Successful outcomes of unilateral vs bilateral pedicle screw fixation for lumbar interbody fusion:A meta-analysis with evidence grading

BACKGROUND Whether it’s better to adopt unilateral pedicle screw(UPS)fixation or to use bilateral pedicle screw(BPS)one for lumbar degenerative diseases is still controversially undetermined.aiM To make a comparison between UPS and BPS fixation as to how they work efficaciously and safely in patients suffering from lumbar degenerative diseases.METHODS We have searched a lot in the databases through 2020 with index terms such as“unilateral pedicle screw fixation”and“bilateral pedicle screw fixation.”Only randomized controlled trials and some prospective cohort studies could be found,yielding 15 studies.The intervention was unilateral pedicle screw fixation;Primarily We’ve got outcomes of complications and fusion rates.Secondarily,we’ve achieved outcomes regarding total blood loss,operative time,as well as length of stay.Softwares were installed and utilized for subgroup analysis,analyzing forest plots,sensitivity,heterogeneity,forest plots,publication bias,and risk of bias.RESULTS Fifteen previous cases of study including 992 participants have been involved in our meta-analysis.UPS had slightly lower effects on fusion rate[relative risk(RR)=0.949,95%CI:0.910 to 0.990,P=0.015],which contributed mostly to this metaanalysis,and similar complication rates(RR=1.140,95%CI:0.792 to 1.640,P=0.481),Δvisual analog scale[standard mean difference(SMD)=0.178,95%CI:-0.021 to 0.378,P=0.080],andΔOswestry disability index(SMD=-0.254,95%CI:-0.820 to 0.329,P=0.402).In contrast,an obvious difference has been observed inΔJapanese Orthopedic Association(JOA)score(SMD=0.305,95%CI:0.046 to 0.563,P=0.021),total blood loss(SMD=-1.586,95%CI:-2.182 to-0.990,P=0.000),operation time(SMD=-2.831,95%CI:-3.753 to-1.909,P=0.000),and length of hospital stay(SMD=-0.614,95%CI:-1.050 to-0.179,P=0.006).CONCLUSION Bilateral fixation is more effective than unilateral fixation regarding fusion rate after lumbar interbody fusion.However,JOA,operation time,total blood loss,as well as length of stay were improved for unilateral fixation.

Combustion mechanism of benzene in iron ore sintering process:experimental and simulation

Benzene is a typical component of volatile organic compounds(VOCs)in the iron ore sintering flue gas.The combustion behavior of benzene directly affects the emission of VOCs in iron ore sintering process.The effects of temperature,benzene,and oxygen concentrations on the conversion ratio of benzene were investigated by experiments and numerical simulation.The experiments were carried out in a tube reactor at temperatures of 773-1098 K,benzene concentrations of 0.01-0.03 vol.%,and oxygen concentrations of 10-21 vol.%.The numerical simulation was performed with the plug flow model in the CHEMKIN program based on a kinetic model that consists of 132 chemical species and 772 elementary step-like reactions.The experimental results reveal that increasing the temperature and benzene concentration could signifi-cantly promote benzene combustion.It is attributed to the increase in the reaction rates of all steps in the pathway for forming CO_(2)and H_(2)O.In addition,due to the large equivalent ratio of oxygen to benzene,the conversion ratio of benzene remained constant at different oxygen concentrations.The simulation results were in good agreement with the experimental results and indicated that six elementary reactions dominated the formations of CO_(2)and H_(2)O.The oxidations of C_(6)H_(5)O,CO,and C_(5)H_(4)O intermediates to CO_(2)were the limiting steps in the reaction pathways.

Preoperative blood markers and intra-abdominal infection after colorectal cancer resection

BACKGROUND Colorectal cancer(CRC)has one of the highest morbidity and mortality rates among digestive tract tumors.Intra-abdominal infection(Iai)is a common postoperative complication that affects the clinical outcomes of patients with CRC and hinders their rehabilitation process.However,the factors influencing abdominal infection after CRC surgery remain unclear;further,prediction models are rarely used to analyze preoperative laboratory indicators and postoperative complications.aiM To explore the predictive value of preoperative blood markers for Iai after radical resection of CRC.METHODS The data of 80 patients who underwent radical resection of CRC in the Anorectal Surgery Department of Suzhou Hospital affiliated with Anhui Medical University were analyzed.These patients were categorized into Iai(n=15)and non-Iai groups(n=65)based on whether Iai occurred.Influencing factors were compared;general data and laboratory indices of both groups were identified.The relationship between the indicators was assessed.Further,a nomogram prediction model was developed and evaluated;its utility and clinical applic-ability were assessed.RESULTS The risk factors for Iai after radical resection of CRC were neutrophil-lymphocyte ratio(NLR),platelet-lymphocyte ratio(PLR),systemic immune-inflammation index(SII),and carcinoembryonic antigen(CEA)levels.NLR was correlated with PLR and SII(r=0.604,0.925,and 0.305,respectively),while PLR was correlated with SII(r=0.787).The nomogram prediction model demonstrated an area under the curve of 0.968[95%confidence interval(CI):0.948-0.988]in the training set(n=60)and 0.926(95%CI:0.906-0.980)in the validation set(n=20).The average absolute errors of the calibration curves for the training and validation sets were 0.032 and 0.048,respectively,indicating a good model fit.The decision curve analysis curves demonstrated high net income above the 5%threshold,indicating the clinical practicality of the model.CONCLUSION The nomogram model constructed using NLR,PLR,SII,and CEA levels had good accur

