Background: This study was performed to evaluate the efficacy and safety of switch maintenance therapy with oral vinorelbine in advanced non-small cell lung cancer (NSCLC) with adenocarcinoma limited to epidermal growth factor receptor (EGFR) wild type. Materials and Methods: In this single randomized trial, patients with advanced stage (IIIB and IV) NSCLC with adenocarcinoma EGFR wild-type status, treated with 6 cycles of platinum based chemotherapy. Patients did not show progression after first-line chemotherapy were randomly assigned to receive switch maintenance with vinorelbine (80 mg/m2, day 1, 8) (group I) or the best supportive care until disease progression (group II). Results: The median progression free survival (PFS) was 9.7 months for group I versus 5.7 months for group II with statistically significant difference between both groups [HR = 1.15;95% CI 1.19 to 1.49;P value = 0.002], while the median overall survival (OS) was 13.2 months for group I versus 11.9 months for group II with no statistically significant differences between both groups [HR = 1.24;95% CI 1.05 to 1.46;P value = 0. 3]. The patients who received oral vinorelbine had tolerable toxicity profile. Conclusion: Switch maintenance therapy with oral vinorelbine, though improve PFS, did not improve OS in patients with NSCLC with adenocarcinoma EGFR wild type.

Objectives:To determine the predictive value of lactate dehydrogenase(LDH)in the diagnosis of septic shock and its association with other prognostic scores in critical pediatric ***:A cross-sectional study was performed at Children’s Hospital of Cairo University between June 2019 and December 2019.A total of 200 pediatric patients were divided into the septic shock group[100 critically ill patients with septic shock from the pediatric intensive care unit(PICU)]and the control group(100 patients with only sepsis).LDH was determined in the first 24 hours of *** sensitivity and specificity of LDH in diagnosis of septic shock were assessed;the levels of related indicators of patients with different etiologies were compared;correlations between LDH,Paediatric Index of MortalityⅡ,and Pediatric Sequential Organ Failure Assessment(pSOFA)were ***:Median LDH was 512μL(406.50-663.00)in the septic shock group and was significantly higher than that[190μL(160.00-264.50)]in the control group(P<0.001).Besides,median LDH in children with chest infecion was higher than that in children with other diagnoses(P=0.047).A good positive correlation was found between pSOFA and LDH(r=0.503,P<0.001).Conclusions:LDH could be a potential inflammatory marker in the diagnosis of septic shock and is valuable for PICU admission decisions.

The Value of Continuous Noninvasive Hemoglobin Monitoring in Intraoperative Blood Transfusion Practice during abdominal Cancer Surgery

Introduction: Patients undergoing major oncological surgery may suffer from severe bleeding. Sometimes, it is difficult to anesthesiologist to take decision about timing of administration blood products to such patients. The aim of this study is to evaluate the use of continuous noninvasive hemoglobin monitoring as a guide for blood transfusion practice. Methods: One hundred patients undergoing elective abdominal cancer surgeries were randomly allocated into two groups, Group I (n = 50): laboratory Hb was obtained at baseline (immediate preoperative), intraoperative (when to suggest transfusion triggering value) and immediate postoperative. Group II (n = 50): The probe of Masimo for SpHb monitoring was applied immediately after induction of anesthesia at the index finger. Laboratory Hb was obtained at baseline (immediate preoperative), intraoperative (when to suggest transfusion triggering value) and immediate postoperative. Results: A number of transfused units of RBC were significantly lower in SpHb group than in control group (p value 0.05). Conclusion: SpHb monitoring had clinically acceptable absolute and trend accuracy. SpHb monitoring altered transfusion decision making and resulted in decreased RBC utilization and decreased RBC costs while facilitating earlier transfusions when indicated.

