Conjugated hyperbilirubinemia presenting in first fourteen days in term neonates

AIM To describe the etiology and characteristics of earlyonset conjugated hyperbilirubinemia(ECHB) presenting within 14 d of life in term *** Retrospective review was performed of term infants up to 28-d-old who presented with conjugated hyperbilirubinemia(CHB) at a tertiary center over a 5-year period from January 2010 to December 2014. CHB is defined as conjugated bilirubin(CB) fraction greater than 15% of total bilirubin and CB greater or equal to 25 μmol/L. ECHB is defined as CHB detected within 14 d of life. "Late-onset" CHB(LCHB) is detected at 15-28 d of life and served as the comparison *** Total of 117 patients were recruited: 65 had ECHB, 52had LCHB. Neonates with ECHB were more likely to be clinically unwell(80.0% vs 42.3%, P < 0.001) and associated with non-hepatic causes(73.8% vs 44.2%, P = 0.001) compared to LCHB. Multifactorial liver injury(75.0%) and sepsis(17.3%) were the most common causes of ECHB in clinically unwell infants, majority(87.5%) had resolution of CHB with no progression to chronic liver disease. Inborn errors of metabolism were rare(5.8%) but associated with high mortality(100%) in our series. In the subgroup of clinically well infants(n = 13) with ECHB, biliary atresia(BA) was the most common diagnosis(61.5%), all presented initially with normal stools and decline in total bilirubin but with persistent CHB. CONCLUSION Secondary hepatic injury is the most common reason for ECHB. BA presents with ECHB in well infants without classical symptoms of pale stools and deep jaundice.

Cap polyposis: A rare cause of rectal bleeding in children

AIM: To evaluate the clinicopathological features and treatment outcomes of cap polyposis in the pediatric population. METHODS: All pediatric patients with histologically proven diagnosis of cap polyposis were identified from our endoscopy and histology database over a 12 year period from 2000-2012 at our tertiary pediatric center, KK Women's and Children's Hospital in Singapore. The case records of these patients were retrospectively reviewed. The demographics, clinical course, laboratory results, endoscopic and histopathological features, treatments, and outcomes were analyzed. The study protocol was approved by the hospital institutional review board. The histological slides were reviewed by a pediatric histopathologist to confirm the diagnosis of cap polyposis. RESULTS: Eleven patients were diagnosed with cap polyposis. The median patient age was 13 years (range 5-17 years); the sample included 7 males and 4 females. All of the patients presented with bloody stools. Seven patients (63%) had constipation, while 4 patients (36%) had diarrhea. All of the patients underwent colonoscopy and polypectomies (excluding 1 patient who refused polypectomy). The macroscopic findings were of polypoid lesions covered by fibrinopurulent exudates with normal intervening mucosa. The rectum was the most common involvement site (n = 9, 82%), followed by the rectosigmoid colon (n = 3, 18%). Five (45%) patients had fewer than 5 polyps, and 6 patients (65%) had multiple polyps. Histological examination of these polyps showed surface ulcerations with a cap of fibrin inflammatory exudate. Four (80%) patients with fewer than 5 polyps had complete resolution of symptoms following the polypectomy. One patient who did not consent to the polypectomy had resolution of symptoms after being treated with sulphasalazine. The growth characterlstlcs, different physlological parameters, photosynthetic activity （^14CO2 fixation）, and the translocatlon rate of photoassimllates In different taro （Colocasia esculenta L. Schott） genotypes was studled In order to determlne the posslble use of these parameters as selectlon crlterla for dlfferent wldely used genotypes of taro （Delta No. 9, 15, 20, 21, and balady）. The results obtalned suggest that Delta No. 21 shows the most slgnlflcant increase In all parameters tested compared wlth the control （balady）, followed by Delta No. 9, 15, and 20, respectively. The results show a positive correlation between photosynthetlc actlvlty, translocatlon efflclency, and total yield. The selected clone Delta No. 21 Is recommended for cultlvatlon In the delta reglon of Egypt.

Material and regenerative properties of an osteon-mimetic cortical bone-like scaffold

The objective of this work was to fabricate a rigid,resorbable and osteoconductive scaffold by mimicking the hierarchical structure of the cortical *** peptide-functionalize nanofiber microsheets were generated with calcium phosphate(CaP)content similar to that of the natural cortical ***,the CaP-rich fibrous microsheets were wrapped around a microneedle to form a laminated microtube mimicking the structure of an ***,a set of the osteon-mimetic microtubes were assembled around a solid rod and the assembly was annealed to fuse the microtubes and form a ***,an array of circular microholes were drilled on the outer surface of the shell to generate a cortical bone-like scaffold with an interconnected network of Haversian-and Volkmann-like *** CaP content,porosity and density of the bone-mimetic microsheets were 240 wt%,8%and 1.9 g/ml,respectively,which were close to that of natural cortical *** interconnected network of microcanals in the fused microtubes increased permeability of a model protein in the *** cortical scaffold induced osteogenesis and vasculogenesis in the absence of bone morphogenetic proteins upon seeding with human mesenchymal stem cells and endothelial colony-forming *** localized and timed-release of morphogenetic factors significantly increased the extent of osteogenic and vasculogenic differentiation of human mesenchymal stem cells and endothelial colony-forming cells in the cortical *** cortical bonemimetic nature of the cellular construct provided balanced rigidity,resorption rate,osteoconductivity and nutrient diffusivity to support vascularization and osteogenesis.

