BACKGROUND Mycobacterium tuberculosis(TB)is the causative agent of TB,a chronic granulo-matous *** disease is prevalent in low-income countries,posing a significant global health *** TB is one of the three *** disease can mimic other intra-abdominal conditions,leading to delayed diagnosis owing to the absence of specific *** gastric outlet obs-truction(GOO)remains a frequent complication,its incidence has declined with the advent of proton pump inhibitors and helicobacter pylori eradication *** TB can cause upper gastrointestinal hemorrhage,obstruction,and malignancy-like *** SUMMARY A 23-year-old male presented with recurrent epigastric pain,distension,nausea,vomiting,and weight loss,prompting a referral to a gastroenterologist *** examination revealed distorted gastric mucosa and signs of chronic ***,treatment was interrupted,possibly owing to vomiting or comorbidities such as human immunodeficiency virus infection or *** surgical intervention revealed a dilated stomach and diffuse thickening of the duodenal *** revealed gastric wall effacement with *** Primary gastric TB is rare,frequently leading to *** its rarity,suspicions should be promptly raised when encountering relevant symptoms,often requiring surgical intervention for diagnosis and treatment.

Acute Transverse Myelitis in Pregnancy—A Case Report and a Literature Review

Purpose: This case report highlights a rare instance of a 26-week pregnant woman presenting with limb numbness, diagnosed with acute transverse myelitis (ATM) following COViD-19 vaccination. Notably, she tested positive for neuromyelitis optica (NMO) but did not exhibit the typical symptoms of blurred vision. Methods: Data were collected from i Seha in governmental hospitals in Bahrain, providing a comprehensive overview of the patient’s clinical journey. Results: The patient was treated with carbamazepine, steroids, and clexane, leading to the successful delivery of a healthy baby at 34 weeks. however, two years postpartum, she reported new episodes of blurred vision and numbness. Subsequent treatment with Rituximab resulted in significant improvement, with a diagnosis of Devic’s disease established. Conclusion: Acute transverse myelitis is a rare condition, particularly during pregnancy, and in this case, symptoms of Devic’s disease were masked by the absence of blurred vision. This condition predominantly affects females in their 20s during childbearing years and can lead to complications such as premature delivery. Effective management includes steroids and antiepileptic drugs, with clexane playing a crucial role in preventing embolisms. A delivery mode may vary based on obstetric factors, and MRi remains the gold standard for diagnosis. Close follow-up is essential for optimal patient outcomes.

Objectives:The objective of the study was to explore the impact of auditory stimulation protocol on auditory response in cerebrovascular stroke(CVS)*** and Methods:A pre-posttest nonequivalent control group quasi-experimental study was conducted at al Mainal hospital at al Kars Einy hospital,Cairo University,among 60 CVS patients,and the sample was randomly assigned through a coin to allocate subjects to study or control group(30 for each group).Auditory stimulation techniques were applied three times per week for the study group for two consecutive *** in the control group received only routine hospital care during the study *** Glasgow Coma Scale(GCS)score for assessing the neurological function and level of arousal and the auditory subscale score of Western Neuro Sensory Stimulation Profile(WNSSP)for assessing auditory response were recorded and compared between the two groups before and after the ***:There were statistically significant differences between the study group and the control group regarding third reading of GCS score(11.5±0.3 vs.10.6±0.9,t=4.676,P=0.000).The scores of all auditory subscales of WNSSP in the study groups increased after implementing the auditory stimulation protocol(Pion:Utilizing the auditory stimulation protocol by nurse proves to be an effective method for enhancing hearing responses in individuals with CVS.

From economic and nutritional points of view,tomato is,historically,considered one of the most important *** signiﬁcant yield reduction,most commercial cultivars of tomato crops are sensitive to moderate levels of ***,high levels of salt stress can negatively affect the yield and quality of tomato ***,this study was conducted to evaluate the yield and fruit quality of three tomato cultivars(Forester,Ghandowra-F1,and Feisty-Red)cultivated hydroponically,under three different levels of nutrient solution *** of tomato fruits was performed based on quantity(number and weight of fruits,and total fruit yield),physical quality(color andﬁrmness),and chemical quality(refractometric index“Brix”,ph,EC,K^(+),Na^(+),and NO_(3)-).Experiments were conducted using three salinity levels of the nutrient solution with electrical conductivity values(dSm^(-1))of 2.5(control),6.0,and *** showed that the studied tomato cultivars were signiﬁcantly inﬂuenced by high salinity(9.5 dSm^(-1))in comparison to the low(2.5 dSm^(-1))and medium(6.0 dSm^(-1))levels of *** average,the highest fruit weight per plant of 1944.84 g and total fruit yield of 4.42 kgm^(-2) were observed at the low salinity level;however,no signiﬁcant differences were obtained in the two yield factors(single fruit weight and total fruit yield)for the low and medium salinity *** the other hand,a signiﬁcant reduction in tomato yield(31%)was associated with the high salinity level compared to the yield at low and medium salinity *** of physical quality parameters showed highly signiﬁcant differences among all salinity *** average,the maximum value of color change(1.72)was associated with the medium salinity level,and the maximum value ofﬁrmness(9.61 Ncm^(-1))was recorded at the high salinity *** levels and tomato cultivars introduced signiﬁcant differences in chemical quality parameters;however,no signiﬁcant differences in these

The impact of chemotherapy-naïve open radical cystectomy delay and perioperative transfusion on the recurrence-free survival: A perioperative parameters-based nomogram

