Clinical profile of disseminated cryptococcal infection-a case series

Objective:To study disseminated cryptococcal infection in a tertiary care hospital in Southern ***:The clinical profile of 12 disseminated cryptococcosis patients with the age group of 28-52 years was retrospectively ***:7(58.3%) presented with fever days and 3(25%) > 30 days whereas 2(16.7%) did not have *** the 12(100%) had headache. 2(16.7%) had altered sensorium,one(8%) seizure.5(41.7%) had diarrhea and vomiting.6(50%) had oral candidiasis,and anemia.9(75%) had elevated erythrocyte sedimentation rate(ESR).6(50%) had neck *** fluid(CSF) pressure was elevated in all 12(100%) patients. blood culture positive for Cryptococcus neqformans(***) in 11(91.7%) and CSF culture positive in all 12(100%),one(8%) had urine culture *** ink preparation was positive in 10(83.3%).Cd4 count was less than 50/microl in 4(33.3%),between 50-100 in 6(50%) and 2(16.7%) in the range of 100-200.6(50%) were treated with parenteral amphotericin b(0.7 mg/kg/d) during intensive phase followed by oral fluconazole 400 mg/d for 8 weeks then maintenance oral fluconazole 200 mg/d.5(41.6%) were treated with fluconazole alone.8(66.7%) improved and 4(33.3%) patients *** those who succumbed to the illness,2(16.7%) received amphotericin and fluconazole,2(16.7%) patients received fluconazole ***:disseminated cryptococcosis can cause considerable mortality in HIV patients and immunocompromised non- HIV *** times,its presentation closely mimics that of *** diagnosis and appropriate treatment should be started as early as possible.

The effect of replacing the anion from N to bi down the group in the periodic table is investigated on SrMg2X2（X = N,P,As,Sb,bi）.A full potential linearized augmented plane wave plus local orbitals method is used along with different exchange–correlation potentials to obtain the lattice constants,phonons,electronic,and optical properties of the Sr Mg2X2（X = N,P,As,Sb,bi） Zintl compounds.A good agreement is achieved and our calculations are validated by previous experimental and theoretical *** compounds have shown stable dynamical behavior with gamma centered longitudinal response having no imaginary *** band structures reveal the semiconducting nature of the *** Pnictogen（X）-p state contributed mainly in the valence band and the Sr-d state forms the conduction of the *** charge transfer and low overlapping of the atomic densities indicates the preferable ionic bonding character of these *** the optical properties,real and imaginary parts of dielectric function,complex refractive index,birefringence,reflectivity,and optical conductivity are *** compounds can be utilized in the optical and optoelectronic devices.

In this work,an Artificial Neural Network(ANN)based technique is suggested for classifying the faults which occur in hybrid power distribution ***,which is generated by the solar and wind energy-based hybrid system,is given to the grid at the Point of Common Coupling(PCC).A boost converter along with perturb and observe(P&O)algorithm is utilized in this system to obtain a constant link *** contrast,the link voltage of the wind energy conversion system(WECS)is retained with the assistance of a Proportional Integral(PI)*** grid synchronization is tainted with the assis-tance of the d-q *** the analysis of faults like islanding,line-ground,and line-line fault,the ANN is *** voltage signal is observed at the PCC,and the discrete Wavelet Transform(dWT)is employed to obtain different *** on the collected features,the ANN classifies the faults in an effi-cient *** simulation is done in MATLAb and the results are also validated through the hardware *** fault analysis is carried out and the results are compared with the existing ***,the Total harmonic distortion(THd)is lessened by 4.3%by using the proposed methodology.

A new lanthanide formate oxalate framework [Eu（C2O4）（HCOO）]n （1） has been synthesized via hydrothermal method and characterized by single-crystal X-ray diffraction （SC-XRd）. The framework crystallizes in the orthorhombic Pnma space group, with a = 7.0984（4）, b = 6.6442（3）, c = 10.6793（6）？, V = 503.68（4）？3, Z = 3, C3HO6Eu, Mr = 285.00 g/mol, dc = 3.7581 g/cm3, F（000） = 520.3796, μ = 12.413 mm-1, the final R = 0.0264 and wR = 0.0628 for 797 observed reflections with I 〉 2σ（I）. The photoluminescence （PL） measurements reveal the significant red emission of the framework is dominated by the （5d0 → 7F2） electronic transition at 614 nm. Further variable-temperature powder X-ray diffraction （VT-PXRd） indicates that framework 1 shows slight negative thermal expansion （NTE） along the a-axis, and positive thermal expansion （PTE） along the b and c axes.

