Orbital apex syndrome after tooth extraction in an immunocompromised patient

A 60 year-old man presented with acute on set of left eye proptosis and ptosis. It was associated with poor vision, eye pain and restriction of eye movement of the same eye after 5 hours post left upper molar tooth extraction. The visual acuity in the left eye was hand movement. There was severe ptosis and proptosis of the left eye. The conjunctiva was chemotic with quite anterior chamber. The pupil was mid dilated and sluggished to light. The ocular movement was restricted in all directions. Fundoscopy of the left eye revealed features of central retinal artery occlusion with hyperaemic disc and subretinal exudates at posterior pole. The right eye appeared normal. Urgent MRI brain and orbit revealed severe left paranasal sinusitis with anterior displacement of the left globe and presence orbital abscess. Patient was managed with Otorhinolaryngology and Neurosurgery teams. He underwent emergency transnasal drainage of abscess. Histopathological examination of unhealthy sinus mucosa showed evidence of fungal infection. However, the culture and sensitivity result was inconclusive. Patient was treated with amphotericin B, ceftriaxone, amoxicillin clavulanate and metronidazole. Patient was detected to have high blood sugar level and was managed accordingly. The proptosis improved with treatment. However, his vision, ptosis and ophthalmoplegia remained static. Assessing the immunocompromised status is important for the management of patient presented as acute orbital apex syndrome to avoid fatal outcome.

Objective:To maintain semen quality of male rabbits during chilled storage by enrichment the tris based diluent with different concentrations of propolis ethanolic ***: Total phenolic and total flavonoid contents, as well as antioxidant activity was determined in propolis ethanolic extract (PEE). The extract was analysed by HPLC for separation and identification of target metabolites. Semen was collected from 10 rabbit bucks, pooled, then divided into five aliquots (each of 500 μL) and diluted each in 5 mL Tris-citric acid-glucose-egg yolk extender (TCGY). The 1st aliquot served as control while PEE was added at concentration of 0.8, 1.2, 1.6 and 2.0 mg/5 mL tris extender in the aliquot 2, 3, 4 and 5 respectively. Diluted semen samples were subjected to cooling at 4℃ for 72 h. Sperm motility, sperm viability, sperm abnormality, sperm membrane integrity and acrosome integrity were evaluated in chilled semen allover the chilling ***:The resluts revealed presence of a considerable amount of total phenolic compounds (98.67 mg GAE/g extract) and total flavonoids (70.16 mg CE/g extract) which were parallel to an antioxidant activity assessed as ABTS, DPPH and FRAP (198.65, 180.18 and 306.17 mM TE/g extract respectively). The dominant phenolic acid was chlorogenic acids (3.959 mg/g extract). Other compounds were found in less amounts rosmarinic acid (3.959 mg/g extract), myrcetin (1.946 mg/g extract), kaempferol (1.089 mg/g extract) and apeginin-7-glucoside (1.113 mg/g extract). Obtained results clearly demonstrated that the addition of (1.2-1.6) mg PEE in the chilled extended rabbit semen proved to be beneficial for maintaining semen characteristics compared to control and the addition of 0.8 and 2 mg ***: The enrichment of rabbit semen tris-basic extender with (1.2-1.6) mg PEE/5 mL tris-extender (as the best and safe concentrations) maintain the sperm characteristics in good condition all over 72 h of chilling.

This paper discusses the technique of casting concrete ofmicropiles with pressures and the consequence of comparisons with normal way of casting concrete （casting with gravity）. Preliminary geotechnical studies have been made in specific area in Sudan to predict the soil parameters and then an experimental work has been done for an estimated number ofmicropiles with different diameters and different techniques of placing concrete with various amount of pressure. This study was carried out to learn the usefulness of this technique in the field of structural foundations in Sudan. Capacities of micropiles were compared in cases of non-pressure casting （normal way of casting concrete） and pressure casting. Through the results, it was found that the entry of pressure factor in the operation of casting concrete increases the capacities of micropiles. The increased value of ultimate load depends on the amount of pressure applied.

Changes in Platelet Indices during Pregnancy as Potential Markers for Prediction of Preeclampsia Development

Objective: To assess whether changes in platelet indices, detectable by simple complete blood count (CBC), during pregnancy could be used as markers for prediction of development of preeclampsia (PE). Methods: A total of 2813 pregnant women who received regular antenatal care until delivery were included. Participants were divided into 3 groups: normotensive pregnant women (n = 2621), women with PE without severe features (n = 169), and women with PE with severe features (n = 23). Blood samples were collected during antenatal visits and/or during the period of in-patient hospital stay, and changes in platelet indices were compared among the three groups. Results: Platelet count (PC) was decreasing while mean platelet volume (MPV) and platelet distribution width (PDW) were increasing as PE progressed. Receiver operating characteristics (ROC) curve analysis showed that PDW had the largest area under curve (AUC) [0.980 (95% CI: 0.964 - 1.000)], making it the best marker for predicting development of PE. Also, PDW showed the most statistically significant correlation with mean arterial pressure (MAP) (r = 0.902, p = 0.000), making it the best marker for predicting severity of hypertension. Conclusion: This study provides evidence that PC decreases while MPV and PDW increase as pregnancy advances, and these changes are more pronounced in PE than normotensive pregnancy. These changes predate development of PE by 2 - 8 weeks and are proportional to the progress of this disorder. The selected platelet indices, especially PDW, have the potential to be utilized as markers for not only prediction of PE development but also severity of hypertension.

