Background: Multiple system atrophy (MSA) is a neurodegenerative condition characterized by variable combinations of parkinsonism, autonomic failure, cerebellar ataxia and pyramidal features. Although the distribution of synucleinopathy correlates with the predominant clinical features, the burden of pathology does not fully explain observed differences in clinical presentation and rate of disease progression. We hypothesized that the clinical heterogeneity in MSA is a consequence of variability in the seeding activity of α-synuclein both between different patients and between different brain regions. Methods: The reliable detection of α-synuclein seeding activity.derived from MSA using cell-free amplification assays remains challenging. Therefore, we conducted a systematic evaluation of 168 different reaction buffers, using an array of pH and salts, seeded with fully characterized brain homogenates from one MSA and one PD patient. We then validated the two conditions that conferred the optimal ability to discriminate between PD- and MSA-derived samples in a larger cohort of 40 neuropathologically confirmed cases, including 15 MSA. Finally, in a subset of brains, we conducted the first multi-region analysis of seeding behaviour in MSA. Results: Using our novel buffer conditions, we show that the physicochemical factors that govern the in vitro amplification of α-synuclein can be tailored to generate strain-specific reaction buffers that can be used to reliably study the seeding capacity from MSA-derived α-synuclein. Using this novel approach, we were able to sub-categorize the 15 MSA brains into 3 groups: high, intermediate and low seeders. To further demonstrate heterogeneity in α-synuclein seeding in MSA, we conducted a comprehensive multi-regional evaluation of α-synuclein seeding in 13 different regions from 2 high seeders, 2 intermediate seeders and 2 low seeders. Preparation of AlN thin films for film bulk acoustic resonator application by radio frequency sputtering

Aluminum nitride （AlN） thin films with high c-axis orientation have been prepared on a glass substrate with an Al bottom electrode by radio frequency （RF） reactive magnetron sputtering. Based on the analysis of Berg＇s hysteresis model, the improved sputtering system is realized without a hysteresis effect. A new control method for rapidly.depositing highly c-axis oriented AlN thin films is proposed. The N2 concentration could be controlled by observing the changes in cathode voltage, to realize the optimum processing condition where the target could be fixed stably in the transition region, and both stoichiometric film composition and a high deposition rate could be obtained. Under a 500 W RF power of a target with a 6 cm diameter, a substrate temperature of 450 ℃, a target-substrate distance of 60 mm and a N2 concentration of 25%, AlN thin film with preferential （002） orientation was deposited at 2.3 μm/h which is a much higher rate than previously achieved. Through X-ray diffraction （XRD） analysis, the full width at half maximum （FWHM） of AlN （002） was shown to be about 0.28°, which shows the good crystallinity and crystal orientation of AlN thin film. With other parameters held constant, any increase or decrease in N2 concentration results in an increase in the FWHM of AlN.

Exact Solution of D-Dimensional Klein-Gordon Oscillator with Minimal Length

Specific modifications of the usual canonical commutation relations between position and momentumoperators have been proposed in the literature in order to implement the idea of the existence of a minimal observablelength. Here, we study a consequence of this proposal in relativistic quantum mechanics by solving in the momentumspace representation the Klein-Gordon oscillator in arbitrary dimensions. The exact bound states spectrum and thecorresponding momentum space wave function are obtained. Following Chang et al. (Phys. Rev. D 65 (2002) 125027),we discuss constraint that can be placed on the minimal length by measuring the energy levels of an electron in a penningtrap.

Systemic lupus erythematosus (SLE) is an autoimmune disease that affects mainly women, presents several clinical manifestations and has no known exact cause. The skin is one of the organs that can be affected, with subacute cutaneous lupus erythematosus (SCLE) being one of the classifications according to cutaneous lupus erythematosus lesions. SCLE is commonly found in young to middle age women and carries characteristics related to sun exposure. In this report, there is a rare case of a male and elderly patient that presents SCLE subgroup initially asymptomatic, with subsequent appearance of macular skin lesions and vitiligo appearance. This is an important case to alert health professionals that the disease can manifest itself and open its clinical profile even in patients of sex and unusual ages.

For the development of 19-plex y STR system and polymorphism studies in local ethnic populations sixteen markers of non-recombining regions (NRy) of y chromosome, which show high power of discrimination among individuals, were selected in this study. Blood samples (600) were collected from the males of three most common castes of Pakistani population (Arain, Awan and Rajput) with different parent lineages. Three markers (DyS385a/b, DyS389I/II and yCAIIa/b) among 16 y STRs are double-targeted regions of the y chromosome and thus provide two polymorphic peaks for each respective primer set. These 16 y-STRs were developed into Megaplex system for simultaneous amplification of all markers within the population. The overall power of discrimination observed in focused populations was 60.5%, 66.5% and 55% in Rajput, Awan and Arain casts respectively. This discrimination power will be helpful in human identification for forensic casework studies including sexual assaults and paternity testing.

