Can mosapride citrate reduce the volume of lavage solution for colonoscopy preparation?

AIM:To evaluate the possibility of reducing the volume of polyethylene glycol(PEG)-electrolyte solution using adjunctive mosapride citrate for colonoscopy preparation. METHODS:This was a single-center,prospective, randomized,investigator-blinded,non-inferiority study involving 252 patients of both sexes,aged from 20 to 80 years,scheduled for screening or diagnostic colonoscopy in our department.A total of 126 patients was randomized to receive 1.5 L PEG-electrolyte solution plus 15 mg of mosapride(1.5 L group),and 126 received 2 L PEG-electrolyte solution plus 15 mg of mosapride(2 L group).Patients completed a questionnaire on the acceptability and tolerability of the bowel preparation *** efficacy of bowel preparation was assessed using a 5-point scale based on the Aronchick *** primary end point was adequate bowel preparation rates(score of excellent/good/fair) vs(poor/inadequate).Acceptability and tolerability,as well as disease detection,were secondary end points. Laparoscopic repair of hiatal hernia with mesenterioaxial volvulus of the stomach

Although mesenterioaxial gastric volvulus is an uncommon entity characterized by rotation at the transverse axis of the stomach, laparoscopic repair procedures have still been controversial. We reported a case of mesenterioaxial intrathoracic gastric volvulus, which was successfully treated with laparoscopic repair of the diaphragmatic hiatal defect using a polytetrafluoroethylene mesh associated with Toupet fundoplication. A 70-year-old Japanese woman was admitted to our hospital because of sudden onset of upper abdominal pain. An upper gastrointestinal series revealed an incarcerated intrathoracic mesenterioaxial volvulus of the distal portion of the stomach and the duodenum. The complete laparoscopic approach was used to repair the volvulus. The laparoscopic procedures involved the repair of the hiatal hernia using polytetrafluoroethylene mesh and Toupet fundoplication. This case highlights the feasibility and effectiveness of the laparoscopic procedure, and laparoscopic repair of the hiatal defect using a polytetrafluoroethylene mesh associated with Toupet fundoplication may be useful for preventing postoperative recurrence of hiatal her-nia, volvulus, and gastroesophageal reflux.

Clinical Outcomes of 67 Patients Treated with Chemoradiotherapy for Primary Thyroid Non-Hodgkin’s Lymphoma in Osaka Medical College

Background: The reports of clinical outcomes of patients treated with chemoradiotherapy for primary thyroid non-Hodgkin’s lymphoma are rare. We report our results of chemoradiotherapy for primary thyroid non-Hodgkin’s lymphoma. Materials and Methods: The subjects were 67 patients with thyroid non-Hodgkin’s lymphoma among 269 patients with malignant lymphoma who received radiotherapy in our hospital during a period between May 1990 and June 2005. The patients included 16 men and 51 women, with a mean age of 66.2 ± 10.7 years (30 - 84 years). The disease stage was stage I in 42 patients, stage II in 24, and unclear in 1. The histologic type was B-cell lymphoma in 66 patients, MALT in 9, diffuse type in 52, follicular type in 5, and diffuse and follicular type in 1. CHOP chemotherapy regimen for malignant lymphoma patients was as follows. Intravenous drip infusion of cyclophosphamide 750 mg/m2, (drip) infusion of doxorubicin 50 mg/m2, and intravenous injection of vincristine 1.4 mg/m2 were administered on day 1, followed by 5 consecutive days of oral prednisolone 100 mg/m2. This regimen was repeated every 3 weeks (21 days) in 6 to 8 courses. Modified CHOP chemotherapy regimen was as follows. Intravenous drip infusion of cyclophosphamide 600 mg/m2, intravenous (drip) infusion of doxorubicin 40 mg/m2, intravenous infusion of vindesine 3 mg/m2, and intravenous drip infusion of prednisolone 60 mg/body were administered on day 1, and intravenous prednisolone was changed to oral prednisolone with the dose tapered gradually. After completing one course of this regimen, two courses of radiotherapy (a total of 36 Gy) were performed, followed by 6 courses of the chemotherapy regimen at lower doses (80% of the initial doses) repeated once a month. Impact of Smoking on Lifestyle and Vitality in College Students

There is a long history of studies on the bad influence of smoking on our bodies, with many study results accumulated to date indicating a decline in the smoking rate. However, the smoking rate in Japan is still higher than that in other countries. In order to reveal the various influences due to smoking, we examined the changes in lifestyle and vitality prior to and the following smoking, targeting college students who are at the early age of starting to smoke. The subjects were 213 college students (83 men and 130 women). Smoking frequency as well as sleeping, eating habits, exercise, and vitality as a lifestyle were surveyed through a questionnaire survey. As a result of comparing smoking and lifestyle between the smoking group and non-smoking group, in terms of sleep, the bedtime and wake-up time tended to be late in the smoking group. Regarding eating habits, there was a tendency for the smoking group to have a high rate of skipping breakfast and a high intake rate of midnight snacks. Regarding exercise, the number of people exercising on a daily basis was small in both the smoking group and the non-smoking group, with no significant difference observed. Therefore, this study revealed that smoking has a negative influence on some aspects of lifestyle. There was no significant relationship in terms of the effects of smoking on vitality. Going forward, it will be necessary to clarify the negative effects of smoking on mental health.

