Experiences of new family physicians finding jobs with obstetrical care in the USA

Objective This study aimed to explore how new family medicine graduates who want to include obstetrics in their scope of practice identify and select jobs and to understand how employment influences scope of practice in family medicine,particularly the ability to provide maternity care and deliver babies.Design Mixed-methods study including a survey and qualitative interviews conducted in 2017.setting We electronically surveyed US family physicians and followed up with a purposeful subsample of these physicians to conduct in-depth,semistructured telephone interviews.Participants 1016 US family medicine residency graduates 2014-2016 who indicated that they intended to deliver babies in practice completed a survey;56 of these were interviewed.Main outcome measures The survey measured the reasons for not doing obstetrics as a family physician.To identify themes regarding finding family medicine jobs with obstetrics,we used a team-based,immersion-crystallisation approach to analyse the transcribed qualitative interviews.results Survey results(49%response rate)showed that not finding a job that included obstetrics was the primary reason newly graduated family physicians who intended to do obstetrics were not doing so.Qualitative interviews revealed that family physicians often find jobs with obstetrics through connections or recruitment efforts and make job decisions based on personal considerations such as included geographical preferences,family obligations and lifestyle.However,job-seeking and job-taking decisions are constrained by employment-related issues such as job structure,practice characteristics and lack of availability of family medicine jobs with obstetrics.Conclusions While personal reasons drove job selection for most physicians,their choices were constrained by multiple factors beyond their control,particularly availability of family medicine jobs allowing obstetrics.The shift from physician as practice owner to physician as employee in the USA has implications for job-seeking behaviours of newly grad

背景足够的产科家庭医生对于确保患者获取必需的产科服务至关重要。然而,既往研究表明执业范围包含产科的家庭医生数量正在逐年减少,如今从事新生儿接生工作的家庭医生不到10%。目的旨在探究希望将产科纳入其执业范围的新毕业家庭医生如何发现和选择工作,并进一步了解当前就业形势对家庭医生的执业范围,特别是产科、新生儿接生方面的影响。方法于2017年进行问卷调查和定性访谈的混合方法研究。对美国家庭医生进行电子问卷调查并在目的性子抽样后对这些医生进行深度半结构化电话访谈。问卷调查总结了家庭医生未从事产科工作的原因,为进一步明确选择从事产科工作的家庭医生的具体情况,采用基于群体的沉浸式方法来转译定性访谈的结果。本研究向2098例毕业于2014—2016年并希望从事新生儿接生工作的美国家庭医学专业实习医生发送调查问卷,回复1016份,回复率48.43%,其中56例接受了电话访谈。结果问卷调查结果显示,未能找到工作范围包含产科的工作是希望从事产科工作的家庭医学毕业生未从事该工作的主要原因。定性访谈结果显示,家庭医生通常通过人脉关系或人才招聘的途径找到产科相关工作,并根据地理位置偏好、家庭义务及生活方式要求等个人考虑因素做出选择。同时,求职过程和择业决策也受到工作结构、执业特点及缺乏产科相工作经验等因素的限制。结论虽然个人意向决定大部分医学生的工作选择,但其选择仍受到多种不可控因素限制,特别是家庭医学工作提供产科相关工作的能力。美国毕业医学生从医院实习生到执业医师的转变同时影响着毕业生的求职选择和医疗机构患者寻求医疗服务的质量。因此,了解就业情况对家庭医生就业范围的影响方式,有助于进一步明确如何协助家庭医生在其希望从事的范围内工作并对其进行相应协助,从而确保每一个家庭可以获得更好的医疗服务。

Effects of Fresh Cupressus sempervirens Leaves Infusion on Growth Performance, Intestinal Microbiota and Haemato-Biochemical Parameters in Broilers

The accumulation of growth-promoting antibiotic residues in animal products and the resistance developed by bacteria in poultry farms has led to a search for natural compounds derived from plants. This study was designed to promote the production performance of broiler chickens using fresh Cupressus sempervirens leaves infusion. Fresh Cupressus sempervirens leaves were harvested, washed, chopped and ground to a paste using a blender and fermented for three days in a closed container at a rate of 500 g/L of water. The solution obtained was filtered and added at the rate of 2, 4, 6, 8 and 10 ml/L of drinking water. The chickens fed on the graded level of the solution were compared to a control ration without an additive and positive control group supplemented with 1 g antibiotic/kg feed. At the finisher phase and throughout the study period, water intake increased significantly (P reasing levels of infusion. Feed intake decreased significantly (P rinking water. Live weight and weight gain were significantly (P r with 6 ml/L, while feed conversion significantly (P reased with the same treatment compared with the control treatment without additives (T0). Carcass characteristics were not significantly (P > 0.05) affected by the inclusion of Cupressus sempervirens infusion. Haematological parameters significantly (P rease independently of the rate of incorporation of the infusion into the drinking water, with the exception of rBCs, MCHT and PCT. Serum content in total protein, globulins, LDL cholesterol and triglycerides were significantly (P ressus sempervirens infusion/litre drinking water as compared to all other treatments. AST, ALT, urea, creatine, albumin, total cholesterol and HDL-cholesterol were not significantly affected. Neonatal pneumothorax in congenital diaphragmatic hernia:Be wary of high ventilatory pressures

