BACKGROUND Metabolic dysfunction-associated steatotic liver disease(MASLD)and metabolic dysfunction-associated steatohepatitis(MASH)are a growing health burden across a significant portion of the global patient ***,these conditions seem to have disparate rates and outcomes between different ethnic *** combination of MASLD/MASH and type 2 diabetes increases the risk of hepatocellular carcinoma(HCC),and Hispanic patients experience the greatest burden,particularly those in South *** To compare outcomes between Hispanic and non-Hispanic patients in the United States,while further focusing on the Hispanic population within Southeast Texas to determine whether the documented disparity in outcomes is a function of geographical circumstance or if there is a more widespread reason that all clinicians must account for in prognostic *** This cohort analysis was conducted with data obtained from TriNetX,LLC(“TriNetX”),a global federated health research network that provides access to deidentified medical records from healthcare organizations *** cohort networks were used:University of Texas Medical Branch(UTMB)hospital and the United States national database collective to determine whether disparities were related to geographic regions,like Southeast *** This study findings revealed Hispanics/Latinos have a statistically significant higher occurrence of HCC,type 2 diabetes mellitus,and liver fibrosis/cirrhosis in both the United States and the UTMB Hispanic/Latino *** mortality in Hispanics/Latinos was lower within the United States group and not statistically elevated in the UTMB *** This would appear to support that Hispanic patients in Southeast Texas are not uniquely affected compared to the national Hispanic population.

肌萎缩侧索硬化(ALS)是一种神经退行性疾病,其特征是中枢神经系统中运动神经元的进行性丧失。星形胶质细胞在ALS的疾病进程中起重要作用。星形胶质细胞通过缝隙连接蛋白家族,如连接蛋白(Connexin,Cx)互相连接。Cx43是中枢神经系统中保持稳态的主要星形胶质细胞连接蛋白。在病理状态下,连接蛋白的表达和功能发生改变。本文发现,在ALS中Cx43异常增高是星形胶质细胞介导的毒性作用机制之一。笔者观察到,在ALS SOD1^(G93A)小鼠模型的疾病进程中,Cx43表达进行性增高。尤其是,在ALS患者的运动皮质和脊髓也能检测到Cx43增高。从SOD1^(G93A)小鼠中分离的星形胶质细胞,与人诱导多能干细胞衍生的星形胶质细胞一样,都检测到Cx43蛋白增高,且为独立于神经元共培养的内源性现象。与对照的星形胶质细胞过表达的野生型SOD1(SOD1^(WT))相比,SOD1^(G93A)星形胶质细胞中增高的Cx43表达可产生重要的功能性影响。本文发现,SOD1^(G93A)星形胶质细胞表现出缝隙连接偶联增强,半通道介导活动升高,细胞内钙离子水平上升。最后,本文检测Cx43蛋白表达升高对MN生存的影响,发现应用pan-Cx43阻滞剂和Cx43半通道阻滞剂均对与SOD1^(G93A)星形胶质细胞共培养的MNs有神经保护作用。这些新发现展示出未被认知的Cx43在ALS相关的运动神经元丢失中所起作用。

Entanglement and Volume Monogamy Features of Permutation Symmetric N-Qubit Pure States with N-Distinct Spinors: GHZ and States

We explore the entanglement features of pure symmetric N-qubit states characterized by N-distinct spinors with a particular focus on the Greenberger-Horne-Zeilinger (GHZ) states and , an equal superposition of W and obverse W states. Along with a comparison of pairwise entanglement and monogamy properties, we explore the geometric information contained in them by constructing their canonical steering ellipsoids. We obtain the volume monogamy relations satisfied by states as a function of number of qubits and compare with the maximal monogamy property of GHZ states.

