1. Introduction Tokyo, one of the largest cities in the world, is the capital of Japan, a heavily earthquake-prone country. The Japanese population and much of its functionality are highly concentrated in Tokyo. The estimated damage to Tokyo as a result of future large earthquakes with a magnitude greater than the Japanese scale 7 is extremely high. The number of casualties could exceed 20 000 due to both the shaking itself and post-earthquake fires.

BACKGROUND Pancreaticoduodenectomy(PD)for advanced gastric cancer is rarely performed because of the high morbidity and mortality rates and low survival ***,neoadjuvant chemotherapy for advanced gastric cancer has improved,and chemotherapy combined with trastuzumab may have a preoperative tumorreducing effect,especially for human epidermal growth factor receptor 2(HER2)-positive *** SUMMARY We report a case of successful radical resection with PD after neoadjuvant S-1 plus oxaliplatin(SOX)and trastuzumab in a patient(66-year-old male)with advanced gastric cancer invading the pancreatic *** esophagogastroduodenoscopy detected a type 3 advanced lesion located on the lower part of the stomach obstructing the pyloric *** tomography detected lymph node metastasis and tumor invasion to the pancreatic head without distant *** findings revealed adenocarcinoma and HER2 positivity(immunohistochemical score of 3+).We performed staging laparoscopy and confirmed no liver metastasis,no dissemination,negative lavage cytological findings,and immobility of the distal side of the stomach due to invasion to the *** gastrojejunostomy was performed at that *** course of SOX and three courses of SOX plus trastuzumab were *** computed tomography showed partial response;therefore,PD was performed after neoadjuvant chemotherapy,and pathological radical resection was *** We suggest that radical resection with PD after neoadjuvant chemotherapy plus trastuzumab is an option for locally advanced HER2-positive gastric cancer invading the pancreatic head in the absence of non-curative factors.

High entropy state of orientationally-disordered clusters in Zr-based bulk metallic glass

A phase transformation of a metastable Zr2Ni intermetallic crystalline phase to vitrify was studied with molecular dynamics(MD)simulations based on a plastic crystal model(PCM),which utilizes the orientational disorder of molecules of plastic crystals in a class of soft *** simulation results showed that computational operations in MD-PCM for the random rotations of the clusters in the crystalline phase around their center of gravity and subsequent annealing lead to form a dense random packing structure from the metastable Zr2Ni *** randomly-rotated clusters provide high degrees of freedom in terms of atomic positions,which results in enhancing the glass-forming ability of the *** critical fraction of the number of rotated clusters for forming the glassy phase(fc~R)is evaluated to be ***,the critical fraction of the number of un-rotated clusters(fc~U=1-fc~R) is close to the critical concentration of site percolation for metallic *** mechanism of the metastable Zr2Ni phase to vitrify with increasing f~R was analyzed with a concept of communal entropy in the free volume theory with solid-and liquid-like cells proposed by Cohen and *** MD-PCM for the metastable Zr2Ni phase suggests that Zr-based bulk metallic glass(BMG)can be regarded as an alloy in a high rotation entropy state of clusters and that glass transition takes place by percolation of the nuclei of a liquid-like glassy phase.

BACKGROUND Gastric cancer(GC)incidence based on the endoscopic Kyoto classification of gastritis has not been systematically investigated using time-to-event *** To examine GC incidence in an endoscopic surveillance *** This study was retrospectively conducted at the Toyoshima Endoscopy *** who underwent two or more esophagogastroduodenoscopies were *** incidence was based on Kyoto classification scores,such as atrophy,intestinal metaplasia(IM),enlarged folds(EFs),nodularity,diffuse redness(DR),and total Kyoto *** ratios(HRs)adjusted for age and sex were calculated using a Cox hazard *** A total of 6718 patients were enrolled(median age 54.0 years;men 44.2%).During the follow-up period(max 5.02 years;median 2.56 years),GC developed in 34 *** average frequency of GCs per year was 0.19%.Kyoto atrophy scores 1[HR with score 0 as reference:3.66,95%confidence interval(CI):1.06 to 12.61],2(11.60,3.82-35.27),IM score 2(9.92,4.37-22.54),EF score 1(4.03,1.63-9.96),DR scores 1(6.22,2.65-14.56),and 2(10.01,3.73-26.86)were associated with GC incidence,whereas nodularity scores were *** total Kyoto scores of 4(HR with total Kyoto scores 0-1 as reference:6.23,95%CI:1.93 to 20.13,P=0.002)and 5-8(16.45,6.29-43.03,P<0.001)were more likely to develop GC,whereas the total Kyoto scores 2-3 were *** HR of the total Kyoto score for developing GC per 1 rank was 1.75(95%CI:1.46 to 2.09,P<0.001).CONCLUSION A high total Kyoto score(≥4)was associated with GC *** endoscopy-based diagnosis of gastritis can stratify GC risk.

