Superficial Temporal Artery to Middle Cerebral Artery Bypass and endovascular Parent Artery Occlusion in the Treatment of Giant Intracranial Aneurysms

The authors are reporting on a study drawn from unpublished dissertation done by the corresponding author when he completed his neurosurgical training in Paris, France in 2004, few years before the advent of flow diverters. The study was a retrospective review of giant intracranial aneurysms treated by superficial temporal artery to middle cerebral artery bypass combined with endovascular occlusion of the parent artery. From 1990 to 2003, 29 consecutive cases of giant cerebral aneurysms, not suitable to selective treatment were managed in that way. Twenty-one medical records had enough data to allow objective evaluation. Sixteen female and five male patients bearing 21 giant aneurysms were involved. Their mean age was 46 years. The aneurysm was revealed by mass effect in 13 cases and subarachnoid hemorrhage in one case. On admission 19 patients presented with unruptured aneurysms and two have sustained a subarachnoid hemorrhage. The balloon occlusion test before the bypass operation was not tolerated in 18 patients. The treatment was completed in 19 patients and 17 of them had parent artery occlusion with latex detachable balloons. The only death of the series occurred before the endovascular treatment. The mean follow-up period was 30 months. After completion of the treatment, 16 (84%) patients had no symptom. Aneurysm recanalization or rupture was not observed after the parent artery occlusion. With the combination of superficial temporal artery to middle cerebral artery bypass + endovascular parent artery occlusion, 90% of giant intracranial aneurysms untreatable selectively were permanently excluded with a good outcome in 95%.

From the backyard to collective gardens in the “blue-courtyard”: Reversing the process of upgrading historic timber-framed courtyard buildings in Madrid

气候变暖促进了植物的春季物候,打破了气候与植物生长之间原有的平衡。随着极端气候事件出现更加频繁,气候与植物生长之间的不匹配性会影响植物的生存、生长和繁殖。为了防止生长季节在细胞内结冰,植物会采用过冷却策略。然而,气候事件的严重程度和物候期可能会影响植物在霜冻事件后的恢复。本研究通过实验探究了4种不同严重程度的极端霜冻事件如何影响青藏高原上两种优势物种钉柱委陵菜(Potentilla saundersiana,仲夏开花)和小龙胆(Gentiana parvula,夏末开花)的过冷却点和枯黄期。探讨了物候期如何影响钉柱委陵菜对霜冻事件的反应。结果发现,两个物种在遭受霜冻损害时都表现出过冷现象。钉柱委陵菜的平均过冷却点为−6.9℃,并受最低温度、持续时间和物候期的影响,而小龙胆的平均过冷却点为−4.8℃,最低温度和持续时间对其过冷却点影响不显著。此外,两个物种在最低温度(−10℃)处理持续4小时后均快速枯黄。本研究首次探索并定量了青藏高原高寒植物的过冷却点。鉴于高寒植物遭遇霜冻事件的概率不断增加,上述结果对了解高寒植物的生长和生存如何应对极端气候的不利影响具有重要意义。

Floristic Diversity, Stand Structure and Plant Life Traits in the Forest-Savanna Mosaic at Ndjole (Centre Cameroon)

With the aim of assessing floristic diversity, stand structure and vegetation ecology, the present work was carried out in the savanna mosaic forest of the locality of Ndjole (Centre Cameroon). A total of 30 floristic plots of 40 m × 40 m (1600 m2) were delineated in the different ecosystems (forest, forest-savanna contact zone and the savanna). Within each plot, all individuals with a diameter at breast height ≥ 10 cm at 1.30 m were taken into account. ANOVA was used to compare the means of the diversity indices of the different ecosystems, and the DUNCAN test at 5% significance level (XLSTAT 2016) to separate these means. A total of 85 species belonging to 67 genera and 33 families were recorded. That is, 68 species in semi-caducifolia forest, 45 in forest-savanna contact zone and 18 in savanna. Urticaceae, Combretaceae, Apocynaceae, Myristicaceae and euphorbiaceae are the most diverse families in the forest. On the other hand, in contact zones, the euphorbiaceae, Fabaceae, Annonaceae and Moraceae families are the most diversified, while in the savannas, the Combretaceae, Fabaceae and Annonaceae families are the most diversified. Shannon’s diversity index values (1.7 to 2.37 bits) reflect low specific diversity;Piélou’s equitability (0.86 to 0.9) shows good distribution of individuals within species. The diametric structure of woody species shows an inverted “J” shape in all plant communities. This indicates a high regeneration potential characterized by a greater number of small trees than large trees. Mesophanerophytes and megaphanerophytes are dominant in forests and forest-savanna contact zones, and microphanerophytes in the savannas. endozoochory is the main mode of diaspore dissemination. Background: Venous thromboembolism (VTe) is a major public health problem due to its increasing frequency, mortality and management cost. This cost may require major financial efforts from patients, especially in developing countries like ours where less than 7% of the population has health insurance. This study aimed to estimate the direct cost of managing VTe in three reference hospitals in Yaoundé. Methods: This was a cross-sectional retrospective study over a three-year period (from January 1st 2018 to December 31 2020) carried out in the Cardiology departments of the Central and General Hospitals, and the emergency Centre of the city of Yaoundé. All patients managed during the study period for deep vein thrombosis and pulmonary embolism confirmed by venous ultrasound coupled with Doppler and computed tomography pulmonary angiography respectively were included. For each patient, we collected sociodemographic and clinical data as well as data on the cost of consultation, hospital stay, workups and medications. These data were analysed using SPSS version 23.0. Results: A total of 92 patient’s records were analysed. The median age was 60 years [48 - 68] with a sex ratio of 0.53. The median direct cost of management of venous thromboembolism was 766,375 CFAF [536,455 - 1,029,745] or $1415 USD. Management of pulmonary embolism associated with deep vein thrombosis was more costly than isolated pulmonary embolism or deep vein thrombosis. Factors influencing the direct cost of management of venous thromboembolism were: hospital structure (p = 0.015), health insurance (p 0.001), type of pulmonary embolism (p = 0.021), and length of hospital stay (p = 0.001). Conclusion: Management of VTe is a major financial burden for our patients and this burden is influenced by the hospital structure, health insurance, type of pulmonary embolism and length of hospital stay.

