Comparative Study between General Anesthesia versus General Anesthesia Combined with Thoracic Epidural Analgesia on Cytokine Response in Laparoscopic Cholecystectomy Patients

Background and Objectives: The main benefits of laparoscopic surgery in comparison to open surgery involve the rapid discharge from recovery room, decreased postoperative hospital stays, reduced postoperative discomfort, easier getting back to work and faster return to ordinary daily life as well as cosmetic surgical wounds. The anesthesia type has an essential role in attenuation of the surgical stress and achievement of these advantages. We aimed to determine the outcome of giving general anesthesia in conjunction with thoracic epidural analgesia (TEA) compared to general anesthesia alone on stress response to surgery and anesthesia by investigating cytokine reaction (interleukin 6 and 8 levels), hemodynamic changes (BP, HR, RR, SPO2), and Visual Analogue Scale (VAS) scores postoperatively in patients subjected for laparoscopic cholecystectomy. Methods: This study included 40 patients aged 20 - 60 years old with American Society of Anesthesiologists physical status (ASA) I and II. They were planned for laparoscopic cholecystectomy at Aswan University Hospital from April 2017 to March 2018. They were randomly allocated into two groups. Group A (n. 20) received general anesthesia only and Group B (n. 20) received general anesthesia in conjunction with thoracic epidural analgesia using fentanyl and bupivacaine in the epidural catheter. Assessment of groundwater suitability for different activities in Toshka district,south Egypt

Globally,groundwater has globally emerged as a crucial freshwater source for domestic,irrigation,and industrial *** evaluation of groundwater quality in the Toshka region is imperative to ensure its suitability for the extensive agricultural and industrial activities underway in this promising,groundwater-dependent development *** is particularly significant as Egypt increasingly relies on groundwater reserves to address freshwater deficits and to implement mega-development projects in barren *** this study,fifty-two samples were collected from the recently drilled wells tapping into the Nubian Sandstone Aquifer(NSA)in the Toshka *** quality was assessed through hydrochemical analysis,Piper diagram,and various indicators such as Na%,SAR,RSC,KR,MH and *** hydrochemical analysis revealed improved groundwater quality characteristics,attributed to continuous recharge from Lake *** Piper diagram categorised most of the water samples as"secondary salinity"water *** all wells proved suitable for irrigation with only two wells unsuitable based on MH values and six wells based on KR *** Total Hardness(TH)values,all samples were classified as"Soft",indicating their suitability for domestic and industrial *** Quality Index(WQI)results concluded that all samples met WHO and FAO guidelines for drinking and irrigation,*** distribution maps,constructed using GIS,facilitate the interpretation of the *** monitoring of quality parameters is essential to detect any deviation from permissible limits.

Does local vaginal estrogen after tension-free transobturator vaginal tape reduce overactive bladder symptoms in postmenopausal women? A prospective randomized, controlled study

Acceleration of coasting enhances pregnancy rate in ICSI cycles at risk for ovarian hyperstimulation syndrome

Objective To assess the efficacy of adding Gn-releasing hormone antagonist（GnR HA） on the day of hC G triggering in a long luteal protocol without withholding the agonist in women who are at a risk to develop ovarian hyperstimulation syndrome（OHSS）.Methods This was a retrospective cohort study conducted upon 50 women who have elevated serum estradiol（E2） level 〉4 500 ng/L at the day of ovum triggering with hC G on a long agonist luteal protocol of controlled ovarian stimulation（COS）. When an exaggerated ovarian response was observed around day 10 of stimulation, immediately the next morning at 6 a.m. gonadotropin administration was stopped or reduced, and a single dose of ganirelix acetate（antagonist） was given sc continuation of the agonist dose hC G. Another serum E2 measurement was done at 6 p.m.（after 12 h of antagonist） then hC G, sc 250 mg and choriogonadotropin α were administered 14 h later after antagonist and documented the reduction of E2 level. Oocyte retrieval was conducted after 34-36 h of hC G administration. The measured outcomes were the level of E2 on the day of hC G injection, number of oocytes and their quality, pregnancy rate and the occurrence of OHSS and its grade in case it *** The total dose for recombinant FSH was 25.3±6.4 ampoules（75 IU/ampoule） while it was 11.0±3.0 ampoules for the urinary hM G. A higher oocyte maturation rate（82.8%） and a high fertilization rate（87.8%） were observed. The mean endometrial thickness was 10.1±1.0 mm on the day of hC G triggering. The higher fertilization rate with the good endometrial thickness observed resulting in a higher pregnancy rate（78.0%, 39/50） with statistically significant（P〈0.05）. A significant reduction of E2 level was documented by a percentage around 40% before hC Ginjection. Influence of temperature on the properties of one-dimensional piezoelectric phononic crystals

The current study investigates the influence of temperature on a one-dimensional piezoelectric phononic crystal using tunable resonant frequencies. Analytical and numerical examples are introduced to emphasize the influence of temperature on the piezoelectric phononic crystals. It was observed that the transmission spectrum of a one-dimensional phononic crystal containing a piezoelectric material（0.7 PMN-0.3 PT） can be changed drastically by an increase in *** resonant peak can be shifted toward high or low frequencies by an increase or decrease in temperature, ***, we deduced that temperature can exhibit a large tuning in the phononic band gaps and in the local resonant frequencies depending on the presence of a piezoelectric material. Such result can enhance the harvesting energy from piezoelectric materials, especially those that are confined in a phononic crystal.

