Objective:We aimed to evaluate the efficacy of topical estrogen after transvaginal tension-free vaginal tape-obturator(TVT-O)in the treatment of de novo overactive bladder symptoms that appear after ***:This is a prospective randomized controlled study performed in the Urology and Gynecology Departments,Kasr Al Ainy Hospital,Cairo University,Cairo,*** hundred and ten postmenopausal females presenting during the period between January 2017 and November 2020 with stress urinary incontinence were included in the *** were divided into two groups,105 patients in Group A(treatment group)and 105 patients in Group B(control group).Patients in Group A underwent transvaginal TVT-O followed by local vaginal estrogen treatment for 6 months,while patients in Group B underwent transvaginal TVT-O *** study included any postmenopausal female with urodynamic stress urinary *** patients had to fulfill a 3-day bladder diary,overactive bladder symptoms score,urine analysis,urodynamic study,and post-voiding residual urine measurement by abdominal ultrasound preoperatively and at 3-month and 6-month ***:At 6-month follow-up,daytime frequency was reduced to 8%in Group A(increased to 21%in Group B)with a statistically significant difference between both groups(p=0.009).At 6-month follow-up,nocturia was 8%in Group A(11%in Group B)with no statistically significant difference between both groups(p=0.469).There was a statistically significant difference between both groups as regards to urinary urgency at 6-month follow-up(p=0.024).There was a statistically significant difference in postoperative wound healing events as regards to cure,hyperemia,gapping,and wound infection 1 week after intervention between both groups(p=0.008).No local or systemic side-effects were reported from local estrogen ***:Local vaginal estrogen treatment given to postmenopausal patients after midurethral sling procedures can reduce the symptoms o

Intelligent traffic control requires accurate estimation of the road states and incorporation of adaptive or dynamically adjusted intelligent algorithms for making the *** this article,these issues are handled by proposing a novel framework for traffic control using vehicular communications and Internet of Things *** framework integrates Kalman filtering and *** smoothing Kalman filtering,our data fusion Kalman filter incorporates a process-aware model which makes it superior in terms of the prediction *** traditional Q-learning,our Q-learning algorithm enables adaptive state quantization by changing the threshold of separating low traffic from high traffic on the road according to the maximum number of vehicles in the junction *** evaluation,the model has been simulated on a single intersection consisting of four roads:east,west,north,and south.A comparison of the developed adaptive quantized Q-learning(AQQL)framework with state-of-the-art and greedy approaches shows the superiority of AQQL with an improvement percentage in terms of the released number of vehicles of AQQL is 5%over the greedy approach and 340%over the state-of-the-art ***,AQQL provides an effective traffic control that can be applied in today’s intelligent traffic system.

Objective:We described the technique and outcomes of robot-assisted repair of uretero-enteric strictures(UES)following robot-assisted radical cystectomy(RARC)and urinary ***:Retrospective review of our RARC database from November 2005 to August 2023 at Roswell Park Comprehensive Cancer center was *** who developed UES and ultimately underwent robot-assisted uretero-enteric reimplantation(RUER)were *** method was used to compute the cumulative incidence recurrence rate of UES after RUER.A multivariable regression model was used to identify variables associated with UES ***:A total of 123(15%)out of 808 RARC patients developed UES,of whom 52 underwent reimplantation(45 patients underwent RUER[n=55 cases]and seven patients underwent open ureteroenteric reimplantation).The median time from RARC to UES was 4.4(interquartile range 3.0e7.0)months,and the median time between UES and RUER was 5.2(interquartile range 3.2e8.9)*** 3-year recurrence rate after RUER is about 29%.On multivariable analysis,longer hospital stay(hazard ratio 1.37,95%confidence interval 1.16e1.61,p<0.01)was associated with recurrent UES after ***:RUER for UES after RARC is feasible with durable outcomes although a notable subset of patients experienced postoperative complications and UES recurrence.

