Long-term outcomes of urethral balloon dilation for anterior urethral stricture: A prospective cohort study

Objective:To prospectively follow up a cohort of anterior urethral stricture disease patients managed with balloon dilation(BD)for 3 years to evaluate the long-term outcomes and to study factors that contribute to ***:This study included men who had urethral BD for significant anterior urethral stricture disease between January 2017 and March *** about the patient age,stricture characteristics,and recurrence date were recorded,along with information on postoperative indwelling catheter use and operative ***,information about the self-calibration procedure was collected and where available,free flow(FF)measurements during the follow-up period were recorded and *** was defined as a lack of symptoms and acceptable FF rates(maximum flow rate>12 mL/s).Results:The final analysis was conducted on 187 *** mean follow-up period was 37 *** long-term overall success rate at the end of our study was 66.8%.Our recurrence rate was 7.4%at 12 months,24.7%at 24 months,and reached 33.2%at the end of our *** time to recurrence ranged from 91 days to 1635 days,with a mean of 670 *** stricture-free survival was significantly shorter with lengthy peno-bulbar(p=0.031)and multiple strictures(p=0.015),and in the group of patients who were not committed to self-calibration protocol(pal self-calibration was the most important factor that may have decreased the incidence of recurrence(odds ratioZ5.85).Adjuvant self-calibration after BD not only reduced the recurrence rate from 85.4%in the non-self-calibration group to 15.1%in the self-calibration one(palso improved the overall stricture-free survival and FF parameters.

Three Keggin-type heteropolyanions, namely H3PMo12O40-13H2O, （NH4）3PMo12O40·4H2O and H3PW12O40·13H2O were prepared and tested in the ring-opening polymerization reaction of tetrahydrofuran. The effects of the counter-cation （H＋, NH4＋） and the peripheral atoms （Mo, W） on the polymerization were investigated. It has been found that when the protons of H3PMo12O40·13H2O were replaced by the ammonium cations the polymerization rate decreased dramatically. Whereas, when the peripheral atoms （Mo） were replaced by their homologous （W）, the polymerization rate increased twofold. As for the viscosity average molecular weight （My） of polymer products, it was found that the high molecular weight （7930） was obtained by using H3PW12O40·13H2O. The molecular weight （My） obtained by H3PMo12O40·13H2O and （NH4）H3PMo12O40·13H2O was 6470 and 6810, respectively.

An experimental and numerical program is carried out in this research to investigate the influence of CFRP sheets on the cyclic behavior of unconfined connections made with recycled *** is partially replaced by silica fume,iron flling and pulverised fuel ash using two different percentages:15%and 20%.Each specimen is partially loaded at the first stage and then specimens are repaired using CFRP *** repaired specimens are then laterally loaded until *** addition,a finite element model is built in ABAQUS and verfed using the experimental *** experimental results have shown that the repaired specimens have regained almost double the capacity of the un-repaired specimens and hence the adopted repair configuration is recommended for retroftting seismically vulnerable RC *** cement replacement percentage by silica fume,fuel ash or iron flling from 15%to 20%has reduced joint carrying capacity and weakened the *** is recommended using 15%pulverised fuel ash or silica fume as cement partial replacement to enhance the strength and ultimate drift of beam-column joints under cyclic *** filling concrete is also recommended but the enhancement is relatively less than that found with pulverised fuel ash concrete and silica fume concrete.

Competition between Coral and algae in Tertiary and Quaternary Reefs: Greenhouse to Icehouse Transitions

