Gliomas are primary brain tumors derived from glial cells of the central nervous system,afflicting both adults and children with distinct characteristics and therapeutic challenges.Recent developments have ushered in novel clinical and molecular prognostic factors,reshaping treatment paradigms based on classi-fication and grading,determined by histological attributes and cellular lineage.This review article delves into the diverse treatment modalities tailored to the specific grades and molecular classifications of gliomas that are currently being discussed and used clinically in the year 2023.For adults,the therapeutic triad typically consists of surgical resection,chemotherapy,and radiotherapy.In contrast,pediatric gliomas,due to their diversity,require a more tailored approach.Although complete tumor excision can be curative based on the location and grade of the glioma,certain non-resectable cases demand a chemotherapy approach usually involving,vincristine and carboplatin.Addi-tionally,if surgery or chemotherapy strategies are unsuccessful,Vinblastine can be used.Despite recent advancements in treatment methodologies,there remains a need of exploration in the literature,particularly concerning the efficacy of treatment regimens for isocitrate dehydrogenase type mutant astrocytomas and fine-tuned therapeutic approaches tailored for pediatric cohorts.This review article explores into the therapeutic modalities employed for both adult and pediatric gliomas in the context of their molecular classification.

The development of bio-sourced materials is essential to ensuring sustainable construction;it is considered a locomotive of the green economy.Furthermore,it is an abundant material in our country,to which very little attention is being given.This work aims to valorize the waste of the trunks of banana trees to be used in construction.Firstly,the physicochemical properties of the fiber,such as the percentage of crystallization and its morphology,have been determined by X-ray diffraction tests and scanning electron microscopy to confirm the potential and the impact of the mode of drying on the quality of the banana fibers,with the purpose to promote the use of this material in construction.Secondly,the results obtained with the gypsum matrix allowed us to note a preponderant improvement in the composite’s thermal properties thanks to the variation of the banana fiber additive.Thirdly,the impact of the nature of the banana fiber distribution(either fiber mixed in matrix or fiber series model)on the flexural and compressive strengths of the composites was studied.The results obtained indicate that the insulation gain reaches up to 40%.It depends on the volume fraction and type of distribution of the banana fibers.However,the thermal inertia of the composites developed,represented by thermal diffusivity and thermal effusivity,was studied.Results indicate a gain of 40%and 25%,respectively,in terms of thermal diffusivity and thermal effusivity of the developed composites compared to plaster alone.Concerning the mechanical properties,the flexural strength depends on the percentage of the volume fraction of banana fibers used,and it can reach 20%more than the flexural strength of plaster;nevertheless,there is a significant loss in terms of the compressive strength of the studied composites.The results obtained are confirmed by the microstructure of the fiber banana.In fact,the morphology of the banana fibers was improved by the drying process.It reduces the amorphous area and improves the cellulosic crystalline surfaces,w

Inertial sensors technologies for navigation applications:state of the art and future trends

Inertial navigation represents a unique method of navigation,in which there is no dependency on external sources of information.As opposed to other position fixing navigation techniques,inertial navigation performs the navigation in a relative sense with respect to the initial navigation state of the moving platform.Hence,inertial navigation systems are not prone to jamming,or spoofing.Inertial navigation systems have developed vastly,from their occurrence in the 1940s up to date.The accuracy of the inertial sensors has improved over time,making inertial sensors sufficient in terms of size,weight,cost,and accuracy for navigation and guidance applications.Within the past few years,inertial sensors have developed from being purely mechanical into incorporating various technologies and taking advantage of numerous physical phenomena,from which the dynamic forces exerted on a moving body could be computed accurately.Besides,the evolution of inertial navigation scheme involved the evolution from stable-platform inertial navigation system,which were mechanically complicated,to computationally demanding strap-down inertial navigation systems.Optical sensory technologies have provided highly accurate inertial sensors,at smaller sizes.Besides,the vibratory inertial navigation technologies enabled the production of Micro-electro-machined inertial sensors that are extremely low-cost,and offer extremely low size,weight and power consumption,making them suitable for a wide range of day-to-day navigation applications.Recently,advanced inertial sensor technologies have been introduced to the industry such as nuclear magnetic resonance technology,coldatom technology,and the reintroduction of fluid-based inertial sensors.On another note,inertial sensor errors constitute a huge research aspect in which it is intended for inertial sensors to reach level in which they could operate for substantially long operation times in the absence of updates from aiding sensors,which would be a huge leap.Inertial sensors error modeling technique

Modelling and control of a grid-connected AC microgrid with the integration of an electric vehicle

