Drainage vs. non-drainage after cholecystectomy for acute cholecystitis:a retrospective study

Many surgeons practice prophylactic drainage after cholecystectomy without reliable evidence,this study was conducted to answer the question whether to drain or not to drain after cholecystectomy for acute calculous cholecystitis.A retrospective review of all patients who had cholecystectomy for acute cholecystitis in Aseer Central Hospital,Abha,Saudi Arabia,was conducted from April 2010 to April *** were extracted from hospital case *** data included clinical presentation,routine investigations and liver function *** data included excessive adhesions,bleeding,bile leak,and drain *** cases such as pericholecystic collections,mucocele and empyema were also *** who needed therapeutic drainage were *** data included hospital stay,volume of drained fluid,time of drain removal,and drain site *** study included 103 patients allocated into two groups;group A（n = 38） for patients with operative drain insertion and group B（n = 65） for patients without drain *** number of patients with preoperative diagnosis of acute non-complicated cholecystitis was significantly greater in group B（80%） than group A（36.8%）（P 〈 0.001）.Operative time was significantly longer in group *** patients who were converted from laparoscopic to open cholecystectomy were in group *** analysis revealed that hospital stay was significantly（P 〈 0.001） longer in patients with preoperative *** was no added benefit for prophylactic drain insertion after cholecystectomy for acute calculous cholecystitis in non-complicated or in complicated cases.

Background: Complications linked to postpartum & pregnancy are one of the major causes of female death. In order to understand the causes of complications and encourage women to take sufficient action in order to receive emergency treatment, a critical step should therefore be taken to minimize complications related to pregnancy, in order to ensure the safety of both women and newborns. Aim of the work: Evaluate the effectiveness of the education programs for pregnant women on obstetric danger signs in rural areas and help minimize the incidence of pregnancy loss and comorbidities. In response to the global food crisis and the imperative to address soil degradation, the international agricultural policy is actively working to alleviate the adverse impacts of soil salinity. As part of this initiative, a field trial spanning two consecutive seasons (2019/20-2020/21) was conducted under saline conditions. The primary objective was to evaluate the influence of various compost sources, including vermicompost at a rate of 0.5 ton·fed-1 and plant residues compost at a rate of 5.0 ton·fed-1, as main plots. Subplots were established by applying agricultural gypsum, both in the presence and absence of gypsum requirements. Additionally, sub-subplots were created by externally applying cobalt at a rate of 10.0 mg·L-1, with one sub-subplot receiving foliar cobalt application and the other not. The trial sought to assess the growth performance, chemical composition, enzymatic antioxidants, yield, and quality of cabbage plants (Brassica oleracea var. capitata L.) cultivated in saline soil. According to the findings, cabbage plants exhibited the most favorable response in terms of plant height, chlorophyll content, carotene levels, leaf area, nitrogen (N), phosphorus (P), potassium (K), head yield, vitamin C, and total dissolved solids (TDS) when treated with vermicompost, followed by plant compost. Conversely, plants grown without compost exhibited the least improvement in performance. Cabbage treated with agricultural gypsum requirements showed better performance than those without gypsum amendment. Moreover, plants subjected to cobalt spray demonstrated the highest growth, yield, and quality parameters compared to those without cobalt foliar application. In contrast, the control group (plants without the studied treatments) displayed the highest levels of enzymatic antioxidants, specifically catalase and peroxidase. AIM:To determine predictors of long term survival after resection of hilar cholangiocarcinoma(HC) by comparing patients surviving > 5 years with those who survived 5 ***:There were 34(14%) long term survivors(5 year survivors) among the 243 *** survivors were younger at diagnosis than those surviving less than 5 years(mean age,50.47 ± 4.45 vs 54.59 ± 4.98,p = 0.001).Gender,clinical presentation,preoperative drainage,preoperative serum bilirubin,albumin and serum glutamic-pyruvic transaminase were similar between the two *** level of CA 19-9 was significantly higher in patients surviving 5 year,includingyoung age(p = 0.001),serum CA19-9(p = 0.0001),non-cirrhotic liver(p = 0.02),major hepatic resection(p = 0.001),caudate lobe resection(p = 0.006),well differentiated tumour(p = 0.03),lymph node status(0.008),R0 resection margin(p = 0.0001) and early postoperative liver cell failure(p = 0.02).CONCLUSION:Liver status,resection of caudate lobe,lymph node status,R0 resection and CA19-9 were demonstrated to be independent risk factors for long term survival.