The objective of this paper is to provide an analytical basis for the quantitative evaluation of damage to a reinforced concrete structure based on the vibration data obtained by using the damage detection technique. A partial reinforced concrete system of a weak beam/strong column moment frame is chosen as an example. A pushover analysis is carried out in order to numerically examine both the story shear-relative displacement characteristics and the associated damage level. In the analysis, a two dimensional nonlinear finite element analysis is employed considering several constitutive models. As a result, the degradation of the stiffness at the damaged story is characterized in association with the story relative displacement. It is also pointed out that the rotation angle of the column-base is highly correlated with the story relative displacement. Based on the analytical findings, quantitative approaches for a structural health monitoring system are suggested considering both the current sensor technologies and those available in the future. Novel Isolation and Identification Methods for Veillonella Species Involved in Halitosis

Purpose: The purpose of the present study was to develop a novel selective medium (OVSM) and useful identification method for oral Veillonella species, to assess the prevalence of this species using the selective medium, and to investigate whether the monitoring of oral Veillonella species levels is useful as a clinical indicator for the diagnosis of halitosis. Methods: To examine bacterial populations in the oral cavity, a novel selective medium (OVSM) and multiplex PCR method were developed for isolating and identifying oral Veillonella species. OVSM consists of tryptic soy agar, sheep blood, sodium lactate, vancomycin, and oxacillin. Results: Oral Veillonella species grew well on OVSM. Oral Veillonella species accounted for 5.4% and 27.2%, respectively, of all bacteria in the tongue biofilm samples of healthy and halitosis subjects. Oral Veillonella species produced larger amounts of volatile sulfur compounds than other representative oral bacteria. Conclusion: These results indicate that monitoring the levels of these organisms is useful as a clinical indicator for the diagnosis of halitosis.

In modern society, the population, wealth and social functions are increasingly concentrated in a few large cities. Such concentration brings about efficiency while at the same time it increases the vulnerability of the society. To address this issue, the Science Council of Japan（SCJ） published a proposal entitled ＂Shape Cities and Societies Safer against Severe Earthquakes＂. It makes a variety of suggestions for enhancing the seismic resilience of large cities, including：（1） comprehensive thinking based on the latest scientific knowledge and rich imagination;（2） selection of sites suitable for residence and social activities;（3） introducing the urban seismic coefficient;（4） promoting the enhancement of the seismic performance of buildings and civil structures;（5） easing the concentration of population and functions;（6） building communities that enable shelter and escape;（7） resilient technology of information and communication system and its effective utilization;（8） preparation and implementation of emergency response after earthquakes;（9） development and application of new structural seismic technology;（10） learning from domestic and foreign earthquake disasters and launching of international cooperation and knowledge sharing and（11） taking actions from a multidisciplinary perspective. The proposal is introduced in this paper.

This paper explores the conversion of raw seawater into drinking water from a religious water literacy (RWL) perspective. RWL is here defined as a socially just water literacy that is based on the respect for the dignity of life of all living beings on the Earth. It discusses the importance of sustainable water sources and proposes purification methods. That is, the study aims to show a brief outline of the drinking water source (inland waters;mainly river water) and their purification technologies in recent times and today and propose the purification method (Freezing-Thawing-UV method) in case of taking the seawater as the source. And, it emphasizes the need for dialogue and cooperation between countries in order to proceed. The paper also discusses the relationship among religion, culture, and civilization, and highlights the similarities between Marx’s historical materialism and Umesao’s ecological view. It further discusses the importance of water purification technology, separation methods, and the potential for seawater as an energy source. The paper concludes by discussing the importance of dialogue, communication and understanding environmental decision-making. In order to carry them forward, it is convinced that citizens in the world are nothing but people with self-awareness of “Bodhisattvas of the Earth” as identities. As a result, the new findings that suggest the possibility of raw seawater into drinking water from a religious water literacy perspective through the lens of Kawakatsu’s maritime view have been obtained.

The Effectiveness of a Portable Fluorescence Spectrophotometer for Early Detection of Oral Cancer

Purpose: To investigate whether photodynamic diagnosis (PDD) using a portable fluorescence spectrophotometer (FC-1) can easily and objectively discriminate between normal and tumor cells at the dental chairside, and to further compare it with PDD that requires speculum examination by focusing on protoporphyrin IX (PPIX). Methods: Three cell lines (2 human oral squamous cell carcinoma-derived cell lines, HSC-2 and HSC-3 cells, and oral keratinocytes, HOK cells) were cultured. 5-Aminolevulinic acid hydrochloride (5-ALA) and deferoxamine mesylate (DFO) were mixed in DMEM, and the mixture was set to Control (DMEM only) and PDD (5-ALA+DFO) groups. And then, a fluorescence was measured under room temperature (RT) and 37°C (Incubation) by using FC-1. In this study, the two conditions were combined with the Control and PDD groups to form the Control/RT, Control/Incubate, PDD/RT, and PDD/Incubate groups. Additionally, the amount of singlet oxygen (1O2) generated by irradiation with 405 nm LED was measured using electron spin resonance spectroscopy to detect PPIX in the cell supernatant after 24 hours. Results: In HSC-2 and HSC-3, the fluorescence intensity values increased significantly at 2 hours between the Control/RT and PDD/RT groups. In addition, there was a significant difference between HSC-2 and HSC-3 compared to HOK. In all cell lines, the fluorescence intensity values of the PDD/Incubate group were significantly higher than those of the PDD/Control group. The amount of 1O2 generated by 405 nm LED irradiation was higher in the cell supernatants of all cell lines in the order of Control/RT Conclusion: It is suggested that PDD using FC-1 can clearly distinguish between normal cells and tumor cells in vitro studies using cell lines at 2 hours und
