Evaluation of Groundwater Quality in the Deep Maastrichtian Aquifer of Senegal Using Multivariate Statistics and Water Quality Index-Based GIS

A regional groundwater quality evaluation was conducted in the deep Maastrichtian aquifer of Senegal through multivariate statistical analysis and a GIS-based water quality index using physicochemical data from 232 boreholes distributed over the whole country. The aim was to 1) identify the water types and likely factors influencing the hydrochemistry, and 2) determine the suitability of groundwater for drinking and irrigation. Results showed that sodium, chloride, and fluoride are highly correlated with electrical conductivity (EC) reflecting the significant contribution of these elements to groundwater mineralization. The principal component analysis evidenced: 1) salinization processes (loaded by Na+, K+, EC, Cl-, F- and HCO3-) controlled by water/rock interaction, seawater intrusion and cation exchange reactions;2) dolomite dissolution loaded by the couple Ca2+ and Mg2+ and 3) localized mixing with upper aquifers and gypsum dissolution respectively loaded by NO3- and SO42-. The hierarchical clustering analysis distinguished four clusters: 1) freshwater (EC = 594 μs/cm) with mixed-HCO3 water type and ionic contents below WHO standard;2) brackish (Na-mixed) water type with moderate mineralization content (1310 μs/cm), 3) brackish (Na-Cl) water type depicted by high EC values (3292 μs/cm) and ionic contents above WHO and 4) saline water with Na-Cl water type and very high mineralization contents (5953 μs/cm). The mapping of the groundwater quality index indicated suitable zones for drinking accounting for 54% of the entire area. The occurrence of a central brackish band and its vicinity, which were characterized by high mineralization, yielded unsuitable groundwater for drinking and agricultural uses. Introduction: Non-communicable diseases constitute a major public health problem, due to their morbi-mortality. The aim was to study knowledge of cardiovascular risk factors among residents of Dakar. Patients and Methods: This was a descriptive cross-sectional study. Our investigations were recruited over a six-month period (October 17, 2021 to March 21, 2022). Results: Two hundred and twenty-six (226) patients were enrolled. The mean age was 45.9 years. The under-40 age group was the most represented with 37.2%. There were 129 men (57.1%), giving a male/female sex ratio of 1.33. The highest level was represented by 159 people (70.4%). Those who responded (heart disease) were in the majority (38.1%). Hypertension was the most common CVD for 101 people (44.7%). One hundred and ninety (190) people (84%) knew the risk factors for CVD. Good knowledge of risk factors concerned 103 people (54.2%). Obesity was the best-known risk factor in 156 cases (69%). The media was the most important source of information for 121 people (53.5%). Hypertensives were the most represented with 8.4%. One hundred and thirty (130) people (57.5%) had previously paid attention to these FDRCVs. Awareness campaigns were the most effective means of communication for 170 cases (75.2%). Sixteen (16) people (35.6%) were taking antihypertensive medication. Walking was the most popular physical activity for 102 people (45%). People with a balanced diet were in the majority, with 174 respondents (77.0%). Conclusion: NCDs are a major cause of morbidity and mortality. It is essential to develop and implement a prevention and management program.

The aim of this study was to investigate the usefulness of combining profiles obtained by using a line probe assay (LPA) originally intended to characterize the resistance of two major anti-tuberculosis drugs to the association of spoligotyping and MIRU-VNTR, in order to improve its discriminatory power. For this purpose, 74 strains of Mycobacterium tuberculosis belonging to the same cluster after spoligotyping were further typed by using the 24 loci MIRU/VNTR. These strains were then tested by the GenoType MTBDRplus, and profiles obtained were analyzed within previously obtained clusters. The combination of spoligotying and MIRU-VNTR led to the consolidation of 56 of them (75.7%) in 9 clusters. Most of the strains (54, 96.4%) were multidrug resistant (MDR). From the 9 initial clusters, the addition of GenoType MTBDRplus helped to define 26 profiles including 11 unique profiles, and 3 original clusters remained undifferentiated. Results obtained express the relevance of combining this method which improved quite significantly the discriminatory power in typing Mycobacterium tuberculosis.