The purpose of this paper is to propose an efficient model and a robust control that ensures good power quality for the AC microgrid(MG)connected to the utility grid with the integration of an electric vehicle(EV).The MG consists of two renewable energy sources:a photovoltaic system(PVS)and a wind turbine system(WTS)based on a permanent magnet synchronous generator(PMSG),with the integration of an EV.These sources are used to supply active and reactive power to the AC bus and the utility grid.Maximum power point tracking(MPPT)based on the perturb-and-observe(PO)method is used to increase the efficiency of the photovoltaic modules and improve overall performance.The MG system includes a 2-MW WTS,a 100-kW PVS and 12 kW provided by the EV.To validate the performance of the proposed system,a series of simulations were conducted using the MATLAB/Simulink?environment.The results demonstrate that the proposed system ensures high performance in terms of power quality,system stability,power tracking and safe integration of the EV.

Cartilage tympanoplasty in the treatment of adhesive otitis media with and without Eustachian tube balloon dilatation

Purpose:To compare cartilage tympanoplasty(CT)combined with eustachian tube balloon dilatation(ETBD)and cartilage tympanoplasty alone as a surgical treatment modality for adhesive otitis media(AdOM)in terms of graft healing,audiological outcomes,and impact on life style,using Chronic Otitis Media Outcome Test 15(COMOT-15).Methods:50 patients with AdOM were randomly classified into 2 groups:25 patients for cartilage tympanoplasty only(CT group)and 25 patients for cartilage tympanoplasty combined with eustachian tube balloon dilatation(CT t ETBD group).Clinical outcomes in both groups were compared at 3 and 6 months of follow up.Results:There was no significant difference in graft healing between the two groups.Postoperative COMOT-15 scores significantly decreased in both groups with a significant difference between the groups with regard to the decrease in COMOT-15 scores at 3 and 6 months of follow-up(P<0.05).Hearing improvement was achieved,as the mean preoperative ABG was 26.5±5.4 and 27.1±4.6 dB,and the mean postoperative ABG at 6 months was 19.4±4.4 and 14.6±3.9 dB in the CT and the CT t ETBD groups,respectively.The difference in the magnitude of ABG reduction in the two groups was significant at 3 and 6 months of postoperative follow-up(P<0.05)in favour of the CT t ETBD group.Conclusion:ETBD can increase the success rate of cartilage tympanoplasty in patients with AdOM by enhancing the audiological outcome and quality of life.

Solar thermoelectric generator and thermoelectric cooler performance:analysis and comparison using a different shape geometry

Thermoelectric devices are one of the technologies used either to generate electricity by applying a temperature difference using thermal energy or as a heating/cooling system by applying an electrical voltage.The number of materials required to produce a product is an important factor in determining its price.Production costs associated with these materials,as well as their availability and quality,play a crucial role in price determination by manufacturers.In this context,a method that employs a uniform volume distribution was implemented.This approach enabled the analysis to focus on other variables,thereby promoting a more precise and relevant evaluation of overall performance.Based on the finite element method,this study investigated the influence of geometric shape,including Rect-leg,Y-leg,Pin-leg and X-leg designs,on the performance of solar thermoelectric generators and thermoelectric coolers.The study was conducted considering the same hot alumina junction surface that receives solar radiation;however,the ef-fective surface,which corresponded to the heat flow area and had a similar area near the exposed surface,varied depending on the chosen leg geometry,thus impacting the heat flux due to the variation in thermal resistance.In the case of a solar thermoelectric generator,the Rect-leg model,having the same effective surface area,presented the lowest heat loss value resulting from convection and radiation in the heat spreader and the hot alumina plate.Under the same conditions,the Y-leg showed the highest value.The Rect-leg design generated,by using thermal and optical concentration,the highest output power of 0.028 and 0.054 W,and efficiency of 3.47%and 4.7%,respectively,whereas the Y-leg generated lower values of 0.006523 and 0.018744 W for power,and 2.83%and 2.71%for efficiency,respectively.In the case of the thermoelectric coolers,the Y-leg generated the highest temperature difference between the hot and cold sides of 67.28 K at an electric current value of 1.8 A,whereas the Rect-leg,Pin-leg and X-leg

Role of cannabinoids and the endocannabinoid system in modulation of diabetic cardiomyopathy

