Introduction: Cystic lymphangiomas are rare benign malformative tumors of the lymphatic system of obscure etiopathogenesis. The cervico-facial location remains the most common (75%). Although benign, these tumors remain potentially fatal, due to possible compression of the upper aero-digestive tract. The aim of this work is to study the epidemiological, diagnostic and therapeutic characteristics of cervico-mandibular congenital cystic lymphangiomas in the pediatric surgery department of the Donka National Hospital (HND) Conakry. Patients and methods: This is a retrospective and descriptive study of 13 files lasting 7 years from January 2015 to December 31, 2021. The files of children whose age is less than or equal to 15 years operated on cervical tumor with histological evidence of cystic lymphangioma were retained. The data were analyzed using SPSS statistical software 21 and anonymously. Results: The incidence of this study was 1.86 cases per year and a sex ratio of 0.62 in favor of girls. The average age was 8 months 19 days. In the antecedents, we only find poorly monitored pregnancies. The average size of the tumors was 11.85 cm. Cervical ultrasound and standard x-ray of the cervical mass were the only examinations performed. Total surgical excision of the cervical tumor was performed in all patients. The mass was polycystic on exploration. The histological examination of the surgical specimens was in favor of a cystic lymphangioma. The surgical consequences were simple in 11 patients (84.62%) and complicated by parietal suppuration in 2 cases (15.38%). There were no cases of recurrence after one year of follow-up. Conclusion: Cervico-mandibular cystic lymphangiomas are the most frequent locations of congenital lymphangiomas in children. Their severity is linked to the risk of compression of the aero-digestive tracts. Their diagnosis must be confirmed by the histology of the surgical specimen. Isolated Ventricular Septal Defect: Ultrasound, Therapeutic and Evolutionary Aspects of 85 Cases in the Cardiology Department of the Ignace Deen National Hospital in Conakry

Introduction: Ventricular septal defect (VSD) is the most common congenital heart disease of all congenital heart defects. The aim of this study was to investigate the echographic, therapeutic and evolutionary aspects of ventricular septal defects (VSD) in the general cardiology department of the Hôpital National Ignace Deen. Methods: A retrospective data collection was carried out from January 2018 to December 2023 including 85 cases of isolated IVC was performed. The variables studied were epidemiological, clinical, paraclinical, therapeutic and evolutionary. Results: Of the 320 patients seen during the study period for congenital heart disease, 85 (26.556%) were isolated IVCs. Age at diagnosis ranged from 3 months to 16 years, with an average age of 3.59 years. The most represented ethnic group was the Fulani (50.58%). The 8.24% came from consanguineous marriage versus 22.35%. 91.76% of children had a history of bronchitis. The most common clinical signs found were systolic murmur (90.58%), growth retardation (51.76%). Only 4 cases (4.70%) had a malformation associated with IVC represented by DiGeorges disease (2.35%) and trisomy 21 (2.35%). Nearly half the patients had type IIb VIC (44.71%). The other half were represented by type 1 (18.82%), type IIa (20%), type III (10.59%) and type IV (5.88%). According to site more than two-thirds of VICs (71.64%) were perimembranous in location, followed by infundibular (16.47%) and muscular (11.76%) VICs. In our study 55.29% presented an indication for both surgical intervention and medical treatment, while 16.47% required only medical treatment. In contrast, 28.23% were placed under exclusive surveillance. Of the 47 patients for whom surgery was indicated, 29 (61.17%) underwent surgical repair, while 18 (38.83%) were awaiting confirmation for surgery. Introduction: Congenital heart disease includes all cardiac and vascular malformations. It accounts for approximately one third of all congenital malformations and is a public health problem, particularly in developing countries. The aim of this study was to analyze the epidemiological, clinical and paraclinical aspects of congenital heart disease. Methods: This was a retrospective descriptive and analytical study based on the records of 135 patients referred for surgery and followed up in the cardiology department of the Ignace Deen University Hospital, collected in November 2022. Results: Hospital prevalence was 5%. The mean age was 71 months, ranging from 1 month to 19 years. The age group over 24 months was the most represented (62%). The M/F sex ratio was 1.36. Urban origin was predominant (58%). The rate of children not attending school or dropping out was high (16%). Siblings with fewer than 4 children were the most common (88%). A heart murmur was the most frequent sign (78%), followed by cyanosis (36%) and heart failure (29%). The association between heart murmurs and CHD was proven with a p-value Conclusion: CHDs represent the main indication for paediatric cardiac surgery and follow-up (95%). We stress the importance of implementing a screening and management strategy for congenital heart disease.