Introduction: Human rabies is a major public health problem in many African countries, including Côte d’Ivoire. The present work aims at describing the eco-epidemiological characteristics of human rabies cases recorded in Côte d’Ivoire. Methods: This was a retrospective and prospective cross-sectional descriptive study based on epidemiological surveillance data that ran from september 2014 to december 2017. The clinical data were extracted from the database of the human rabies epidemiological surveillance management service of the National Institute of Public Hygiene’s rabies centre. These data cover the period from 1 January 2012 to 31 December 2017. Results: Human rabies is endemic in Côte d’Ivoire, with most people exposed in the south and especially in the west of the country. There was no association between any of the climatological parameters (temperature, humidity, rainfall) and the occurrence of human rabies during the months of exposure to rabies infection. Conclusion: Ultimately, effective prevention and control of human rabies requires a thorough understanding of the links between climatological parameters and rabies. Health authorities must take ownership of these results if we are to achieve our goal of eliminating rabies by 2030.

Simplified seismic scenario analysis of existing masonry buildings accounting for local site effects

This study discusses the effects of local sites and hazard amplification on the seismic vulnerability assessment of existing masonry *** this context,a rapid seismic evaluation procedure was implemented on an old masonry building stock in the historical center Galata,located inİstanbul,to determine the seismic risk priority of the built *** scenarios were generated for all soil classes,different moment magnitudes,and source-to-site distances to obtain more accurate results for the seismic vulnerability assessment of the studied building ***,damage distributions estimated under nine different scenarios with/without site effects were compared and illustrated in maps to discuss changes in vulnerability owing to amplification *** this study,by re-examining the rapid seismic evaluation procedure by including geo-hazard-based assessment,the importance of site effects on the vulnerability and risk assessment of built heritage was *** proposed framework integrating field data and local site effects is believed to advance the current applications for vulnerability assessment of masonry buildings and provide an improvement in the application of rapid seismic assessment procedures with more reliable results.

Totally Occluded Coarctation of the Aorta in a Young Adult

We report the case of a 20-year-old female patient presenting with a totally occluded coarctation of the aorta. The patient was admitted for the evaluation of resistant grade III high blood pressure. The physical examination revealed a blood pressure gradient between the upper and lower limbs, absent femoral pulses, a diffuse continuous murmur over the chest, and hyperpulsatility of the carotid arteries and sternal notch. A transthoracic echocardiography outlined a narrowing in the caliber of the isthmic aorta without acceleration of flow or gradient. Thoracic CT angiography revealed a complete aortic coarctation with interruption of continuity between segment 3 and the descending aorta.

FSCIL-eACA: Few-Shot Class-Incremental Learning Network Based on embedding Augmentation and Classifier Adaptation for Image Classification

The ability to learn incrementally is critical to the long-term operation of AI systems. Benefiting from the power of few-shot class-incremental learning(FSCIL), deep learning models can continuously recognize new classes with only a few samples. The difficulty is that limited instances of new classes will lead to overfitting and exacerbate the catastrophic forgetting of the old classes. Most previous works alleviate the above problems by imposing strong constraints on the model structure or parameters, but ignoring embedding network transferability and classifier adaptation(CA), failing to guarantee the efficient utilization of visual features and establishing relationships between old and new classes. In this paper, we propose a simple and novel approach from two perspectives: embedding bias and classifier bias. The method learns an embedding augmented(eA) network with cross-class transfer and class-specific discriminative abilities based on self-supervised learning and modulated attention to alleviate embedding bias. Based on the adaptive incremental classifier learning scheme to realize incremental learning capability,guiding the adaptive update of prototypes and feature embeddings to alleviate classifier bias. We conduct extensive experiments on two popular natural image datasets and two medical datasets. The experiments show that our method is significantly better than the baseline and achieves state-of-the-art results.
