Objective To evaluate the differences in 24-hour ambulatory blood pressure (BP) in older patients with hypertension treated with the five major classes of antihypertensive drugs,as monotherapy or dual combination therapy,to improve daytime and nighttime BP control. Methods We enrolled 1920 Chinese community-dwelling outpatients aged ≥ 60 years and compared ambulatory BP values and ambulatory BP control (24-hour BP < 130/80 mmHg;daytime mean BP < 135/85 mmHg;and nighttime mean BP < 120/70 mmHg),as well as nighttime BP dip patterns for monotherapy and dual combination therapy groups. Results Patients’ mean age was 71 years,and 59.5% of patients were women. Calcium channel blockers (CCBs) constituted the most common (60.3% of patients) monotherapy,and renin–angiotensin system (RAS) blockers combined with CCBs was the most common (56.5% of patients) dual combination therapy. Monotherapy with beta-blockers (BB) provided the best daytime BP control. The probabilities of having a nighttime dip pattern and nighttime BP control were higher in patients receiving diuretics compared with CCBs (OR = 0.52,P = 0.05 and OR = 0.41,P = 0.007,respectively). Patients receiving RAS/diuretic combination therapy had a higher probability of having controlled nighttime BP compared with those receiving RAS/CCB (OR = 0.45,P = 0.004). Compared with RAS/diuretic therapy,BB/CCB therapy had a higher probability of achieving daytime BP control (OR = 1.27,P = 0.45). Conclusions Antihypertensive monotherapy and dual combination drug therapy provided different ambulatory BP control and nighttime BP dip patterns. BB-based regimens provided lower daytime BP,whereas diuretic-based therapies provided lower nighttime BP,compared with other antihypertensive regimens.

People-centric activity recognition is one of the most critical technologies in a wide range of real-world applications,including intelligent transportation systems, healthcare services, and brain-computer interfaces. Large-scale data collection and annotation make the application of machine learning algorithms prohibitively expensive when adapting to new tasks. One way of circumventing this limitation is to train the model in a semi-supervised learning manner that utilizes a percentage of unlabeled data to reduce the labeling burden in prediction tasks. Despite their appeal, these models often assume that labeled and unlabeled data come from similar distributions, which leads to the domain shift problem caused by the presence of distribution gaps. To address these limitations, we propose herein a novel method for people-centric activity recognition,called domain generalization with semi-supervised learning(DGSSL), that effectively enhances the representation learning and domain alignment capabilities of a model. We first design a new autoregressive discriminator for adversarial training between unlabeled and labeled source domains, extracting domain-specific features to reduce the distribution gaps. Second, we introduce two reconstruction tasks to capture the task-specific features to avoid losing information related to representation learning while maintaining task-specific consistency. Finally, benefiting from the collaborative optimization of these two tasks, the model can accurately predict both the domain and category labels of the source domains for the classification task. We conduct extensive experiments on three real-world sensing datasets. The experimental results show that DGSSL surpasses the three state-of-the-art methods with better performance and generalization.

aiM: To investigate the levels of serum soluble intercellular adhesion molecules-1 (sICAM-1) and neutrophilic expression of CD18 in patients with various stages of diabetic retinopathy and to determine their different expression pattern in the development of diabetic retinopathy(DR). METHODS: Levels of serum sICAM-1 and CD18 on the surface of neutrophile were measured in 41 DR patients, they were classified in three subgroups according to the stage of retinopathy as determined by fund's ophthalmoscopy; 10 control subjects were also studied. sICAM-1 were measured by enzyme-linked immunosorbent assay and CD18 by flow cytometry. RESULTS: The neutrophilic CD18 expression and serum sICAM-1 level were all significantly elevated in all diabetic subgroups compared to control subjects (P <0.01). The differences of CD18 and sICAM-1 among the diabetic subgroups were significant in CD18 but not in sICAM-1. The progression of retinopathy was associated with an increase both in CD18 and in sICAM-1 levels by simple correlation analysis (beta =0.74, P<0.001; beta =0.38, P<0.01, respectively). But stepwise multiple regression analysis revealed that only CD18 Was independent determinant of retinopathy (beta =1.04, P<0.01). CONCLUSION: Our results confirm the contribution of endothelial and neutrophilic activation in the development of DR as indicated by increased levels of CD18 and sICAM-1. However, a direct implication of CD18 and ICAM-1 in the progression of DR can be supported only in the CD18 but not ICAM-1. CD18 and ICAM-1 may play different role in the development of diabetic retinopathy.