Diabetic complications,chiefly seen in long-term situations,are persistently deleterious to a large extent,requiring multi-factorial risk reduction strategies beyond glycemic control.Diabetic cardiomyopathy is one of the most common deleterious diabetic complications,being the leading cause of mortality among diabetic patients.The mechanisms of diabetic cardiomyopathy are multi-factorial,involving increased oxidative stress,accumulation of advanced glycation end products(AGEs),activation of various pro-inflammatory and cell death signaling pathways,and changes in the composition of extracellular matrix with enhanced cardiac fibrosis.The novel lipid signaling system,the endocannabinoid system,has been implicated in the pathogenesis of diabetes and its complications through its two main receptors:Cannabinoid receptor type 1 and cannabinoid receptor type 2,alongside other components.However,the role of the endocannabinoid system in diabetic cardiomyopathy has not been fully investigated.This review aims to elucidate the possible mechanisms through which cannabinoids and the endocannabinoid system could interact with the pathogenesis and the development of diabetic cardiomyopathy.These mechanisms include oxidative/nitrative stress,inflammation,accumulation of AGEs,cardiac remodeling,and autophagy.A better understanding of the role of cannabinoids and the endocannabinoid system in diabetic cardiomyopathy may provide novel strategies to manipulate such a serious diabetic complication.

Effect of reperfusion strategy on QT dispersion in patients with acute myocardial infarction:Impact on in-hospital arrhythmia

BACKGROUND Myocardial ischemia and ST-elevation myocardial infarction(STEMI)increase QT dispersion(QTD)and corrected QT dispersion(QTcD),and are also associated with ventricular arrhythmia.AIM To evaluate the effects of reperfusion strategy[primary percutaneous coronary intervention(PPCI)or fibrinolytic therapy]on QTD and QTcD in STEMI patients and assess the impact of the chosen strategy on the occurrence of in-hospital arrhythmia.METHODS This prospective,observational,multicenter study included 240 patients admitted with STEMI who were treated with either PPCI(group I)or fibrinolytic therapy(group II).QTD and QTcD were measured on admission and 24 hr after reperfusion,and patients were observed to detect in-hospital arrhythmia.RESULTS There were significant reductions in QTD and QTcD from admission to 24 hr in both group I and group II patients.QTD and QTcD were found to be shorter in group I patients at 24 hr than those in group II(53±19 msec vs 60±18 msec,P=0.005 and 60±21 msec vs 69+22 msec,P=0.003,respectively).The occurrence of in-hospital arrhythmia was significantly more frequent in group II than in group I(25 patients,20.8%vs 8 patients,6.7%,P=0.001).Furthermore,QTD and QTcD were higher in patients with in-hospital arrhythmia than those without(P=0.001 and P=0.02,respectively).CONCLUSION In STEMI patients,PPCI and fibrinolytic therapy effectively reduced QTD and QTcD,with a higher observed reduction using PPCI.PPCI was associated with a lower incidence of in-hospital arrhythmia than fibrinolytic therapy.In addition,QTD and QTcD were shorter in patients not experiencing in-hospital arrhythmia than those with arrhythmia.

BACKGROUND Management of superficial bowel neoplasia(SBN)in early stages is associated with better outcomes.The last few decades experienced a paradigm shift in the management of SBN with the introduction of advanced endoscopic resection techniques(ERTs).However,there are no clear data about the aspects of ERTs in Egypt despite the growing gastroenterology practice.AIM To investigate the knowledge,attitude,and practice of ERTs toward management of SBN among Egyptian practitioners and the suitability of the endoscopy units’infrastructures toward these techniques.METHODS An online 2-pages questionnaire was used.The first page comprised demographic data,and questions for all physicians,about the knowledge(11 questions)of and attitude(5 questions)toward ERTs as a therapeutic option for SBN.The second page investigated the practice of ERTs by endoscopists(6 questions)and the infrastructures of their endoscopy units(14 questions).The survey was disseminated through July 2021 and the data were collected in an excel sheet and later analyzed anonymously.RESULTS The complete responses were 833/2300(36.2%).The majority of the participants were males(n=560,67.2%),middle-aged(n=366,43.9%),consultants(n=464,55.7%),gastroenterologists(n=678,81.4%),spending≥15 years in practice(n=368,44.2%),and were working in university hospitals(n=569,68.3%).The majority correctly identified the definition of SBN(88.4%)and the terms polypectomy,endoscopic mucosal resection(EMR),and endoscopic submucosal dissection(ESD)(92.1%,90.2%,and 89.1%respectively).However,26.9%,43.2%and 49.5%did not recognize the clear indication of polypectomy,EMR,and ESD respectively.Although 68.1%of physicians are convinced about the ERTs for management of SBN;only 8.9%referred all candidate cases for ERTs.About 76.5%of endoscopists had formal training in the basic polypectomy techniques while formal training for EMR and ESD was encountered only in 31.9%and 7.2%respectively.About 71.6%and 88.4%of endoscopists did not perform EMR or ESD in the last one year.Consequentl
