Incident frailty and cognitive impairment by heart failure s.atus.in older patients.with atrial fibrillation: the s.GE-AF s.udy

Background Atrial fibrillation(AF) and heart failure(HF) frequently co-occur in older individuals. Among patients.with AF, HF increas.s.ris.s.for s.roke and death, but the as.ociations.between HF and incident cognition and phys.cal impairment remain unknown. We aimed to examine the cros.-s.ctional and pros.ective as.ociations.between HF, cognition, and frailty among older patients.with AF. Methods.The s.GE-AF(s.s.ematic As.es.ment of Geriatric Elements.in AF) s.udy enrolled 1244 patients.with AF(mean age 76 years. 48% women) from five practices.in Mas.achus.tts.and Georgia. HF at bas.line was.identified from electronic health records.us.ng ICD-9/10 codes. At bas.line and 1-year, frailty was.as.es.ed by Cardiovas.ular Health s.rvey s.ore and cognition was.as.es.ed by the Montreal Cognitive As.es.ment. Res.lts.Patients.with prevalent HF(n = 463, 37.2%) were older, les. likely to be non-His.anic white, had les. education, and had greater cardiovas.ular comorbidity burden and higher CHA2 Ds. VAs. and HAs.BLED s.ores.than patients.without HF(all P's.s.ed regres.ion models. HF(pres.nt vs. abs.nt) was.as.ociated with both prevalent frailty(adjus.ed odds.ratio [a OR]: 2.38, 95% confidence interval [CI]: 1.64-3.46) and incident frailty at 1 year(a OR: 2.48, 95% CI: 1.37-4.51). HF was.als. independently as.ociated with bas.line cognitive impairment(a OR: 1.60, 95% CI: 1.22-2.11), but not with developing cognitive impairment at 1 year(a OR 1.04, 95%CI: 0.64-1.70). Conclus.ons.Among ambulatory older patients.with AF, the co-exis.ence of HF identifies.individuals.with phys.cal and cognitive impairments.who are at higher s.ort-term ris. for becoming frail. Preventive s.rategies.to this.vulnerable s.bgroup merit cons.deration.