The HKC （Hong Kong Convention） adopted by the IMO in 2009 is aimed at ensuring that ships, when being recycled after reaching the end of their operational lives, do not pose any unnecessary risk to human health and safety of the marine environment. The text of the ship recycling Convention was developed with input from the IMO （International Maritime Organization） Member States and relevant NGOs （Non-Governmental Organizations）, and in co-operation with the ILO （International Labor Organization） and the Parties to the Basel Convention. The Convention intends to address all the issues around ship recycling, taking into account that ships sold for scrapping may contain environmentally hazardous substances. It addresses concerns raised about the working and environmental conditions at many of the world ship recycling locations. Regulations in the new Convention cover： the design, construction, operation and preparation of ships so as to facilitate safe and environmentally sound recycling, without compromising the safety and operational efficiency of ships; the operation of ship recycling facilities in a safe and environmentally sound manner; and the establishment of an appropriate enforcement mechanism for ship recycling, incorporating certification and reporting requirements. This paper will present and discuss the evaluation of the Hong Kong Convention. Also it will discuss implementing the Hong Kong convention to improve the recycling business, highlighting the effect of the convention on the maritime industry.

In this paper, the Penman-Monteith method was applied to evaluate the reference crop evapotranspiration. A reliable estimation of the reference evapotranspiration (ETo) is of critical importance and required accurate estimates to close the water balance. The aim of this paper is estimating the reference evapotranspiration (ETo) as preliminary to use for groundwater modeling in the area. Based on FAO-Penman-Monteith method, ETo calculator software was applied. Meteorological data within this study were obtained from two gauges stations (Xing ping and Wu gong) and available literatures. The results indicated that the values of ETo for a period (1981-2009)—29 years—in two stations approximately the same. Specifically, is ranged between 0.4 - 6.9 mm /day, 0.4 - 6.7 mm/day and the average value is 2.6 mm/ day, 2.6 mm/day in Xing ping and Wu gong respectively. In addition, the maximum values were occurred in summer season (May, June and July). The result also found that the correlation coefficient ≈ 1. Moreover, “ETo” was increasing by recent years. The reference crop evapotranspiration for some crops were calculated.

Hydrogeochemical Variations of Groundwater of the Northern Jabal Hafit in Eastern Part of Abu Dhabi Emirate, United Arab Emirates (UAE)

This study is aimed to assess and evaluate the suitability of groundwater of the area located to the south-east of Al-Ain area in the UAE using hydrogeochemcical approach. The chemical analyses of groundwater samples collected from the study area showed that the groundwater salinity is high which resulted from heavy groundwater pumping. High chloride concentrations in groundwater of Mubazarah and Neima might be attributed to the entrapped saline water within the limestone sequence of Jabal Hafit, or it can be the agricultural activities as it clear from the positive relationship between Cl- and Br-. High sulphate concentrations in groundwater might be related to the presence of gypsum and anhydrite within the limestone sequence of Jabal Hafit. The anionic and cationic compositions of groundwater indicate that the chloride and sodium ions are the dominant and presence of bicarbonate and sulphate may reflect the mixing of such water by the recent freshwater through the existing structural lineaments within the study area. The hydrochemical parameters indicate a relative increase in the concentration of calcium, magnesium and sulphate ions and this could reflect the influence of carbonates and evaporite sediments.

Microbiological Quality of Freshly Prepared, Packaged Fruit and Milk Juices Sold in Cafés, Shops, and Supermarkets in Hargeisa, Somaliland

Background: Due to their delicious taste, high nutritional content, and health benefits, fruit juices are well-known drinks in many countries and are now an essential component of the modern diet. Objective: Determining the microbiological quality of both packaged and freshly made fruit and milk juices. Method: The spread-plate approach was employed to isolate and count the bacteria. 90 ml of sterile peptone water were blended with 10 ml of well-mixed, packed, and freshly made fruit juices. The samples were sequentially diluted (101 - 105) in accordance with the Indian Manual of Food Microbiological Testing Methods. Results: From eight samples of imported packaged fruit and milk juice, the average of total coliform, staphylococci, and viable bacterial counts were zero, 1.39 × 102, and 2 × 102 CFU/ml, respectively. In contrast, from three samples of locally produced fruit and milk juice, the average of total coliform, staphylococci, and viable bacterial counts were zero, 5.83 × 102, and 2.73 × 103 CFU/ml, respectively. Four samples of handmade prepared fruit and milk juices had a mean of total coliform, staphylococci, and viable bacterial count of 1.441 × 104, 4.1 × 103, and 2.35 × 105 CFU/ml, respectively. Conclusion: 33.3% of the results from microbiological analysis of freshly made fruit and milk juices met the permissible range of the Revised Microbiological Standards for Fruit and Vegetables and Their Products, which were published in 2018 and as well as the Hong Kong Center for Food Safety, whereas 66.7% of the microbiological analyses of freshly prepared fruit and milk juices were above the permissible reference range of GSO standard 2000. 12.5% of the investigated imported and packed fruits and milk juices had one failed test (TSC), which was above the acceptable limit, 87.5% of the tested samples of fruit and milk juices fulfilled the necessary standards of TCC, TVBC, and TSC. 100% of the tested locally manufactured fruit and milk juices complied with TSC, T
