Optimum Timing and Complication of Completion Thyroidectomy for Differentiated Thyroid Cancer

Background: Despite improved preoperative diagnostics, incidental postoperative detection of differentiated thyroid cancer in the final histology is still common. In most of these cases, completion thyroidectomy is recommended by national and international guidelines, although secondary surgery is associated with an increased operative risk. The optimal timing of completion thyroidectomy is still controversial. Patients and Methods: The patients admitted to surgical oncology department, SECI, with diagnosis of differentiated thyroid cancer;during the period from January 2008 to December 2015;were rewired for age, sex, type of 1st operation, histopathological result, type of 2nd operation and time interval between the 2 operation, complication of 2nd operation and morbidity. 118 patients underwent completion thyroidectomy;those patients were divided according to timing of completion operation into 3 groups: Group A is from one week to 3 months and include 64 patients;Group B is from 3 - 6 months and include 30 patients;Group C is more than 6 months and include 24 patients. Clinical complications and oncologic outcomes were analyzed. The mean follow-up was 80 ± 10 months. Result: we record 118 patients under completion thyroidectomy. Ages range from 79 to 13 years. Papillary thyroid cancer were 96 and follicular thyroid cancer were 22. The overall rates of transient and persistent postoperative hypocalcemia were 19.5% and 4.2%, respectively. The rates of persistent hypocalcemia were found in group A and B but not in group C. Transient or persistent vocal cord paresis was observed in 9 (7.6%) and 3 patients (2.5%). The incidence of persistent vocal cord paresis (VCP) was significantly higher in groups A and B than in group C. There was no significant difference regarding survival among the 3 groups;however recurrence is higher in group A. Conclusion: Considering perioperative morbidity and oncologic outcomes, completion thyroidectomy should be performed at least 3 to 6

Cotton is considered as a major cash crop of the world. It earns huge foreign exchange by its valuable products;fiber, lint, cotton seed oil, hull and a lot more. Being an important fiber crop, it earns huge foreign exchange by contributing to textile and seed oil industry. This review summarizes cotton biology, its diversity and domestication, genome assembly, constraints in its production and methods to improve cotton plant to fulfill the need of textile and oil industry. But cotton is facing enormous biotic and abiotic stresses with insect pests being most prominent. Massive destruction caused by insects needs to be controlled for maintaining fruitful cotton crop production. Conventional breeding approaches are limited to improving single trait and integrate stable genes within plant genome in approximately 7 - 8 years. Improved biotechnological procedures have paved new pathways to target genes specifically and improve cotton germplasm in lesser time than conventional breeding.

Robust <i>H</i><sub>∞</sub>Observer-Based Tracking Control for the Photovoltaic Pumping System

In this paper, we propose a H∞ robust observer-based control DC motor based on a photovoltaic pumping system. Maximum power point tracking is achieved via an algorithm using Perturb and Observe method, with array voltage and current being used to generate the reference voltage which should be the PV panel’s operating voltage to get maximum available power. A Takagi-Sugeno (T-S) observer has been proposed and designed with non-measurable premise variables and the conditions of stability are given in terms of Linear Matrix Inequality (LMI). The simulation results show the effectiveness and robustness of the proposed method.

Simple and Efficient Synthesis of Novel Glycosyl Thiourea Derivatives Derived from Thiophene as Potential Antitumor Agents

The practical synthesis of pseudonucleosides incorporating thiourea derivative by coupling of monosaccharides （D-glucose and D-galactose） per-O-acetylated glycosyl isothiocyanates and different heterocyclic hydrazide derivatives is reported. The method involves the preparation ofper-O-acetylated glycosyl isothiocyanates from per-O-acetylated sugars （two-step synthesis）, which couple with heterocyclic hydrazides from amines to give thiourea-linked pseudonucleosides. all newly synthesized pseudo-nucleosides were assayed against human lung cancer-cell lines （PG） and human liver cancer-cell lines （BEL-7402） in vitro. The 6,6-dimethyl-benzothiophen-3-carbo-hydrazide-4-one pseudonucleosides showed moderate inhibition against these two cancer-cell lines with ECs0 from 22.8 to 76.4 mM and from 54.9 to 82.4 mM, respectively. And the other compounds did not demonstrate any significant cytotoxicity even at concentrations up to 200 mM.

Research on organic solar cells has a craze importance because they show very interesting properties including their flexibility and the opportunity to be made into large surfaces. However, their stability and performance should be significantly improved compared to their current state. A nominal output of around 10% will be the goal for the coming years. The use of organic materials for photovoltaic applications is the subject of intense research in recent years. This work is based in part on the development of new conjugated polymers. In this paper, we present the synthesis and characterization of poly [(thiophene-2,5-diyl)-co-(benzylidene)] PTB catalysed by Maghnite-H+, used in the active layer of the solar cell organic heterojunction with PCBM (derivative of C60) was used as a junction of the solar cell: Glas/ITO/BCP/C60/PTB/Au/al. A current density of short circuit of about Jcc 0.1mA/cm2 was obtained for this structure with a yield of around 0.15%.