Background Patients with congenital diaphragmatic hernia(CDH)require invasive respiratory support and higher ventilator pressures may be associated with barotrauma.We sought to evaluate the risk factors associated with pneumothorax in CDH neonates prior to repair.Methods Weretrospectivelyreviewednewborns born withCDHbetween 2014and 2019who developeda pneumothorax,and we matched these cases to patients with CDH without pneumothorax.results Twenty-sixpatientswere included(n=13per group).The pneumothorax group required extracorporeal life support(ECLS)more frequently(85%vs 54%,p=0.04),particularly among type A/B defects(31%vs 7%,p=0.01).The pneumothorax group had higher positive end-expiratory pressure(PEEP)within 1 hour of birth(p=0.02),at pneumothorax diagnosis(p=0.003),and at ECLS(p=0.02).The pneumothorax group had a higher mean airway pressure(Paw)at birth(p=0.01),within 1 hour of birth(p=0.01),and at pneumothorax diagnosis(p=0.04).Using multiple logistic regression with cluster robust SEs,higher Paw(Or 2.2,95%Cl 1.08 to 3.72,p=0.03)and PEEP(Or 1.8,95%CI 1.15 to 3.14,p=0.007)were associated with an increased risk of developing pneumothorax.There was no difference in survival(p=0.09).Conclusions Development of a pneumothorax in CDH neonates is independently associated with higher Paw and higher PEEP.A pneumothorax increases the likelihood of treated with ECLS,even with smaller defect.

MicrorNA in pancreatic ductal adenocarcinoma and its precursor lesions

Pancreatic ductal adenocarcinoma(PDAC) is the 4^(th) deadliest cancer in the United States, due to its aggressive nature, late detection, and resistance to chemotherapy. The majority of PDAC develops from 3 precursor lesions, pancreatic intraepithelial lesions(PanIN), intraductual papillary mucinous neoplasm(IPMN), and mucinous cystic neoplasm. Early detection and surgical resection can increase PDAC 5-year survival rate from 6% for Stage Ⅳ to 50% for Stage Ⅰ. To date, there are no reliable biomarkers that can detect PDAC. MicrorNAs(mirNA) are small noncoding rNAs(18-25 nucleotides) that regulate gene expression by affecting translation of messenger rNA(mrNA). A large body of evidence suggests that mirNAs are dysregulated in various types of cancers. MirNA has been profiled as a potential biomarker in pancreatic tumor tissue, blood, cyst fluid, stool, and saliva. Four mi rNA biomarkers(mir-21, mir-155, mir-196, and mir-210) have been consistently dysregulated in PDAC. Mir-21, mir-155, and mir-196 have also been dysregulated in IPMN and PanIN lesions suggesting their use as early biomarkers of this disease. In this review, we explore current knowledge of mirNA sampling, mir NA dysregulation in PDAC and its precursor lesions, and advances that have been made in using mirNA as a biomarker for PDAC and its precursor lesions.

Improving the reading Skills of ALS Learners Through Project rICE(reading Intervention and Comprehension Enhancement)

One of the major problems in the education sector is the reading skills of learners.Students enrolled in the Alternative Learning System(ALS)also experienced this scenario due to several circumstances.This study focused on the improvement of reading comprehension skills of ALS learners by means of project rICE(reading Intervention and Comprehension Enhancement)as well as the issues and challenges.A quasi-experimental design was utilized in this study which was participated by 54 learners,composed of 27 learners under the experimental group and 27 under the control group.The result revealed that the learners in the experimental group changed from frustration level to instructional level after the implementation of project rICE while the learners in the control group changed from instructional level to inde-pendent level,thereby signifying a significant difference and the effectiveness of the project.Additionally,issues with time and resources were often encountered by the learners.

The effects of plant density of Melastoma malabathricum on the erosion rate of slope soil at different slope orientations

Malaysia’s cut slopes,especially for road lines accommodation,are prone to erosions and landslides.These problems mainly occur due to lack of vegetation cover and strong erosive forces.In addition,the topography factors have also become a major factor affecting soil degradation.Thus,this study is aimed at determining the effects of planting density of a selected species,namely Melastoma malabathricum;one,two,and three seedlings per box,on the erosion rate at selected slopes of different orientation(morning and evening sun) at the Guthrie Corridor Expressway,Selangor.In six months of observation,treatment with three seedlings/box on the morning sun slope showed a lower erosion rate by 69.2%than those with the same treatment on the evening sun slope.In addition,the treatment of the three seedlings recorded at month six(final observation) had the highest reduction of soil saturation level(STL),by 23.6%.Furthermore,the physiological values of the species studied,grown on the morning sun slope,were higher in terms of stomatal conductance and photosynthetic rate by 12.1%and 15.8%(three seedlings/box),respectively.The overall results showed that plant density was inversely related to the STL and erosion rate on the slope.In conclusion,a planting density of three seedlings/box and morning sun orientation gave positive effects on the plant’s physiological performance of the slope,reducing the STL,as well as alleviating the erosion rate of slope soils.