背景:全膝关节置换术(TKA)是缓解慢性膝关节炎患者关节疼痛和增强关节功能的重要手段。TKA的并发症常常与大量失血相关,并直接导致术后血红蛋白水平下降,从而高达69%的患者需要异体输血来应对术后贫血。这已被证明是增加不良结果风险的独立因素,包括延长住院时间和增加术后感染的易感性。由丹麦学者Henrik Kehlet于1997年首次提出的加速康复外科(ERAS)已成为现代外科实践中的一个关键方法。ERAS最初在腹部手术中实施,逐渐扩展到包括骨科和泌尿科在内的各个医学专业。然而,尽管ERAS方案的采用日益增加,系统评估关节置换术后的血液指标仍然有限。这种多方面的策略在减轻手术压力、加速术后恢复、减少住院时间和提高整体功能结果方面发挥着重要作用,同时保持低并发症率。之前的研究已证明ERAS在减少失血和优化全髋关节置换术结果方面的有效性。问题是这些发现是否也适用于TKA。THA和TKA之间的关键区别,包括患者人口统计学、手术时间和软组织损伤程度,强调了优化每项手术中患者护理的需要。TKA患者通常比THA患者平均年龄更大,皮肤对皮肤时间更长,并且由于股骨和胫骨截骨术,软组织损伤增加。ERAS方案有效控制围手术期失血量。ERAS组术后CRP和ESR水平较低,表明炎症和凝血活动减少,这可能归因于ERAS计划提供的全面护理,其中包括人性化和以患者为中心的干预措施,以减轻焦虑。ERAS手术已显示出相当大的好处,例如通过利用术前优化、术中程序和术后管理等循证策略,最大限度地减少围手术期失血、降低并发症和改善营养状况。WOMAC、OKS和SF-12的理想结果表明ERAS方案有利于改善术后短期内的短期恢复和生活质量结果。目的:评估在加速康复外科(ERAS)下进行单侧全膝关节置换术(TKA)的患者的血液指标管理效果。材料与方法:回顾性研究了2019年1月至2023年10月在我们医院接受单侧TKA的患者。通过使用0.2校准器进行倾向评分匹配后,这些病例被分为两组:非ERAS组(n=51)接受标准护理,ERAS组(n=51)接受全面ERAS管理。术后比较了主要血液参数/血液指标,同时在第3个月随访时也评估了临床结果。结果:ERAS组术后Hb明显高于非ERAS组(P<0.05)。非ERAS组术前ESR明显高,而ERAS组CRP明显低于非ERAS组(P<0.05)。ERAS组的LOS和Alb明显少于非ERAS组(P<0.05)。WOMAC和OKS得分在第1个月和第3个月均在ERAS组。结论:ERAS方案在术后第一天到第三个月的术后期间提供了良好且现实的临床和功能结果,对接受单侧初次TKA的患者满意度很高。此外,ERAS方案已被证明可以有效减少围手术期失血、最大限度地减少术后并发症并维持这些患者的最佳营养水平。

Accurate software cost estimation in Global Software Development(GSD)remains challenging due to reliance on historical data and expert *** models,such as the Constructive Cost Model(COCOMO II),rely heavily on historical and accurate *** addition,expert judgment is required to set many input parameters,which can introduce subjectivity and variability in the estimation ***,there is a need to improve the current GSD models to mitigate reliance on historical data,subjectivity in expert judgment,inadequate consideration of GSD-based cost drivers and limited integration of modern technologies with cost *** study introduces a novel hybrid model that synergizes the COCOMO II with Artificial Neural Networks(ANN)to address these *** proposed hybrid model integrates additional GSD-based cost drivers identified through a systematic literature review and further vetted by industry *** article compares the effectiveness of the proposedmodelwith state-of-the-artmachine learning-basedmodels for software cost *** the NASA 93 dataset by adopting twenty-six GSD-based cost drivers reveals that our hybrid model achieves superior accuracy,outperforming existing *** findings indicate the potential of combining COCOMO II,ANN,and additional GSD-based cost drivers to transform cost estimation in GSD.
