Efficacy of Some Antibiotics in Curing Resistant <i>Escherichia coli</i>Infection

There is growing interest in re-evaluation of older antibiotics with the wide spread of pathogen resistance, especially gram negative bacteria, which impair treatment of some infections. In contrast various studies have reported that some antibiotics have efficacy in clearing resistant bacterial infections. On account of that it was interesting to evaluate the efficacy of erythromycin, chloramphenicol and/or tenoxicam in curing and/or relieving wound infection of highly resistant Escherichia coli and investigate the possible mechanisms beyond their antibacterial activity. This was achieved through evaluating highly resistant E. coli strains in vitro using agar dilution and in vivo rat models of E. coli infected wound and acute inflammation by carrageenin, where possible mechanisms were evaluated through measuring immunological mediators and histopathological examination. This study revealed that in vivo, erythromycin alone or in combination with tenoxicam significantly improved the healing of infected skin wounds with E. coli irresspective of resistancy in vitro. In addition to the improvement of immunological mediators involved in inflammatory reaction, oxidative stress and in cytokines expression as response to the bacterial infection in vivo. On the other hand chloramphenicol neither alone nor in combination with tenoxicam, achieved any significant effect. Tenoxicam didn’t show antimicrobial activity alone nor in combination with tested antibiotics in vitro, but it has shown synergestic activity in combination with tested antibiotics in vivo. Thus we concluded that immunomodulatory activity of erythromycin through anti-inflammatory and antioxidant effects was the possible mechanisms by which this antibiotic had healed infection with resistant E. coli in vivo, despite its resistancy to this antibiotic in vitro.

High Protein Diet that Cause Weight Loss and Lower Blood Glucose Level Have a Serious Impact on the Kidney Functions of Male Diabetic Obese Albino Rats

Background: High protein (HP) diets are increasingly being recommended as one of the management strategies for weight control in overweight and obese individuals. The health benefits of high protein diets are well-established, but the mechanisms of action on body systems responsible for the changes in body weight and glycaemic control are not well-clear. Objective: The present study aimed to examine the effect of HP diets on the kidney functions of diabetic obese albino rats. Material and Methods: Eighty male adult male albino rats were used in this study. The animals were divided into eight equal groups (10 rats for each). Type 2 DM and obesity were induced. At the end of the 12 weeks, samples were collected for biochemical analysis. Results: The high protein diet led to significant decrease in BW, FI, BG, TC, LDL, TG, Lactate dehydrogenase, albumin, urine pH and urine citrate;while serum insulin, HDL, urea, creatinine, total protein, urine volume and urinary excretion of Ca were significantly higher in high protein diet groups. Conclusion: A high protein intake in diabetic obese albino rats for 12 weeks led to changes in the serum and urine levels of markers of renal function which indicated abnormalities in the functions of the kidney.

Occult hepatitis B virus(HBV)infection,by definition,is a state in which infection with this virus does not manifest with the conventional diagnostic laboratory criteria reserved for the obvious form of HBV *** a result,occult HBV infection is commonly a surprise finding discovered accidently during the evaluation of other apparent liver diseases,such as hepatitis C virus(HCV)infection or non-alcoholic fatty liver disease and,more importantly,their evolution into life-threatening hepatocellular *** infection with HCV and occult HBV is rarely considered when assessing these more obvious conditions,and in an attempt to offer a better understanding of this phenomenon,this study attempted to shed some light onto the uniqueness of occult HBV infection by addressing the natural history of HBV and HCV infections,as well as non-alcoholic fatty liver *** was carried out by taking into account the exclusive integration process undertaken by the HBV genome into infected host hepatocytes,with consideration given to conditions which afford reactivation of the occult infection and stress on the molecular mechanisms that underlie occult HBV ***,the clinical outcome of occult HBV infection and its relation to hepatocellular carcinoma is analyzed.

Lightweight deep convolutional neural networks(CNNs)present a good solution to achieve fast and accurate image-guided diagnostic procedures of COVID-19 ***,advantages of portable Ultrasound(US)imaging such as simplicity and safe procedures have attracted many radiologists for scanning suspected COVID-19 *** this paper,a new framework of lightweight deep learning classifiers,namely COVID-LWNet is proposed to identify COVID-19 and pneumonia abnormalities in US *** to traditional deep learning models,lightweight CNNs showed significant performance of real-time vision applications by using mobile devices with limited hardware *** main lightweight deep learning models,namely MobileNets,ShuffleNets,MENet and MnasNet have been proposed to identify the health status of lungs using US *** image dataset(POCUS)was used to validate our proposed COVID-LWNet framework *** classes of infectious COVID-19,bacterial pneumonia,and the healthy lung were investigated in this *** results showed that the performance of our proposed MnasNet classifier achieved the best accuracy score and shortest training time of 99.0%and 647.0 s,*** paper demonstrates the feasibility of using our proposed COVID-LWNet framework as a new mobilebased radiological tool for clinical diagnosis of COVID-19 and other lung diseases.

