficulties and therapeutic possibilities that are practically non-existent in the African environment.

From a harmonized structural map at the scale of a portion of SW Burkina Faso (90 × 80 km2), we targeted prospective zones on the basis of structural criteria and structure-geology relationships. The map was constructed from an aeromagnetic image, and the geology was deduced from old geological maps. The procedure involved extracting the magnetic fabric and deducing the deformation gradients, which led to the identification of shear zones. Analysis and interpretation of the structural geometry of the obtained structural pattern led us to target areas of interference between structures or geology-structures interplays, which are conducive to the deposition of metallic elements. The subsequent overlay of known gold prospects and deposits documented in the region revealed the validity of the predefined prospects. This result reinforces the applicability of the methodology as a first-order regional approach to mineral exploration.

电子邮件是最常用的互联网应用与Web技术之一。电子邮件可以用于各种应用场景,涉及到工作、生活等方面,与传统的通信手段相比,更加方便快捷。除了成本低,更有利于信息传播等特点,是众多公司的主要通信方式。使用电子邮件进行敏感或机密文件交换时往往会存在一些问题,自1970年标准化邮件应用协议的设计及应用至今,互联网安全问题比较少。但是,电子邮件的出现也带来了其它不良后果,比如:垃圾邮件,病毒等。高级的网络攻击如网络钓鱼邮件、附件和其它恶意链接等。这些电子邮件攻击方式经常绕过基于签名的保护系统,如防病毒软件和垃圾邮件过滤器等。本文介绍了一种安全的电子邮件系统,尤其是如何通过使用对称密钥和公钥技术设计并实现的个人电子邮件系统。该系统模型包括邮件管理模块、消息加密模块和会话密钥加密模块,其中邮件管理模块用于发送和接收电子邮件;消息加密模块用于执行电子邮件加密和解密;会话密钥加密模块是通过管理公共和私有密钥对会话密钥进行加密。该系统的设计与实现概括如下:·在邮件加密操作之前,邮件管理模块负责发送和接收电子邮件。邮件服务器Postfix用于发送,Dovecot邮件服务器用于接收,此系统在Linux平台上设计实现;·消息加密模块通过AES加密方法使用对称密钥加密电子邮件或消息,并使用OpenSSL库随机生成会话密钥;·会话密钥加密模块通过非对称加密技术RSA加密方法对会话密钥进行加密。对于公钥和私钥,可以使用OpenSSL库来获取所有相关参数,如公共和私有密钥指数,模数等;·为了让用户读取邮件,开发解密程序先通过收件端的私钥获得会话密钥,然后通过使用会话密钥对消息进行解密。设计实现的邮件系统可以对当前电子邮件系统的安全性加以改进。设计方案使用了混合加密方法,包括对称和非对称加密方法以加强邮件系统的安全性。用户可以使用RSA加密电子邮件并使用安全的信道将会话密钥发送到接收端,这样接收端将收到两个消息,第一个是消息的密文,第二个是加密的会话密钥。系统采用上述安全控制措施,并在Linux shell命令行界面中实现用户间电子邮件的可靠、透明传输。
