Giant Omphalocele Ruptured: An Operated Case and Review of the Literature

Background: The omphalocele is a congenital defect of closure of the abdominal wall at the level of the umbilical ring with evisceration of the intra-abdominal organs in the form of a bag covered with a membrane. This membrane can rupture and become an extreme emergency that can put the infant’s life at stake. Aim: The purpose of this case report is to present a case of ruptured omphalocele operated successfully and describe the factors of good prognosis. Case Presentation: A female new-born with a giant omphalocele. This omphalocele ruptured during conservative treatment. An emergency surgery was organised and the defect was closed by using a prosthesis. The postoperative course was simple. During the 12 months follow-up, we have seen no complication. Conclusion: The management of ruptured omphaloceles is a surgical and anesthetic challenge. The mortality of this condition can be improved in our context by setting up well-equipped neonatal resuscitation units.

Prevalence and Factors Influencing Malnutrition among Children under Five Years of Age in a Peri-Urban Environment in Abidjan

Introduction: Child malnutrition is a major public health problem in Côte d’Ivoire in sub-Saharan Africa. This study aimed to determine the prevalence and factors influencing malnutrition in children under five in the peri-urban area of Abidjan, the economic capital. Patients and Methods: We carried out a prospective cross-sectional analytical study from May 3rd to October 31st 2019 at the General Hospital of Yopougon Attié in a peri-urban district of Abidjan, the economic capital. The study included 522 children aged 0 to 59 months who were received for a consultation, with the informed consent of their parents. The assessment of nutritional status was based on WHO growth standards. Statistical comparisons were made using the Chi-2 test for a significant p-value below 5%. Results: The population was predominantly female (sex ratio of 0.91) with an average age of 8.21 months and overall modest socio-economic conditions. The prevalence of malnutrition was as follows: 3.45% of malnutrition by excess, including 0.57% of obesity, 16.09% of stunting (5.75% severe), 12.07% of underweight (3.45% severe), 8.04% emaciation (1.91% severe). Three risk factors were identified for default malnutrition: age less than 6 months (p = 0.022), low birth weight (p = 0.003), and prematurity (p Conclusion: Malnutrition by deficiency is common in peri-urban areas in Abidjan with the main risk factors being age less than 6 months, low birth weight and prematurity. Fighting against those risk factors could help improve the nutritional status of children under five in the peri-urban environment of Abidjan.

Epidemiological, Clinical Progress Aspects of Congenital Heart Disease with Neonatal Revelation at the Mother-Child Hospital of Bingerville (HME) Concerning 98 Cases from January 2021 to December 2022 (Côte d’Ivoire)

Introduction The distribution of congenital heart disease (CHD) in sub-Saharan Africa is highly imprecise and varies from one region to another due to the inequality of diagnostic facilities. The aim of this study was to determine the in-hospital prevalence of congenital heart disease in children at the Mother-Child hospital of Bingerville (HME) by specifying the diagnostic, therapeutic and evolutionary aspects. Materials and methods We conducted a retrospective, descriptive, cross-sectional study at HME of Bingerville from January 2021 to December 2022. All newborns with congenital heart disease confirmed by echocardiography were included in the study. Results Of 656 admissions to the neonatology department over the study period, congenital heart disease accounted for 14.9% (98/656) of cases. In our series, 76.7% were diagnosed before the 1st week of life, with a mean chronological age of 5.18 days and extremes of 0 and 46 days. There were as many male patients (50%) as female (50%), i.e. a sex ratio of 1. These newborns were premature in 60.2% of cases, with a mean and median gestational age of 34 weeks’ amenorrhea. Most were left-right shunts (90.8%). Persistent ductus arteriosus (PDA) (48.9%) predominated, followed by atrial septal defect (38.7%), ventricular septal defect (13.3%), common trunk artery (CTA) (3.1%) and open septal pulmonary atresia (OSPA) (1%) as the primary cyanogenic heart disease. Pulmonary arterial hypertension (PAH) (50%) was primary in 38.8% and secondary (61.2%). The mortality rate was 30.6%, and all CTA patients died (100%), with a significant statistical relationship (p = 0.027). Progression under treatment was marked by clinical stabilization (68/98) in 69.4% of cases. Conclusion: Congenital heart disease is relatively common at the Bingerville HME. Access to echocardiography should be facilitated in neonatology departments for rapid diagnosis and optimal management of congenital heart disease in newborns.