Could Bladder Inflation Prior to Cesarean Section Prevent Urinary Tract Injury in High Risk Group? A Randomized Controlled Trial

Background: Accidental urinary tract particularly bladder injury during cesarean delivery has a significant maternal morbidity, as it may lead to extended operative time, infection of urinary tract and sometimes development of urinary tract fistulae. Objective: To find out the efficacy of urinary bladder inflation immediately prior to cesarean section (CS) procedure in minimizing incidence of accidently urinary tract injury in high risk patients. Setting: Obstetrics and Gynecology Department, Faculty of Medicine, South Valley University, Qena, Egypt. Duration: From August 2017 to November 2018. Study Design: A prospective randomized controlled trial. Methods: Seventy six pregnant women recruited from attendants of outpatient antenatal care unit of obstetrics and gynecology department who planned for cesarean delivery and carried one or more risk factors for urinary tract injury. Patients randomly were classified into 2 groups (group I included 38 cases, underwent bladder inflation using triple way Foley’s catheter immediately before CS and group II included 38 cases, and underwent bladder deflation with 2 ways Foley’s catheter immediately before CS. Results: The overall incidence of urinary tract injury was significantly higher in group II (7 cases = 18.4%) than in group I (2 cases = 5.2%) with p value ely significantly higher in group II (5 cases = 13.1%) than group I (2 cases = 5.2%) with p value 0.01;ureteric or combined vesico-ureteric injuries had been reported only in group II (1 case = 2.6% and 1 case = 2.6%) respectively with no case reported in group I (p There was a highly statistically significant difference between group I and group II as regard to hospital stay (p < 0.001) but mildly significant differences in operative time and remote urinary tract fistulae (p Conclusions: There was significant reduction in urinary bladder injury, ureteric injury, operative time and hospital stay. Timed Intercourse versus Intrauterine Insemination with Mild Ovarian Stimulation for Unexplained Infertility

Background: Infertility is unexplained in about 22% - 28% of infertile couples. Spontaneous pregnancy may occur in them. If did not happen, Expectant management will be via clomiphene citrate (CC) administration, intrauterine insemination (IUI), and IVF & ICSI. Aim: to assess the effectiveness of intrauterine insemination with mild controlled ovarian stimulation compared with expectant management in couples with unexplained infertility more than one year. Methods: 160 couples with unexplained infertility were selected, The couples were randomly divided into two groups: Group (A) “80 couples”: Intrauterine insemination (IUI) with mild controlled ovarian stimulation (combination of CC + hMG) using prepared semen and was performed 36 hours after hCG injection, Group (B) “80 couples”: Couples had no ovulation induction. They encouraged for timed intercourse in the most fertile days of female cycle guided by folliculometry for 6 months. Data were collected quantitatively, coded and analyzed using SPSS. The power of study is 80% and 95% confidence interval. Result: The pregnancy rate was calculated in both groups: In group (A) (IUI/COS): OPR (Ongoing pregnancy rates) was 27.5% and PR (pregnancy rates)/cycle was 8.6%. In group (B) (Expectant management): OPR was 25% and PR/cycle was 5.3%. Conclusion: Mild controlled ovarian stimulation CC + hMG with IUI offers no statistical significance in terms of pregnancy outcomes over expectant management in this study however significance in the response to stimulation.

Ampelographic and Genetic Diversity Assessment of Some Local Grape Genotypes under Egyptian Conditions

Grape (Vitis vinifera L.) is assumed as one of the most valuable and popular fruit crops all over the world. In this study, ten of local grape germplasm were characterized considering phenotypic diversity and genetic features under Egyptian conditions. Selected local grapes;Baltim Eswid, Edkawy, Matrouh Eswid, Bez el-Naka, Bez el-Anza, Romy Abiad, Romy Ahmer, Gharibi, Fayoumi and Banaty, were spread geographically among seven national governorates. A total of 58 attributes were characterized grape germplasm for the distinctness of vine parts. For molecular characterization, 9 nuclear SSR loci were analysed. Data revealed a broad sense of similarity at the level of studied morphological attributes, where the highest similarity (0.93) was between Romy Ahmer and Romy Abiad. A total of 24 alleles, ranging from 124 - 253 bp in size, were detected at the nine tested loci with an average of 2.67 alleles per locus. The amplification products in all of the 9 SSRs loci showed polymorphism among the 10 grapevine cultivars. The genetic relatedness among most of the cultivars was in accordance with their identification based on ampelographic properties. Data of phenotypic and molecular analysis revealed high variability of Vitis germplasm in Egypt and contributed substantially to our awareness of valuable local grape genetic resources which are on the verge of extinction.