Hepatitis B virus outreach to immigrant population in Greater Boston Area: Key to improving hepatitis B knowledge

aim To characterize the understanding of hepatitis B virus(HBV) and determine if outreach improves HBV understanding among Greater Boston Area immigrants. METHODS Six outreach sessions were held in various community venues in the Greater Boston Area. Verbal consent was obtained from participants prior to starting each session. Each session included a pre-session questionnaire, followed by a teaching session, and then a post-session questionnaire. In person interpreters were present for translation during the teaching session and assistance for questionnaire completion when needed. The questions were developed based on the HBV clinical experience of physicians who serve largely immigrant populations. Questionnaires included Likerttype scale, open-ended, and true-false questions. All results were anonymous. RESULTS One hundred and one people participated in this study. Participants were 30% male with ages ranging from 19 to 87 years. The study population included immigrants from 21 countries, as well as seven United States-born participants. The greatest numbers of participants were from Somalia(44%), Morocco(10%), and Cameroon(8%). Pre session questionnaires revealed that 42% of participants were unaware that HBV can cause cancer, and 50% were unaware that therapies for HBV exist. Our brief teaching intervention led to improved scores on post session questionnaires. For example, at baseline, 58% of participants responded correctly to the question "HBV infection can cause scarring of the liver and liver cancer", whereas 79% of participants responded correctly after the teaching session(P = 0.01). Furthermore, the mean of total correct answers in the true or false portion of the questionnaire increased from 5.5 to 7.6(P < 0.001).CONCLUSION A teaching session targeting Boston Immigrants atrisk for HBV helped improve scores on HBV knowledge questionnaires. Outreach may empower at-risk patients to pro-actively seek HBV care.

Introduction-Objective: Tuberculosis (TB) is the leading cause of death from a single infectious agent worldwide. The predictors of mortality due to TB are rarely evaluated in Senegal. The aim of our study was to identify factors associated with related TB death in two treatment centers in Dakar, Senegal. Method: We conducted a prospective descriptive-analytical study dealing with TB patients followed in the Department of Infectious and Tropical Diseases at FANN Teaching Hospital and Dakar Principal Hospital and in the Department of Pulmonology at Dakar Principal Hospital from March 1st, 2019 to February 29th, 2020. Univariate and multivariate logistic regressions were performed to identify the associated factors of death. Results: Two hundred eighty-two patients in the Department of Infectious and Tropical Diseases at FANN teaching hospital (57%), in the Department of Infectious and Tropical Diseases at Dakar Principal Hospital (31%), and in the Department of Pulmonology at Dakar Principal Hospital (12%) were included in the study. The mean age was 39 ± 16 years and the male to female ratio was 2.2. Isolated pulmonary TB, isolated extrapulmonary TB, and both pulmonary and extrapulmonary TB were present in 33.3%, 30.5% and 36.2% of cases, respectively. Twenty-two patients died, corresponding to a mortality rate of 7.8%. Factors independently associated with death were age ≥ 60 years (26.2 [3.6 - 191.2]) compared to 16 - 40 years’ age group, HIV infection (7.2 [1.4 - 36.9]), neurological localization of TB (13.19 [3.2 - 54.3]), and hemoglobin level Conclusion: Tuberculosis remains a fatal disease despite therapeutic advances. Better knowledge of associated factors of death from TB may help to reduce its mortality.

Background: Malaria is the most widespread parasitic disease and remains a public health priority worldwide. The severe form is fatal if not treated early and appropriately. The aim was to carry out a descriptive and analytical study of the factors associated with mortality during severe malaria in children attending emergency departments in Dakar, Senegal. Methods: This is a prospective, observational and analytical study conducted over a 6-month period (July 1 to December 31, 2022), focusing on children hospitalized for severe malaria according to WHO severity criteria. Results: A total of 403 patients were hospitalized, including 78 cases of severe malaria (19.35%). Males predominated (60.26%) (sex ratio 1.51). The average age was 6.56 years [8 months - 14 years], with the [5 - 10 years] age group the most represented (40.26%). The average consultation time was 5.33 days (1 - 19 days). The main reasons for consultation were fever (70.51%), vomiting (24.35%) and convulsions (14.10%). Biological signs of severity were severe anemia (17.95%), renal failure (6.4%) and hypoglycemia (3.85%). Thrombocytopenia was noted in 52.56% of patients, including 32.05% of severe cases (sis in 61.41% of cases. Hyponatremia was noted in 39.74% of cases and hyperkalemia in 2 patients. Artesunate was the main drug used (93.59% of cases). Mortality was estimated at 1.5%. Factors leading to death were coma (P < 0.01), respiratory distress (P Conclusion: Malaria is still a public health problem, with a high mortality rate in emergency departments. Reducing this mortality rate requires effective management of the factors associated with death.