The Impact of a Heterogeneous Surface on Spatiotemporal Uncertainties of Sensible Heat, Latent Heat, and CO<sub>2</sub>Flux Measured over the Secondary Forest

The turbulent fluxes, such as sensible and latent heat fluxes and CO2 flux, are globally observed over various terrestrial areas in order to understand the interaction between biosphere and atmosphere. Although the turbulent flux observations are generally performed on a horizontally homogeneous surface, the spatial distribution of the soil moisture is not homogeneous even on cultivated land with homogeneous vegetation, indicating that the development of each plant would be different and that the plant physiology, such as photosynthesis and growth, would be heterogeneous. In this study, to clarify the impact of a heterogeneous surface on spatiotemporal uncertainty of turbulent fluxes, a simultaneous flux observation experiment was conducted at different heights (20 m and 30 m) above the ground surface in a secondary seasonal tropical forest located in the Tak Province, Thailand. We defined ε as the spatial uncertainty of the turbulent flow flux, as proposed by Kim et al. (2011b) [1], and observed that ε of CO2 flux was high, whereas ε of sensible and latent heat fluxes were low. This is likely to be caused by spatial uncertainty such as a heterogeneous surface. The CO2 environment was heterogeneous;however, sensible and latent heat environments were homogeneous because the source area received insolation uniformly. Therefore, the analytical results for the CO2 flux presented a different pattern from those exhibited by the analytical results of the latent and sensible heat fluxes.

Distributed Hydrological Model for Assessing Flood Hazards in Laos

Many natural disasters have recently occurred in Laos. Among them, flooding has been the greatest problem. Land use change (deforestation and urbanization) and climate change have played significant roles, and it is important to understand the impacts of these changes on flooding. We have developed an integrated hazard map based on a combination of four hazard maps of flooding, land use change and climate change to assess hazard areas at the national scale. The hazard map was developed using the analytical hierarchy process (AHP) and a hazard index. Finally, we divided the map into four hazard area categories, which include low, medium, intermediate and high. Based on this analysis, the integrated hazard map of Laos indicates that low hazard areas cover 87.44% of the total area, medium hazard areas cover 8.12%, and intermediate and high hazard areas respectively cover 2.42% and 2% of the land area. We compared the results with historical events to confirm that the proposed methodology is valid.

High Density Plasma Heating by EC-Waves Injected from the High-Field Side for Mode Conversion to Electron Bernstein Waves in LHD

To realize an excitation of electron Bernstein waves （EBW） via mode conversion from X-mode waves injected from the high magnetic field side （HFS）, new inner-vessel mirrors were installed close to a helicM coil in the large helicM device （LHD）. 77 GHz electron cyclotron （EC） wave beams injected from an existing EC-wave injection system toward the new mirror are reflected on the mirror so that the beams are injected to plasmas from HFS. Evident increases in the electron temperature at the plasma core region and the plasma stored energy were observed by the HFS beam injection to the plasmas with the line-average electron density of 7.5~ 1019 m-3, which is slightly higher than the plasma cut-off density of 77 GHz EC-waves, 7.35~ 1019 m-3. The heating efficiency evaluated from the changes in the time derivative of the plasma stored energy reached ,,~70%. Although so far it is not clear which is the main cause of the heating effect, the mode-converted EBW or the X-mode wave itself injected from the HFS, an effective heating of high-density plasma over the plasma cut-off of EC-wave was successfully demonstrated.

Efficient Numerical Optimization Algorithm Based on New Real-Coded Genetic Algorithm, AREX + JGG, and Application to the Inverse Problem in Systems Biology

In Systems Biology, system identification, which infers regulatory network in genetic system and metabolic pathways using experimentally observed time-course data, is one of the hottest issues. The efficient numerical optimization algorithm to estimate more than 100 real-coded parameters should be developed for this purpose. New real-coded genetic algorithm (RCGA), the combination of AREX (adaptive real-coded ensemble crossover) with JGG (just generation gap), have applied to the inference of genetic interactions involving more than 100 parameters related to the interactions with using experimentally observed time-course data. Compared with conventional RCGA, the combination of UNDX (unimodal normal distribution crossover) with MGG (minimal generation gap), new algorithm has shown the superiority with improving early convergence in the first stage of search and suppressing evolutionary stagnation in the last stage of search.

Clinicopathological features of alpha-fetoprotein producing early gastric cancer with enteroblastic differentiation

AIM To investigate clinicopathological features of early stage gastric cancer with enteroblastic differentiation(GCED).METHODS We retrospectively investigated data on 6 cases of early stage GCED and 186 cases of early stage conventional gastric cancer(CGC: well or moderately differentiated adenocarcinoma) who underwent endoscopic submucosal dissection or endoscopic mucosal resection from September 2011 to February 2015 in our *** was defined as a tumor having a primitive intestine-like structure composed of cuboidal or columnar cells with clear cytoplasm and immunohistochemical positivity for either alpha-fetoprotein, Glypican 3 or SALL4. The following were compared between GCED and CGC: age, gender, location and size of tumor, macroscopic type, ulceration, depth of invasion, lymphatic and venous invasion, positive horizontal and vertical margin, curative resection *** Six cases(5 males, 1 female; mean age 75.7 years; 6 lesions) of early gastric cancer with a GCED component and 186 cases(139 males, 47 females; mean age 72.7 years; 209 lesions) of early stage CGC were investigated. Mean tumor diameters were similar but rates of submucosal invasion, lymphatic invasion, venous invasion, and non-curative resection were higher in GCED than CGC(66.6% vs 11.4%, 33.3% vs 2.3%, 66.6% vs 0.4%, 83.3% vs 11% respectively, P < 0.01). Deep submucosal invasion was not revealed endoscopically or by preoperative biopsy. Histologically, in GCED the superficial mucosal layer was covered with a CGC component. The GCED component tended to exist in the deeper part of the mucosa to the submucosa by lymphatic and/or venous invasion, without severe stromal reaction.
