Everolimus immunosuppression reduces the serum expression of fibrosis markers in liver transplant recipients

AIM: To evaluate the expression of serum fibrosis markers in liver transplantation(LT) recipients on everolimus monotherapy compared to patients on an anti-calcineurin ***: This cross-sectional case-control study included LT patients on everolimus monotherapy(cases)(E)(n = 30) and matched controls on an anticalcineurin regimen(calcineurin inhibitors, CNI), paired by etiology of liver disease and time since LT(n = 30). Clinical characteristics, blood tests and elastography were collected. Serum levels of transforming growth factor-β(TGF-β), angiopoietin-1, tumor necrosis factor(TNF), platelet derived growth factor, amino-terminal propeptide of type Ⅲ procollagen(PⅢNP), hyaluronicacid(HA), VCM-1(ng/m L), interleukin(IL)-10, interferoninducible protein 10(IP-10), vascular endothelial growth factor and hepatocyte growth factor(HGF)(pg/m L) were determined by enzyme-linked immunosorbent assay. Expression of these markers between E and CNI was compared. Stratified analysis was done according to factors that may influence liver fibrosis. Variables are described with medians(interquartillic range) or ***: A total of 60 patients [age: 59(49-64), hepatitis C virus(HCV): n = 21(35%), time from LT: 73 mo(16-105)] were included. Patients had been on everolimus for a median of 15 mo. No differences in inflammatory activity, APRI test or liver elastography were found between the groups. No significant differences were observed between the groups in serum levels of PⅢ NP, metalloproteinase type = 1, angiopoietin, HGF, IP-10, TNF-α, IL-10 and vascular cell adhesion molecule. Patients on E had a lower expression of TGF-β [E: 12.7(3.7-133.6), CNI: 152.5(14.4-333.2), P = 0.009] and HA [E: 702.89(329.4-838.2), CNI: 1513.6(691.9-1951.4), P = 0.001] than those on CNI. This difference was maintained in the stratified analysis when recipient age is more than 50 years(TFG-β1: P = 0.06; HA: P = 0.005), in patients without active neoplasia(TFG-β1, P = 0.009; HA: P =

Photolysis of a mixture of anthracene and benzo[a] pyrene at ultra-trace levels in natural water with disinfection purposes

The photodegradation of anthracene(AN)and benzo[a]pyrene(BaP),two priority polycyclic aromatic hydrocarbons(PAHs),was examined at ultra-trace levels in surface water to elucidate their behaviour under several irradiance values and types of *** emitting flux and the spectrum of the lamps were found to develop a crucial role in AN and BaP degradation since removal efficiencies of the target contaminants higher than 99%were found after 15 min of irradiation under an ultraviolet C(UVC)irradiance of 0.63 mW/cm^2,corresponding to a fluence of 560.25 mJ/cm^*** the other hand,although ultraviolet A(UVA)lamps exhibited a higher irradiance compared to that of UVC lamps,they were not efficient for degrading the target *** removal kinetic studies corroborated these findings,being the AN elimination rate in surface water higher than that in deionized water at optimal operating *** potential was also measured.A rapid microbial load inactivation,in terms of total coliforms naturally contained in the water matrix studied,was evidenced within 15 min of treatment for the fluence ***,after 24 hr in the dark,a regrowth was ***,photolysis products more toxic than the parent compounds were found,which were not removed even by extending the treatment *** this regard,it can be concluded that the individual action of UVC light for removing AN and BaP with disinfection purposes is not an efficient treatment;therefore,the use of radiation in combination with other kinds of treatments is required.

Multifocal intraocular lenses(IOLs) are currently usually implanted for the treatment of cataracts because they have been proved to be superior to monofocal IOLs with respect to spectacle independence. In turn, they are associated with a higher prevalence of dysphotopsia symptoms that is one of the most common causes of patient dissatisfaction. Neuroadaptation seems to play a major role in the optimal adaptation to multifocal IOLs. In this context, the development of strategies that facilitate the neuroadaptation process to multifocality might be an effective strategy to reduce patients' dissatisfaction. Video games have been proved to be effective for the improvement of visual acuity and for the promotion of neuroplasticity in elderly subjects and other populations with cortical-related visual impairment. This narrative review highlights the physiological potential of video games as a perceptual strategy to improve visual acuity and promote neuroplasticity in patients using multifocalIOLs, although research is still needed to confirm these benefits in this specific population, with only one comparative study to this date providing evidence of them.

Encapsulation of Almond Essential Oil by Co-Extrusion/Gelling Using Chitosan as Wall Material

Encapsulation confers protection to substances as essential oils from processes like oxidation, evaporation or uncontrolled release. In this study almond oil capsules were obtained by co-extrusion/gelling technique. Chitosan was used as shell material and sodium triphosphate pentabasic as cross linking agent. Different encapsulation process variables were studied: cross-linker concentration, nozzles size and potential. Optical microscopy was used to determine the capsules morphology and degradability tests were performed in order to study capsules degradation over time. Results showed that nozzles size and cross linking concentration are key variables to consider in the encapsulation process. Degradability tests showed rapid weight loss.

Immunotherapy and immunoescape in colorectal cancer

Immunotherapy encompasses a variety of interventions and techniques with the common goal of eliciting tumor cell destructive immune responses. Colorectal carcinoma often presents as metastatic disease that impedes curative surgery. Novel strategies such as active immunization with dendritic cells (DCs), gene transfer of cytokines into tumor cells or administration of immunostimulatory monoclonal antibodies (such as anti-CD137 or anti-CTLA-4) have been assessed in preclinical studies and are at an early clinical development stage. Importantly, there is accumulating evidence that chemotherapy and immunotherapy can be combined in the treatment of some cases with colorectal cancer, with synergistic potentiation as a result of antigens cross-presented by dendritic cells and/or elimination of competitor or suppressive T lymphocyte populations (regulatory T-cells). However, genetic and epigenetic unstable carcinoma cells frequently evolve mechanisms of immunoevasion that are the result of either loss of antigen presentation, or an active expression of immunosuppressive substances. Some of these actively immunosuppressive mechanisms are inducible by cytokines that signify the arrival of an effector immune response. For example, induction of 2, 3 indoleamine dioxygenase (IDO) by IFNγ in colorectal carcinoma cells. Combinational and balanced strategies fostering antigen presentation, T-cell costimulation and interference with immune regulatory mechanisms will probably take the stage in translational research in the treatment of colorectal carcinoma.