目的:探讨完全性左束支传导阻滞(CLBBB)患者QRS时限与左室射血分数(LVEF)的相关性。方法:选择2012年2月至2013年6月在我院经心电图诊断为左束支传导阻滞的患者213例。根据QRS波时限,患者被分为CLBBB组(182例)和不完全性左束支传导阻滞组(ICLBBB组,31例)。应用线性相关分析分析QRS波时限与LVEF的相关性,使用受试者工作特征曲线(ROC)分析QRS波时限预测LVEF<50%的最佳临界点。结果:与ICLBBB组比较,CLBBB组QRS波时限[(104.61±8.85)ms比(149.36±17.25)ms]显著升高,LVEF[(54.26±4.96)%比(45.22±12.57)%]显著降低,P均<0.01。线性相关分析显示,CLBBB患者QRS波时限与LVEF呈显著负相关(r=-0.55,P=0.001)。ROC曲线分析QRS波时限预测LVEF<50%的最佳临界点为151ms,曲线下面积为0.79(P=0.001),敏感性为68.1%,特异性为83.5%。结论:完全性左束支阻滞患者的QRS波时限与左室射血分数显著负相关,可以作为临床预测左室射血分数减低的一个简便指标。

The principle of a navigation constellation composed of SIGSO communication satellites

The Chinese Area Positioning System （CAPS）, a navigation system based on geostafionary orbit （GEO） communication satellites, was developed in 2002 by astronomers at Chinese Academy of Sciences. Extensive positioning experiments of CAPS have been performed since 2005. On the basis of CAPS, this paper studies the principle of a navigation constellation composed of slightly inclined geostationary orbit （SIGSO） communication satellites. SIGSO satellites are derived from GEO satellites which are near the end of their operational life by inclined orbit operation. Considering the abundant frequency resources of SIGSO satellites, multi-frequency observations could be conducted to enhance the precision of pseudorange measurements and ameliorate the positioning performance. A constellation composed of two GEO satellites and four SIGSO satellites with an inclination of 5° can provide service to most of the territory of China with a maximum position dilution of precision （PDOP） over 24 h of less than 42. With synthetic utilization of the truncated precise code and a physical augmentation factor in four frequencies, the navigation system with this constellation is expected to obtain comparable positioning performance to that of the coarse acquisition code of the Global Positioning System （GPS）. When the new method of code-carrier phase combinations is adopted, the system has the potential to possess commensurate accuracy with the precise code in GPS. Additionally, the copious frequency resources can also be used to develop new anti-interference techniques and integrate navigation and communication.

光催化水解制氢为解决能源危机和环境污染问题提供了一种可行的策略.本文通过第一性原理计算,提出了单层Janus Ga_(2)SSe是一种有效的水解光催化剂.结构稳定性分析表明,单层Janus Ga_(2)SSe具有能量和动力学稳定性.电子性质表明,单层Janus Ga_(2)SSe属于间接带隙半导体,有利于降低载流子的复合几率.光催化性能分析表明,单层Janus Ga_(2)SSe的能带边缘位置适合PH=0~14宽范围的光催化水解.此外,单层Janus Ga_(2)SSe具有较高的紫外光和可见光吸收、较大的内建电场、较小的载流子有效质量保证了光生载流子有效分离并快速迁移到反应活性位点参与水的氧化还原反应.研究结果为单层Janus Ga_(2)SSe的合成及其作为水解光催化剂的潜在应用提供了有价值的理论指导.
