Stent migration, which causes issues in stent therapy for esophageal perforations, can counteract the therapeutic effects and lead to complications. Therefore, techniques to regulate stent migration are important and lead to effective stent therapy. Here, in these cases, we placed a removable fully covered self-expandable metallic stent(FSEMS) in a 52-year-old man with suture failure after surgery to treat Boerhaave syndrome, and in a 53-yearold man with a perforation in the lower esophagus due to acute esophageal necrosis. At the same time, we nasally inserted a Sengstaken-Blakemore tube(SBT), passing it through the stent lumen. By inflating a gastric balloon, the lower end of the stent was supported. When the stent migration was confirmed, the gastric balloon was lifted slightly toward the oral side to correct the stent migration. In this manner, the therapy was completed for these two patients. Using a FSEMS and SBT is a therapeutic method for correcting stent migration and regulating the complete migration of the stent into the stomach without the patient undergoing endoscopic rearrangement of the stent. It was effective for positioning a stent crossing the esophagogastric junction.

Robotic hepatectomy remains controversial to date,despite continuous advances in the technology and more surgeons gaining experience with the *** et al.(1)recently published an interesting article in Hepatobiliary Surgery and Nutrition,titled"Selecting incision-dominant cases for robotic liver resection:towards outpatient hepatectomy with rapid recovery."The authors evaluated 97 cases of robotically-assisted liver resection(RLR)arguing that it is difficult to reach the superior-posterior liver segments with the straight instruments used in conventional laparoscopic liver resection(LLR).The articulated instruments of the surgical robot are better suited for hepatectomies in these difficult-to-reach areas of the *** concluded that cases where the incision of an open approach would be large and affect recovery results in the highest likelihood of patients benefitting from a robotic *** think that the authors present an interesting perspective by focusing on the strong points of RLR compared to LLR.

The extracellular matrix of trabecular bone has a large surface exposed to the bone marrow and plays important roles such as hematopoietic stem cell niche formation and *** vitro reproduction of trabecular bone microenvironment would be valuable not only for developing a functional scaffold for bone marrow tissue engineering but also for understanding its biological ***,we analyzed and reproduced the initial stages of trabecular bone formation in mouse femur *** identified that the trabecular bone formation progressed through the following steps:(i)partial rupture of hypertrophic chondrocytes;(ii)calcospherite formation on cell membrane nano fragments(CNFs)derived from the ruptured cells;and(iii)calcospherite growth and fusion to form the initial three-dimensional(3D)structure of trabecular *** reproducing the initial trabecular bone formation in vitro,we collected CNFs from cultured cells and used as nucleation sites for biomimetic calcospherite ***,almost the same 3D structure of the initial trabecular bone could be obtained in vitro by using additional CNFs as a binder to fuse biomimetic calcospherites.

In February 2007,a 41-year-old Japanese male was admitted to our hospital with increasing upper abdominal pain.A contrast-enhanced computed tomography(CT)scan of the abdomen demonstrated a well-demarcated,hypodense cystic mass with a thickened wall in the *** laboratory results were within normal limits,except for increased carcinoembryonic antigen,carbohydrate antigen 19-9,DUPAN-2 and *** patient was diagnosed as having a mesenteric malignant cyst,and during a laparotomy,a right hemicolectomy with mesenteric cystectomy was performed without rupture in March *** the microscopic findings,there was a well-differentiated adenocarcinoma in the inner surface of the cyst and in the fibrous connective tissue of the hypertrophic cystic *** tumor cells were immunohistochemically reactive to cytokeratin(CK)7,CK18 and *** remnant of the malignancy was detected in the resected margin of the colon,cyst,liver or peritoneum nor was an uptake detected in an 18[F]-fluorodeoxyglucose positron emission tomography/CT examination of other ***,the malignancy was concluded to be a serous cystadenocarcinoma of the *** months after the operation,the patient died from peritonitis carcinomatosa due to a small intestine *** report suggests mesenteric cystadenocarcinomas originating in the ovary,oviduct and intestinal mucosa,but these were ruled out in our *** this report,we discuss a case of the malignant transformation of a cyst into adenocarcinoma,which to our knowledge has never been previously reported in a male patient.

