Role of biomarkers in community-acquired pneumonia management

Background:The impact of the coronavirus disease 2019(COVID-19)pandemic on patient decision making remains *** study aimed to investigate the effect of the pandemic on treatment preferences of patients with proximal ureteral stones.Materials and methods:Retrospective data regarding treatment preferences of patients diagnosed with symptomatic proximal ure-teral stones between July 2018 and November 2021 at a single center were *** from 493 patients were analyzed according to 2 groups,including patients diagnosed during the COVID-19 pandemic and those diagnosed during an equivalent period of time be-fore the pandemic.Results:Preference for conservative treatment increased during the COVID-19 pandemic(p=0.009).In patients who had previously undergone shock wave lithotripsy(SWL),the preference for SWL decreased and the preference for conservative treatment increased during the COVID-19 pandemic(p=0.042).Multiple logistic regression analysis revealed a significant correlation between a preference for conservative treatment during the pandemic and no prior spontaneous stone passage(p=0.003;odds ratio[OR],2.48;95％con-fidence interval[CI],1.45-4.23),no hydronephrosis(p=0.035;OR,3.57;95％Cl,1.34-9.49),and a visual analog scale score of 4 or less(p=0.018;OR,1.97;95％Cl,1.15-3.38).Conclusions:A significant increase in the preference for conservative treatment was observed among patients diagnosed during the pandemic,and patients with a history of SWL demonstrated a preference shift from SWL to conservative treatment.

BACKGROUND The February 6,2023,twin earthquakes in Türkiye caused significant structural damage and a high number of injuries,particularly affecting the spine,which underscores the importance of understanding the distribution and nature of vertebral injuries in disaster *** To investigate the distribution of radiological findings of vertebral injuries in patients referred to a major tertiary center during the February 6,2023 twin earthquakes in Tü*** With the approval of the institutional ethics committee,1216 examinations of 238 patients transferred from the region to a tertiary major hospital after the twin earthquakes of February 6,2023,were retrospectively analyzed for spine *** Spine computed tomography(CT)scans were performed in 192 of 238 patients with a suspected spinal injury,42 of whom also had an magnetic resonance imaging(MRI).In 86 of 192 patients(44.79%;M:F=33:53)a spinal fracture was detected on CT and in 33 of 42 patients(78.57%;M:F=20:13)a spinal injury was found on *** the 86 patients in whom vertebral injury was detected,fractures were detected in the Denis-B group in 33,Denis-C in 4,Denis-D in 20 and Denis-E in 11 *** the vertebral bodies:40"compression fractures",17"burst fractures",5"translational dislocation fractures",5"flexion-distraction fractures"and 58"prolonged forced fetal posture fractures"were *** addition,isolated transverse or spinous process fractures were found in eighteen *** Our study highlights the prevalence and diverse spectrum of spinal injuries following the February 6,2023 twin earthquakes in Turkey underscoring the urgent need for effective management strategies in similar disaster scenarios,and emphasizing the"prolonged forced fetal posture"damage we encountered in earthquake victims who remained under the collapse for a long time.

We present a retrospective analysis of three cases of cardiac hydatidosis,who underwent surgery between 2010 and *** patients had a lesion in the interventricular septum,whereas one patient had the lesion in apicoinferior wall of *** diagnosis was made by echocardiography,but magnetic resonance imaging was utilized to assess cyst activity and extend of *** patients were placed on cardiopulmonary *** postoperative complication or death *** patients discharged uneventfully and all of them were free from hydatid disease at two years *** concluded that cardiac hydatid cysts should be removed surgically regardless of their location or extent,even in asymptomatic patients.

Mechanical properties and microstructure of composites produced by recycling metal chips

In this study, the processing and mechanical properties of porous metal matrix composites(MMCs) composed of spheroidal cast iron chips(GGG40) and bronze chips(CuSn10) and formed by hot isostatic pressing were investigated. Bronze chips(CuSn10) were used as a matrix component, and spheroidal cast iron(GGG40) chips were used as a reinforcement component. The MMCs were produced with different CuSn10 contents(90 wt%, 80 wt%, 70 wt%, and 60 wt%). The hot isostatic pressing process was performed under three different pressures and temperatures. The produced MMCs were characterized using density tests, Brinell hardness tests, and compression tests. In addition, the consolidation mechanism was investigated by X-ray diffraction(XRD) analysis and scanning electron microscopy. The test results were compared with those for bulk CuSn10 and bulk GGG40. Mechanical tests results revealed that the metallic chips can be recycled by using hot pressing and that the mechanical properties of the produced MMCs were similar to those of bulk CuSn10. XRD and microscopy studies showed that no intermetallic compounds formed between the metallic chips. The results showed that the CuSn10 and GGG40 chips were consolidated by mechanical interlocking.

