The impact of chemotherapy-naïve open radical cystectomy delay and perioperative transfusion on the recurrence-free survival: A perioperative parameters-based nomogram

Objective: To develop and internally validate a nomogram to predict recurrence-free survival (RFS) including the time to radical cystectomy (RC) and perioperative blood transfusion (PBT) as potential ***: Patients who underwent open RC and ileal conduit between January 1996 to December 2016 were split into developing (n=948) and validating (n=237) cohorts. The time to radical cystectomy (TTC) was defined as the interval between the onset of symptoms and RC. The regression coefficients of the independent predictors obtained by Cox regression were used to construct the nomogram. Discrimination, validation, and clinical usefulness in the validation cohort were assessed by the area under the curve, the calibration plot, and decision curve ***: In the developing dataset, the 1-, 5-, and 10-year RFS were 83.0%, 47.2%, and 44.4%, respectively. On multivariate analysis, independent predictors were TTC (hazards ratio [HR] 1.07, 95% confidence interval [CI] 1.05-1.08, p<0.001), PBT (one unit: HR 1.40, 95% CI 1.03-1.90, p=0.03;two or more units: HR 1.72, 95% CI 1.29-2.29, p<0.001), bilateral hydronephrosis (HR 1.54, 95% CI 1.21-1.97, p<0.001), squamous cell carcinoma (HR 0.60, 95% CI 0.45-0.81, p=0.001), pT3-T4 (HR 1.77, 95% CI 1.41-2.22, p<0.001), lymph node status (HR 1.53, 95% CI 1.21-1.95, p<0.001), and lymphovascular invasion (HR 1.28, 95% CI 1.01-1.62, p=0.044). The areas under the curve in the validation dataset were 79.3%, 69.6%, and 76.2%, for 1-, 5-, and 10-year RFS, respectively. Calibration plots showed considerable correspondence between predicted and actual survival probabilities. The decision curve analysis revealed a better net benefit of the ***: A nomogram with good discrimination, validation, and clinical utility was constructed utilizing TTC and PBT in addition to standard pathological criteria.

MODELING AND OPTIMIZATION OF CYCLIC HOIST SCHEDULES IN AN ELECTROPLATING LINE

This paper deals with the modeling, analysis and optimization of a specific kind of real industrial problems. This class of problems is known in the literature as Cyclic Hoist Scheduling Problem （CHSP）. In such class of problems, several jobs have to flow through a production line according to an ordered bath sequence. The CHSPs appear in the manufacturing facilities to achieve a mass production and to search a repetitive sequence of moves for the hoist. In this paper, we develop P-Temporal Petri Net models to represent the behavior and validate certain qualitative properties of the basic production line. Afterward, complex configurations of the production line are modeled and their properties such as reachability of desired functioning （cyclic operation）, deadlock-free, resource sharing and management are checked and validated. A mathematical analysis and a simulation study of all proposed Petri net models are carried out using mathematical fundaments of Petri nets and a Visual Object Net ＋＋ tool. The second part of the paper deals with the development of a mixed integer linear programming models to optimize processing of each line configuration. Optimal manufacturing plans of the studied system with cyclic processing sequences are defined and the feasibility of optimal cyclic scheduling of each configuration is proved.

In this study,a nonlinear three-dimensional hydrocode numerical simulation was carried out using AUTODYN-3D to investigate the effect of blasting of a high explosive material(TNT)against several configurations of the composite *** numerical models were carried out to study the effect of varying the thickness of the walls and the effect of adding an air layer or aluminum foam layer inside two layers of concrete in mitigating the effect of blast waves on the structure *** results showed that increasing the thickness of walls has a good effect on mitigating the effect of blast *** a layer of air was added,the effect of blast waves was exaggerated,while when a layer of aluminum foam was added the blast wave effects were mitigated with a reasonable percentage.

