Infants with congenital deafness caused by severe bilateral inner ear malformations frequently suffer from severe hearing loss and poor balance. Unfortunately, the use of hearing aids is usually ineffective in recovering hearing, necessitating cochlear implants. We report a case of a 6-year-old boy with congenital deafness and bilateral inner ear malformations(right side, incomplete partition type I [IP-I]; left side, common cavity deformity). Hearing aids had a remarkable effect in this patient, enabling sufficient and favorable hearing recovery such as to allow the patient to engage in daily conversations. Per-rotatory nystagmus was recorded on an electronystagmogram for both right and left rotations in a damped rotational chair test. It is rare for deaf children with severe bilateral inner ear malformation to demonstrate favorable development in hearing and good equilibrium function. Our findings suggest that auditoryevestibular hair cells in this patient may have been partially preserved despite IP-I in the right ear and common cavity deformity of the left ear.

Utilizing lightweight Al alloys in various industrial applications requires achieving precise pressure tightness and leak requirements.Vacuum pressure impregnation(VPI)with thermosetting polymers is commonly used to address leakage defects in die-cast Al alloys.In this study,the efficacy of the VPI technique in sealing alloy parts was investigated using a combination of nondestructive micro X-ray computed tomography(micro XCT)and a standard leak test.The results demonstrate that the commonly used water leak test is insufficient for determining the sealing performance.Instead,micro XCT shows distinct advantages by enabling more comprehensive analysis.It reveals the presence of a low atomic number impregnates sealant within casting defects,which has low grey contrast and allows for visualizing primary leakage paths in 3D.The effective atomic number of impregnated resin is 6.75 and that of Al alloy is 13.69 by dual-energy X-ray CT.This research findings will contribute to enhancing the standard VPI process parameters and the properties of impregnating sealants to improve quality assurance for impregnation in industrial metals.

Multicentric recurrence of intraductal papillary neoplasm of bile duct after spontaneous detachment of primary tumor:A case report

BACKGROUND Intraductal papillary neoplasm of the bile duct(IPNB)rarely recurs in a multicentric manner.We encountered a patient with multiple recurrences of the gastric subtype of IPNB one year after spontaneous detachment of the primary tumor during peroral cholangioscopy(POCS).CASE SUMMARY A 68-year-old woman on maintenance hemodialysis because of lupus nephritis had several cardiovascular diseases and a pancreatic intraductal papillary mucinous neoplasm(IPMN).She was referred to our department for dilation of the common bile duct(CBD)and a tumor in the lumen,detected using ultrasonography.She had no complaints,and blood tests of hepatobiliary enzymes were normal.Magnetic resonance cholangiopancreatography(MRCP)showed a papillary tumor in the CBD with a filling defect detected using endoscopic retrograde cholangiography(ERC).Intraductal ultrasonography revealed a papillary tumor and stalk at the CBD.During POCS,the tumor spontaneously detached with its stalk into the CBD.Pathology showed low-intermediate nuclear atypia of the gastric subtype of IPNB.After 1 year,follow-up MRCP showed multiple tumors distributed from the left hepatic duct to the CBD.ERC and POCS showed multicentric tumors.She was alive without hepatobiliary symptoms at least two years after initial diagnosis of IPNB.CONCLUSION The patient experienced gastric subtype of IPNB without curative resection.Observation may be reasonable for patients with this subtype.

During the coronavirus disease 2019(COVID-19)pandemic,the Japanese government implemented several infection control measures,such as states of emergency(SoE)and school closures,and encouraged universal masking and hand hygiene.Initial activity restrictions and improved hygiene were found to be associated with declines in the incidence of infuenza during the 2019–2020 winter season[1].Prior studies have found that the incidence of seven pediatric infections,such as pharyngoconjunctivitis and varicella,decreased during and after the school closures in Japan[2].As activity restrictions continue to be lifted,there is growing concern that pediatric infections may suddenly return given that the pool of susceptible children increased during the pandemic[3].In this study,we use national infectious disease sentinel data from January 2012 to October 2021 to assess changes in the reported number of pediatric infections in Japan during the COVID-19 pandemic.We hypothesize that the reporting of many infectious diseases decreased during the early stages of the pandemic but ultimately returned to normal when activity restrictions were lifted.

An iliopsoas hematoma can occur either spontaneously or secondary to trauma or bleeding tendency due to hemophilia and anticoagulant therapy.Although liver cirrhosis is commonly associated with coagulopathy, iliopsoas hematoma is very rare.We herein,present a case of bilateral iliopsoas hematoma in a patient with alcoholic cirrhosis,and review the literature on muscle hematoma associated with cirrhosis.A 56-year-old man with alcoholic cirrhosis was admitted in a state of shock with anemia.The cause of anemia could not be detected,and the patient was treated conservatively. The site of bleeding was not detected with either gastroduodenal endoscopy or upper abdominal computed tomography,the latter of which did not include the iliopsoas muscle.He died on the 10th day of admission and bilateral iliopsoas hematomas were found on autopsy.An iron stain was positive in the iliopsoas muscle.Eight cases of muscle hematoma associated with cirrhosis,including the present case, were found in a review of the literature.Four of these cases involved the rectus abdominis muscle,3 involved the iliopsoas muscle and 1 involved combined muscles.Alcoholic cirrhosis accounted for 75%of the cases.One case(12.5%)was associated with virus- related cirrhosis,and another with combined virus- and alcohol-related cirrhosis.The mortality rate was 75%despite early diagnosis and low risk scores for cirrhosis.Muscle hematoma in patients with cirrhosis isclosely related to alcoholism,and the mortality rate of the condition is extremely high.In conclusion,muscle hematoma should be recognized as an important complication of cirrhosis.