Evolution of the Physico-Chemical Quality of the Water in the Manantali Dam Reservoir from 1989 (One Year after Impoundment) to 2022

The Manantali dam is located on the Bafing River in the Kayes region of Mali. It is the main tributary of the Senegal River. The water released from the dam provides year-round access to water for drinking, irrigation and livestock watering. The Manantali dam regulates the river’s flow, generates energy and supports agricultural development in all three countries (Mali, Mauritania, and Senegal). The aim of this article is to monitor changes in the physico-chemical quality of the water in the dam’s reservoir from 1989 (one year after the dam was impounded) to 2022. In order to carry out this work, we analysed the evolution of physico-chemical parameters in Stations 1 and 3 of the dam, on the basis of fluctuating water levels in the reservoir. The results obtained show a similarity in the parameters measured at the two stations, except for iron content, which is higher at Station 3. The average pH is weakly basic (7.44 at Station 1 and 7.29 at Station 3) and the average water temperature is between 26.5°C at Station 1 and 26.2°C at Station 3. The average conductivity of the water at Station 1 is 38.8 μS/cm and 39.8 μS/cm at Station 3, attesting to low mineralization of the water. The oxygen content of 5.75 mg/L at Station 1 and 5.00 mg/L at Station 3 shows good oxygenation of the water, which is favorable for the development of most fish and aquatic plant species. Ammonium levels of 0.02 mg/L at Station 1 and 0.06 mg/L at Station 3 show that the water is not contaminated. In addition, the Water Quality Index (WQI) was calculated, showing excellent water quality at Station 1 and good quality at Station 3. Overall, the results obtained show that the water is of good quality, enabling the authorities of the Organization for the Development of the Senegal River (OMVS) to achieve their development objectives.

Study of the Biological Quality of the Water in the Manantali Dam Reservoir: Analysis of the Fish Fauna and Plankton Communities

The aim of this study of the Manantali dam reservoir is to investigate the biological quality of the water through inventories and analysis of the ichthyofauna and planktonic communities (zooplankton and phytoplankton). Inventories of ichthyofauna and plankton communities were carried out quarterly from 2003 to 2022. The fish populations studied on the basis of bioindicators (species richness, diversity index, equitability index, diversification index) identified 29 species belonging to 10 families. The Cichlidae family is the most represented, while the Schilbeidae and Tetraodontioae families are less represented in terms of number of species. The species Hydrocynus forskalii and Brycinus nurse with relative abundances of 40.12% and 21.4% respectively, both from the family Alestidae, are the most abundant. The species H. forskalii has a frequency of occurrence of 100%. The specific diversity index of 2.9 and the equitability index of 0.6 show an average diversity of fish species. The diversification index of 2.9 indicates a theoretical number of habitats of 2 to 3. The planktonic community is made up of zooplankton and phytoplankton. The inventory of zooplankton shows the presence of 28 zooplankton species belonging to 9 families of the three zooplankton groups Rotifera, Cladocera and Copepoda. Analysis of the frequencies of occurrence of the various taxa showed that the characteristic species of Lake Manantali are the omnipresent rotifers Keratella cochlearis, Keratella quadrata and Trichocerca chattoni. For phytoplankton, the inventories carried out show the presence of 65 species belonging to 5 families (Chlorophycees, Diatomees, Dinophycees, Chrysophycees and Cyanophycees). An analysis of the frequency of occurrence of the various taxa shows that the characteristic species of Lake Manantali are Chlorophycees, with 7 species: Desmidium baileyi, Micrasterias alata, Sphaerocystis schroeteri, Spondylosium sp., Staurastrum heimii, Staurastrum subanchora, Staurodesmu

Most newborn deaths occur in two regions of the world, South Asia and sub-Saharan Africa. In Senegal, much progress has been made over the past two decades with a reduction in overall infant mortality by 38%. However, the decline in neonatal mortality has been slower during the same period. The objective of this study is to assess neonatal mortality, to determine the different causes and to make recommendations for improving care in rural areas. This is a retrospective study with a descriptive and analytical aim carried out in the pediatrics department of the Diourbel regional hospital, 130 km from Dakar, the Senegalese capital, over a 12-month period from January 1 to January 31, December 2018. All newborns hospitalized in the pediatric ward during the study period were included. Overall mortality was 30.6%, newborns between 0 and 7 days accounted for 90.6% of deaths, INBORNs accounted for 62.3% of deaths. Newborns whose mother’s age was between 20 and 35 years of age accounted for 69.3% of deaths. Newborns of first-time mothers accounted for 40.2% of deaths and those of multiparous mothers 31%. Newborns who did not reach term accounted for 58.9% of deaths. Newborns admitted for respiratory distress represented the majority of deaths 45.5% (n = 93) followed by those with low birth weight 32.5 (n = 65) followed by those with neurological manifestations 30.5 (n = 62). Conclusion: Neonatal mortality is very high in the Diourbel region and the main cause is the lack of human resources and a very insufficient technical platform. The fight against this mortality involves improving the technical platform and recruiting sufficient and well-trained staff.
