Evaluation of a Barrier Repair Cream Containing Pseudo-Ceramide for Practical Use by Hairdressers with Hand Skin Disorders Due to Daily Exposure to Chemical Irritants

Hairdressers are exposed to numerous skin-damaging factors, such as wet work, skin irritants, and thermal changes. Skin protection is a very important factor for the prevention of occupational skin disorders. Therefore, we evaluated a hand cream that was specially formulated for highly stressed skin with regard to its capability to reduce dry, rough, and scaly skin. The objective of this study was to conduct a controlled use test of a barrier repair cream containing pseudo-ceramide and demonstrate its efficacy for hairdressers with dry, sensitive hands and/or occupational skin disorders. A total of 30 hairdressers (29 females and 1 male, aged 20 - 67) participated in the study. All subjects suffered from dry hands. Subjects applied the test cream on their hands for 28 days (four weeks). For each subject, the skin surface topography of the hands was measured by the Surface Evaluation of Living Skin (SELS) system at day 14 (week 2) and day 28 (week 4). Dryness, scaling, cracking, redness, and itchiness were evaluated and graded by experts at week 2 and week 4, and then compared to baselines. All 30 subjects completed the study. A comparison of the changes from baseline to endpoint revealed a statistically significant reduction in skin roughness, scaling, and volume measured according to SELS. These data were confirmed by photographic documentation. The results were further supported by the expert evaluation. For the symptoms dryness and scaling, improvement could be observed in almost all regions of the hands. In addition, according to subjective assessments of overall hand skin condition and the hand cream, subjects reported clear improvement over time (p value below 0.001) and good efficacy of the product. In this study, the test hand cream showed excellent skin compatibility. Therefore, the hand cream may be effective for hairdressers with dry, sensitive hands and/or occupational skin disorders.

AIM: To examine the long term survival of geriatric patients treated with percutaneous endoscopic gastrostomy (PEG) in Japan. METHODS: We retrospectively included 46 Japanese community and tertiary hospitals to investigate 931 consecutive geriatric patients (≥ 65 years old) with swallowing difficulty and newly performed PEG between Jan 1st 2005 and Dec 31st 2008. We set death as an outcome and explored the associations among patient’s characteristics at PEG using log-rank tests and Cox proportional hazard models. RESULTS: Nine hundred and thirty one patients were followed up for a median of 468 d. A total of 502 deaths were observed (mortality 53%). However, 99%, 95%, 88%, 75% and 66% of 931 patients survived more than 7, 30, 60 d, a half year and one year, respectively. In addition, 50% and 25% of the patients survived 753 and 1647 d, respectively. Eight deaths were considered as PEG-related, and were associated with lower serum albumin levels (P = 0.002). On the other hand, among 28 surviving patients (6.5%), PEG was removed. In a multivariate hazard model, older age [hazard ratio (HR), 1.02; 95% confidence interval (CI), 1.00-1.03; P = 0.009], higher C-reactive protein (HR, 1.04; 95% CI: 1.01-1.07; P = 0.005), and higher blood urea nitrogen (HR, 1.01; 95% CI: 1.00-1.02; P = 0.003) were significant poor prognostic factors, whereas higher albumin (HR, 0.67; 95% CI: 0.52-0.85; P = 0.001), female gender (HR, 0.60; 95% CI: 0.48-0.75; P < 0.001) and no previous history of ischemic heart disease (HR, 0.69; 95% CI: 0.54-0.88, P = 0.003) were markedly better prognostic factors. CONCLUSION: These results suggest that more than half of geriatric patients with PEG may survive longer than 2 years. The analysis elucidated prognostic factors.

Improved Hemolytic Performance of Blood Pump with Fluorine-Doped Hydrogenated Amorphous Carbon Coating

Fluorine-doped hydrogenated amorphous carbon (a-C:H:F) film was deposited on a flow-straightener, impeller and diffuser surface (SUS 304) of an enclosed-impeller type flow blood pump using the ionization deposition method with a source gas of C6F5H. The surface characteristics of the a-C:H:F film were examined using atomic force microscopy, X-ray photoelectron spectroscopy, and measurements of surface roughness, friction and surface potential. The a-C:H:F film tends to increase surface roughness and the negative surface charge. In addition, the surface energy and friction decrease with fluorine dopant in the a-C:H film. To estimate the hemolytic performance of a blood pump with the a-C:H:F film coating, the amount of hemolysis was measured using a mock circulatory system (in vitro test) with 500 mL of pig blood containing sodium citrate. In vitro test was conducted for 180 min with the blood flow and pump head maintained at 5 L/min and 100 mmHg, respectively. The a-C:H:F film coating reduced the amount of hemolysis and improved the hemolytic performance. Decreasing the surface energy and negative surface charge of the a-C:H:F film contributes to the improvement of the hemolytic performance. The a-C:H:F film coating is thus expected to be utilized in medical technology as a surface coating technology for artificial heart blood pumps.