AIM To investigate the causes of the recurrent carpal tunnel syndrome(CTS) and implemented surgical *** Four hundred and eighty-seven patients, who were diagnosed with CTS and underwent surgical intervention between October 2016 and September 2007, were evaluated in this retrospective study. The age, gender, physical evaluation findings, electrophysiological examination reports and implemented surgical treatment methods were *** Thirty-nine of the cases were operated due to recurrent CTS. Further examination of the patients with recurrent CTS revealed that ten cases had diabetic polyneuropathy, three cases had hypothyroidism, two cases had rheumatoid arthritis and one case had systemic amyloidosis. Postoperative electromyography confirmed the neuropathy was due to systemic diseases. The remaining 23 patients with recurrent CTS did not have any systemic disease and all of them had applied previously to another health center. CONCLUSION We concluded that the recurrence rates in CTS might be decreased with exploration and incision of the entire transverse ligament. Damage to the motor and sensory branches of the median nerve could be avoided with an incision on the ulnar side.

Astrocytes perform many functions in the brain and spinal *** metabolism is important for astroglial cells and astrocytes are the only cells with insulin receptors in the *** common antibiotic penicillin is also a chemical agent that causes degenerative effect on neuronal *** aim of this study is to show the effect of insulin and glucose at different concentrations on the astrocyte death induced by penicillin on primer astroglial cell *** is well known that intracranial penicillin treatment causes neuronal cell death and it is used for experimental epilepsy model *** studies showed that insulin and glucose might protect neuronal cell in case of proper ***,the present study is about the effect of insulin and glucose against astrocyte death induced by *** this purpose,newborn rat brain was extracted and then mechanically dissociated to astroglial cell suspension and finally grown in culture *** were maintained for 2 weeks prior to being used in these *** concentrations of insulin（0,1,3 nM）and glucose（0,3,30 mM）were used in media without penicillin and with 2 500μM *** decreased the viability of astroglial cell *** highest cell viability appeared in medium with 3 nM insulin and 3 mM glucose but without ***,in medium with penicillin,the best cell survival was in medium with 1 nM insulin but without glucose. We concluded that insulin and glucose show protective effects on the damage induced by penicillin to primer astroglial cell ***,cell survival depends on concentrations of insulin and glucose *** results of this study will help to explain cerebrovascular pathologies parallel to insulin and glucose conditions of patient after intracranial injuries.

Effect of transcatheter aortic valve implantation on QT dispersion in patients with aortic stenosis

Background QT dispersion （QTd） is a predictor of ventricular arrhythmia. Ventricular arrhythmia is an important factor influencing morbidity and mortality in patients with aortic stenosis. Surgical aortic valve replacement reduced the QTd in this patients group. However, the effect of transcatheter aortic valve implantation （TAVI） on QTd in patients with aortic stenosis is unknown. The aim of this study was to investigate the effect of TAVI on QTd in patients with aortic stenosis. Methods Patients with severe aortic stenosis, who were not candi-dates for surgical aortic valve replacement due to contraindications or high surgical risk, were included in the study. All patients underwent electrocardiographic and echocardiographic evaluation before, and at the 6th month after TAVI, computed QTd and left ventricular mass index （LVMI）. Results A total 30 patients were admitted to the study （mean age 83.2 ± 1.0 years, female 21 and male 9, mean valve area 0.7 ± 3 mm2）. Edwards SAPIEN heart valves, 23 mm （21 patients） and 26 mm （9 patients）, by the transfemoral approach were used in the TAVI procedures. All TAVI procedures were successful. Both QTd and LVMI at the 6th month after TAVI were significantly reduced com-pared with baseline values of QTd and LVMI before TAVI （73.8 ± 4 ms vs. 68 ± 2 ms, P=0.001 and 198 ± 51 g/m2 vs. 184 ± 40 g/m2, P=0.04, respectively）. There was a significant correlation between QTd and LVMI （r=0.646, P〈0.001）. Conclusions QTd, which malign ventricular arrhythmia marker, and LVMI were significantly reduced after TAVI procedure. TAVI may decrease the possibility of ventricu-lar arrhythmia in patients with aortic stenosis.