Delay-Dependent Robust H_∞ Control for Uncertain 2-D Discrete State Delay Systems Described by the General Model

This paper considers the problem of delay-dependent robust optimal H∞ control for a class of uncertain two-dimensional (2-D) discrete state delay systems described by the general model (GM). The parameter uncertainties are assumed to be norm-bounded. A linear matrix inequality (LMI)-based sufficient condition for the existence of delay-dependent g-suboptimal state feedback robust H∞ controllers which guarantees not only the asymptotic stability of the closed-loop system, but also the H∞ noise attenuation g over all admissible parameter uncertainties is established. Furthermore, a convex optimization problem is formulated to design a delay-dependent state feedback robust optimal H∞ controller which minimizes the H∞ noise attenuation g of the closed-loop system. Finally, an illustrative example is provided to demonstrate the effectiveness of the proposed method.

The Feasibility of a Novel Dual-Task Exercise Program Which Integrates Balance, Gaze, Mobility and Cognition in Community Dwelling Older Adults: Protocol for a Randomized Clinical Pilot Trial

Background: Mobility limitations and cognitive impairments which are common with ageing often coexist, causing a reduction in the levels of physical and mental activity and are prognostic of future adverse health events and falls. Consequently, multi-task training paradigms that simultaneously address both mobility and cognition benefit healthy ageing are important to consider in rehabilitation as well as primary prevention. Objectives: An exploratory RCT is being conducted to: a) describe the feasibility and acceptability of the study design and process, procedures, resources and management in two game-based dual-task training programs delivered in the community;b) to explore the lived experiences of the study participants who completed their respective exercise programs. A secondary objective is to obtain preliminary data on the therapeutic effectiveness of the two dual-task training programs. Methods: Thirty healthy older community dwelling participants aged 70 - 85 with previous history of falls will be recruited and randomized to either dual- task treadmill walking (experimental group) or dual-task recumbent bicycle (control group). Data analysis: The qualitative data will be analyzed by two investigators using a content analysis approach. For the quantitative data, outcome measures will be collected pre and post intervention and included measures to assess core balance, spatial-temporal gait variables, visual tracking and cognitive function, as well as, balance and gait analysis under dual-task conditions. Discussion: This research will demonstrate the feasibility of the dual-task training programs in the community, and demonstrate the system’s ability to improve targeted and integrated (dual-task) aspects of balance, mobility, gaze, and cognitive performance. A blended analysis of balance, mobility gaze and cognition will also contribute to a better understanding of the functional consequences of decline in physical and mental skills with age. Adverse Drug Reactions in Management of Multi Drug Resistant Tuberculosis, in Tertiary Chest Institute

Background: Multidrug resistant tuberculosis is a global threat. Effective treatment is implemented as per RNTCP guidelines. But the drugs used have great potential to develop adverse drug reactions. Such drug reactions if not managed optimally can lead to unfavourable treatment outcome. Hence, the study is to know the occurrence of adverse drug reactions. Aims: To study the occurrence of adverse drug reactions in treatment of multidrug resistant tuberculosis and hence the factors affecting the treatment. Settings and Design: Retrospective analysis of patients treated with standardised regimen for MDR-TB, as per RNTCP guidelines at a tertiary chest institute between august 2011 and December 2014. Methods and Material: Retrospective analysis of 607 patients’ records reviewed for the occurrence of adverse drug reactions. All adverse reactions are noted and diagnosed either clinically or by laboratory evidence. Results: Among the 607 patients included in the study, majority had one or more adverse drug reactions. The most common was gastritis (71.7%), which was easily treatable, and the least common was visual impairment (0.2%). Only 1.7% discontinued the treatment citing adverse drug reactions and 10.5% required permanent discontinuation of the offending drug. Conclusion: Treatment of MDR-TB is challenging mainly due to the long duration of treatment and the potential adverse reactions of the drugs used. These reactions are frequent but majority of them can be successfully managed without treatment interruption. Training the peripheral health centre workers to identify and refer the patients with adverse reaction bears a major impact on treatment outcome.