In this article, we study modified nano-hydroxyapatite (HAp) by nano manganese oxide (Mn3O4) as adsorbent material to remove iron ions from groundwater. Different parameters were studied to option optimum conditions of removing such as contact time, pH, initial concentration, a dosage of adsorbent, agitation speed and temperature. Kinetics studies included first order (R2 = 0.915), pseudo-first order (R2 = 0.936), second order (R2 = 0.948), pseudo-second order (R2 = 0.995), Elovich equation model (R2 = 0.977), intraparticle diffusion (R2 = 0.946), Natarajan and Khalaf (R2 = 0.915) were carried out, the obtained results revealed that the pseudo-second order is the best to describe the adsorption process because the correlation coefficient is approaching one (R2 = 0.995). Adsorption isotherm was calculated by using Freundlich, Langmuir and Temkin constants, adsorption capacity from Langmuir model was 0.606 mg/g. Thermodynamic parameters (ΔG, ΔH = ?51 KJ/mol, and ΔS = ?142 (KJ/mol)) for the adsorption process were also calculated and discussed.

Sarcoidosis is a non caseating granulomatous disease of unknown etiology. Many factors may be attributed to the occurrence of that granuloma including the chemical factors. We present this case aiming to highlight Synthol injection which is chemical substance frequently used by athletics to increase their muscle size as it can trigger the occurrence of musculocutanous sarcoidosis. In conclusion Synthol should be added to the chemical list which can induce musculocutanous sarcoidosis.

Copper as well as copper base composites reinforced with coated and uncoated 1 wt% diamond, graphite particles or short carbon fibers are prepared by powder metallurgy process. The reinforcement particles were encapsulated with silver as well as copper layer by using the electroless deposition technique to investigate the influence of the reinforcement surface coating on the microstructure, density, electrical and thermal properties of the sintered samples. The coated and the uncoated powders were cold compacted at 600 MPa, and then sintered at 1173 K (900°C) for 2 h under hydrogen atmosphere. The phase composition, morphology and microstructure of the prepared powders as well as the copper base sintered composites were investigated using X-ray diffraction analysis (XRD) and Scanning Electron Microscope (SEM) equipped with an Energy Dispersive Spectrometer (EDS) respectively. The density of the sintered composites was measured by Archimedes method. The copper base consolidated composites had a density up to 96% and the reinforcement coated particles were distributed uniformly within the copper matrix better than the uncoated one. The electrical resistivity at room temperature and the heat transfer conduction of the produced samples were measured in a temperature range between 323 K (50°C) and 393 K (120°C). The results observed that the sintered materials prepared from the coated powder have lower electrical resistivity than the sintered materials prepared from the mixed powders. On the other hand the thermal conductivity values were calculated using the heat transfer conduction values by means of the Fourier formula. The results observed that the thermal conductivity of copper is (391 W/m·K), 1 wt% diamond/Cu is (408 W/m·K), 1 wt% graphite coated silver/Cu is (393 W/m·K), 1 wt% Cu coated short carbon fiber/Cu is (393 W/m·K), graphite/Cu is (383 W/m·K) and short carbon fiber/Cu is (382 W/m·K). This paper introduces an Improved RC6(IRC6)cipher for data encryption based on data-dependent *** proposed scheme is designed with the potential of meeting the needs of the Advanced Encryption Standard(AES).Four parameters are used to characterize the proposed *** parameters are the size of the word(w)in bits,the number of rounds(r),the length of the secret key(b)in bytes,and the size of the block(L)in *** main feature of IRC6 is the variable number of working registers instead of just four registers as in RC6,resulting in a variable block size for plaintext and *** IRC6 cipher is designed to improve the robustness against attacks by increasing the diffusion for each round and providing greater security with fewer *** effectiveness of the proposed IRC6 scheme is verified against theoretical *** proposed IRC6 scheme depends on full diffusion and confusion mechanisms regardless of the utilized block *** proposed IRC6 scheme saves 70%of the encryption time and 64%of the decryption time of *** simulation results prove that the IRC6 achieves a better encryption/decryption time compared to the ***,the proposed IRC6 is anticipated to fulfill the market needs and system security requirements.

