Pericarditis in Cardiology at a Regional Hospital

Introduction: Pericarditis is an inflammation of the pericardium with or without pericardial fluid effusion. Its prevalence is difficult to determine given the many forms that are not symptomatic. In Africa, its prevalence was 6.3% in Gabon in 2020 and 7.2% in Mali in 2022. In Europe, an italian study estimates the incidence of acute pericarditis at 27.7 cases per 100,000 people per year. In another study conducted in Finland over a period of 9 years, the incidence of pericarditis requiring hospitalisation was 3.32 cases per 100,000 people per year. The aim of our study was to describe the clinical and paraclinical characteristics of pericarditis observed in the cardiology department of the regional hospital in Mali. Methodology: This was a single centre cross-sectional study from 30 January 2018 to 30 June 2020 in the cardiology department of the ségou regional hospital. All consenting patients, regardless of age or sex hospitalised in the department for pericarditis confirmed on cardiac ultrasound were included. Data were collected using an individual patient follow-up form recording sociodemographic, clinical, biological, electrocardiographic and echocardiographic data, as well as the course of the disease. Results: Out of 879 patients hospitalized, the hospital frequency was 7.28%. Females predominated, with a sex ratio of 0.42. More than half the patients were aged 45 or younger (59.4%). The mean age of patients was 41.8 ± 18.1 years. Cardiovascular risk factors were dominated by hypertension and smoking (46.9% and 12.5% respectively). The reasons for consultation were dyspnoea (84.3%), chest pain (54.7%), cough (71.9%) and fever (34.4%). Physical signs included muffled heart sounds (76.6%), tachycardia (70.3%), pericardial friction (17.2%) and signs of peripheral stasis in 53.1% of cases. We observed elevated C-reactive protein (CRP) in 57.8% of cases, hypercreatininaemia in 37.5% and positive HIV serology in 3.1%. The major radiographic signs were cardiomegal

In this study, toxic equivalent factors and sediment quality guidelines were employed for the assessment of toxicity potential of PAHs in 2 sediment cores collected from the center (RC) and shoreline (Rs) of Reforme lake, sE Nigeria over the last ~1 century. Boundary cross plots of isomeric ratios such as Ant/Ant + Phe (>0.1) vs. Fl/Fl + Pyr (>0.4) and BaA/BaA + Chry (>0.35) vs. Icdp/Icdp + Bper (>0.2) indicated biomass combustion/domestic coal utilization as the main sources of PAHs input. The result revealed toxicity unit indices (TU1, 1.22 - 1.57) for the two cores were above the thresholds of no effect. The highest TU1 value found in the near-bottom layer (Rs5, 20 - 25 cm) of the Rs corresponding to geological time-frame ~1930-1947 coincided with the period of inhabitation by the European settlers along the lake’s catchments when coal or coal products utilization for domestic and recreational purposes was at its peak. On the other hand, TU2 values were s, revealing unlikely PAH effects to resident organisms. Total toxicity equivalency factors (TEqFs) for the surface sediment of Rs and RC were 9.29 ng/g TEqFs and 9.16 ng/g TEqFs, respectively, and indicated that more attention should be paid to BaPyr accumulation in the lake’s sediment.

Ibeno, one of the major oil producing communities in south-south Nigeria is a coastal commercial fishery hub that houses Axon-Mobil operational base and pharmaceutical/plastic industries. shrimp species (Macrobrachium vollenhovenii) is a major aquatic food frequently consumed by inhabitants of the coastal area and those living hinterland, thus, being a major route of human exposure to 2-methylnaphthalene, phenanthrene and didodecyl phthalate in the area. The purpose of the study was to evaluate factors that determine bioaccumulation and assess the potential cancer and non-cancer risk posed by these contaminants through human (adult and children) exposure via shrimp’s consumption using gas chromatography-mass spectroscopy. The associated sediment showed higher mean concentrations of 2-methylnaphthalene, phenanthrene and didodecylphthalate at the two sites relative to those in fresh shrimp samples and factors such as size, lipid content, physicochemical property and environmental condition influenced the uptake of these contaminants. Besides water loss, the traditional drying process enhanced the levels of phenanthrene and didodecylphthalate in dry shrimps most likely due to combustion process and relatively low volatility, respectively and lowered the level of 2-methylnapthalene linked to its relatively high volatility. The potential of cancer and non-cancer development in human were highest via dry large shrimp consumption and followed the sequence: dry small shrimp > fresh large shrimp > fresh small shrimp and were within the UsEPA reference standards. Although children were more vulnerable, the exposed individuals may not exhibit notable health-associated adverse effects in the near future. Thus, adequate advocacy is needed to sensitize those living in the catchments who often prefer dry shrimp in their meals on the adverse health implications of these contaminants for their survival and the need to maintain the health of the ecosystem.