The competition between coral and algae in marine reefs is pervasive through geologic time;that competition determines the structure and composition of reef communities, which we see in the fossil record. However, the relationships between coral and calcareous algae in reefs are poorly understood. To study this relationship, several hand samples and thin sections were examined from nine different foralgal reef localities around the world. Foralgal reefs typically extend from about 20 m depth or shallower on the seaward side of the reef. The first section is Salt Mountain, alabama, which preserves a Paleocene reef. It contains a high percentage of red coralline algae with benthic foraminifera. The second section is IDOP-U1376, IIA Limestone, it is Middle Eocene, in the form of an isolated reef sandwiched between two igneous beds. The third section is the Utoe’ Limestone, New Caledonia, it is Middle Eocene in age and is composed mainly of grain-boundstone units with some igneous interlayered. The fourth section is the Darnah Formation in the West-Darnah roadcut section, Northeast Libya, it is Middle Eocene in age, it is composed of highly fossiliferous limestone (corals, red coralline algae, echinoids, mollusks, foraminifers, and bryozoans). The fifth section, the al Bayda Formation (algal Limestone Member) in Northeast Libya, is in the Drayanah—al Abyar roadcut, Northeast Libya, it has several species of algae but also includes a high percentage of buildups of coral species. The sixth section is the Oligo-Miocene al Faidiyah Formation (al Fatayah Cement Quarry) limestone unit in Northeast Libya. The seventh section is (Core-core 20) late-early to middle Miocene Limestone Unit-Cicuco Field, NW Colombia. The eighth section is the Benghazi Formation at Benghazi Cement Quarry, in Northeast Libya, it is fossiliferous limestone, consisting of coral, algae, mollusks, and echinoids. Sodium-glucose cotransporter-2 inhibitors (SGLT-2 inhibitors) have transformed diabetes management by targeting renal glucose reabsorption. Designed initially as antidiabetic agents, their ability to lower blood glucose levels independently of insulin is well-documented. Beyond glycemic control, emerging research has unveiled their profound cardiorenal benefits. By inhibiting SGLT-2 protein, these drugs enhance glucose excretion in urine, reducing blood glucose levels. This mechanism has translated into significant cardiovascular and renal protection, establishing SGLT-2 inhibitors as pivotal in managing not only diabetes but also cardiovascular and renal diseases. Recent studies have illuminated the broader therapeutic potential of SGLT-2 inhibitors beyond diabetes. Evidence indicates their efficacy in managing heart failure, chronic kidney disease (CKD), and cardiovascular complications in individuals with or without diabetes. This expanded therapeutic landscape has catalyzed a paradigm shift in SGLT-2 inhibitor use, positioning them as key agents in the cardiorenal metabolic continuum. Moreover, their role in the secondary prevention of cardiovascular events and slowing CKD progression in T2DM patients has garnered considerable attention. This consensus-based review aims to offer practical guidance in an algorithmic approach to primary care healthcare professionals to optimize SGLT-2 inhibitors utilization and maximize their benefits. The review seeks to empower clinicians to effectively manage patients who may benefit from SGLT-2 inhibitor therapy by addressing common initiation barriers and optimizing treatment strategies. Additionally, it aims to raise awareness among primary care physicians regarding the multifaceted benefits of these medications and overcome clinical inertia in their adoption into routine clinical practice.

A Participatory GIS Approach to Identify Critical Land Degradation Areas and Prioritize Soil Conservation for Mountainous Olive Groves(Case Study)

Climate change scenarios predict an increase in the frequency of heavy rainfall events in some areas. This will increase runoff and soil erosion, and reduce agricultural productivity, particularly on vulnerable mountainous agricultural lands that is already exhibiting high rates of soil erosion. Haphazard implementation of soil and water conservation （SWC） interventions on scattered fields is inefficient in reducing soil erosion. The objective of this study was to identify areas at high risk of erosion to aid the design and implementation of sustainable SWC using GIS analysis and farmers＇ participation approach. A 25 m digital elevation model （DEM） was used to derive layers of flow accumulation, slope steepness and land curvature, which were used to derive an erosion-risk （priority） map for the whole watershed. Boundaries of farmers＇ fields were mapped and verified by the community and each field was classified into high, moderate or low erosion risk. Fields with low flow accumulation （top of hill） and/or steep slope and/or convex slope were assigned high erosion risk and therefore high implementation priority. The study showed that more than 54% of the fields were classified into high erosion risk areas. Accordingly, a community-watershed plan was established, revised and approved by the *** loans to implement SWC measures were distributed to 100 farmers based on the priorities of their fields. Judged by local farmers and using 16 randomly selected fields, 90% of the targeted areas were correctly identified using the erosion risk map. After two years, the conservation measures had led to marked improvement of soil conservation. The approach is simple and easy to comprehend by the community and provides scientific basis to prioritize the implementation of SWC and to target the most degraded areas, which amplify the impact of these in reducing the vulnerability to land degradation.