Laayoune and the Foum El Oued aquifers are in hydraulic communication only at the level of Oued Saguia El Hamra. The present study has accordingly made use of all the hydrogeological, hydrological, geological and geophysical data that preceded the watershed of Oued Saguia El Hamra in its downstream part. These data are by no means omplementary with the objective of having a better understanding of the boundary line between Laayoune and Foum El Oued aquifers and the origin of feeding the sources of Oued Saguia El Hamra. This study will focus only on the previous geophysical studies where a reinterpretation of electrical soundings has proved useful as a result of the recent well-logging results. It makes it possible to highlight the presence of a significant rise in the truncated marly substratum of Oued Saguia El Hamra and depressions (left and right banks) which could correspond to stream channels or depressed areas. At the level of the Wadi bed, there has been a regular immersion of the conductive level roofs from east to west towards Foum El Oued favoring the flow of wastewater from the zone and spraying the brackish water sources towards the groundwater of Foum El Oued. In the light of the reinterpretation of electric polls, plus as well as the geophysical surveys by electrical tomography and high definition made at the right and left banks of the Oueed Saguia El-Hamra, it was possible to verify the existence of dry ridge separating the two webs of Laayoune and Foum El Oued and stream channels or depressed areas of the left bank for drainage of brackish water to sources located along Oued Saguia El Hamra. The true resistivity models tomography profiles confirm the presence of the backbone at the left and right banks and the graben of the left bank for underground water drainage of the web Laayoune to sources welling the river Saguia El Hamra. AIM: To evaluate the efficacy and safety of botulinum toxin type A(BTX-A) in the management of patients with anismus. METHODS: An organized search of published literature was conducted using electronic databases including: Pub Med/MEDLINE, and Cochrane Central Register of Control ed Trials, also an internet-based search using "Google Scholar" service was conducted. Both comparative and observational studies were included. We excluded irrelevant articles, editorials, case reports, reviews, and meta-analyses. The studies that followed the patients less than 6 mo were excluded. Variables collected were demographic data of the patients, technique of BTX-A injection and number of sessions, short-term and longterm clinical improvement, post-injection changes in electromyography(EMG), defecography, manometry, and balloon expulsion test, and complications recorded after BTX-A ***: Seven studies comprising 189 patients were included in the review. The median age of the patients was 41.2 years and female-to-male ratio was 1.3:1. The median dose of BTX-A injected per procedure was 100 IU(range, 20-100 IU). Lateral injection was done in five trails and combined lateral and posterior injections in two trials. Three studies used endorectal ultrasonographyguided technique, one study used EMG-guided technique,whereas the remaining three studies used manual palpation with the index finger. The median percentage of patients who reported initial improvement of symptoms was 77.4%(range 37.5%-86.7%), this percentage declined to a median of 46%(range 25%-100%) at 4 mo after injection of BTX-A. Rates of improvement evaluated by balloon expulsion test, EMG, and defecography ranged between(37.5%-80%),(54%-86.7%), and(25%-86.6%), respectively. Fourteen(7.4%) patients developed complications after injection of BTX-A. Complication rates across the studies ranged from 0% to 22.6%. CONCLUSION: Initial satisfactory improvement of symptoms after BTX-A injection remarkably deteriorate

AIM:To assess the effectiveness of hepatitis B virus(HBV) vaccination program among fully vaccinated ***:A national community based crosssectional study was carried out in 6 governorates representing Egypt. A total of 3600 children aged from 9 mo to 16 years who were fully vaccinated with HBV vaccine during infancy were recruited. Face to face interviews were carried out and sera were evaluated for hepatitis B surface antigen(HBsA g),anti-HBV core antibodies(total) and quantitative detection of hepatitis B surface antibody using enzyme linked immunoassays techniques. Samples positive to HBs Ag/anti-HBV core antibodies were subjected to quantitative HBV-DNA detection by real time polymerase chain reaction with 3.8 IU/L detection limit. RESULTS:Sero-protection was detected among 2059 children(57.2%) with geometric mean titers 75.4 ± 3.6 IU/L compared to 3.1 ± 2.1 IU/L among nonseroprotected children. Multivariate logistic analysis revealed that older age and female gender were the significant predicting variables for having non seroprotective level,with adjusted odds ratio 3.3,9.1and 14.2 among children aged 5 to 0.05. C ON C LU S I ON :T he E gy pt ian c ompuls or y H B V vaccination program provides adequate protection. Occult HBV infection exists among apparently healthy vaccinated children. Adherence to infection control measures is mandatory.