Mulching type-induced soil moisture and temperature regimes and water use efficiency of soybean under rain-fed condition in central Japan

Soybean(Glycine max)is a high water-demand crop and grown under moderate temperature in *** protect the crop from hot summer and to utilize rainfall for its cultivation,selection of appropriate mulching material(s)is *** optimum production of the crop,soil moisture and temperature regimes as well as water use efficiency(WUE)of the crop were investigated under straw,grass,paper,plastic and bare soil(control)mulching under rain-fed condition at Gifu university farm in *** mulching treatments,compared to the control,lowered soil temperature by 2°C at 5 cm depth and 0.5°C at 15 and 25 cm *** plastic and straw mulching stored the highest quantity of soil moisture at 5 and 15 cm depths;the bare soil stored the lowest *** 25 cm depth,soil-moisture content was the highest under paper mulch but invariable under the other *** mulching reduced evaporation rate from the soil surface and,consequently,the reduced soil-water consumption(SWC)from the root zone augmented WUE of *** paper mulching,by conserving soil-moisture and reducing soil temperature,provided better crop growth attributes,while the plastic mulching improved WUE of green ***,the plastic mulch performed best in reducing soil-water consumption and increasing WUE,while the paper mulch was good for soil-moisture conservation and temperature modification that increased soybean yield.

Geochemistry and spectrometric prospection of younger granites and granitic pegmatites bearing uranium mineralization at *** El Rakeb area, Central Eastern Desert, Egypt

The present study deals with the detailed field geology,mineralogy,geochemistry,and spectrometric prospection of rare metals and radioactive minerals associated with granitic pegmatites in the Gabal Um LassafAbu Dob-Um Atelia-Kadabora-Kab El Rakeb area(LDAKK),which are situated in the central Eastern Desert of *** study area is composed of granodiorites and *** latter is dissected by granitic pegmatites with a discriminatory distribution of economically important mineralization(rare-metals and U-Th minerals).The granitic pegmatites display a zoning structure composed of feldspar-rich zones enveloping pockets of mica(mostly muscovite) and all zoning internal quartz ***,the monzogranite and granitic pegmatites are composed of potash-feldspar,albite,and quartz with a small amount of biotite and *** mineralogical investigation of the highly radioactive zones in the granitic pegmatite shows enrichment in kasolite,uranothorite,thorite,xenotime,columbite,zircon,bismuth,and ***,the granitic pegmatites are enriched with high field strength elements(HFSE;Zr,Nb,Th,and U) and ion lithophilic elements(LILE;Rb,and Pb) and depleted in Ba,Sr,and *** are strongly peralkaline to metalumainous except for two samples,which are *** are formed by partial melting of metagreywackes at *** studied granitic pegmatites could belong to the NYF family(Niobium-Yttrium-Fluorine family) according to the geochemistry of the trace *** studied granitic pegmatites show deep negative Eu anomaly and display a strong M-type tetrad effect of rare earth *** field spectroscopy measurements showed the localization of the radiometric anomalous associated with the granitic pegmatites,which are intruded in the granitic host *** eU and eTh contents reached up to 316 and 1367 ppm,*** and thorium occurrences in the investigated granitic pegmatites are related to magmatic(syngenetic)ori

The effect on wound healing of pazopanib and bevacizumab compared with corticosteroid in experimental glaucoma filtration surgery

AIM: To compare the effects of bevacizumab and pazopanib with corticosteroids on wound healing after trabeculectomy. METHODS: In the study, 35 New Zealand white rabbits were randomly divided into five groups. Apart from the first group, limbus-based trabeculectomy was performed for the eyes of rabbits. No postoperative treatment was administered for group Ⅰ. Topically administered saline, prednisolone acetate(1%), bevacizumab 5 mg/mL, pazopanib 5 mg/mL for group Ⅱ, Ⅲ, Ⅳ and Ⅴ respectively were applied for groups 6 h daily for 28 d. On day 28 of the experiment, eyes were enucleated and histologically and immunohistochemically ***: The fibroblast counts of groups Ⅳ and Ⅴ were determined to be lower than those of groups Ⅱ and Ⅲ(P0.05). The immunohistochemical staining intensity of fibroblast growth factor β(FGF-β) and vascular endothelial growth factor(VEGF) was determined to be lower in groups Ⅳ and Ⅴ than in groups Ⅱ and Ⅲ(P0.05). The platelet derived growth factor β(PDGF-β) intensity was lower in group Ⅴ than in groups Ⅱ, Ⅲ and Ⅳ(P0.05). CONCLUSION: Bevacizumab and pazopanib might be good alternatives of corticosteroid treatment on delaying wound healing in glaucoma surgery.

Modelling the Segregation of Impurities During Solidification With Turbulent Electromagnetic Stirring

This paper proposes an estimate of the stirring intensity needed to maintain an efficient segregation of impurities towards the liquid when crystallizing semiconductors such as silicon,with rapid solidification rates(several cm/h).The method,valid far from stagnation points or detachments,is based on the properties of turbulent boundary layers,with a normal velocity of the liquid towards the solid/liquid interface due to solidification,that has the same effect as boundary layer suction in *** transition between the diffusive regime(no segregation),and the convective regime(efficient segregation)occurs if the friction at the wall is greater than a threshold depending on the solidification rate.A chart is given to estimate the convecto-diffusive parameter from the ratio between stirring and solidification velocity,and the Reynolds number.

Nonalcoholic fatty liver disease(NAFLD)is quickly becoming one of the most prominent causes of liver disease *** increasing incidence of NAFLD is tied to the obesity epidemic and the subsequent metabolic derangements brought along with *** efforts to elucidate the mechanism and causes of the disease have answered some questions,but much remains unknown about *** aim of this article is to discuss the current knowledge regarding the pathogenesis of the disease,as well as the current and future diagnostic,preventative,and therapeutic options available to clinicians for the management of NAFLD.