Plants affect the horizontal transmission of a new densovirus infecting the green peach aphid Myzus persicae by modulating honeydew production

In a tritrophic context of plant-insect-entomopathogen,plants play important roles in modulating the interaction of insects and their pathogenic viruses.Currently,the influence of plants on the transmission of insect viruses has been mainly studied on baculoviruses and some RNA viruses,whereas the impact of plants on other insect viruses is largely unknown.Here,we identified a new densovirus infecting the green peach aphid Myzus persicae and tested whether and how host plants influence the transmission of the aphid densovirus.The complete single-stranded DNA genome of the virus,M.persicae densovirus 2,is 5727 nt and contains inverted terminal repeats.Transcription and phylogenetic analysis indicated that the virus was distinct from other a few identified aphid densoviruses.The virus abundance was detected highly in the intestinal tract of aphids,compared with the lower level of it in other tissues including head,embryo,and epidermis.Cabbage and pepper plants had no obvious effect on the vertical transmission and saliva-mediated horizontal transmission of the virus.However,the honeydew-mediated horizontal transmission among aphids highly depended on host plants(65%on cabbages versus 17%on peppers).Although the virus concentration in the honeydew produced by aphids between 2 plants was similar,the honeydew production of the infected aphids reared on peppers was dramatically reduced.Taken together,our results provide evidence that plants influence the horizontal transmission of a new densovirus in an aphid population by modulating honeydew secretion of aphids,suggesting plants may manipulate the spread of an aphid-pathogenic densovirus in nature.

Intelligent reflecting surfaces-assisted millimeter wave communication:Channel estimation based on deep learning

In response to the challenge posed by the complexity of the system and the difficulty in obtaining accurate channel state information(CSI)for millimeter wave communication assisted by intelligent reflecting surfaces(IRS),we propose a deep learning-based channel estimation scheme.The proposed scheme employs a hybrid active/passive IRS architecture,wherein the least square(LS)algorithm is initially utilized to acquire the channel estimate from the active elements.Subsequently,this estimation is interpolated to obtain a preliminary channel estimation and ultimately refined into an accurate estimate of the channel using the channel super-resolution convolutional neural network(Chan-SRCNN)deep learning network.The simulation results demonstrate that the proposed scheme surpasses LS,orthogonal matching pursuit(OMP),synchronous OMP(SOMP),and deep neural network(DNN)channel estimation algorithms in terms of normalized mean squared error(NMSE)performance,thereby validating the feasibility of the proposed approach.

Electrically tunable phase-change metasurface for dynamic infrared thermal camouflage

Dynamic infrared thermal camouflage technology has attracted extensive attention due to its ability to thermally conceal targets in various environmental backgrounds by tuning thermal emission.The use of phase change materials(PCMs)offers numerous advantages,including zero static power,rapid modulation rate,and large emissivity tuning range.However,existing PCM solutions still encounter several practical application challenges,such as temperature uniformity,amorphization achievement,and adaptability to different environments.In this paper,we present the design of an electrically controlled metal-insulator-metal thermal emitter based on a PCM metasurface,and numerically investigate its emissivity tunability,physical mechanisms,heat conduction,and thermal camouflage performance across different backgrounds.Furthermore,the influence of the quench rate on amorphization was studied to provide a guidance for evaluating and optimizing device structures.Simulation results reveal that the thermal emitter exhibits a wide spectral emissivity tuning range between 8 and 14μm,considerable quench rates for achieving amorphization,and the ability to provide thermal camouflage across a wide background temperature range.Therefore,it is anticipated that this contribution will promote the development of PCM-based thermal emitters for practical dynamic infrared thermal camouflage technology with broad applications in both civilian and military domains.