本文介绍了在载重卡车的增压柴油机上既要获得良好的燃油经济性,又要具有极好的驱动能力的一些技术解决方法和研制过程。为获得良好的燃油经济性,首先要考虑怎样减少发动机本身的摩擦损失,同时又要保持所要求的马力和扭矩特性。高增压的小型发动机具有这种可能性,但它又有一些严重的缺点。最近研制了一台新型的较高增压、中冷柴油机。在这台发动机上采用了许多新技术,如发动机工作客积的最佳化;可变控制的惯性进气系统,带有超后弯叶轮增压器;电子控制的燃油喷射定时器(电子定时控制)以及闸门式排气制动系统等等。新近研制的卡车安装了这种发动机,该发动机的燃油经济性比原来发动机提高20～30％。为获得柴油机良好的燃油经济性,新研制的较高增压小型涡轮增压发动机带有一个空气——空气式中冷器和一个电子定时控制的燃油喷射装置。它主要是通过减少自身摩擦损失来获得良好的燃油经济性。这种发动机的燃烧系统是以日野微混合系统(HMMS)为基础的,这种系统的燃烧周期非常短,这是由于气缸内空气的高紊流能量促进了燃油微粒与空气的混合。同时,在这台发动机上还采用了可变控制的惯性进气系统,带有超后弯叶轮的涡轮增压器以及闸门式排气制动系统等等。本论文阐述了这个基本设计方案及其细节。

Easy detecting signal of cognitive decline in healthy community-dwelling elderly people

The relationship between changes of elderly people’s participation in social activities and their cognitive functions was examined. Healthy, middle and upper-middle aged people (n = 407) responded to the Nagoya University Cognitive Assessment Battery and to a questionnaire inquiring about their participation in social activities. The results suggested that those whose participation in social activities increased in the last 6 months showed better verbal functioning than those whose participation was unchanged or decreased. Moreover, those whose social participation decreased showed inferior memory function compared to those whose social participation was unchanged, or increased. Furthermore, those whose social participation increased had better information processing speed than those whose social participation decreased. These results suggest that changes in social activities might act as an important signal that is indicative of the cognitive decline in elderly people, which would be useful to local health care managers in the elderly people’s community.

An appendiceal neurofibroma(ANF) is a rare neoplasm associated with neurofibromatosis type 1(NF-1),an inheritable neurocutaneous disorder that involves multiple systems including the intraabdominal *** diverticulitis occasionally ruptures in the absence of intense abdominal pain,which can lead to serious *** reports highlight the association between appendiceal diverticulum and appendiceal neoplasms; however,there is still little information on the association between appendiceal diverticulitis and ANF in NF-1.A 51-year-old Japanese male with NF-1 was referred to the division of surgery for mild right lower quadrant *** was suspected he had perforated acute appendicitis with periappendiceal abscess based on clinical manifestations and findings of computed *** emergency appendectomy was *** pathological examination revealed diffusely proliferated tumor cells of a neurofibroma,coexistent with multiple appendiceal diverticulums,leading to the diagnosis ofperforated appendiceal diverticulitis associated with *** he developed a remnant abscess,he recovered with the conservative treatments of antibiotics and *** case suggests that appendiceal diverticulitis might be a complication of appendiceal involvement of NF-1,and that it occasionally ruptures in the absence of intense abdominal *** should recognize that NF-1 can cause various abdominal manifestations.

Chronological changes in the liver after temporary partial portal venous occlusion

AIM:To investigate time-dependent changes caused by temporal portal vein obstruction and subsequent reperfusion in the lobe with or without an occluded portal ***:The portal vein(PV)of the anterior lobe of the liver of a male Wistar rat(8 wk-old)was obstructed(70%)for 12,24,36 and 48 h,respectively,and models were sacrificed at 48 h after reperfusion(each group:n=10).The histological changes and the status of liver regeneration were compared between a liver biopsy performed on each lobe after temporary obstruction of the portal vein in the same rat liver,and the liver extracted at the time of sacrifice(48 h after reperfusion).RESULTS:With regard to the obstructed lobe,the liver weight/body weight ratio significantly decreased according to obstruction *** the other hand,in thenon-obstructed lobe,there were no significant differences within each *** duration of PV occlusion did not seem to be strong enough to introduce liver weight *** of liver regeneration was brought about in the non-occluded lobe by 12-h occlusion,and was sustained even at 48 h after *** obstructed lobe atrophied with the passage of time in the obstructed ***,the proliferating-cell nuclear antigen labeling index also increased at 48 h after reperfusion,and a repair mechanism was ***:Temporary blood flow obstruction of the portal vein may become a significant trigger for liver regeneration,even with an obstruction of 12 h.

Pancreaticoduodenal artery aneurysm associated with coeliac artery occlusion from an aortic intramural hematoma

Pancreaticoduodenal artery aneurysms are a rare type of visceral artery aneurysm, whose rupture is associated with high mortality. These aneurysms are of particular interest because local haemodynamic change caused by coeliac artery obstruction plays an important role in their development. However, the pathophysiological mechanism of coeliac artery obstruction is not completely understood. Pressure from the median arcuate ligament is most frequently reported cause. Although it is well-known that stenosis or occlusion of the visceral vessels may be caused by aortic syndrome, reports of pancreaticoduodenal artery aneurysm associated with coeliac artery occlusion due to aortic syndrome are extremely rare. Our case indicates a new aetiology for a pancreaticoduodenal artery aneurysm and demonstrates the rapid deterioration of the patient affected.
