Efficacy and safety of treatment of hepatitis C virus infection in renal transplant recipients

AiM: To assess the efficacy and safety of combinedpegylated interferon and ribavirin therapy in hepatitis C virus (HCV) infection in renal transplant recipients. METHODS: This is a retrospective chart review of post renal transplant patients who were positive for anti-HCV and HCV-RNA, and who have received treatment with combination of pegylated interferon and ribavirin between October 2003 and December 2008. Only patients with stable graft function and absence of evidence of cirrhosis and who received the therapy for continuous 48 wk were included. Nineteen patients (13 male and 6 female) were identified and included. The patient's complete blood count, liver and kidney prof ile, and cal- culated glomerular f iltration rate (GFR) were monitored every 6-8 wk while on treatment. HCV-RNA was tested at 12 wk for early virological response, at 48 wk for end of treatment response (ETR), and then retested at 24, and 48 wk after completion of therapy for sustained virological response (SVR). Liver biopsies were obtained before treatment from all patients and graft kidney biopsies were performed as required. RESULTS: Of the entire cohort, 9 patients (47.4%) showed an ETR and 8 had SVR (42.1%). Of the 8 patients with abnormal alanine aminotransferase (alT) levels at baseline, 78.9% had their alT normalized (including the virological non responders). alT was normal in all responders at the end of therapy and at 24 wk post therapy (100%). Only one patient (5.3%) developed an increase in creatinine and decline in GFR from baseline towards the end of treatment. This patient's kidney biopsy revealed borderline rejection. There was no impact on response by HCV-genotype, initial HCV RNA load, age or sex of the patient or duration post transplant before commencement of therapy. all patients tolerated treatment in the same way as non-transplant with no unusual or increased occurrence of side effects. Effectiveness of external prestressing in enhancing the non-ductile hanger failure mechanism in reinforced concrete inverted T-beams

Recently,inverted T-beams have been used in reinforced concrete(RC)bridges to support transverse precast *** T-beams,contrary to practice with conventional beams,are loaded on the flanges upper *** loading configuration causes hanger failure due to the generation of vertical tensile stresses near the bottom of the *** key purpose of this study is to investigate the efficiency of vertical external prestressing stainless-steel bars in mitigating non-ductile hanger failure in reinforced concrete inverted *** experimental study on six inverted-T beams,including two un-strengthened specimens,was carried *** study showed that the value of the prestressing level had a considerable impact on the performance of hanger mechanism in relation to crack pattern,ultimate loads,cracking behavior,load-deflection,strains,and *** experimental results indicated that the suggested method for strengthening inverted T-beams had efficacy in reducing the seriousness of the non-ductile hanger failure and resulted in a strength increase of up to 53% when compared to that of the un-strengthened ***,two analytical models for estimating the hanger capacity and the average crack width of the strengthened RC inverted T-beams were *** models that were proposed exhibited a high degree of agreement with the experimental results.

Exploring the impact of tacrolimus on kidney transplant recipients: insights into immunological outcomes

To minimize graft rejection, it is crucial to reduce immunosuppression. in the present study, we investigated immunological markers in the blood of transplant patients who did not receive tacrolimus. We also assessed alloreactive CD4+CD25+Fox P3+ Tregs both before transplantation and after tacrolimus reduction. The comparative analysis included 50 individuals experiencing early rejection after tacrolimus discontinuation and 100 non-rejectors. Before reducing tacrolimus, rejectors exhibited elevated levels of memory CD8+ T cells compared to Tregs, while non-rejectors displayed a lower ratio of memory CD4+ T cells to Tregs. Following tacrolimus removal, rejectors showed an increase in na?ve T cells and a decrease in effector T cells. After transplantation, CD4+ Treg levels declined, but their anti-donor regulatory function remained consistent in both rejectors and non-rejectors. immunological correlation revealed an association between acute rejection after tacrolimus discontinuation and the pre-reduction ratio of memory T cells to regulatory T cells. These indicators identified individuals whose immunosuppression could be safely reduced.