3D-Analysis of Soil-Foundation-Structure Interaction in Layered Soil

The analysis of building structure in contact with soil involves an interactive process of stresses and strains developed within the structure and the soil field. The response of Piled-Raft Foundation system to the structure is very challenging because there is an important interplay between the component of building structure and the soil field. Herein, soil-foundation-structure interaction of buildings founded on Piled-Raft Foundation is evaluated through 3D-Nonlinear Finite Element Analyses using PLAXIS3D FOUNDATION code. The soil settlements and forces demand of the high-rise building structures and foundation is computed. The parametric study affecting the soil-foundation-structure response has been carried out. The parameters such as construction phasing, sequential loading, building aspect ratios, soil failure models and thickness proportion of soil field stiff layer, are considered. It is concluded that the interaction of building foundation-soil field and super-structure has remarkable effect on the structure.

Performance Analysis of a Rooftop Hybrid Connected Solar PV System

In the present work,a 5-kW hybrid PV solar system was installed on the roof of a house in Diyala,Iraq(33.77°N,45.14°E elevation 44 m).The system consists of two strings,where each string consists of nine polycrystalline PV modules with 355 Wp in series,and the two strings are in *** energy storage system(ESS)consists of two parallel strings,each with four 12 V and 150 Ah tubular deep cycle batteries in series.A hybrid inverter of 5 kW rated power was operated in different *** results showed that May’s monthly energy consumption was about 822.9 and 1085 kWh,*** percentage distribution of the DC energy produced was about 1%system energy losses,27.9%was used to charge the ESS,34.3%was used to feed the grid,and the remaining 37.64%was used to share the *** energy percentage sharing the load was 16.67%from ESS,33.33%from the PV system,and 50%*** average daily reference,array,and final yields were 6.07,4.327,and 3.991 h/day,*** average array and load efficiencies were 12.3%and 92.24%,with the performance ratio at 65.4%.

Enhanced bio-catalytic and tolerance properties of an indigenous cellulase through xerogel immobilization

Today, demand exists for cost-effective production of industrially important enzymes from entire scientific sectors. By keeping in mind the extensive industrial applications of cellulase, this study was performed to immobilize the indigenous enzyme produced from Trichoderma viride under pre-optimized SSF of an agricultural waste material, wheat straw. To enhance the bio-catalytic and tolerance properties of the present enzyme gel matrix immobilization engineering was applied. Previously, 2.33~fold purified novel cellulase was immobilized in to a xerogel matrix of TMOS and PTMS. FTIR spectroscopy confirmed the successful immobilization of cellulase. The free and immobilized cellulase was characterized and stability profile showed that after 24 h incubation, immobilization enhanced the thermo-stability up to 75% against 80℃ as compare to the free enzyme. Xerogel matrix immobilization enhanced the catalytic efficiency of entrapped enzyme than that of the free cellulase. Among activators/inhibitors SDS, EDTA, and Hg2+ showed inhibitory effect while, gel matrix immobilization enhanced 80% tolerance capacity of the cellulase against inactivating agents.

Colonoscopy a True Therapeutic Icon

Colonic polypectomy remained the first colonoscopic therapeutic intervention which was successfully accomplished with the help of an early snare. This therapeutic procedure was executed by Dehyle in Europe and Shinya in USA in 1971 [1]. Thus, these two colonoscopists acquired the pioneer’s position of therapeutic colonoscopy. Now the colonoscopy has achieved a special status in treating colorectal diseases because of its unique therapeutic aspect. The common uses of therapeutic colonoscopy are broadly classified in to the four major categories because of its unique therapeutic properties. These uses include resection and ablation, hemostasis, decompression with or without recanalization and foreign body extraction. Colonic perforation remained an absolute contraindication to therapeutic colonoscopy which is analogous to diagnostic colonoscopy. Good Communication plays a vital role in accomplishment of successful therapy with the help of colonoscopy. Consent process remained a complex process which should be well organized and tailored as per patient’s needs. There are lot of advancements in colonoscopy in recent years and many new procedures and treatment modalities have become available to treat colorectal diseases with the help of colonoscope. The recent advancement in colonoscopy has led to having many novel techniques. This therapeutic colonoscopy has made a significant change in current colorectal cancer management as polyps can be treated in the early stage, thus reducing the incidence of colorectal cancer. This is the basis of the United Kingdom existing bowel screening program, which is only feasible because of colonoscopy-induced interventions. The successful uses of these techniques obviate the need for a major surgical operation and colorectal disease can be treated with minimally invasive colonoscopic methods. These attributes make colonoscopy a true therapeutic icon.

Geospatial Database Template for Urban Management in Fez (Morocco)

In order to develop an urban GIS of Fez, we began with the scoping and feasibility study of the system. This strategic review has identified the characteristics of the urban areas, their problems, needs and existing potential, both in terms of data, equipment or personnel. This contribution focuses on a conceptual modeling of the geospatial database, the proposed template for the system and the quality assurance plan for the project. According to the feasibility study conducted during the first phase, the system must have an architecture respecting the intervention of actors in the city. Therefore, the implementation of the system requires an adequate hardware and software platform for optimum operation of the proposed urban GIS. After identifying technical choices, the financing plan of this scenario has been proposed. This arrangement is structured in three parts: Hardware, Software and Development/Training.
