Papanicolaou smear interpretation with progestin-only hormonal birth control methods

Objective To define the relationship between the use of progestogen-only contracep- tion and the occurrence of abnormal cervical cytology and cervical intraepithelial lesion. Methods The present study was designed as a case-control study including 200 progestogen-only contraceptive （POC） users recruited as cases （group A） and 50 women who were non-hormonal contraceptive users as a control group （group B）. Positive Papanicolau （Pap） smear was reported in cervical cell abnormalities from atypical squamous cell of undetermined significance （ASCUS） to invasive cancer. Pap smear was evaluated according to the Bethesda system classification 2001. Cases with posi- tive Pap smears were subjected to visual inspection of the cervix with 5.0% acetic acid （VIA test） and punch biopsy. Results Statistical analysis of Pap smear results in this study showed epithelial cell abnormalities in 44 cases （22.0%） in group A and 9 cases （18.0%） in group B showing no statistically significant difference （P〉O. 05）. ASCUS abnormality was reported in 31 cases （15.5%） of group A and 6 cases of group B （12.0%）. Low-grade squamous intraepithelial lesion （LSIL） was found in 11 cases （5.5%） and 3 cases （6.0%） of group A and group B, respectively. High-grade squamous intraepithelial lesion （HSIL） was found in 2 cases （1.0%） of group A and none of group B. The incidence of abnormal Pap smears in group A did not greatly vary between different methods of POCs. Among Depo medroxyprogesterone acetate （DMPA） users, ASCUS was found in 17 cases （16.8%）, LSIL was found in 11 cases （10.8%） and HSIL was found in 2 cases （1.9%）. While in progesterone-only pills （POPs） users and Implanon~ users, ASCUS was found in 10 cases （12.8%） and 4 cases （20.0%） respectively with no higher epithelial abnormalities found. Levonorgestrel intrauterine device （LNG-IUD） was used by 1 case in group A and was not associated with Pap smear abnormality. Minimal perisigmoidal adhesions in cases with normally appearing peritoneum as a sure sign of superficial peritoneal endometriosis

Objective To test for the presence of perisigmoidal adhesions in cases with suspected endometriosis（EMS）.Methods A prospective cohort controlled study was performed. Seventy-four women with no clinical or ultrasonic evidence of ovarian cysts or EMS were included in the study. All cases were admitted to undergo laparoscopy for different indications. In all cases, thorough inspection of pelvic peritoneum was done. Then these patients were divided into two groups. Group A had no perisigmoidal adhesions（15 patients） and group B had mild filmy perisigmoidal adhesions. In both groups, documentations of visible endometriotic lesions, bipolar cauterization of selected parts of uterosacral ligaments, ovarian beds and sides of Douglas’ s pouch and histopathology assessment were done in all cases from suspicious, positive endocoaggulation and perisigmoid *** Twenty-seven patients with visible EMS had perisigmoidal adhesions. Thirtytwo patients with normal appearing peritoneum with perisigmoidal adhesions had positive electrocoagulation test in one or more pre-selected parts of the pelvic *** patients with normally appearing peritoneum without perisigmoidal adhesions had negative electrocoagulation test（bipolar cauterization turned white）.Conclusion Presence of mild filmy perisigmoidal adhesions causing tenting of the sigmoid colon could be a strong evidence of the presence of at least minimal or subtle EMS and should warrant careful inspection of the peritoneum cavity before exclusion of EMS.

