Role of Vaginal Progesterone in Prevention of Preterm Labor in Women with Previous History of One or More Previous Preterm Births

Objective: To evaluate whether prophylactic administration of 200 mg vaginal progesterone can reduce the incidence of preterm birth in women with documented history of preterm birth Setting: Obstetrics and Gynecology Department, Zagazig University Hospital, Egypt. Methods: Ninety patients with previous history of preterm birth prior to 37 week presenting with singleton pregnancy between 20 - 24 weeks were randomly allocated to receive either the progesterone 200 mg vaginal suppository or no treatment. Results: The incidence of preterm labor before 37 weeks of gestation was significantly lower in the study group than in the control group (22.2% vs. 53.3%) especially in earlier gestational ages. While, the mean birth weight was significantly higher in the study group than in control group (2872.67 ± 565.76 gm vs. 2487.78 ± 742.40 gm). The neonatal morbidities and mortality associated with preterm labor were significantly lower in the study group than in the control group as shown by lower incidence of neonatal RDS (13.3% vs. 31.1%;P = 0.043) and lower incidence of the need for NICU admission (15.6% vs. 35.5%;P = 0.03). Conclusion: Administration of prophylactic vaginal progesterone (200 mg, daily) can significantly reduce the rate of preterm birth before 37, 32 and 28 wks of gestation among women with previous spontaneous preterm birth. In addition, the rates of RDS and admission to NICU were significantly decreased among infants of women assigned to progesterone treatment. Also, there was an additional benefit of vaginal progesterone for prevention of preterm birth in women who had prior spontaneous preterm birth and cervical length 25 mm.

Simultaneous Repair of Para-Umbilical Hernia during Cesarean Section (CS): A Novel Approach

Background: Pregnant women that are complaining from paraumbilical hernia postpone its repair until they get birth. We hypothesized that it will be better to perform hernia repair of such type of hernia simultaneously during performing cesarean section (CS) which will help to decrease future morbidity re-operation, avoid complications and further skin incision. In this study we aimed to compare the value of performing para-umbilical hernia repair simultaneously during performing CS through the same skin incision with performing para-umbilical hernia repair simultaneously during performing CS through another infra- or supra-umbilical skin incision and performing para-umbilical hernia repair electively later on after healing of a CS skin incision in relation to clinical recovery and patient satisfaction. Patients and Methods: This is a prospective cohort study, where we included 45 pregnant female patients who will give birth by CS, and we have divided them into 3 groups: the first group of patients (A) included 15 patients that undergoing paraumbilical hernia repair by pre-peritoneal mesh insertion through CS incision, the second group of patients (B) in-cluded 15 patients that undergoing paraumbilical hernia repair by infra- or supra-umbilical incision during CS incision and the third group of patients (C) included 15 patients that undergoing paraumbilical hernia repair by infra- or supra-umbilical incision later on after healing of the CS wound. We have evaluated advantages of that novel approach e.g. operation time, severity of pain, peri-partum and post-operative complications, financial cost, duration of hospital stay, clinical recovery, mesh rejection, and patient satisfaction. Results: In group A there is shorter duration of hospital stay, no new skin incision (p 0.002). Tectonic Style Associated with the Doleritic Dykes of the Téra-Ayorou Pluton (Liptako, Western Niger)

The doleritic dykes present in the Téra-Ayorou pluton crosscut the basement of the Nigerien Liptako and are part of the system of intrusive mafic dykes in the Paleoproterozoic domain of the Léo-Man ridge. In the previous work, emphasis was placed on the petrology, geochemistry and geochronology of the Liptako doleritic dykes. This study aims to analyze the tectonic style of intrusive doleritic dykes in the Téra-Ayorou pluton. The characterization of the deformation which affected the doleritic dykes of the Téra-Ayorou pluton is important for the evaluation of their economic potential. To this end, measurements of tectonic structure planes were taken in the field, and samples were taken from the chilled margin and cores of dolerite dykes, before being processed in the laboratory. Analysis of the tectonic structures collected revealed a brittle tectonic pattern, characterizing a phase of deformation subdivided into two episodes D1 and D2. Episode D1, with its subvertically dipping normal faults and simple N70˚ - N110˚ fractures, is compatible with N-S extension. On the other hand, episode D2, which created shear corridors by reactivation of pre-existing fractures and oriented N150˚ - N170˚, is associated with a WSW-ENE extension. These results open up prospects for the exploration of gold, uranium mineralization and diamonds in the north-west of Niger.