Fast adaptive regression-based model predictive control

Model predictive control(MPC)is an optimal control method that predicts the future states of the system being controlled and estimates the optimal control inputs that drive the predicted states to the required *** computations of the MPC are performed at pre-determined sample instances over a finite time *** number of sample instances and the horizon length determine the performance of the MPC and its computational cost.A long horizon with a large sample count allows the MPC to better estimate the inputs when the states have rapid changes over time,which results in better performance but at the expense of high computational ***,this long horizon is not always necessary,especially for slowly-varying *** this case,a short horizon with less sample count is preferable as the same MPC performance can be obtained but at a fraction of the computational *** this paper,we propose an adaptive regression-based MPC that predicts the best minimum horizon length and the sample count from several features extracted from the time-varying changes of the *** proposed technique builds a synthetic dataset using the system model and utilizes the dataset to train a support vector regressor that performs the *** proposed technique is experimentally compared with several state-of-the-art techniques on both linear and non-linear *** proposed technique shows a superior reduction in computational time with a reduction of about 35–65%compared with the other techniques without introducing a noticeable loss in performance.

The antiretroviral treatment (ART) has significantly reduced the number of new HIV/AIDS infections and related deaths. However, cases of immunovirological discordance (IVD) are found in various locations. The objective of this study was to determine the profile of People living with HIV (PLHIV) with IVD and to identify associated factors. We conducted a cross-sectional study based on the records of PLHIV under ART for at least 6 months, followed at Hpital de Jour Donka from 2015 to 2017, and having both viral load (CV) and CD4 T-cell count. Prevalence of IVD was 34.57%, with 23.87% for immunological discordance (ID) and 10.7% for virological discordance (VD). Females were predominant (66.26%), and male gender influenced IVD with a statistically significant difference (p = 0.006) and was associated with VD (p = 0.007). The average age was 38.77 11.30 years. PLHIV were classified at WHO stages 3 and 4 (86.01%). The median initial haemoglobin level was 11.5 g/L [3.2 - 12]. The mean initial CD4 T-cell count was 272.84 cells/mm3 201.6. The median initial viral load (VL) was 147,337 copies/mL [1092 - 31,675,000]. The initial CD4 T-cell count 3 was associated with IVD with a statistically significant difference (p = 0.0009) and correlated with ID (p = 0.000). Prurigo was associated with IVD with a statistically significant difference (p = 0.003). Cerebral toxoplasmosis was not associated with IVD but was associated with ID (p = 0.04). This study allowed us to describe the profile of PLHIV with IVD. The main associated factors were male gender, initial CD4 T-cell count 3, toxoplasmosis, prurigo, and herpes zoster.

New Polymer Syntheses Part 60: A Facile Synthetic Route to Polyamides Based on Thieno[2,3-b]thiophene and Their Corrosion Inhibition Behavior

Polyamides containing thieno[2,3-b]thiophene moiety were prepared via a simple polycondensation reaction of the diaminothieno[2,3-b]thiophene monomer la with different kinds of diacid chlorides （including oxalyl, adipoyl, sebacoyl, isophthaloyl, terephthaloyl, 4,4＇-azodibenzoyl, 3,3＇-azodibenzoyl, p-phenylene diacryloyl） in the presence of LiC1 and NMP as a solvent through lowtemperature solution polycondensation. The chemical structures of model compound and synthesized polyamides were confirmed by FTIR, nuclear magnetic resonance spectroscopy （including 1 H-NMR and 13 C-NMR） and elemental analysis. In addition, the thermal stability, crystallinity structure and surface morphology of synthesized polyamides were characterized via thermogravametric analysis （TGA）, wide-angle X-ray diffraction analysis （WAXD） and scanning electron microscopy （SEM）. Also, the corrosion inhibition behavior of selected examples ofpolyamides was investigated; the inhibitive effect of the investigated polymers for carbon steel in 1.0 mol＇L-1 HCI was studied using potentiodynamic polarization （PDP） and electrochemical impedance spectroscopy （EIS） methods. PDP results displayed that the polyamides containing thieno[2,3-b]thiophene moiety can be as mixed-type inhibitors. The inhibition efficiency （P, %） was found to be in the range from 67.13% to 96.01%. There is an increase in P by the synthesized polymers in comparison to the starting monomer. The adsorption of these polymers was found to obey Langmuir adsorption isotherm.