Garden pea productivity and qualities are hampered in zinc(Zn),boron(B),and molybdenum(Mo)deficient ***,the combination of micronutrients(i.e.,Zn,B,and Mo)and rhizobium is necessary to increase the productivity and quality of garden peas,since this management for garden peas is neglected in ***,the present study was made to assess the effectiveness of rhizobium inoculant singly or in combination with the micronutrients(i.e.,Zn,B,and Mo)on growth,yield,nutrient uptake,and quality of garden *** were:T_(1)=Control,T_(2)=Rhizobium inoculation at 50 g/kg seed,T_(3)=T_(2)+Zn_(3)Mo1,T_(4)=T_(2)+B_(2)Mo1,T_(5)=T_(2)+Zn_(3)B_(2),T_(6)=T_(2)+Zn_(3)B_(2)Mo1 and T_(7)=Zn_(3)B_(2)*** treatments were arranged in a randomized complete block design and repeated all treatments in three *** application of 3 kg Zn,2 kg B,and 1 kg Mo ha^(−1)with inoculation of Rhizobium at 50 g kg^(−1)seed(T_(6))facilitated to increase of 44.8%in the green pod and 29.7%seed yield over *** same treatment contributed to attaining the maximum nodulation(25.3 plant^(−1)),Vitamin C(43.5 mg 100 g^(−1)),protein content(22.2%),and nutrient uptake as well as accumulation in garden *** all treatment combinations,treatment T_(6)was found superior to others based on microbial activities,soil fertility,and *** results of the study found that the application of 3 kg Zn,2 kg B,and 1 kg Mo ha^(−1)in combination with Rhizobium inoculation(50 g kg^(−1)seed)can improve the yield and quality of garden *** results of the study have the potential for the areas,where there is no use of Rhizobium inoculant or Zn,B,and Mo fertilizer for cultivation of garden pea.

Micropunch grafting for healing of refractory chronic venous leg ulcers

Background: Chronic venous ulcers are a serious problem for both patients and physicians. The CEAP classification(clinical manifestations(C), etiologic factors(E), anatomic distribution of disease(A), and underlying pathophysiologic findings(P)) for chronic venous disorders(CVD) was developed in 1994. Published papers on CVD use all or part of the CEAP. Micropunch grafts, which are a variation of skin grafts, have been used with great success in plastic surgery for both esthetic and reconstructive purposes. This study aimed to assess the outcomes of micropunch grafting in the treatment of chronic refractory venous ***: Patients with chronic venous ulcers who did not respond to conservative treatment for more than 6months were included in the study. All patients underwent ulcer coverage by micropunch skin grafts using a micrometer. The patients were discharged on the same day as the surgery. Micropunch skin grafts were manually counted per square centimeter intraoperatively and during follow-up using Dermlite DermatoschopeTM II Pro *** satisfaction was assessed by using a ***: Twenty patients underwent the surgery. Most of the patients were middle-aged males. The ulcers were predominantly small-and medium-sized. The mean operation time was 167.00 ± 86.01 min. After 6 months, the survival rate of the planted micrografts was 84%. Three ulcers had graft loss and two patients had an infection at the ulcer site, which was managed conservatively. The patients were followed-up for 14 ***: Micropunch grafting is a useful and convenient method for the treatment of difficult venous ulcers and can be performed on an outpatient basis.

Preservability of rabbit semen after chilled storage in tris based extender enriched with different concentrations of Propolis ethanolic extract (PEE)

Objective:To maintain semen quality of male rabbits during chilled storage by enrichment the tris based diluent with different concentrations of propolis ethanolic ***: Total phenolic and total flavonoid contents, as well as antioxidant activity was determined in propolis ethanolic extract (PEE). The extract was analysed by HPLC for separation and identification of target metabolites. Semen was collected from 10 rabbit bucks, pooled, then divided into five aliquots (each of 500 μL) and diluted each in 5 mL Tris-citric acid-glucose-egg yolk extender (TCGY). The 1st aliquot served as control while PEE was added at concentration of 0.8, 1.2, 1.6 and 2.0 mg/5 mL tris extender in the aliquot 2, 3, 4 and 5 respectively. Diluted semen samples were subjected to cooling at 4℃ for 72 h. Sperm motility, sperm viability, sperm abnormality, sperm membrane integrity and acrosome integrity were evaluated in chilled semen allover the chilling ***:The resluts revealed presence of a considerable amount of total phenolic compounds (98.67 mg GAE/g extract) and total flavonoids (70.16 mg CE/g extract) which were parallel to an antioxidant activity assessed as ABTS, DPPH and FRAP (198.65, 180.18 and 306.17 mM TE/g extract respectively). The dominant phenolic acid was chlorogenic acids (3.959 mg/g extract). Other compounds were found in less amounts rosmarinic acid (3.959 mg/g extract), myrcetin (1.946 mg/g extract), kaempferol (1.089 mg/g extract) and apeginin-7-glucoside (1.113 mg/g extract). Obtained results clearly demonstrated that the addition of (1.2-1.6) mg PEE in the chilled extended rabbit semen proved to be beneficial for maintaining semen characteristics compared to control and the addition of 0.8 and 2 mg ***: The enrichment of rabbit semen tris-basic extender with (1.2-1.6) mg PEE/5 mL tris-extender (as the best and safe concentrations) maintain the sperm characteristics in good condition all over 72 h of chilling.

