Total hip arthroplastys (THAs) for hemophilic spontaneous ankylotic hips are rare. We performed one-stage bilateral THA for hemophilic patient and I would like to report the results of the case. A 61-year-old male had been diagnosed with mild hemophilia A as a neonate. He had severe pain in both hips and moderate pain in both knees. His hips were ankylotic and had no mobility. Uncemented simultaneous bilateral THAs were performed under general anesthesia. This case suggests that one-stage THA can be a good option for hemophilic hips, provided that sufficient substitution therapy is used.

Today, latent heat storage technology has advanced to allow reuse of waste heat in the middle-temperature range. This paper describes an approach to develop a latent heat storage system using middle-temperature waste heat (~100oC - 200oC) from factories. Direct contact melting and solidification behavior between a heat-transfer fluid (oil) and a latent heat storage material mixture were observed. The mixture consisted of mannitol and erythritol (Cm = 70 mass %, Ce = 30 mass %) as a phase-change material (PCM). The weight of the PCM was 3.0 kg and the flow rate of the oil, foil, was 1.0, 1.5, or 2.0 kg/min. To decrease the solidified height of the PCM mixture during the solidification process, a perforated partition plate was installed in the PCM region in the heat storage vessel. PCM coated oil droplets were broken by the perforated partition plate, preventing the solidified height of the PCM from increasing. The solidification and melting processes were repeated using metal fiber. It was found that installing the metal fiber was more effective than installing the perforated partition plate to prevent the flow out problem of the PCM.

Cognitive impairment is a symptom of schizophrenia and strongly associated with impaired social function. In recent years, cognitive interventions (cognitive remediation therapies) have been implemented and their results have also been reported. Intervention studies have been conducted using a variety of methods that differ in terms of terminology, approach, and targeted cognitive domains. In this study, we examined trends in recent cognitive remediation research conducted subsequent to the meta-analyses of McGurk et al. (2007) and Wykes et al. (2011). We identified studies conducted between 2009 and 2013 to examine computer-assisted cognitive remediation and collated and analyzed the literature describing their major results and trends. Our results indicated improved cognitive performance subsequent to cognitive remediation therapy using computer programs. We found promising outcomes, particularly in reasoning and problem solving, verbal learning and memory, verbal working memory, and attention and vigilance, which replicated the findings of Wykes et al. (2011).

Background: In actual clinical settings, an occupational therapist (OT) encounters a patient with moderate Alzheimer’s disease (AD) walking with an anxious expression on the ward. The OT walks with the patient and encourages them to look at the symbol tree in the center of the ward. Additionally, when the patient talks to the OT, the OT empathizes with them and responds appropriately. As a result, many patients feel less anxious and become calmer. Therefore, in this study, the OT walked with an elderly female patient in her 80s with moderate AD and measured the patient’s autonomic nervous system responses. The study aimed to verify to what extent the results matched the OT’s clinical observational perspective. Methods: An OT with over 10 years of experience and a good relationship with the patient conducted one-on-one walking sessions with the patient. We divided the sessions into five-time periods: when the patient and OT were sitting on a sofa, and when they were walking. We implemented the same approach to the patient as usual. Therefore, we prompted the patient to look at the symbol tree during the walking session. Additionally, when the patient talked to the OT, the OT empathized and gave appropriate responses. We checked the changes in the patient’s autonomic nervous system responses in each time period. Results: This study showed that when walking with an OT, patient with moderate AD had a significant increase in parasympathetic nervous system (PNS) activity. Therefore, it is suggested that the involvement of professionals such as the environment and the OT during walking training for Alzheimer’s patients may lead to improved mental stability. Conclusion: This study suggests that walking together with an OT may have a positive effect on the PNS of female patient with AD. BACKGROUND Dedifferentiated chondrosarcoma(DDCS)accounts for 10%of all chondrosarcomas and has the poorest outcome,with a 5-year survival rate of 7%-25%.DDCS commonly occurs in the femur and pelvis,whereas DDCS of the finger is extremely ***,the histological findings of preexisting solitary enchondroma samples are important and valuable for diagnosing malignant *** SUMMARY We report our experience with DDCS in the proximal phalanx of the left middle finger of an 87-year-old *** had undergone surgery for enchondroma,with curettage and artificial bone grafting,11 years ago,in the same *** years after the primary surgery,the left middle finger gradually started to enlarge,and the growth speed increased in the past *** radiographs showed an expansive osteolytic lesion with calcifications and residual grafting *** to the suspicion of malignancy,we performed ray *** findings revealed an abrupt transition between the low-grade chondrosarcoma and dedifferentiated sarcoma *** dedifferentiated components showed the features of a high-grade undifferentiated pleomorphic *** patient was diagnosed with DDCS arising from a preexisting *** had no local recurrence or distant metastasis and died of pneumonia 6 years and 10 months after the second *** The histological findings of a precursor lesion showed a typical enchondroma,suggesting that DDCS can arise from enchondroma.