Egyptian heavy vacuum gas oil hydrotreating over Co-Mo/CNT and Co-Mo/γ-Al2O3 catalysts

The catalytic activity of CoMoS /CNT towards the Egyptian heavy vacuum gas oil hydrotreating was studied. The delivered CNT was functionalized with 6 mol /L HNO_3. The CNT were loaded with 12% MoO_3( by weight) and 0.7 Co /Mo atomic ratio with impregnation methods. The γ-Al_2O_3 catalyst was also prepared by impregnation method to compare both catalysts *** analysis tools such XRD,Raman spectroscopy,TEM,and BET were used to characterize the catalysts. The autoclave reactor was used to operate the hydrotreating experiments. The hydrotreating reactions were tested at various operating conditions of temperature 325-375 ℃,pressure 2-6 MPa,time 2-6 h,and catalyst /oil ratio( by weight) of 1 ∶75,1 ∶33 and 1 ∶10. The results revealed that the CoMoS /CNT was highly efficient for the hydrotreating more than the CoMoS /γ-Al_2O_3. Also, the hydrodesulfurization( HDS) increased with increasing catalyst /oil ratio. Additionally,results showed that the optimum condition was temperature 350℃,pressure 4 MPa,catalyst /oil ratio of 1 ∶75 for 2 h. Furthermore,even at low CoMoS /CNT catalyst /oil ratio of 1 ∶75,an acceptable HDS of 77.1% was achieved.

The Impact of Simulation Education on Self-Efficacy in Pre-Registration Nursing Students

This literature review primarily aims to explore and synthesise the previous studies in simulation education research conducted over the past five years related to the effects of simulation training on the self-efficacy of undergraduate pre-registration nursing students. The second aim of this study is to explore additional outcome variables that were examined in the previous studies. Five electronic databases were searched systematically. These databases were medLINE, CINAHL Plus, Scopus, Embase and PsycINFO. The PICO model was employed to identify the search terms, with a thesaurus being used to provide synonyms. Reference lists of relevant articles were examined and hand searches of journals were also undertaken. The quality of each study was assessed using the Simulation Research Rubric (SRR). A total of 11 studies were included. All studies explored the impact of simulation education on undergraduate pre-registration nursing. Six studies explored nursing students’ competence and performance and two papers examined their critical thinking. Problem solving, learning motivation, communication skills and knowledge acquisition were examined once. The majority of studies indicated that simulation training has a positive impact on pre-registration nursing students’ self-efficacy and other outcome variables. Furthermore, the study results indicate that simulation training is more dependable than traditional training, and students were extremely satisfied with the simulation training. However, most of the studies included in this review had several gaps, including study design, sample size and dissimilarities between the scales used. Further research with large samples, reliable and valid instruments, and outcomes measures (such as critical thinking and transferability of skills) is required to provide better insight into the effectiveness of simulation in undergraduate nursing education. .

Disinfection of Microbes by Magnetized DC Plasma

A different combined effects for helium gas discharge such as: magnetic field strength, breakdown voltage, applied power, applied pressure, cathode fall thickness, edge effect, distribution of the electron temperature and density, and finally exposure time for Staphylococcus aureus substrate over slides at the cathode edge, are discussed under the influence of cold, nonthermal plasmas, ultra low pressure, and presence of the magnetic field for disinfection of bacteria for short exposure times, compatible to International Commission on Non-Ionizing Radiation Protection, Health Phys (ICNIRP) for healing applications. Furthermore, analyses of the experimental data of initial and final densities of cells alive, using survival curves, showed an impressive inhibitory effect of plasma discharge to the remaining survival of bacterial ratio under the influence of the magnetic field.

Evaluation of Left Ventricular Function after Percutaneous Recanalization of Chronic Coronary Occlusions: The Role of Two-Dimensional Speckle Tracking Echocardiography

Background: The recanalization of a chronic total coronary occlusion is the possible way to improve left ventricular (LV) function through the recovery of hibernating myocardium. Aim: The aim of this study is to evaluate the role of 2D speckle tracking in evaluation of the left ventricular (LV) systolic function in chronic total occlusion (CTO) patients before and at 1 day as well as 3 months after percutaneous coronary intervention (PCI). Patients and Methods: A prospective observational study included 40 patients diagnosed with coronary angiography to have a chronic total occlusion. Percutaneous coronary revascularization was performed according to standard practices with the femoral approach. Conventional 2D echocardiography was used to assess LV functions and wall motion abnormalities scoring index (WMAI). Using speckle-tracking echocardiography was to measure global longitudinal strain (GLS) and. Follow-up of patients was done at day 1 and 3 months later after PCI. Results: Forty patients were included in this study, with a mean age of 58.55 ± 7.98 years. GLS and WMAI difference at baseline and follow-up shows a positive correlation with left ventricular ejection fraction (LVEF) changes at baseline and follow-up (p Conclusion: The results of this study provide evidence to support the clinical use of 2D-STE to monitor the early changes of LV function. In patients undergoing CTO revascularization, change in GLS was more sensitive predictors for LV function improvement at 3-month follow-up.