The band structure, magnetism, charge distribution, and optics parameters of TMO3–h-bN hybrid systems are investigated by adopting first-principles study(FPS) calculations. It is observed that the TMO3 clusters add finite magnetic moments to bilayer h-bN(bL/h-bN), thereby making it a magnetic two-dimensional(2d) material. Spin-polarized band structures for various TMO3–bL/h-bN hybrid models are calculated. After the incorporation of TMO3, bL/h-bN is converted into semimetal or conducting material in spin up/down bands, depending on the type of impurity cluster present in bL/h-bN lattice. Optics parameters are also investigated for the TMO3–bL/h-bN complex systems. The incorporation of TMO3 clusters modifies the absorption and extinction coefficient in visible range, while static reflectivity and refraction parameter increase. It can be surmised that the TMO3 substitution in bL/h-bN is a suitable technique to modify its physical parameters thus making it functional for nano/opto-electronic applications, and an experimental approach can be adapted to reinforce the outcomes of this study.

The next-generation wireless networks are expected to provide higher capacity,system throughput with improved energy *** of the key technologies,to meet the demand for high-rate transmission,is deviceto-device(d2d)communication which allows users who are close to communicating directly instead of transiting through base stations,and d2d communication users to share the cellular user chain under the control of the cellular *** a new generation of cellular network technology,d2d communication technology has the advantages of improving spectrum resource utilization and improving system throughput and has become one of the key technologies that have been widely concerned in the ***,due to the sharing of cellular network resources,d2d communication causes severe interference to existing cellular *** of the most important factors in d2d communication is the spectrum resources utilization and energy consumption which needs considerable attention from research *** address these issues,this paper proposes an efficient algorithm based on the idea of particle swarm *** main idea is to maximize the energy efficiency based on the overall link optimization of d2d user pairs by generating an allocation matrix of spectrum and *** d2d users are enabled to reuse multiple cellular user’s resources by enhancing their total energy efficiency based on the quality of service constraints and the modification of location and speed in particle *** constraint also provides feasibility to solve the original fractional programming *** results indicate that the proposed scheme effectively improved the energy efficiency and spectrum utilization as compared with other competing alternatives.

A dark pulse mode-locked laser is experimentally demonstrated using the indium tin oxide(ITO)coated dshape fiber as a saturable absorber(SA).Using the polishing wheel technique,a d-shape single mode fiber was fabricated.A 60-nm-thick layer of ITO was deposited over the d-shape fiber using the electron beam deposition *** SA has a saturation intensity of 40.32 MW/cm^2 and a modulation depth of 3.5%.A stable dark pulse mode-locked laser was observed at a central wavelength of 1559.4 nm with repetition rate 0.98 MHz,pulse width 370 ns and signal-to-noise ratio 61 db.

A rapid, efficient, and facile synthesis of an assortment of C-2 substituted imidazopyrazines has been achieved by utilizing the palladium catalyzed Suzuki cross-coupling of 2-bromo-1H-imidazo[4,5-b]pyrazine with various boronic acids under microwave irradiation. The utilization of（A-^（ta）phos）_2PdCl_2 as a catalyst in combination with Cs F as base and dME-H_2O（4：1） as the solvent system at 100℃ procured the diaryls in acceptable to excellent yields. Prominent features of this developed methodology include short reaction times, fewer side products, and exceptional tolerance to a wide variety of functional groups.

Effect of seedling age and water depth on morphological and physiological aspects of transplanted rice under high temperature

To study the effect of high temperature, rice seedlings 20, 30, 40 and 50 d were kept at 5, 10, 15 and 20 cm water depth in a water pool. Meteorological findings indicated that water temperature varied up to 10 cm but became stable below this depth. deep water inflicted higher tiller mortality, minimal increase in dry weight of aerial parts and leaf area, decrease in root length, and decrease in root dry weight especially at 20 cm water depth and produced an unbalanced T/R ratio (top versus root dry weight). However, deep water tended to increase plant length. These parameters, however, excel in shallow water. Older seedlings, with the exception of root dry weight, could not perform well compared to young seedlings in all physiological and morphological aspects. The study revealed that seedlings, particularly young ones, stand well in shallow water and can cope with high temperature.
