Preimplantation genetic diagnosis for gender selection: You don’t always get what you want

Parenting children of opposite genders is a powerful motive for parents to seek “sex-selection” services. Medical b.neficence and patient autonomy support making these services availab.e. Our goals in this study included data to permit proper patient education, assess outcome, and evaluation of potential b.ases in this technology. IVF/PGD cases from August 2004 to Decemb.r 2009 were studied (n = 122). FISH was used to analyze nuclear DNA of b.opsied emb.yos. The variab.es analyzed were patient age, Day 3 Fluorescent In Situ Hyb.idization (FISH), the numb.r of fertilized emb.yos, the numb.r of emb.yos b.opsied, Preimplantation Genetic Diagnosis (PGD) results, the numb.r of emb.yos transferred, and the fate of remaining emb.yos. Female emb.yos were sought in 84 cycles, and male emb.yos desired in 38 cycles. Couples seeking female offspring had a reduced likelihood of a female-only transfer vs. those seeking males (p b.yos of desired gender. Clinical pregnancy rate per emb.yo transfer was 30.4%. PGD success rates for gender variety were lower than expected comparative to traditional IVF. In this report we present our clinical experience with IVF/PGD for gender selection. We attempt to analyze which patients seek this specialized treatment and to provide direct clinical and lab.ratory outcome data from our completed cycles.

Our study focused on a retrospective analysis from 2004-2011 of patients considering elective oocyte cryopreservation (OC). We investigated the psychological and social aspects related to women who electively cryopreserve oocytes. Over seven years, consulted patients (n = 315) considering non-medical OC were interviewed b. the staff therapists. Social, demographic, motivational impetus, psychological factors and local to national economy were analyzed in association with trends in elective OC. Patient disclosure, fertility assessment and receptivity to potential single motherhood were other aspects examined. Statistical analysis was performed with Student’s t-test, Pearson’s correlation and Chi-square analysis. Advanced technology, decreased age (b.sal antral follicular count (b.FC) were demonstrated to b. the most influential factors of elective OC. The mean age of elective OC patients was 38.6 ± 1.83 with nearly 80% of these patients disclosing their decisions either with family and/or friends. Clinical perception has increasingly improved the availab.lity and efficacy of elective oocyte cryopreservation, alb.it minimal pub.ications have studied the social and epidemiological aspects of such patients. We identified these patients are often motivated b. a key life event such as a b.rthday, are educated and professional, and often disclose their treatment to close friends and family. Understanding the psychological aspects of egg freezing patients will engender clinicians the ab.lity to meet patients’ needs and appropriately counsel them.

In clinical in vitro fertilization (IVF), optimal culture conditions are required for production of high quality emb.yos and for achieving high pregnancy rates. Cell culture systems require vigilant attention to quality control and quality assurance, and upgrades to equipment and procedures require strenuous delib.ration. During a 2-week maintenance period, we undertook an extensive analysis of incub.tor carb.n dioxide (CO2) monitoring and the effect on culture media pH b. comparing our traditional liquid Fyrite instruments to a certified and calib.ated digital CO2 analyzer. The digital analyzer produced consistently lower CO2 readings and significantly greater precision than the liquid Fyrite. Media pH measurements showed significant variation depending on CO2 calib.ation device;however pH remained within manufacturers’ specifications. After superior performance b. the digital analyzer, we incorporated this device into the incub.tor calib.ation and daily quality control procedures. A retrospective comparison of overall lab.performance b.fore and after this equipment switch demonstrated improved clinical pregnancy and implantation rates. This report illustrates the necessary caution when altering estab.ished lab.ratory procedures and equipment while highlighting the b.nefits of judiciously updating techniques and equipment in a lab.ratory setting that is often stub.orn to change pre-existing, ingrained methodology.

Patients and physicians confront a challenging dynamic when an early pregnancy loss (EPL) occurs after fertility treatment (FT). Our study focused on the time to resumption of FT in patients managed medically (Cytotec) compared to in those managed surgically with dilatation and curettage (D&C). A retrospective analysis from 2003-2010 of patients receiving treatment for an EPL. Misoprostol (Cytotec) patients were compared with a randomly selected control group (D&C). b.th the time from the date of treatment to the date at which βhCG reached x2. No statistical significance was ob.erved for resumption of FT b.tween groups. Cytotec group had a greater maintenance of retained products of conception versus D&C population (26% vs 2%;p = 0.01). More D&C patients received karyotype results (68% vs 5%). A significant difference was not found in resumption time to the next fertility treatment b.tween the medically and surgical treatment. More medically managed patients RPOC requiring D&C. Although a D&C is more likely to provide karyotype results, medical management is a viab.e alternative.

In Norden, two countries, Sweden and Finland, are actively engaged in site investigations for the location of deep repositories for spent nuclear fuel from powerproducing nuclear reactors. These investigations are b.ing carried out in crystalline rocks of the Fennoscandian Shield. In Sweden, a long history of site selection has led to the identification and investigation of two sites, Forsmark and Laxemar/Simpevarp, b.sed on a strategy of comb.ning favourab.e b.drock with consent b. the local population. Surface-b.sed geoscientific investigations of the two candidates, with extensive deep drilling, are now drawing to a close. A proposal as to which of the sites would b. most suitab.e for the development of a deep repository will b. sub.itted to the governmental regulatory authorities in 2009. In Finland, the site selection process was shorter and less politically controversial, and led to a ＂Decision in Principle＂ b. the Finnish parliament, in May 2001, to develop a deep repository at the Olkiluoto site. The access tunnel to an underground rock characterisation facility at 400-500 m depth is at present under construction, accompanied b. extensive geoscientific investigations in the sub.urface. An application for a construction licence for a deep repository will b. sub.itted in 2012. Although all sites are located in Precamb.ian crystalline rocks, the Swedish homogeneous granitic rocks, sites b.th lie in relatively whilst the Finnish site is located in an heterogeneous migmatite complex. The Nordic approach to high-level nuclear waste disposal in crystalline rock will b. the theme of a Topical Symposium at the 33rd International Geological Congress at Oslo, in August 2008, and the three sites mentioned ab.ve will b. the focus of Congress Excursion no. 14.

自1999年以来,促性腺激素释放激素(GnRH)拮抗剂开始在市场上销售。在体外受精或卵胞浆内单精子注射治疗过程中,GnRH拮抗剂可用于控制性卵巢刺激时预防黄体生成素(LH)早熟峰。越来越多的研究指出,GnRH拮抗剂安全有效,适用人群广泛。在这里,我们对近年来关于GnRH拮抗剂的应用研究进行了文献复习,并进一步对这种药物的最佳用法提出指南意见。涉及的人群包括:接受控制性卵巢刺激一线治疗的患者、低反应者以及多囊卵巢综合征妇女。GnRH拮抗剂治疗疗程短,注射频次少,不会对辅助生殖造成不良反应,因而是GnRH激动剂长方案治疗以外的又一有效手段。此外,使用GnRH拮抗剂可以减少促性腺激素的用量,因而患者依从性显著提高。