Objective: To develop and internally validate a nomogram to predict recurrence-free survival (RFS) including the time to radical cystectomy (RC) and perioperative blood transfusion (PBT) as potential ***: Patients who underwent open RC and ileal conduit between January 1996 to December 2016 were split into developing (n=948) and validating (n=237) cohorts. The time to radical cystectomy (TTC) was defined as the interval between the onset of symptoms and RC. The regression coefficients of the independent predictors obtained by Cox regression were used to construct the nomogram. Discrimination, validation, and clinical usefulness in the validation cohort were assessed by the area under the curve, the calibration plot, and decision curve ***: in the developing dataset, the 1-, 5-, and 10-year RFS were 83.0%, 47.2%, and 44.4%, respectively. On multivariate analysis, independent predictors were TTC (hazards ratio [hR] 1.07, 95% confidence interval [Ci] 1.05-1.08, pit: hR 1.40, 95% Ci 1.03-1.90, p=0.03;two or more units: hR 1.72, 95% Ci 1.29-2.29, pilateral hydronephrosis (hR 1.54, 95% Ci 1.21-1.97, pinoma (hR 0.60, 95% Ci 0.45-0.81, p=0.001), pT3-T4 (hR 1.77, 95% Ci 1.41-2.22, ph node status (hR 1.53, 95% Ci 1.21-1.95, phovascular invasion (hR 1.28, 95% Ci 1.01-1.62, p=0.044). The areas under the curve in the validation dataset were 79.3%, 69.6%, and 76.2%, for 1-, 5-, and 10-year RFS, respectively. Calibration plots showed considerable correspondence between predicted and actual survival probabilities. The decision curve analysis revealed a better net benefit of the ***: A nomogram with good discrimination, validation, and clinical utility was constructed utilizing TTC and PBT in addition to standard pathological criteria.

Effects of Transjugular intrahepatic Portosystemic Shunt on Renal and Pulmonary Function in hepatic Decompensation with and without hepatorenal and hepatopulmonary Syndromes:A Review

Cirrhosis is often characterized by decreased liver function,ranging from a compensated,typically asymptomatic phase to a decompensated phase characterized by the appearance of ascites or variceal bleeding,and ultimately hepatorenal syndrome(hRS)or hepatopulmonary syndrome(hPS).The latter two complications are associated with a poor prognosis and limited treatment *** cases of ascites or variceal bleeding resistant to medical therapy,transjugular intrahepatic portosystemic shunt(TiPS)is effective and *** blood by TiPS diverts portal blood to the systemic circulation,potentially increasing systemic blood volume and benefiting renal ***,TiPS could also divert nitric oxide to the systemic circulation,potentially worsening systemic hypotension and perfusion,which could be detrimental to renal *** evidence indicates that TiPS often improves renal function in patients with portal hypertension,with or without *** studies have shown persistently decreased renal function after ***,these data are insufficient to support a recommendation for the use of TiPS specifically for *** patients without pre-existing hPS,TiPS does not appear to significantly affect pulmonary gas *** of TiPS in hPS have been inconsistent;some studies have shown improvement,but effects were *** studies have shown a persistent decline in pulmonary function after *** evidence supports the need for large randomized controlled trials to investigate the beneficial effects of TiPS for *** pulmonary function data are less clear regarding TiPS for *** aim of the current report was to review the literature regarding the effects of TiPS on renal and pulmonary function in hepatic decompensation,with or without the development of hRS or hPS.

intelligent traffic control requires accurate estimation of the road states and incorporation of adaptive or dynamically adjusted intelligent algorithms for making the *** this article,these issues are handled by proposing a novel framework for traffic control using vehicular communications and internet of Things *** framework integrates Kalman filtering and *** smoothing Kalman filtering,our data fusion Kalman filter incorporates a process-aware model which makes it superior in terms of the prediction *** traditional Q-learning,our Q-learning algorithm enables adaptive state quantization by changing the threshold of separating low traffic from high traffic on the road according to the maximum number of vehicles in the junction *** evaluation,the model has been simulated on a single intersection consisting of four roads:east,west,north,and south.A comparison of the developed adaptive quantized Q-learning(AQQL)framework with state-of-the-art and greedy approaches shows the superiority of AQQL with an improvement percentage in terms of the released number of vehicles of AQQL is 5%over the greedy approach and 340%over the state-of-the-art ***,AQQL provides an effective traffic control that can be applied in today’s intelligent traffic system.

Objective:To evaluate the combination therapy of pyronaridine tetraphosphate and diminazene aceturate against Babesia in vitro and in ***:Bioinformatic analysis was performed using atom pair *** in vitro combination test was performed against Babesia bovis and Theileria ***,the in vivo chemotherapeutic efficacy of pyronaridine tetraphosphate in combination with diminazene aceturate was investigated against the growth of Babesia microti in mice using a fluorescence inhibitory ***:Pyronaridine tetraphosphate and diminazene aceturate exhibited nearly similar molecular *** in vitro combination of pyronaridine tetraphosphate and diminazene aceturate was synergistic on Babesia bovis and additive on Theileria *** addition,5 mg/kg pyronaridine tetraphosphate combined with 10 mg/kg diminazene aceturate inhibited Babesia microti growth significantly compared with those observed after treatment with 25 mg/kg diminazene aceturate alone from day 6 post treatment to day 12 post *** combination therapy also normalized the hematological parameters of infected ***:An oral dose of pyronaridine tetraphosphate combined with a subcutaneous dose of diminazene aceturate inhibits Babesia in vitro and in mice,suggesting it might be a new paradigm for the treatment of babesiosis.