Introduction: Syphilis consists in a systemic infect contagious pathology with a chronic character. The etiological agent consists of an anaerobic spirochete bacterium, Treponema pallidum. Laboratory diagnosis can be made through a direct investigation of the etiologic agent or non-treponemal (VDRL) and treponemal (FTA-Abs) serological tests. False-negative results are rare. Objectives: The present report presents a rare case of a man with a secondary syphilis syndrome who was initially undiagnosed due to the prozone effect, since he presented VDRL in low titers, and was later confirmed with a new diluted serum sample and VDRL and FTA-Abs, which were positive. Conclusion: A close look should be given to patients who, although the clinical condition is compatible, complementary exams may not be, and clinical cases should be carefully evaluated so that the patient is not treated late.

Assessing Secure OpenID-Based EAAA Protocol to Prevent MITM and Phishing Attacks in Web Apps

To secure web applications from Man-In-The-Middle(MITM)and phishing attacks is a challenging task *** this purpose,authen-tication protocol plays a vital role in web communication which securely transfers data from one party to *** authentication works via OpenID,Kerberos,password authentication protocols,***,there are still some limitations present in the reported security *** this paper,the presented anticipated strategy secures both Web-based attacks by leveraging encoded emails and a novel password form pattern *** proposed OpenID-based encrypted Email’s Authentication,Authorization,and Accounting(EAAA)protocol ensure security by relying on the email authenticity and a Special Secret Encrypted Alphanumeric String(SSEAS).This string is deployed on both the relying party and the email server,which is unique and *** first authentication,OpenID Uniform Resource Locator(URL)identity,is performed on the identity provider side.A second authentication is carried out by the hidden Email’s server side and receives a third authentication *** Email’s third SSEAS authentication link manages on the relying party(RP).Compared to existing cryptographic single sign-on protocols,the EAAA protocol ensures that an OpenID URL’s identity is secured from MITM and phishing *** study manages two attacks such as MITM and phishing attacks and gives 339 ms response time which is higher than the already reported methods,such as Single Sign-On(SSO)and *** experimental sites were examined by 72 information technology(IT)specialists,who found that 88.89%of respondents successfully validated the user authorization provided to them via *** proposed EAAA protocol minimizes the higher-level risk of MITM and phishing attacks in an OpenID-based atmosphere.

Importance of Primary Health Care in Medicine: Simultaneous Analysis of Two Colleges, under the Vision of Teachers and Students

Introduction: The involvement of undergraduate student at the beginning of their training in activities based on Primary Health Care (PHC), provides preventive and health promoting actions, and ensures the evaluation of the vulnerability of communities and people. Objectives: To analyze the opinion of students and teachers of two medicine higher education institutions on the relevance of Primary Health Care in this course. METHODS: Students and teachers should be effectively enrolled in order for the study to be conducted, being chosen in a random manner and without interferences on the part of the researchers so that the work was as reliable as possible. A cross-sectional study was conducted with interviews structured in questionnaires about what they thought about PHC. To some students and teachers of the medical course of Valen?a and Grande Rio University (UNIGRANRIO). Before receiving the questionnaire, all interviewed signed a Free and Informed Consent Term, which included the objectives and methodology of the work. Results: 310 students and 51 teachers participated in the study, where 91.43% of the students and 100% of the teachers affirmed that primary care plays a fundamental role in medical education. 94.86% of students and 97.91% of teachers, claimed that the discipline allows a greater contact between undergraduates and community. When questioned about the encouragement that teachers give to students to pursue a career in PHC, 57.87% of the students and 70.83% of the professors said they lacked in such a stimulus. Conclusion: PHC is still very neglected by governmental policies, educational institutions and by the own teachers, that often, turn they attention only to tertiary medicine and, as a consequence, a discouragement of this area occur. Therefore, a higher PHC value is necessary, allowing a more humane look at the patient, valuing their feelings, anguishes and their pathological framework.

Psychological interventions for adherence, metabolic control, and coping with stress in adolescents with type 1 diabetes:a systematic review

Background The aim of this review was to summarize and identify the variations in the effectiveness of psychological interventions on adherence,metabolic control,and coping with stress in adolescents with type 1 diabetes(T1D).Methods An electronic search of literature was performed using PubMed(NLM),Embase(Ovid),CINAHL Plus(EBSCO-host),PsycINFO(Ovid),and Google *** search was limited to include articles reported the effect of one of the psychological interventions:cognitive behavioral therapy(CBT),coping skills training(CST),stress management,or psychotherapy *** report included peer-reviewed articles published in English from January 1990 until May 2019in adolescents with T1D,and summarizes the results of 24 studies that met the inclusion *** Psychological interventions showed differential effects on adherence,metabolic control,and coping with stress in adolescents with *** interventions using principles of CBT appear to have a superior positive effect on regimen adherence compared with other types of psychological *** contrast,metabolic control was significantly improved with the implementation of CST and some forms of *** management and psychotherapy interventions showed significant promises for adolescents to cope with their diabetes-related daily *** However,the findings may add some promises to diabetes management in adolescents,additional research to understand the effect of these interventions is needed.

Amyloidosis is the result of fibrous and insoluble amyloid protein deposition in extracellular spaces of tissues and organs, including the skin. Through the analysis of medical records, clinical condition and laboratory tests, this article proposes a rare case report of an adult patient with symptoms and diagnosis of polymyositis, histopathology of nodular amyloidosis with skin lesions suggestive of lupus. The patient is still being followed at the dermatology clinic of Valença Medical School, in order to improve the prognosis and prevent the progression of existing symptoms.