Diagnostic Accuracy of Strain Imaging of the Left Ventricle in Detection of Obstructive Coronary Artery Disease

Background: Speckle tracking echocardiography using average global strain and strain rate (SR) parameters for early detection of high risk patients with coronary artery disease (CAD) has gained a substantial clinical interest. Objective: Assessment of the diagnostic accuracy of strain imaging of the left ventricle in detection of obstructive coronary artery disease. Methods: One hundred patients were enrolled. They were divided into 3 groups: Group I (n = 40 patients) presented with ST segment elevation myocardial infarction (STEMI), Group II (n = 40 patients) presented with non-ST segment elevation myocardial infarction (NSTEMI) or unstable angina (UA) and Group III (n = 20 patients) with normal coronary angiography served as a control group. All patients were subjected to 2D speckle tracking echocardiography (2D STE) to assess LV longitudinal strain and strain rate (SR). Sensitivity, specificity and diagnostic accuracy of 2D STE in prediction of CAD and its severity using Gensini score were assessed. Results: The mean age for Group I, II and III was 52.20 ± 11.83, 51.97 ± 14.53 and 52.75 ± 10.75 respectively. Objective:The objective of this study was to assess the prevalence and level of social media addiction among nursing students in the Faculty of Nursing,Cairo *** and Methods:A descriptive cross‑sectional research design was conducted on samples of 340 students at the Faculty of Nursing,Cairo *** were collected through demographic background information sheet and Social Networking Addiction ***:All the students were addicted as 6.76%were severely addicted and 60.59%and 32.65%were moderately and mildly addicted,*** relations were found between social media addiction and students’age(χ^(2)=11.331,P=0.003),educational level(χ^(2)=20.239,P=0.003),and grade point average(χ^(2)=19.378,P=0.013).Conclusion:Internet addiction was prevalent among all students but at different levels,so early screening of students for Internet addiction using the Internet Addiction Scale is important to provide early treatment and prevent hazards to health.

Utilization of Egyptian Ilmenite Ore for Steel Surface Preparation in Petroleum Field

The construction of gas and oil plants include many items such as steel structures, equipments, piping, tanks and vessels at different conditions such as offshore, onshore, above and underground shall be protected against corrosion by coating. The protective value of a coating system is proportional to the surface preparation;the objectives of the surface preparation are the surface cleanliness and the surface profile of the substrate. The surface preparation is usually achieved by blast cleaning using different types of abrasive. Due to its high hardness and low cost, silica sand has been used extensively for impact abrasion. It is, however, very brittle with most grain disintegrating on impact, causing excessive levels of siliceous dust which is a major health hazard and causes serious environmental pollution. Consequently, it has been banned as impact abrasive material in most countries around the world. Considerable research has been done in the last twenty years to find an impact abrasive material as hard as silica and that respects. So it was important to search for sand substitution among the local abundant minerals in Egypt. Waste product obtained from ilmenite ore extracted from the mine was found to be the most suitable alternative abrasive. This work shows the abrasive types, the occurrence of the ilmenite ore. The chemical properties, physical properties and microscopic examination of the waste were investigated. The obtained result indicates that, the waste product obtained from ilmenite ore can be utilized for abrasion as sand substitution.