The Problem of Rehospitalisation for Heart Failure at the Cardiology Department of the Hôpital National Ignace Deen

Introduction: Despite current therapeutic advances, heart failure in sub-Saharan Africa remains a common, serious and costly disease, particularly due to rehospitalizations. The objective of this work was to determine the proportion of rehospitalizations for heart failure and to identify etiological factors. Methodology: This was a retrospective descriptive study with a duration of 8 months from April 1 to November 30, 2021. This study included all patients rehospitalized in the department for Heart Failure and who agreed to participate in our study. Results: During the period of our study, 437 patients were hospitalized in the HF department, among which we collected 126 cases of rehospitalization for HF with a frequency of 28.83%. The mean age of our patients was 46.32 ± 18.98 years with the extremes of 15 to 84 years. The most affected age group was between 35 and 44 years old in 24 cases, i.e. a frequency of 19%. We observed a female predominance of 64 cases, i.e. a frequency of 50.8% compared to 62 cases, i.e. a frequency of 49.2% with a sex ratio (M/F) equal to 0.96. 98 cases of our patients, i.e. a frequency of 77.8%, were mutual insurance companies who felt they had the necessary support from those around them. In our sample, the underlying heart disease was mainly represented by valvular heart disease in 59 cases, followed by hypertensive heart disease in 42 cases with the respective frequencies of 46.82% and 33.33%. The majority of our patients were rehospitalized (1 - 3) times after a first episode of HF flare-up in 117 cases or 92.9%. Irregularity at control and therapeutic break were the most common decompensation factors with frequencies of 75.8% and 74.2% respectively. Lipid Profile of Patients Seen in Consultation at the Cardiology Department at the Ignace DEEN National Hospital (City of Conakry, Republic of Guinea)

The study addressed a crucial public health concern of dyslipidemia and its management, with a focus on the Ignace DEEN National Hospital patients, filling a research gap in the Republic of Guinea. It contributed valuable epidemiological data on dyslipidemia in the studied population, emphasizing gender, age, and socio-professional categories, which could inform tailored prevention and management strategies in improving the health care of dyslipidemia patients by proposing diets to help these patients better manage their disease and a food system for preventing dyslipidemia. The study focused on the lipid profile of patients seen at the cardiology department at the Ignace DEEN National Hospital Republic of Guinea (Conakry). This was a prospective, descriptive study lasting six (6) months from January 24 to July 23, 2021;aimed at determining the frequency of lipid abnormalities and pathologies related to the lipid profile. A total of 523 people were included in this study with a female predominance of 59.34% compared to 40.66% of males for a ratio of 1.5 in favor of women. The age groups of 45 - 54 and 55 - 65 were the most affected with respective frequencies of 45.42% and 30.03%. The most represented socio-professional category was that of housewives, i.e. 45.78%, followed by traders 18.68%. our series was dominated by the married couple, i.e. 77.29%. Dyslipidemia constitutes a modifiable cardiovascular risk factor in patients seen in the cardiology department at the Ignace DEEN Hospital;its control must be strict through the use of hygienic and dietary rules and hypolipidemia medications.