On the Large Helical Device （LHD） where nested magnetic surfaces are surrounded by the ergodic field layer, edge transport barrier （ETB） was produced in neutral-beam-injection （NBI） heated plasmas through transition and non-transition processes. The former case is the ETB formation by L-Htransition, where characteristics of L-H transition observed in a tokamak plasma are clearly recognized. The confinement improvement is the modest （- 10%）, compared with the Iss95 international stellarator scaling. The threshold power for the transition is comparable or slightly lower than the ITER scaling law established by tokamaks and compact tori. The ETB is formed inside the ergodic field layer of the vacuum field. The ETB formation destabilizes edge coherent modes such as m/n = 1/1, 2/3 and 1/2, of which rational surfaces are in the magnetic hill. The formed ETB is partially and transiently destroyed by these coherent edge MHD modes and edge localized modes （ELMs） typically observed in Ha signals. The latter ETB is observed in a plasma with large reversed NBI-driven current more than 100 kA at Bt = 1 T. In these plasmas, the edge magnetic shear is enhanced by the current and the rotational transform in the core region is expected to be appreciably reduced. Thus reduced rotational transform in the plasma central region will enhance outward heat and particle fluxes toward ergodic edge layer. The ETB with steep electron temperature gradient up to - 5 keV/m is formed by blocking enhanced outward heat flux.

Considering the variation in metabolic evaluation and medical management of kidney stone disease,this consensus review was created to discuss the metabolic activity of nephrolithiasis,define the difference between single and recurrent stone formers,and develop a schema for metabolic and radiologic follow-up.A systematic review of the literature was performed to identify studies of metabolic evaluation and follow-up of patients with *** single and recurrent stone formers share many similarities in metabolic *** study group determined that based on an assessment of risk for stone recurrence and metabolic activity,single and recurrent stone formers should be evaluated comprehensively,including two 24 h urine studies on a random *** medication and dietary recommendations are effective for many patients in reducing the risk of stone ***-up of those with stone disease should be obtained depending on the level of metabolic activity of the patient,the risk of chronic kidney disease and the risk of osteoporosis/osteopenia.A standard scheme includes a baseline metabolic profile,a repeat study 3e6 months after initiation of treatment,and then yearly when stable,with abdominal imaging obtained every 1-2 years.

The relationship between Nigerian Bonny Light crude oil (NBLCO) induced hypoglycaemia and endogenous serum insulin concentration;the role of antioxidant vitamin C or E supplementation was the focus of this study. Forty adult male Wistar rats were randomly divided into group I, which served as the control, group II, which was oral gavaged 6 ml/kg of NBLCO, groups III and IV, which were in addition to 6 ml/kg of NBLCO supplemented with 9 ml/kg and 1 mg/kg of vitamin E or C, respectively for 28 days. Results showed that NBLCO significantly (p se effects exerted by NBLCO were however significantly (p sed by vitamin E or C supplementation. The NBLCO significantly (p sting blood glucose (FBG) when compared with control, the antioxidant vitamins supplementation significantly (p sed the crude oil effect. The mean serum insulin level in NBLCO, vitamin E or C supplemented groups is not significantly different from the control. There was no significant correlation between FBG and fasting serum insulin concentrations in all the groups on day 28. It has been demonstrated in this study that direct oral ingestion of crude oil (NBLCO) could reduce food intake, body weight and cause hypoglycemia;the hypoglycemia may not be a function of serum insulin concentration. Interestingly, the hazardous effects of NBLCO could be ameliorated with antioxidant vitamin C or E supplementation.

Congenital abnormalities of the kidneys and urinary tract (CAKUT) represent all the morphological abnormalities associated with a disorder of the embryonic development of the kidneys and their excretory tract. Its prevalence is underestimated in developing countries compared to developed countries. This was a cross-sectional and descriptive study between January 1st, 2015 and February 28th, 2020, carried out in the pediatric department of Aristide Le Dantec Hospital. The prevalence of CAKUT was 8.77% for all pediatric nephrologic pathologies (n = 58), the median age at diagnosis was 77.28 months and the sex ratio was 1.6. The circumstances of the discovery of CAKUT were dominated by urinary tract infection (n = 21), the antenatal diagnosis was poor (n = 13) and confusion between cysts and calyx dilation in antenatal was noted. Ultrasound was the main postnatal imaging test requested. The most common renal abnormality was kidney cysts (n = 18) (cystic kidneys and multicystic dysplastic kidney) and the most common urological abnormality was the posterior urethral valves (n = 8). Kidney failure was a pejorative factor in the evolution of these CAKUT.