It is important to establish techniques to manufacture the parts for aircraft structures at a lower cost in the current aircraft industry. To reduce tool cost, it is necessary to improve the prediction accuracy for aircraft part dimensions, which many aircraft manufacturers have attempted to do by introducing simulation technologies. Aircraft structures are primarily made of aluminum alloys, which have a greater springback tendency than steel. Therefore, it is necessary to develop simulation technologies to predict springback. In this research, two examples of applying simulation technologies to the main plastic forming are described. One ishydro-pressure forming. Hydro-pressure forming can reduce the number of dies by applying hydraulic pressure using a rubber pad. The other is stretch forming in which a desired shape is obtained by stretching a sheet over a tool surface. In this research, the results of a finite element method (FEM) were compared with those of experiments and theoretically calculated results to determine the accuracy of the FEM simulation. The FEM results were considerably similar to the theoretical value when appropriate simulation parameters were selected. Although the springback angles of the hydro-pressure forming simulation were in good agreement with the results of experiments, the curvature radius of the stretch forming after springback was smaller than the experimental values.

Case of gastric neuroendocrine carcinoma showing an interesting tumorigenic pathway

Here, we report a case of gastric neuroendocrine carcinoma showing an interesting tumorigenic pathway. A 57-year-old Japanese woman presented with epigastric tenderness, and distal gastrectomy was performed. In the surgical specimen, histologically, the tumor tissue was composed of three subtypes of tumor components showing different histological architecture and cellular atypia, diagnosed as neuroendocrine tumor(NET) G2, NET G3, and neuroendocrine carcinoma(NEC) components. Immunohistochemically, the Ki-67-positive rates of NET G2, NET G3, and NEC components were 6.5%, 99.5% and 88.1%, respectively. Although allelic imbalance(AI) on chromosomes 1p, 3p, 8q, TP53, 18q and 22 q was commonly found in all components, AI of 4p was found in NET G3 and NEC components(but not in the NET G2 component). In contrast, AIs of 5q and 9p were found in only the NEC component. Thus, we showed the progression from NET G2 to NEC, via NET G3, within the same tumor.

Histological Findings of Bone Marrow Lesions on Magnetic Resonance Images in Patients with Varus Knee Osteoarthritis

Bone marrow lesions (BMLs) on magnetic resonance (MR) images in knee osteoarthritis patients are considered to predict the severity and progression of the disease. We evaluated the histological findings of BMLs on MR images of the subchondral area of the medial femoral condyle in varus osteoarthritic knees. In 24 patients with varus knee osteoarthritis who underwent total knee arthroplasty (TKA), sagittal T1- and T2-weighted MR images of the affected knee were acquired before TKA. During TKA, resected bone pieces from the distal medial femoral condyle were obtained. Sagittal specimens obtained from the center of the bone pieces were histologically examined. Twenty patients had BMLs. Histological findings of BMLs in the subchondral area showed various features, such as fibrovascular tissue, cyst formation, active bone remodeling with bone formation and bone resorption, and hyaline cartilage. BMLs were not found in four patients;histological findings of these patients showed normal bone marrow tissue with normal-thickness trabeculae. Subchondral bony end plate in knees with BMLs was usually thin or destroyed, while that without BMLs was thick or normal. The condition of the subchondral bony end plate would explain the differences in the severity and progression between patients with or without BMLs.

Influence of Psychiatric Problems on Clinical Outcomes during the First 12 Months after Primary Total Knee Arthroplasty

Background: Total knee arthroplasty (TKA) is a useful treatment option for advanced knee osteoarthritis. Excellent clinical outcomes after TKA have been widely recognized, but the influence of psychiatric problems on them has not been focused on until quite recently. This study aimed to assess the influence of psychiatric problems on clinical outcomes after TKA in Japanese patients using two assessment scales developed in Japan because the Japanese cultural lifestyle is specifically characterized by bending to the floor and standing up. Methods: Clinical outcomes and psychiatric problems were evaluated using the Japanese Knee Osteoarthritis Measure (JKOM) and Brief Scale for Psychiatric Problems in Orthopaedic Patients (BS-POP), respectively. A total of 115 TKA patients were evaluated preoperatively and at 3, 6, and 12 months after TKA. The patients were classified into four groups (groups A-D) based on the BS-POP score. The JKOM scores were then compared between the two groups (groups A and D) with the worst and least psychiatric problems. The JKOM improvement rate between pre- and postoperative status in both groups A and D was also calculated. Results: The total JKOM score was significantly poorer in group A than in group D preoperatively and at 3, 6, and 12 months after TKA. The improvement rate showed no significant difference between groups A and D. Conclusion: Psychiatric problems influence both the poorer post- and preoperative clinical outcomes. However, a similar improvement rate in both groups A and D has indicated that TKA can be an effective treatment even for patients with psychiatric problems.