We report a case of lipoma in the antum of the stomach which prolapsed into the duodenal bulb and caused a duodenal ulcer, speculated to have been induced by the friction of its top against the duodenal mucosa. Although the ulcer healed after the administration of a proton pump inhibitor, the symptom of epigastric discomfort continued, suggested to be due to prolapsing. Therefore, a laparoscopic operation was conducted. The incidence of lipoma of the stomach is rare, and cases of its prolapse into the duodenum are very few. Furthermore, it is extremely rare for it to cause a duodenal ulcer. Because these features made this case clinically interesting, we report it here.

Introduction: The aim of this study was to observe an inhibition of bone resorption and osteoclastogenesis by iguratimod (IGU, T-614), a disease-modifying anti-rheumatic drug, using adjuvant-induced arthritis (AIA) rats and receptor activator of nuclear factor kappa-B ligand (RANKL)-stimulated RAW264.7 cells. Methods: The bone mineral density and 3D morphometric parameters of hind paws in AIA rats were measured using micro computed tomography (μCT) imaging. The activity of osteoclast cells was estimated based on tartrate-resistant acid phosphatase (TRAP) staining in specimens from the rats. In vitro TRAP activity was investigated using RANKL-stimulated RAW264.7 cells. The amount of nuclear factor of activated T-cells, cytoplasmic, calcineurin-dependent 1 (NFATc1) protein was measured by western blot analysis. The expression of Nfatc1, its regulator genes, its upstream factors, and osteoclast-functional genes were investigated. Results: In addition to the suppression of bone resorption and lesions of bone trabeculae of AIA rats, IGU significantly decreased the number of TRAP-positive cells in the calcaneal bones. Moreover, this drug inhibited the differentiation of RANKL-stimulated RAW264.7 cells into osteoclasts, which were identified morphologically and functionally. IGU decreased the amount of NFATc1 protein and improved the altered expression of NFATc1-associated genes and osteoclast-functional genes. Conclusions: IGU suppressed osteoclastogenesis and bone resorption via the RANKL-NFATc1 pathway, suggesting such effect would be expected in clinical use.

Is There Any Relationship between Total Hip Arthroplasty and Urinary Incontinent?

Introduction: In my daily practice as a hip surgeon, I have observed some circumstances where urinary incontinence (UI) improves after total hip arthroplasty (THA). We investigated UI symptoms before and after THA at our facility and considered the factors that influence UI. Patients and Method: The subjects were 113 female patients who underwent primary THA in our facility. An anterior lateral approach was used in all cases. Using the International Consultation on Incontinence Questionnaire-Short Form (ICIQ-SF), we conducted an investigation into the presence of UI before and 2 weeks after THA. An improvement in UI was defined as a decrease of 1 point or more, a worsening is defined as an increase of 1 point or more. Results: The responses from the 113 subjects were analyzed. Of the 113 patients, prior to THA, UI was prevalent among 59 patients and was absent among 54 patients. In the group where UI was prevalent, it improved after THA in 50 patients (85%), remained unchanged in 5 (8%) and worsened in 4 (7%). In the group where UI was absent, 49 patients (91%) remained unchanged and UI appeared in 5 (9%). Compared with the non-prevalence group (62 patients), the prevalence group (50 patients) had a noticeable improvement rate of internal rotation of the surgical hip side (P Conclusion: UI greatly reduces the quality of life (QOL). In this study, there is a possibility that THA improves UI.
