BACKGROUND Management of superficial bowel neoplasia(SBN)in early stages is associated with better *** last few decades experienced a paradigm shift in the management of SBN with the introduction of advanced endoscopic resection techniques(ERTs).However,there are no clear data about the aspects of ERTs in Egypt despite the growing gastroenterology *** To investigate the knowledge,attitude,and practice of ERTs toward management of SBN among Egyptian practitioners and the suitability of the endoscopy units’infrastructures toward these *** An online 2-pages questionnaire was *** first page comprised demographic data,and questions for all physicians,about the knowledge(11 questions)of and attitude(5 questions)toward ERTs as a therapeutic option for *** second page investigated the practice of ERTs by endoscopists(6 questions)and the infrastructures of their endoscopy units(14 questions).The survey was disseminated through July 2021 and the data were collected in an excel sheet and later analyzed *** The complete responses were 833/2300(36.2%).The majority of the participants were males(n=560,67.2%),middle-aged(n=366,43.9%),consultants(n=464,55.7%),gastroenterologists(n=678,81.4%),spending≥15 years in practice(n=368,44.2%),and were working in university hospitals(n=569,68.3%).The majority correctly identified the definition of SBN(88.4%)and the terms polypectomy,endoscopic mucosal resection(EMR),and endoscopic submucosal dissection(ESD)(92.1%,90.2%,and 89.1%respectively).However,26.9%,43.2%and 49.5%did not recognize the clear indication of polypectomy,EMR,and ESD *** 68.1%of physicians are convinced about the ERTs for management of SBN;only 8.9%referred all candidate cases for *** 76.5%of endoscopists had formal training in the basic polypectomy techniques while formal training for EMR and ESD was encountered only in 31.9%and 7.2%*** 71.6%and 88.4%of endoscopists did not per

BACKGROUND Although endoscopic ultrasound(EUS)is now widely available and has an established role in adults,the utility of EUS and EUS-guided fine needle aspiration(EUS-FNA)in pediatrics is insufficiently described compared to adults and is supported by only a few *** To report the experience of a single tertiary center in the use of EUS and EUS-FNA in a pediatric population and to further assess its safety,feasibility,and clinical impact on *** A retrospective study of 13 children(aged 18 years or younger)identified from our medical database was conducted.A retrospective review of demographic data,procedure indications,EUS findings,and the clinical impact of EUS on the subsequent management of these patients was *** During the 4-year study period,a total of 13(1.7%)pediatric EUS examinations out of 749 EUS procedures were performed in our *** mean age of these 8 females and 5 males was 15.6 years(range:6-18).Six of the 13 EUS examinations were pancreatobiliary(46.1%),followed by mediastinal 2/13(15.4%),peri-gastric 2/13(15.4%),abdominal lymphadenopathy 1/13(7.7%),tracheal 1/13(7.7%)and rectal 1/13(7.7%).Overall,EUS-FNA was performed in 7 patients(53.8%)with a diagnostic yield of 100%.The EUS results had a significant impact on clinical care in 10/13(77%)*** complications occurred in these patients during or after any of the *** EUS and EUS-FNA in the pediatric population are safe,feasible,and have a significant clinical impact on the subsequent management;thus avoiding invasive and unnecessary procedures.

Production and Characterization of a Composite Based on Plaster and Juncus Maritimus Plant Fibers

Nowadays,materials with a limited impact on the environment are required in the construction *** the interesting properties of natural elements such as natural fibers,it seems advantageous to use them to reinforce materials while protecting the environment and guaranteeing economic *** these lines,this research was devoted to studying the effect of untreated natural fibers extracted from the Juncus maritimus plant(from Southern Morocco)on ***,the effect of the percentage of added fibers on the fluidity of the plaster was evaluated by means of the Marsh’s cone method,that is,by measuring the time taken by a known volume of plaster to flow from a cone through a short ***,the highly porous microstructure of the plaster was observed using a scanning electron microscope(SEM).The diffractogram of Juncus maritimus fibers was built by means of a X-ray diffraction(XRD)*** values obtained for the wet thermal conductivity of the composites decreased from 0.4853 W/m.K for the reference specimen to 0.4324 W/m K for a fiber volume content of 40%.The densities were calculated to determine the lightness of each *** mechanical strengths[flexural and compressive]were evaluated for different percentages of rush *** addition of 20%volume fiber was found to improve the flexural strength to 3.63 MPa compared to the sample without fiber(3.36 MPa),thereby ensuring good toughness of the considered ***,a reduction in compressive strength was observed,due to the poor adhesion between the fibers and the plaster matrix.

