Circulating microRNA 9-3p and serum endocan as potential biomarkers for hepatitis C virus-related hepatocellular carcinoma

Development of new lateral-flow immunochromatographic strip using colloidal gold and mesoporous silica nanoparticles for rapid diagnosis of active schistosomiasis

Objective: To develop a new sandwich based lateral flow immunochromatographic strip for rapid detection of circulating Schistosoma mansoni antigen in serum and urine samples of patients with active schistosomiasis. Methods: This lateral flow immunochromatographic strip was prepared by using anti-Schistosoma mansoni soluble egg antigen monoclonal antibody conjugated gold nanoparticles (MAb-AuNPs) as antigen-detecting antibody, while crystalline material (MCM)-41-MAb bioconjugate was immobilized at the test line as antigen-capturing antibody. Both antigen capturing and detecting antibodies formed sandwich complexes with circulating Schistosoma mansoni antigen in the positive samples. Sandwich complexes immobilized at the test line gave distinct red color. The assay reliability was examined by using urine and serum samples of 60 Schistosoma mansoni infected patients, 20 patients infected with parasites other than Schistosoma, and 20 healthy individuals as negative controls. Results were compared with those obtained via sandwich enzyme linked immunosorbent assay (ELISA). Results: The detection limit of circulating Schistosoma mansoni antigen by lateral flow immunochromatographic strip was lower (3 ng/mL) than the detection limit by ELISA (30 ng/mL). The sensitivity and specificity of lateral flow immunochromatographic strip in urine samples were 98.3% and 97.5%, respectively compared to 93.5% and 90.0% by ELISA. In serum samples, they were 100.0% and 97.5%, respectively compared to 97.0% and 95.0% by ELISA. The strip test took approximately 10 min to complete. Conclusions: This new lateral flow immunochromatographic strip offers a sensitive, rapid, and field applicable technique for diagnosis of active schistosomiasis.

Sustainable Guideline for Developing the E-Waste Sector in Egypt

The amount of electronic waste generated globally is alarming especially that it is on the rise. The impact of the generated amounts is not only Environmental. The electronic waste sector is unregulated in most developing countries;this results in the informal sector being heavily involved in managing and handling it. Due to the lack of regulations, informal handlers are exposed to hazardous materials that affect both their health and health of the communities living in their proximity. Moreover, the informal sector involves illegal child and women labor under harsh conditions. In addition to the social impact, the lack of regulations leads to missing major economic opportunities associated with developing a recycling industry for the safe handling and material recovery from the generated e-waste volumes. Electronic waste may contain up to 60 different materials including precious and rare-earth metals with estimated equivalent economic value of 48 billion Euros. Egypt is one the countries which lack policies and regulations dedicated to e-waste, hence the Egyptian economy is missing the economic potential and job creation opportunities associated with developing the e-waste sector as well as facing major Environmental and health challenges due to the improper handling of the generated amounts. This paper briefly reviews the current situation of e-waste in Egypt. The paper then proposes an adapted guideline for the sustainable management of e-waste in Egypt. The guideline is based on the international regulations and guidelines especially those of countries similar to the Egyptian socioeconomic context. The produced guideline focuses on establishing a collection and recycling framework for e-waste in Egypt with EPR at its center. The guideline also establishes a financing system to help foster the recycling industry in Egypt while fulfilling the three main pillars of sustainability.

Validation of High Performance Liquid Chromatography with Fluorescence Detector Methods for Determination of Aflatoxins in Different Food and Animal Feed Samples

Method validation for quantitative analysis of aflatoxins, AFB1, AFB2, AFG1 and AFG2 in sorghum, peanut butter, groundnut and animals feed is presented. Aflatoxins were extracted with a mixture of methanol: acetonitrile: water (60:30:10) and cleaned with Alfa test IAC chromatography before analysis by High Performance Liquid Chromatography coupled with fluorescence detection (HPLC-FLD) by adopting an isocratic chromatographic system using a mobile phase consisting acetic acid: acetonitrile: methanol (59:14:27), the separation of the four aflatoxins was achieved in less than 15 minutes. Calibration curves were linear over the concentration range from 2 - 18 ng/mL for AFB1 and AFG1, 0.4 - 3.6 ng/mL for AFB2 and AFG2, respectively. The LOD and LOQ in spiked samples were found to be 0.02 and 0.05 μg/kg for both AFB1 and AFG1, 0.01 and 0.03 μg/kg for both AFB2 and AFG2. The mean recovery values were in range from 84.2% to 96.9% was obtained. Five samples were found to be contaminated with aflatoxins and the total aflatoxins ranged from 0.02 to 3.26 μg/kg were obtained. Nineteen different samples were found to be contaminated with aflatoxins;the total aflatoxins ranged from 0.27 to 10.48 μg/kg were obtained. The highest total aflatoxins value was obtained in animal feeds.

Problem of living liver donation in the absence of deceased liver transplantation program:Mansoura experience

We report our experience with potential donors for living donor liver transplantation(LDLT), which is the first report from an area where there is no legalized deceased donation program. This is a single center retrospective analysis of potential living donors(n = 1004) between May 2004 and December 2012. This report focuses on the analysis of causes, duration, cost, and various implications of donor exclusion(n = 792).Most of the transplant candidates(82.3%) had an experience with more than one excluded donor(median = 3). Some recipients travelled abroad for a deceased donor transplant(n = 12) and some died before finding a suitable donor(n = 14). The evaluation of an excluded donor is a time-consuming process(median = 3 d, range 1 d to 47 d). It is also a costly process with a median cost of approximately 70 USD(range 35 USD to 885 USD). From these results, living donor exclusion has negative implications on the patients and transplant program with ethical dilemmas and an economic impact. Many strategies are adopted by other centers to expand the donor pool; however, they are not all applicable in our locality. We conclude that an active legalized deceased donor transplantation program is necessary to overcome the shortage of available liver grafts in Egypt.