Efficacy of Some Antibiotics in Curing Resistant <i>Escherichia coli</i>Infection

There is growing interest in re-evaluation of older antibiotics with the wide spread of pathogen resistance, especially gram negative bacteria, which impair treatment of some infections. In contrast various studies have reported that some antibiotics have efficacy in clearing resistant bacterial infections. On account of that it was interesting to evaluate the efficacy of erythromycin, chloramphenicol and/or tenoxicam in curing and/or relieving wound infection of highly resistant Escherichia coli and investigate the possible mechanisms beyond their antibacterial activity. This was achieved through evaluating highly resistant E. coli strains in vitro using agar dilution and in vivo rat models of E. coli infected wound and acute inflammation by carrageenin, where possible mechanisms were evaluated through measuring immunological mediators and histopathological examination. This study revealed that in vivo, erythromycin alone or in combination with tenoxicam significantly improved the healing of infected skin wounds with E. coli irresspective of resistancy in vitro. In addition to the improvement of immunological mediators involved in inflammatory reaction, oxidative stress and in cytokines expression as response to the bacterial infection in vivo. On the other hand chloramphenicol neither alone nor in combination with tenoxicam, achieved any significant effect. Tenoxicam didn’t show antimicrobial activity alone nor in combination with tested antibiotics in vitro, but it has shown synergestic activity in combination with tested antibiotics in vivo. Thus we concluded that immunomodulatory activity of erythromycin through anti-inflammatory and antioxidant effects was the possible mechanisms by which this antibiotic had healed infection with resistant E. coli in vivo, despite its resistancy to this antibiotic in vitro.

A Finite element Approximation of the Stokes Equations

In this work, a numerical solution of the incompressible Stokes equations is proposed. The method suggested is based on an algorithm of discretization by the unstable of Q1 – P0 velocity/pressure ?nite element approximation. It is shown that the inf-sup stability constant is O(h) in two dimensions and O( h2) in three dimensions. The basic tool in the analysis is the method of modi?ed equations which is applied to ?nite difference representations of the underlying ?nite element equations. In order to evaluate the performance of the method, the numerical results are compared with some previously published works or with others coming from commercial code like Adina system.

Effect of Oxygen Therapy by Venturi Mask versus Non Invasive Ventillation on the Outcome of Patients Who Devolope Hypoxia after Open Heart Surgery

Background: Cardiothoracic surgery is a common cause of acute respiratory failure and is associated with increased morbidity and mortality. We aimed to compare the outcomes of open heart surgery patients treated using oxygen delivery devices with patients who receive NIV as a first-line therapy for hypoxemic respiratory failure. Materials and Methods: 40 patients who developed acute hypoxemic respiratory failure after open heart surgery and admitted to cardiothoracic ICU 20 patients received NIV and 20 patient received oxygen by venture mask. For all patients the following measurements were performed before and after CPAP AND Venture use: CBC, blood urea, serum creatinine body temperature, chest X-ray, Arterial blood gases (arterial pH, sodium bicarbonate, pcO2, SpO2 and PaO2-to-FiO2 ratio). Results: Mean PO2 and SO2 have increased after using of both venture and Cpap, increase in both PCO2 and HCO3 levels after using Venturi mask, CPAP mask was superior to venturi mask in avoiding the need of intubation, decreasing The ICU stay median length and also median length of hospitalization, all were lower in CPAP group than venture group. Also the mortality rate was lower in CPAP group than the venturi group. Conclusion: Using CPAP mask in severe AHRF following open heart surgery can avoid intubation, decreases the levels of tachypnea and arterial hypoxemia, decreases ICU stay, the length of hospitalization and also decreases the mortality rate compared with patients receiving high-concentration oxygen therapy with venture mask.