High-density linkage maps are essential tools for genome analysis of various biological traits. Our developed compact multi-gel system, HEGS （high efficiency genome scanning） is a high-throughput and high-cost-performance electrophoresis apparatus. Using this system, a high-density （average interval 2.3 cM） map with 1 065 AFLP and 63 SSR markers was constructed from recombinant inbred lines of a japonica and indica hybrid in just two months of electrophoreses by a single person. More than 50% of the mapped AFLP markers were commonly polymorphic for several combinations between japonica and indica rice and 15% were applicable for genetically closer crosses between upland and lowland types of japonica rice. This system can be used for rapid analyses of all kinds of markers.

Background Our previous studies have shown that electrical stimulation from the skin surface to the tendon region(Tendon Electrical Stimulation:TES)can elicit a force sensation,and adjusting the current parameters can control the amount of the sensation.TES is thought to present a proprioceptive force sensation by stimulating receptors or sensory nerves responsible for recognizing the magnitude of the muscle contraction existing inside the tendon,so it can be a proprioceptive module of a small-size,low-cost force feedback device.But there is also suspect that TES presents only strong,noisy skin sensation.From previous study,it was found that TES has some limitation on varying sensations.Methods In this study,in addition to characterizing the proprioceptive sensation induced by TES,we constructed a multimodal presentation system reproducing a situation in which force is applied to the hand was offered,so as to investigate whether TES contributed to the reproduction of haptics cooperating with other modalities,rather than disturbing them.Specifically,we used vibration to present a cutaneous sensation and a visual head mounted display(HMD)system to present simultaneous images.Using this system,we also evaluated the efficacy of TES itself and that of the multimodal system involving TES.Results We found that TES,along with visual and vibration stimulation,contributed to the perception of a certain force.Conclusions Thus,TES appears to be an effective component of multimodal force sense presentation systems.

In a severe accident of a nuclear power reactor,coolant channel blockage by solidified molten core debris may significantly influence the core degradations that follow.The moving particle semi-implicit(MPS)method is one of the Lagrangian-based particle methods for analyzing incompressible flows.In the study described in this paper,a novel solidification model for analyzing melt flowing channel blockage with the MPS method has been developed,which is suitable to attain a sufficient numerical accuracy with a reasonable calculation cost.The prompt velocity diffusion by viscosity is prioritized over the prompt velocity correction by the pressure term(for assuring incompressibility)within each time step over the“mushy zone”(between the solidus and liquidus temperature)for accurate modeling of solidification before fixing the coordinates of the completely solidified particles.To sustain the numerical accuracy and stability,the corrective matrix and particle shifting techniques have been applied to correct the discretization errors from irregular particle arrangements and to recover the regular particle arrangements,respectively.To validate the newly developed algorithm,2-D benchmark analyses are conducted for steady-state freezing of the water in a laminar flow between two parallel plates.Furthermore,3-D channel blockage analyses of a boiling water reactor(BWR)fuel support piece have been performed.The results show that a partial channel blockage develops from the vicinity of the speed limiter,which does not fully develop into a complete channel blockage,but still diverts the incoming melt flow that follows to the orifice region.

Large lingual heterotopic gastrointestinal cyst in a newborn:A case report

BACKGROUND Heterotopic gastrointestinal cysts have gastrointestinal epithelium in the cyst wall and rarely occur in the oral cavity.Most are found in the neonatal period.However,heterotopic gastrointestinal cysts that are diagnosed as a congenital tongue cyst by routine ultrasonography are extremely rare.CASE SUMMARY A 12-day-old female presented with swelling of the anterior tongue.The obstetrician had detected significant tongue swelling on fetal ultrasonography in the 35 th gestational week.The female was born by cesarean delivery at gestational week 39.She soon became dyspneic,and the cyst was aspirated.After the aspiration,her breathing recovered and she started breastfeeding.The cyst was excised under general anesthesia on the 67 th day.Histopathologic examination showed that that cyst wall consisted of a lining of columnar gastrointestinal-type epithelium and pseudostratified ciliated epithelium.The patient restarted breastfeeding 3 h after surgery.The postoperative course was uneventful.CONCLUSION Airway distress and feeding difficulty were successfully avoided by cyst aspiration,and surgical resection was performed with no perioperative complications.

Orthodontic tooth separation activates the hypothalamic area in the human brain

OBJECTIVES： An animal experiment clarified that insertion of an orthodontic apparatus activated the trigeminal neurons of the medulla oblongata. Orthodontic tooth movement is known to be associated with the sympathetic nervous system and controlled by the nucleus of the hypothalamus. However, the transmission of both has not been demonstrated in humans. The purpose of this study were to examine the activated cerebral areas using brain functional magnetic resonance imaging（MRI）, when orthodontic tooth separators were inserted, and to confirm the possibility of the transmission route from the medulla oblongata to the hypothalamus.METHODS： Two types of alternative orthodontic tooth separators（brass contact gauge and floss） were inserted into the right upper premolars of 10 healthy volunteers. Brain functional T2＊-weighted images and anatomical T1-weighted images were taken.RESULTS： The blood oxygenation level dependent（BOLD） signals following insertion of a brass contact gauge and floss significantly increased in the somatosensory association cortex and hypothalamic area.CONCLUSION： Our findings suggest the possibility of a transmission route from the medulla oblongata to the hypothalamus.