Comparative Analysis of ARIMA and LSTM Model-Based Anomaly Detection for Unannotated Structural Health Monitoring Data in an Immersed Tunnel

Structural Health Monitoring(SHM)systems have become a crucial tool for the operational management of long tunnels.For immersed tunnels exposed to both traffic loads and the effects of the marine environment,efficiently identifying abnormal conditions from the extensive unannotated SHM data presents a significant challenge.This study proposed amodel-based approach for anomaly detection and conducted validation and comparative analysis of two distinct temporal predictive models using SHM data from a real immersed tunnel.Firstly,a dynamic predictive model-based anomaly detectionmethod is proposed,which utilizes a rolling time window for modeling to achieve dynamic prediction.Leveraging the assumption of temporal data similarity,an interval prediction value deviation was employed to determine the abnormality of the data.Subsequently,dynamic predictive models were constructed based on the Autoregressive Integrated Moving Average(ARIMA)and Long Short-Term Memory(LSTM)models.The hyperparameters of these models were optimized and selected using monitoring data from the immersed tunnel,yielding viable static and dynamic predictive models.Finally,the models were applied within the same segment of SHM data,to validate the effectiveness of the anomaly detection approach based on dynamic predictive modeling.A detailed comparative analysis discusses the discrepancies in temporal anomaly detection between the ARIMA-and LSTM-based models.The results demonstrated that the dynamic predictive modelbased anomaly detection approach was effective for dealing with unannotated SHM data.In a comparison between ARIMA and LSTM,it was found that ARIMA demonstrated higher modeling efficiency,rendering it suitable for short-term predictions.In contrast,the LSTM model exhibited greater capacity to capture long-term performance trends and enhanced early warning capabilities,thereby resulting in superior overall performance.

Attitude Tracking Control for Rigid Spacecraft with Arbitrary Convergence Time

In this paper, attitude tracking control with arbitrary convergence time for rigid spacecraft is considered. First, a novel time-varying sliding function is proposed to achieve free-will arbitrary time convergence when the system states reside on the sliding surface. With such a sliding function, an attitude tracking controller is designed to guarantee that the states of the closed-loop system converge to the sliding surface within a predetermined time in the presence of external disturbances. The free-will arbitrary time convergences of the closed-loop system and sliding function are illustrated by numerical simulations.

Interpretation and characterization of rate of penetration intelligent prediction model

Accurate prediction of the rate of penetration(ROP)is significant for drilling optimization.While the intelligent ROP prediction model based on fully connected neural networks(FNN)outperforms traditional ROP equations and machine learning algorithms,its lack of interpretability undermines its credibility.This study proposes a novel interpretation and characterization method for the FNN ROP prediction model using the Rectified Linear Unit(ReLU)activation function.By leveraging the derivative of the ReLU function,the FNN function calculation process is transformed into vector operations.The FNN model is linearly characterized through further simplification,enabling its interpretation and analysis.The proposed method is applied in ROP prediction scenarios using drilling data from three vertical wells in the Tarim Oilfield.The results demonstrate that the FNN ROP prediction model with ReLU as the activation function performs exceptionally well.The relative activation frequency curve of hidden layer neurons aids in analyzing the overfitting of the FNN ROP model and determining drilling data similarity.In the well sections with similar drilling data,averaging the weight parameters enables linear characterization of the FNN ROP prediction model,leading to the establishment of a corresponding linear representation equation.Furthermore,the quantitative analysis of each feature's influence on ROP facilitates the proposal of drilling parameter optimization schemes for the current well section.The established linear characterization equation exhibits high precision,strong stability,and adaptability through the application and validation across multiple well sections.

Accidental placement of venous return catheter in the superior vena cava during venovenous extracorporeal membrane oxygenation for severe pneumonia: A case report

BACKGROUND Venovenous extracorporeal membrane oxygenation(V-V ECMO)has become an important treatment for severe pneumonia,but there are various complications during the treatment.This article describes a case with severe pneumonia success-fully treated by V-V ECMO,but during treatment,the retrovenous catheter,which was supposed to be in the right internal vein,entered the superior vena cava directly in the mediastinum.The ECMO was safely withdrawn after multidiscip-linary consultation.Our experience with this case is expected to provide a reference for colleagues who will encounter similar situations.CASE SUMMARY A 64-year-old man had severe pulmonary infection and respiratory failure.He was admitted to our hospital and was given ventilation support(fraction of inspired oxygen 100%).The respiratory failure was not improved and he was treated by V-V ECMO,during which the venous return catheter,which was supposed to be in the right internal vein,entered the superior vena cava directly in the mediastinum.There was a risk of massive mediastinal bleeding if the catheter was removed directly when the ECMO was withdrawn.Finally,the patient underwent vena cava angiography+balloon attachment+ECMO with-drawal in the operating room(prepared for conversion to thoracotomy for vascular exploration and repair at any time during surgery)after multidiscip-linary consultation.ECMO was safely withdrawn,and the patient recovered and was discharged.CONCLUSION Patients may have different vascular conditions.Multidisciplinary cooperation can ensure patient safety.Our experience will provide a reference for similar cases.