Validity of QRS Configuration and Myeloperoxidase Level as Determinants of CAD Severity and Prognosis in Patients with STEMI

Background: Terminal QRS distortion and fragmentation (fQRS) with elevated myeloperoxidase (MPO) were linked to poor cardiovascular outcomes in acute coronary syndrome. We aimed to investigate these parameters in early prediction of coronary artery disease severity based on SYNTAX score and in-hospital adverse events in STEMI patients. Methods: A total of 215 patients with first STEMI admitted for primary PCI were included in the study. They were divided according to the admission ECG into group I with QRS distortion or fQRS, group II with combined QRS distortion and fQRS, and group III without QRS distortion or fQRS. Myeloperoxidase and troponin I levels, ST resolution ratio, left ventricular EF%, and severity of coronary artery lesions using SYNTAX risk score were measured. Results: MPO level, SYNTAX score, and in-hospital mortality were higher in group I and II and were higher in group II compared to group I. By regression analysis, QRS distortion, fQRS, and MPO > 412 ng/ml were independent predictors of both CAD severity and in-hospital mortality. DM was an independent predictor of CAD severity (OR: 2.851, P 0.012) while high SYNTAX score was an independent predictor of in-hospital mortality (OR: 6.113, P 0.001). Adding MPO level to any QRS configuration pattern increased predictive value for the detection of CAD severity that was more evident in the combined QRS distortion and fragmentation. Conclusion: Terminal QRS distortion, fragmentation, or combined QRS distortion and fragmentation have a significant value in predicting in-hospital adverse events and CAD severity

Chronic Kidney Disease in Sub-Saharan Africans: A Study of 462 Patients

Chronic kidney disease is a global public health problem due to its increasing prevalence as well as its main risk factors such as hypertension and diabetes. However, in Africa, few studies have been done on chronic kidney disease. The aim of our study is to describe the epidemiological, clinical, paraclinical and therapeutic aspects of chronic kidney disease. It was a retrospective and descriptive study carried out from the first of January 2004 to the 31st of December 2013 at Principal hospital in Dakar. Records of any patient aged 18 and over with chronic kidney disease were included. Chronic kidney disease was defined according to the KDIGO 2012 recommendations. Among the 8873 patient records used during our study, 462 presented with chronic kidney disease, which was a hospital prevalence of 5.2%. The sex ratio was 1.61. The mean age of the patients was 58.37 ± 19.97 years. There were 75.32% of the patients who were aged 50 and over. The mean serum creatinine was 49.14 ± 56.83. The mean glomerular filtration rate was 27.47 ± 19.86 ml/min/1.73 m2. Chronic renal failure was diagnosed in 92% of patients, including 34.9% in the end-stage of the renal disease. The mean proteinuria was 3.07 ± 4.92 g/24 h. Leukocyturia was present in 34.17% of patients. Hematuria was present in 25% of patients. Hypertension and diabetes were the most common causes, found in 61.25% and 35.93% of patients, respectively. Hemodialysis was performed in 49 patients. Peritoneal dialysis was performed in 2 patients. One patient had undergone a kidney transplant. This study establishes the relatively high prevalence of chronic kidney disease and its risk factors including hypertension and diabetes. Evaluation of Gallium Arsenide Thermal Expansion Coefficient by Extended X-Ray Absorption Fine Structure

Negative thermal expansion of gallium arsenide has been investigated through temperature dependent Extended X-ray Absorption Fine Structure (EXAFS) measurements. The bond thermal expansion coefficient αbond has been evaluated and compared to negative expansion coefficient αtens due to tension effects. The overall thermal expansion coefficient is the sum?of?αbond?and αtens. Below 60 K, αtens is greater than αbond? yielding to a negative expansion in this temperature region. Tension effects are progressively overcome by the stretching effects in the region 60 - 300 K. The asymmetry of nearest neighbors distribution is not negligible since the gaussian approximation underestimates the bond expansion by about 0.00426 Å. This error decreases when the temperature is lowered. The accuracy in the thermal expansion evaluation and the connection between third cumulant and thermal expansion are discussed.