An attempt was made to isolate yeast strains from different sources (flowers, trees exudates, fruits and cheese) which have a fast growth rate and high content of carotenoids. Seventy cultures of pigmented yeast were isolated. Forty six yeast isolates were selected and identified by simplified identification method which showed that all isolates belong to Rhodotorula glutinis. These isolates were screened in two steps. Seventeen promising isolates of Rhodotorula glutinis were selected after the primary screening. Then 6 yeast isolates were chosen and passed through the second screening. The most promising isolate (isolated from pin cushion flower, Scabiosa atropurpurea) was selected according to its total carotenoids content expressed as torulene (μg?g–1 and μg?L–1). This isolate was reclassified using a number of morphological, biochemical, and physiological characteristics which revealed that the yeast isolate agree well with those of Rhodotorula glutinis Var. glutinis. Dry biomass, amount and proportion of individual carotenoids and lipid content of the selected identified yeast were determined.

随着城市人口的增加,气候变化已成为全球共同关注的问题。人类引起的大气成分变化是全球气候变化的主要原因。在地方和区域层面,城市化和土地利用的变化也发挥着重要作用。然而,与能源使用相关的排放占这些干扰的大部分。2017年美国气候科学报告指出,如果年排放量继续像2000年以来那样快速上升,模型预测到本世纪末,全球平均气温将至少上升5华氏度,甚至可能上升10.2度。根据模型预测,即使到2050年年排放量逐渐上升并开始急剧下降,气温仍将比20世纪上半叶至少高出2.4度,甚至可能高出5.9度。巴格达被认为是世界上最热的城市之一,伊拉克巴格达市的特点是亚热带沙漠气候,天气炎热,夏季炎热而漫长,一年长达七个月。巴格达的每日最高温度通常接近50°C。炎热的气候使人们通过人工降温系统来调节建筑物的室内温度,不仅增加了能源消耗,而且将热能传递到建筑物周围,导致室外环境变差,室外温度升高导致城市热岛现象的出现。此外,许多研究表明,中东炎热干燥城市(如巴格达)的大部分城市形态风格都是基于当代规划技术创建的。在不考虑炎热干燥地区恶劣的当地条件的情况下,这些传统是从普遍存在温带和凉爽气候的西方环境中继承下来的。本论文的研究对象是巴格达的城市环境和住宅建筑。该研究方法依靠描述性和分析性的方法来实现其目标并达到预期目标,首先,通过研究和分析来阐明巴格达的气候特征和城市气候的影响。其次,调查城市住区外部环境、室外空间的规划要素及其遇到的问题,如朝向、开洞、植被覆盖、绿化、保温材料的使用、个体的设计特点等、建筑,以及居住区生活质量的自我评估。最后,提出了能够降低热气候负面影响的改善设计策略,并提出适合该区域的技术措施,为城市居住区规划设计和建筑设计提供参考。

AIM To evaluate the evolution, trends in surgical approaches a n d r e c o n s t r u c t i o n t e c h n i q u e s, a n d i m p o r t a n t lessons learned from performing 1000 consecutive pancreaticoduodenectomies(PDs) for periampullary *** This is a retrospective review of the data of all patients who underwent PD for periampullary tumor during the period from January 1993 to April 2017. The data were categorized into three periods, including early period(1993-2002), middle period(2003-2012), and late period(2013-2017).RESULTS The frequency showed PD was increasingly performed after the year 2000. With time, elderly, cirrhotic and obese patients, as well as patients with uncinate process carcinoma and borderline tumor were increasingly selected for PD. The median operative time and postoperative hospital stay decreased significantly over the periods. Hospital mortality declined significantly, from 6.6% to 3.1%. Postoperative complications significantly decreased, from 40% to 27.9%. There was significant decrease in postoperative pancreatic fistula in the second 10 years, from 15% to 12.7%. There was a significant improvement in median survival and overall survival among the *** Surgical results of PD significantly improved, with mortality rate nearly reaching 3%. Pancreatic reconstruction following PD is still debatable. The survival rate was also improved but the rate of recurrence is still high, at 36.9%.