BACKGROUND Pancreatic cystic lesions(PCLs) are common in clinical practice. The accurate classification and diagnosis of these lesions are crucial to avoid unnecessary treatment of benign lesions and missed opportunities for early treatment of potentially malignant *** To evaluate the role of cyst fluid analysis of different tumor markers such as cancer antigens [e.g., cancer antigen(CA)19-9, CA72-4], carcinoembryonic antigen(CEA), serine protease inhibitor Kazal-type 1(SPINK1), interleukin 1 beta(IL1-β), vascular endothelial growth factor A(VEGF-A), and prostaglandin E2(PGE2)], amylase, and mucin stain in diagnosing pancreatic cysts and differentiating malignant from benign *** This study included 76 patients diagnosed with PCLs using different imaging modalities. All patients underwent endoscopic ultrasound(EUS) and EUS-fine needle aspiration(EUS-FNA) for characterization and sampling of different *** The mean age of studied patients was 47.4 ± 11.4 years, with a slight female predominance(59.2%). Mucin stain showed high statistical significance in predicting malignancy with a sensitivity of 87.1% and specificity of 95.56%. It also showed a positive predictive value and negative predictive value of 93.1% and 91.49%, respectively(P < 0.001). We found that positive mucin stain, cyst fluid glucose, SPINK1, amylase, and CEA levels had high statistical significance(P < 0.0001). In contrast, IL-1β, CA 72-4, VEGF-A, VEGFR2, and PGE2 did not show any statistical significance. Univariate regression analysis for prediction of malignancy in PCLs showed a statistically significant positive correlation with mural nodules, lymph nodes, cyst diameter, mucin stain, and cyst fluid CEA. Endoscopic retrograde cholangiopancreatography in periampullary diverticulum: The challenge of cannulation

Periampullary diverticulum(PAD) is duodenal outpunching defined as herniation of the mucosa or submucosa that occurs via a defect in the muscle layer within an area of 2 to 3 cm around the papilla. Although PAD isusually asymptomatic and discovered incidentally during endoscopic retrograde cholangiopancreatography(ERCP), it is associated with different pathological conditions such as common bile duct obstruction, pancreatitis, perforation, bleeding, and rarely carcinoma. ERCP has a low rate of success in patients with PAD,suggesting that this condition may complicate the technical application of the ERCP procedure. Moreover, cannulation of PAD can be challenging, time consuming, and require the higher level of skill of more experienced endoscopists. A large portion of the failures of cannulation in patients with PAD can be attributed to inability of the endoscopist to detect the papilla. In cases where the papilla is identified but does not point in a suitable direction for cannulation, different techniques have been described. Endoscopists must be aware of papilla identification in the presence of PAD and of different cannulation techniques, including their technical feasibility and safety, to allow for an informed decision and ensure the best outcome. Herein, we review the literature on this practical topic and propose an algorithm to increase the success rate of biliary cannulation.

Solar thermoelectric generator and thermoelectric cooler performance:analysis and comparison using a different shape geometry

Cartilage tympanoplasty in the treatment of adhesive otitis media with and without Eustachian tube balloon dilatation

Purpose:To compare cartilage tympanoplasty(CT)combined with eustachian tube balloon dilatation(ETBD)and cartilage tympanoplasty alone as a surgical treatment modality for adhesive otitis media(AdOM)in terms of graft healing,audiological outcomes,and impact on life style,using Chronic Otitis Media Outcome Test 15(COMOT-15).Methods:50 patients with AdOM were randomly classified into 2 groups:25 patients for cartilage tympanoplasty only(CT group)and 25 patients for cartilage tympanoplasty combined with eustachian tube balloon dilatation(CT t ETBD group).Clinical outcomes in both groups were compared at 3 and 6 months of follow ***:There was no significant difference in graft healing between the two *** COMOT-15 scores significantly decreased in both groups with a significant difference between the groups with regard to the decrease in COMOT-15 scores at 3 and 6 months of follow-up(P<0.05).Hearing improvement was achieved,as the mean preoperative ABG was 26.5±5.4 and 27.1±4.6 dB,and the mean postoperative ABG at 6 months was 19.4±4.4 and 14.6±3.9 dB in the CT and the CT t ETBD groups,*** difference in the magnitude of ABG reduction in the two groups was significant at 3 and 6 months of postoperative follow-up(P<0.05)in favour of the CT t ETBD ***:ETBD can increase the success rate of cartilage tympanoplasty in patients with AdOM by enhancing the audiological outcome and quality of life.