The role of polymorphic cytochrome P450 gene(CYP2B6)in B-chronic lymphocytic leukemia(B-CLL)incidence and outcome among Egyptian patients

Cytochromes P450(CYPs)play a prominent role in catalyzing phase i xenobiotic biotransformation and account for about 75%of the total metabolism of commercially available drugs,including *** gene expression and enzyme activity of CYPs are variable between individuals,which subsequently leads to different patterns of susceptibility to carcinogenesis by genotoxic xenobiotics,as well as differences in the efficacy and toxicity of clinically used *** research aimed to examine the presence of the CYP2B6*9 polymorphism and its possible association with the incidence of B-CLL in Egyptian patients,as well as the clinical outcome after receiving cyclophosphamide *** was isolated from whole blood samples of 100 de novo B-CLL cases and also from 100 sex-and age-matched healthy *** presence of the CYP2B6*9(G516T)polymorphism was examined by PCR-based allele specific amplification(ASA).Patients were further indicated for receiving chemotherapy,and then they were followed *** CYP2B6*9 variant indicated a statistically significant higher risk of B-CLL under different genetic models,comprising allelic(T-allele vs.G-allele,OR=4.8,pinant(GT+TT ***,OR=5.4,pide chemotherapy,we found that the patients with variant genotypes(GT+TT)were less likely to achieve remission compared to those with the wild-type genotype(GG),with a response percentage of(37.5%vs.83%,respectively).in conclusion,our findings showed that the CYP2B6*9(G516T)polymorphism is associated with B-CLL susceptibility among Egyptian *** variant greatly affected the clinical outcome and can serve as a good therapeutic marker in predicting response to cyclophosphamide treatment.

in this paper,we present a comprehensive system model for industrial internet of Things(iioT)networks empowered by Non-Orthogonal Multiple Access(NOMA)and Mobile Edge Computing(MEC)*** network comprises essential components such as base stations,edge servers,and numerous iioT devices characterized by limited energy and computing *** central challenge addressed is the optimization of resource allocation and task distribution while adhering to stringent queueing delay constraints and minimizing overall energy *** system operates in discrete time slots and employs a quasi-static approach,with a specific focus on the complexities of task partitioning and the management of constrained resources within the iioT *** study makes valuable contributions to the field by enhancing the understanding of resourceefficient management and task allocation,particularly relevant in real-time industrial *** results indicate that our proposed algorithmsignificantly outperforms existing approaches,reducing queue backlog by 45.32% and 17.25% compared to SMRA and ACRA while achieving a 27.31% and 74.12% improvement in Qn ***,the algorithmeffectively balances complexity and network performance,as demonstratedwhen reducing the number of devices in each group(Ng)from 200 to 50,resulting in a 97.21% reduction in complexity with only a 7.35% increase in energy *** research offers a practical solution for optimizing iioT networks in real-time industrial settings.

Study of scalar particles through the Klein–Gordon equation under rainbow gravity effects in Bonnor–Melvin-Lambda space-time

in our investigation,we explore the quantum dynamics of charge-free scalar particles through the Klein–Gordon equation within the framework of rainbow gravity,considering the Bonnor–Melvin-Lambda(BML)space-time *** BML solution is characterized by the magnetic field strength along the axis of the symmetry direction which is related to the cosmological constantΛand the topological parameterαof the *** behavior of charge-free scalar particles described by the Klein–Gordon equation is investigated,utilizing two sets of rainbow functions:(i)f(χ)=■,h(χ)=1 and(ii)f(χ)=1,h(χ)=1+βХ/*** 0ith E representing the particle's energy,Ep is the Planck's energy,andβis the rainbow *** obtain the approximate analytical solutions for the scalar particles and conduct a thorough analysis of the obtained ***,we study the quantum dynamics of quantum oscillator fields within this BML space-time,employing the Klein–Gordon *** also,we choose the same sets of rainbow functions and obtain the approximate eigenvalue solution for the oscillator ***,we demonstrate that the relativistic approximate energy profiles of charge-free scalar particles and oscillator fields get influenced by the topology of the geometry and the cosmological ***,we show that the energy profiles of scalar particles receive modifications from the rainbow parameter and the quantum oscillator fields by both the rainbow parameter and the frequency of oscillation.