This study aims to enhance the healthcare services for diabetic patients in the administrative region of Kindia by suggesting dietary interventions to assist diabetics in better managing their condition. Conducted over a period of six months, from February 18 to July 18, 2021, this prospective and descriptive cross-sectional study involved 220 diabetic patients. Among these, 48 patients (22%) maintained balanced glucose levels (2 g/l). Positive glycosuria was observed in 54% of the patients, whereas 46% demonstrated normal glycosuria. An analysis of urinary parameters revealed that 15% of the patients had abnormal Ketone Bodies. Normal HbA1c levels (9%) HbA1c levels. Hematological assessments indicated significant variation: 56% of the patients had low hemoglobin levels, 4% suffered from hyper-eosinophilia, and 1% each from hyper-basophilia and hyper-hemoglobinemia. The anemic profile was characterized as mild anemia in 75%, moderate anemia in 20%, and severe anemia in 5%. Furthermore, 25% of patients were affected by Microcytic anemia and 75% by Normocytic anemia. Demographically, women constituted 65% of the study group compared to 35% of men. The most represented age bracket was 41 to 60 years, accounting for 52% of the patients, while those between 61 and 80 years comprised 36%. Every district in Kindia was impacted by diabetes.

近几十年来,环境问题成为人类科技发展中最为担心的问题之一。以信息通信技术(ICT)为例,环境问题必将影响未来的网络技术、架构和使用惯例。我们需要一些新的无线网络技术和方法来减少碳的排放,进而实现人们常说的绿色网络。异构蜂窝网络的广泛应用和部署既带来了挑战也带来了机遇。机遇包括,虽然异构蜂窝网络对有限的频率资源进行复用提高了频谱效率,但它也会在蜂窝内带来严重的小区间干扰。另外,蜂窝内的宏小区和普通小区之间也会有更多的合作机会。为实现未来的绿色通信,在当前以小区部署密集来提升用户通话质量的场景下,能量效率(EE)将成为衡量通信技术性能的非常重要的一项指标。本文拟利用当前异构网络的潜在合作多样性来解决网络中的干扰问题并提高网络的能量效率。为分析普通小区和宏小区相互之间复杂混乱的交互行为,本文重点对采用大规模多输入多输出(MIMO)的2层异构网络(HetNets)中的宏单元及其能源效率进行研究,并开发出了一种检验异构网络能量效率的分析方法。本文提出了一种新颖的基于议价合作博弈的框架来分析网络中的干扰和能量效率。其中,一种新的可调节的效用函数应用于BCG中来共同解决异构网中能量有效性的问题。通过使用提出的BCG框架,本文推导出了封闭式的能源协调方案,并提出了以干扰抑制和能量节省为目标的共同干扰感的知能量协调机制此外,简化的算法可以减少目前大量部署的小区中的信令开销。最后,仿真结果证明了本文提出的方案的有效性,并表明大规模输入输出技术的使用能够显著改善网络的能量效率。

The relationship between fire severity and burning efficiency for estimating wildfire emissions in Mediterranean forests

Forests are exposed to changing climatic conditions reflected by increasing drought and heat waves that increase the risk of wildfire ignition and *** variables such as rain and wind as well as vegetation structure,land configuration and forest management practices are all factors that determine the burning potential of *** assessment of emissions released by vegetation combustion is essential for determining greenhouse gases and air *** estimation of wildfire-related emissions depends on factors such as the type and fraction of fuel(i.e.,live biomass,ground litter,dead wood)consumed by the fire in a given area,termed the burning *** approaches estimate live burning efficiency from optical remote sensing *** study used a data-driven method to estimate live burning efficiency in a Mediterranean *** severity estimations from Lands at imagery(dNBR),which relate to fuel consumption,and quantitative field data from three national forest inventory data were combined to establish the relationship between burning severity and live burning *** proxies explored these relationships based on dNBR interval classes,as well as regres sion *** correlation results between live burning efficiency and dNBR for conifers(R=0.63)and broad-leaved vegetation(R=0.95)indicated ways for improving emissions *** estimations by severity class(low,moderate-low,moderate-high,and high)are provided for conifers(0.44-0.81)and broad-leaves(0.64-0.86),and regression models for the live fraction of the tree canopy susceptible to burning(7 branches,and leaves).The live burning efficiency values by severity class were higher than previous studies.