Early detection of hepatocellular carcinoma co-occurring with hepatitis C virus infection: A mathematical model

AIM: To develop a mathematical model for the early detection of hepatocellular carcinoma (HCC) with a panel of serum proteins in combination with α-fetoprotein (AFP).METHODS: Serum levels of interleukin (IL)-8, soluble intercellular adhesion molecule-1 (sICAM-1), soluble tumor necrosis factor receptor II (sTNF-RII), proteasome, and β-catenin were measured in 479 subjects categorized into four groups: (1) HCC concurrent with hepatitis C virus (HCV) infection (n = 192); (2) HCV related liver cirrhosis (LC) (n = 96); (3) Chronic hepatitis C (CHC) (n = 96); and (4) Healthy controls (n = 95). The R package and different modules for binary and multi-class classifiers based on generalized linear models were used to model the data. Predictive power was used to evaluate the performance of the model. Receiver operating characteristic curve analysis over pairs of groups was used to identify the best cutoffs differentiating the different ***: We revealed mathematical models, based on a binary classifier, made up of a unique panel of serum proteins that improved the individual performance of AFP in discriminating HCC patients from patients with chronic liver disease either with or without cirrhosis. We discriminated the HCC group from the cirrhotic liver group using a mathematical model (-11.3 + 7.38 × Prot + 0.00108 × sICAM + 0.2574 × β-catenin + 0.01597 × AFP) with a cutoff of 0.6552, which achieved 98.8% specificity and 89.1% sensitivity. For the discrimination of the HCC group from the CHC group, we used a mathematical model [-10.40 + 1.416 × proteasome + 0.002024 × IL + 0.004096 × sICAM-1 + (4.251 × 10-4) × sTNF + 0.02567 × β-catenin + 0.02442 × AFP] with a cutoff 0.744 and achieved 96.8% specificity and 89.7% sensitivity. Impact of cirrhosis on surgical outcome after pancreaticoduodenectomy

Prognostic Value of Retinal Nerve Fiber Layer and Ganglion Cell Complex after Optic Chiasmal Decompression

This paper is an attempt to ascertain the role of the optical coherence tomography by measuring the retinal nerve fiber layer thickness and ganglion cell complex area to predict postoperative visual outcome after chiasmal decompression. 16 eyes scheduled for chiasmal decompression surgery were assessed before and 3 months after surgery with standard automated perimetry and OCT (optical coherence tomography). Preoperative RNFL (retinal nerve fibre layer) thickness and GCC (ganglion cell complex) area were compared with 20 normal control eyes. 13 cases were operated by microscopic assisted endoscopic endonasal transsphenoidal approach;the remaining 3 cases were operated transcranially. Spearman’s correlation analysis was used to evaluate the relationship between preoperative RNFL thickness, GCC area, postoperative mean deviation and temporal visual field sensitivity (1/Lambert). Preoperative measurements of RNFL thickness and all GCC area were significantly reduced in the patients compared with normal control. 3 months postoperative evaluation showed improvement of the visual field, but reduction in global and sectorial RNFL thickness except for nasal sector. Moreover, absolute postoperative (not pre-post change) visual field parameters were significantly correlated to preoperative RNFL (P = 0.00399 for mean deviation, P = 0.0023 for temporal sensitivity), GCC thickness (P = 0.00736 for mean deviation, P = 0.0469 for temporal sensitivity), with FLV (focal loss value) (P = 0.0012 for mean deviation, P = 0.0021 for temporal sensitivity) showed a higher correlation. Reduced RNFL thickness mainly, and GCC area minimally, were associated with the worst visual field outcome. FLV is a new prognostic value.

A Cradle-to-Cradle Novel Approach for Wastewater Management in Sustainable Urban Communities

Water, a valuable resource to human lives, is being abused and driven to scarcity. This scarcity is leading some countries and areas to face difficulty in accessing drinking water. As the UN recently stated “by 2050 water shortages and harder access will be reached by around 2/3rd of the world total population” [1], thus, there is a high need to treat and reuse wastewater for domestic purposes, which will lead to less reliance on fresh water as an initial water source. Greywater—defined as the water produced in domestic houses including sinks and bathroom showers, and excluding any blackwater mix which is collected from toilets—is a type of wastewater. Greywater accounts for up to 75% of the daily water produced [2] while it has fewer contaminants when compared to blackwater. This makes greywater a focal point for treatment, and reusing to conserve fresh water and approach net zero water concept. Even though the definition of greywater is the same globally, its criteria can differ from one country to another, from one building to another, or even from the same person’s usage along the day. Accordingly, several treatment methods evolved over years aiming at treating the produced greywater for reuse mainly in irrigation and toilet flushing. The objective of this paper is to demonstrate a novel net zero wastewater approach applying cradle-to-cradle concept for urban communities;while also proposing a sustainable greywater treatment technique that is environmentally friendly, cost-effective and socially acceptable.
